OBJECTIVES: To determine the level of cultural competence of European critical care nurses. DESIGN: A multi-country survey performed in 2017 as a part of the European project Multicultural Care in European Intensive Care Units. METHOD: Online survey of critical care nurses in 15 European countries (n = 591) using the Healthcare Provider Cultural Competence Instrument consisting of 49 items divided into five subscales: awareness and sensitivity, behaviour, patient-centred orientation, practice orientation and self-assessment. Descriptive and correlational analyses were performed. RESULTS: Critical care nurses scored highest for 'awareness and sensitivity' (M = 5.09, SD = 0.76), and lowest for 'patient-centred communication' (M = 3.26, SD = 0.94). Nurses from northern and southern Europe scored higher across all subsets of the cultural competence instrument (all subsets, p < 0.001) than nurses from central Europe. Speaking other languages significantly correlated with higher scores in all subscales (all > 0.05) except 'practice orientation'. Previous education on multicultural nursing significantly correlated with higher scores in all subscales (all > 0.01) except patient-centred communication; and visits to other countries was negatively correlated with all subscales (all, p > 0.001) except patient-centred communication. CONCLUSION: Being exposed to cultural diversity in different ways, like living in a multicultural country, speaking a second language and visiting other countries may influence development of cultural competence. Therefore, programmes which facilitate multicultural clinical practice are strongly recommended in nursing education.

V štátoch vyspelej Európy je najčastejšou príčinou kardiopulmonálneho zlyhania náhla zástava obehu. Mortalita sa pohybuje okolo 65-95 % pri zástave mimo zdravotníckeho zariadenia a 40-50 % pri náhlej zástave obehu v nemocnici. Prognóza pacientov je neistá, ale môže byť zlepšená včasnou a efektívnou kardiopulmonálnou resuscitáciou. Cieľom kardiopulmonálnej resuscitácie je zyýšiť počet prežívaní u obetí kardiopulmonálneho zlyhania. Za úspešnú resuscitáciu považujeme resuscitáciu, výsledkom ktorej je primeraná kvalita života. Pri obnovení krvného obehu s pretrvávajúcim závažným neurologickým deficitom je považovaná resuscitácia za primárne úspešnú, ale pre pacienta neprospešnú. V predkladanom príspevku sa zaoberáme ošetrovateľskou starostlivosťou o pacienta v poresuscitačnom období. Účinnosť a úspešnosť liečebného postupu a realizovaných ošetrovateľských intervencií treba hodnotiť v širších súvislostiach. V súčasnosti sa u pacientov, ktorí boli resuscitovaní z dôvodu komorovej fibrilácie a u ktorých pretrváva bezvedomie po obnovení obehu, odporúča aplikovať metódu liečebnej hypotermie 32-34°C počas 12-24 hodín. Metóda liečebnej hypotermie je jednou z ľahko dostupných možností, ktoré môžu prispieť k pozitívnemu výsledku. Ošetrovateľská starostlivosť o pacienta v poresuscitačnom období patrí medzi najnáročnejšie. Kontinuálny monitoring vitálnych funkcií, správne naplánované a realizované ošetrovateľské intervencie sú súčasťou ošetrovateľskej starostlivosti o pacientov s neistou prognózou.
In the advanced European states, the most common cause of cardiopulmonary failure is sudden circulatory arrest. Mortality is around 65-95% when a shut down happens outside the medical facility and 40-50% at a sudden shutdown in the hospital. Patient prognosis is uncertain but may be improved by early and effective cardiopulmonary resuscitation. The aim of cardiopulmonary resuscitation is to increase the number of survivors in cardiopulmonary failure victims. For successful resuscitation we consider resuscitation resulting in adequate quality of life. In resuscitation of the blood circulation with persistent severe neurological deficiency, resuscitation is considered to be primarily successfuls but does not benefit the patient. In the present paper we deal with nursing care of the patient in the postresuscitation period. The effectiveness and success of the treatment process and the nursing interventions should be evaluated in a wider context. Currently at the patients who have been resuscitated due to ventricular fibrillation and who remain unconscious after resumption of circulation it is recommending to apply the 32-34 ° C hepatic hypotermy treatment method for 12-24 hours. The method of hepatic hypotermia is one of the readily available options that can contribute to a positive outcome. Nursing care for a patient in the postresuscitation period is one of the most demanding. Continuous monitoring of vital functions, properly planned and implemented nursing interventions are part of nursing care for patients with uncertain prognosis.
