Dyssynchronie srdeční kontrakce je významným faktorem vzniku srdečního selhání. Vzniká na podkladě vynuceného šíření srdečního vzruchu pracovním myokardem při dysfunkci převodního systému. U dětí a mladistvých je tato situace obvykle následkem operace vrozené srdeční vady vedoucí k raménkové blokádě nebo na podkladě nevhodně aplikované trvalé kardiostimulace. Aktivace srdečních komor cestou pracovního myokardu je mnohem pomalejší a směr jejího šíření je abnormální, dochází k významnému opoždění aktivace volné stěny komory oproti mezikomorové přepážce. Tím je narušena synchronie kontrakce nutná k efektivní funkci komory. Vzniká nehomogenita v rozložení práce myokardu vedoucí k redistribuci perfuze, spotřeby kyslíku a úrovně metabolismu ve prospěch více zatížených segmentů, proces provází změny na subcelulární úrovni. Rozvíjí se asymetrická hypertrofie postižené komory spolu s její dilatací a sníženou funkcí – dyssynchronní kardiomyopatie. Rozvoji dyssynchronní kardiomyopatie lze předcházet pečlivou volbou místa dlouhodobé stimulace či ji lze léčit resynchronizační terapií levé, resp. pravé komory.
Dyssynchrony of cardiac contraction is an important factor in development of heart failure. It is caused by surrogate conduction of action potential by working myocardium in case of conduction system dysfunction. In children and adolescent, this situation is often caused by damage to conduction system by cardiac surgery for congenital heart defect leading to bundle branch block or by inappropriate cardiostimulation. Activation of ventricle via working myocardium is much slower and vector of spreading is abnormal. It leads to significantdelay of free wall activation in contrast to early activation of interventricular septum. This disrupts synchrony of ventricular activation which is mandatory for effective ventricular contraction. There is an inhomogeneity in the distribution of myocardial work, leading to redistribution of perfusion, oxygen consumption and levels of metabolism in favor of more loaded segments, there are also changes at the subcellular level. Asymmetric hypertrophy of the affected ventricle develops together with its dilatation and reduced function – so called dyssynchronopathy. The development of dyssynchronopathy can be prevented by careful selection of the site of long-term stimulation or it can be treated by resynchronization therapy of the left, resp. right ventricle.

Human natural killer (HNK)-1 antibody is an established marker of developing cardiac conduction system (CCS) in birds and mammals. In our search for the evolutionary origin of the CCS, we tested this antibody in a variety of sauropsid species (Crocodylus niloticus, Varanus indicus, Pogona vitticeps, Pantherophis guttatus, Eublepharis macularius, Gallus gallus, and Coturnix japonica). Hearts of different species were collected at various stages of embryonic development and studied to map immunoreactivity in cardiac tissues. We performed detection on alternating serial paraffin sections using immunohistochemistry for smooth muscle actin or sarcomeric actin as myocardial markers, and HNK-1 to visualize overall staining pattern and then positivity in specific myocyte populations. We observed HNK-1 expression of various intensity distributed in the extracellular matrix and mesenchymal cell surface of cardiac cushions in most of the examined hearts. Strong staining was found in the cardiac nerve fibers and ganglia in all species. The myocardium of the sinus venosus and the atrioventricular canal exhibited transitory patterns of expression. In the Pogona and Crocodylus hearts, as well as in the Gallus and Coturnix ones, additional expression was detected in a subset of myocytes of the (inter)ventricular septum. These results support the use of HNK-1 as a conserved marker of the CCS and suggest that there is a rudimentary CCS present in developing reptilian hearts. Wolffův-Parkinsonův-Whiteův (WPW) syndrom je definován přítomností akcesorní dráhy a s ní souvisejících symptomů. Typickým EKG obrazem preexcitace je krátký interval PQ, přítomnost vlny delta a rozšíření komplexu QRS na povrchovém EKG. Podstatou syndromu preexcitace je existence akcesorní dráhy, která umožňuje převod vlny depolarizace ze síní na komory mimo atrioventrikulární (AV) uzel a predisponuje ke vzniku arytmií a náhlé smrti. Nejčastěji se vyskytující arytmií u pacientů s WPW syndromem je atrioventrikulární reentry tachykardie. Není přítomna u všech pacientů s preexcitací [1-4]. Až u jedné třetiny pacientů s atrioventrikulární reentry tachykardií (AVRT) se vyskytuje fibrilace síní, která může být díky rychlému převodu spojkou převedena na svalovinu komor a vést k rozvoji život ohrožující fibrilace komor. Nejúčinnější léčbou WPW syndromu je radiofrekvenční ablace akcesorní dráhy [2,5-8]. Článek popisuje kasuistiku 40leté ženy po kardiopulmonální resuscitaci pro fibrilaci komor, která byla primomanifestací WPW syndromu u této pacientky. Zároveň uvádí patofyziologii, klinický obraz a možnosti léčby pacientů s WPW syndromem.
Wolff-Parkinson-White syndrome (WPW) is defined as a condition involving an accessory pathway associated with symptoms. A typical ECG pattern of a pre-excitation shows a short PQ interval, presence of delta wave and a broad QRS complex on surface ECG. The underlying mechanism involves an accessory pathway, which enables conduction of a depolarization wave from atria to ventricles bypassing the AV node and predisposes to arrhythmias and sudden cardiac death. The most common arrhythmia in patients with WPW syndrome is atrioventricular reentrant tachycardia. However, it is not present in all patients with pre-excitation [1-4]. Up to 1/3 of patients with AVRT experience atrial fibrillation, which may be conducted to ventricular myocardium via the accessory pathway and lead to a life-threatening ventricular fibrillation. The most effective treatment of the WPW syndrome is a radiofrequency catheter ablation [2,5-8]. This paper describes a case of a 40-year-old woman after a cardiopulmonary resuscitation for ventricular fibrillation, which was a primary manifestation of the WPW syndrome. It focuses on pathophysiology, clinical pattern and treatment possibilities of patients with WPW syndrome.

Recent progress in search of mechanisms regulating thespatio-temporal formation of specialized conduction tissues pushes us further in our understanding of the developmental molecular mechanisms of myocardial patterning. It is not clear which mechanisms direct the fates of chamber myocytes to differentiate intoventricular CCS. Similarly, the formation and nature of connections between its most distal component, the Purkinje fibers, and the working myocytes, is unresolved. The process of remodelling of originally ring-like AV junction into AV node and accompanying fibrous insulation between the atria and ventricles is poorly understood, especially on the molecular level. Perturbation of this process can result in abnormal atrio-ventricular connections, manifesting as Wolf-Parkinson-White syndrome of ventricular pre-excitation. Understanding the signalling mechanisms involved in CCS development may be of significance to clinicians and basic researchers studying adult cardiac disease. Congenital abnormalities in CCS, as well as ectopic or inappropriate induction of CCS tissue in the mature heart, may be processes that contribute to, or cause, cardiac conduction disturbance and arrhythmia in adults. Understanding the signalling mechanisms that give rise to normal development of the CCS may thus provide insight into cardiac disease.
