The worldwide incidence of abnormally invasive placenta is rapidly rising, following the trend of increasing cesarean delivery. It is a heterogeneous condition and has a high maternal morbidity and mortality rate, presenting specific intrapartum challenges. Its rarity makes developing individual expertise difficult for the majority of clinicians. The International Society for Abnormally Invasive Placenta aims to improve clinicians' understanding and skills in managing this difficult condition. By pooling knowledge, experience, and expertise gained within a variety of different healthcare systems, the Society seeks to improve the outcomes for women with abnormally invasive placenta globally. The recommendations presented herewith were reached using a modified Delphi technique and are based on the best available evidence. The evidence base for each is presented using a formal grading system. The topics chosen address the most pertinent questions regarding intrapartum management of abnormally invasive placenta with respect to clinically relevant outcomes, including the following: definition of a center of excellence; requirement for antenatal hospitalization; antenatal optimization of hemoglobin; gestational age for delivery; antenatal corticosteroid administration; use of preoperative cystoscopy, ureteric stents, and prophylactic pelvic arterial balloon catheters; maternal position for surgery; type of skin incision; position of the uterine incision; use of interoperative ultrasound; prophylactic administration of oxytocin; optimal method for intraoperative diagnosis; use of expectant management; adjuvant therapies for expectant management; use of local surgical resection; type of hysterectomy; use of delayed hysterectomy; intraoperative measures to treat life-threatening hemorrhage; and fertility after conservative management.

Placenta accreta je v současnosti jednou z hlavních indikací k provedení postpartální hysterektomie. Je to diagnóza spojená s významnou fetomaternální morbiditou a mortalitou. V etiopatogenezi poruchy se jako hlavní příčina uplatňuje myometrální trauma. Současná prenatální diagnostika (ultrasonografie, magnetická rezonance) umožňuje časnou selekci rizikových pacientek a jejich referenci do perinatologických center s možností zajištění multioborové péče.
Placenta accreta is the most common indication to perform peripartum hysterectomy. The diagnosis is related to significant foetomaternal morbidity and mortality. Myometrial damage was identified as a major risk factor. Prenatal diagnosis (ultrasound, MRI) can be helpful in selection of patients in risk. Placenta accreta je těhotenskou patologií, jejíž incidence se pohybuje kolem 1:2500 porodů. Stoupá však se vzrůstajícím počtem císařských řezů, a nabývá tedy na významu. Za standardní postup v případě průkazu placenta accreta je považováno operační řešení – hysterektomie. Za určitých podmínek je však možno přistoupit ke konzervativnímu postupu, který umožní zachování dělohy. Níže uvádíme dvě kazuistiky, kdy bylo po konzultaci s pacientkou rozhodnuto o konzervativním postupu, který vedl k úspěšnému zvládnutí této těhotenské patologie. V obou případech byla diagnóza akrétní placenty stanovena až s určitým odstupem od porodu, a to na základě klinického a ultrazvukového nálezu. Rozhodnutí o konzervativním postupu mohlo být učiněno díky tomu, že ani u jedné pacientky nevznikla život ohrožující situace, která by si vyžádala akutní operační řešení. Placenta accreta is an obstetric pathology with incidence around 1:2500 deliveries. This incidence is increasing due to increasing number of caesarean sections. Hysterectomy is considered to be a standard solution in this situation. Anyway in some cases we can decide to perform a conservative way which means that uterus is saved. There are two case reports below in which we decided to proceed conservatively. We consulted the patient before doing this and in both cases we were succesful in keeping the uterus. In both cases we were able to determine diagnosis of placenta accreta up to some time after delivery. The diagnosis was made according to ultrasonography and clinical examination. We were able to manage conservative way of treatment because there were no situations that could cause an acute operating solution. Key words: placenta accreta, conservative treatment, hysterectomia.

Placenta accreta is one of the leading causes of peripartum hemorrhage. The goal of this article is to review anesthetic management of parturients with placenta accreta and to examine a modern approach to massive peripartum hemorrhage. RECENT FINDINGS: The incidence of placenta accreta is rising in parallel with the increased rate of cesarean delivery. If accreta is diagnosed or suspected preoperatively, anesthetic management can be optimized. Even with the best possible management, the blood loss associated with placenta accreta can resemble that of a major trauma. The use of Damage Control Resuscitation strategies to guide transfusion may improve morbidity and mortality. SUMMARY: Careful planning and close communication are essential between anesthesiology, obstetric, interventional radiology, gynecologic oncology, blood bank, and specialized surgical teams when taking care of a patient with placenta accreta.
