Předčasné ovariální selhání (POF – premature ovarian failure, POI – premature ovarian insuficiency) je u ženy s reprodukčními plány závažný stav, který více méně vylučuje mít vlastní biologické dítě. Kromě chybění funkčních oocytů v ovariích jde také o předčasný nedostatek pohlavních hormonů, který má celkový negativní vliv na zdraví. Článek nás provede péčí jak v ambulanci registrujícího gynekologa, tak léčbou v centru reprodukční medicíny. Diagnóza a léčba POF nám názorně ilustruje některé endokrinologické principy a souvislosti.
Premature ovarian failure (POF, POI – premature ovarian insufficiency) is a serious situation for a woman with reproductive plans that more or less precludes having her own biological child. In addition to the lack of functional oocytes in the ovaries, it is also a premature lack of sex hormones, which has an overall negative effect on health. The article guides us through care both in the clinic of the gynecologist and through treatment in the center of reproductive medicine. The diagnosis and treatment of POF illustrates some endocrinological principles and connections.

BACKGROUND AND PURPOSE: Acupoint stimulation and Chinese herbal medicines (CHM) are widely used in the treatment of premature ovarian insufficiency (POI), but the efficacy and safety remain controversial. This systematic review aims to evaluate the efficacy and safety of acupoint stimulation and CHM for POI. METHODS: Seven databases were searched and collected studies comparing acupoint stimulation and CHM with hormone replacement therapy (HRT) from inception to July 31, 2019. The methodological quality of the included trials was assessed in line with the criteria of the Cochrane risk of bias assessment tool. RESULTS: Meta-analysis was performed in 14 trials, which contained a total of 1030 women with POI. The acupoint stimulation and CHM presented advantages in normalizing of menstrual cycle (RR 2.06, 95% CI 1.62 to 2.61, P < 0.00001) and improving perimenopausal symptoms (RR 2.00, 95% CI 1.56 to 2.56, P < 0.00001) when compared with HRT. After treatment, compared with HRT, acupoint stimulation and CHM effectively decreased the level of follicle stimulating hormone (MD -2.88, 95% CI -5.00 to -0.76, P = 0.008) and increased the level of estradiol (SMD 0.88, 95% CI 0.06 to 1.71, P = 0.04). By contrast, there were no significant between-group differences in the level of luteinizing hormone (MD -3.24, 95% CI -6.77 to 0.29, P = 0.07) and adverse effects (RR 0.31, 95% CI 0.04 to 2.54, P = 0.28). CONCLUSION: This meta-analysis suggested that acupoint stimulation and CHM can serve as complementary therapies to alleviate menstrual disorders, perimenopausal symptoms, and serum sex hormone levels in POI females.

Předčasné ovariální selhání je heterogenní onemocnění, které přináší řadu zdravotních rizik. Musíme myslet na to, že nasazení antikoncepce u dospívajících dívek může toto onemocnění dlouhodobě maskovat. Metody asistované reprodukce přinášejí naději na vlastní dítě i ženám s předčasným ovariálním selháním. Substituce hormonálního deficitu je důležitá pro odstranění nepříjemných pocitů spojených s předčasným ovariálním selháním i pro snížení rizika pozdních následků.
Premature ovarian failure is a heterogeneous disease that brings about several health risks. We must consider that the use of contraception in adolescent girls can mask this disease for a long time. Assisted reproductive technology has brought hope to women with premature ovarian failure to have their own child. Substitution of hormonal deficiency is important for eliminating unpleasant feelings associated with premature ovarian failure as well as for reducing the risk of late effects.

V důsledku ovariální suprese nebo ablace dochází k výskytu menopauzálních symptomů. Nejčastěji se vyskytují vazomotorické symptomy, vaginální atrofie, poruchy spánku. Dále dochází ke změně kostní denzity, ke změně nálad až výskytu depresí, objevuje se i kardiální toxicita. Jejich management by měl být komplexní. U žen s karcinomem prsu není možno použít hormonální substituční terapii – tzn. estrogeny, popř. kombinaci estrogenů s gestageny. Je nutno volit alternativy nefarmakologické nebo farmakologické, event. jejich kombinaci. Vhodné je zavést známá režimová opatření – snažit se omezit teplé nápoje, kořeněná jídla, konzumaci kávy, alkoholických nápojů a oblékat se ve více vrstvách, které mohou být postupně odstraněny. Dále je možné je doplnit o další nefarmakologické či farmakologické metody. Z nefarmakologických metod je možno využít akupunktury, kognitivní behaviorální terapie, z farmakologických skupin jsou využívány nejčastěji léky ze skupiny antidepresiv – SSRI (selektivní serotonin reuptake inhibitor), SNRI (serotonin norepinefrine reuptake inhibitor), dále gabapentin nebo pregabalin. Ze skupiny SSRI má nejvíce dat k dispozici paroxetin, ale možno je využít i dalších preparátů – citalopramu, escitalopramu nebo sertralinu. Paroxetin není vhodné použít u pacientek léčených tamoxifenem vzhledem k metabolizaci obou preparátů na cytochromu CYP2D6. Ze skupiny SNRI je k dispozici venlafaxin. Progestační látky byly testovány především na pacientkách s karcinomem prsu a podání obou látek – medroxyprogesteron acetátu nebo megestrol acetátu prokázalo benefit v řešení návalů horka. Vaginální atrofii lze řešit lokálním podáním lubrikačních gelů, popř. krátkodobou lokální aplikací estrogenů. Vzhledem k polymorfním symptomům je při jejich řešení vhodná multioborová spolupráce.
Ovarian suppression or ovarian ablation used in treatment of breast carcinoma results in temporary or permanent menopause and associated menopausal symptoms – most frequently vasomotoric symptoms (hot flashes, sweats), vaginal atrophy, sleep disturbances. Patients can also experience frequent decrease in bone density (osteopenia, osteoporosis), mood swings or depression, less frequently cardiac toxicity. Managements of these symptoms is complex. As hormonal replacement therapy (estrogens or combined estrogen/gestagen therapy) is contraindicated in women with breast carcinoma, other available options include non-hormonal pharmacological or non-pharmacological methods or their combinations. Women should be advised about cooling techniques and how to avoid known triggers; these measures should be combined with other non-pharmacological and pharmacological intervention. Non-pharmacological methods include the use of acupuncture or cognitive behavioral therapy. Some tips to help stay cool and decrease hot flashes – avoid hot beverages, spicy food, limit coffee or alcohol intake, dress in layers of clothing that can be removed if necessary. Pharmacological options include most frequently antidepressants – SSRI (selective serotonin reuptake inhibitor), SNRI (serotonin norepinephrin reuptake inhibitor), or alternatively gabapentin or pregabali. A very promising drug is paroxetine with a lot of clinical trials. Only this drug has FDA approval for the indication of hot flashes. Paroxetine can lead to disproportional changes in plasma levels of drug in CYP2D6 metabolism and thus it is not suitable for combination of paroxetine with tamoxifen. Several studies demonstrated the effectiveness of the newer generation of SSRI – citalopram, escitalopram, sertralin and duloxetin in ameliorating hot flashes. Venlafaxine in dose 75 or 150 mg has been associated with a 61% reduction in hot flashes frequency if compared to 27% reduction with placebo. Medroxyprogesterone acetate and megestrol acetate were investigated especially in patients with breast cancer history and both drugs demonstrate an effect in hot flashes treatment. Management of vaginal atrophy is challenging. Vaginal dryness/atrophy can be relieved with use of topical lubricants/gels or possibly in highly symptomatic patients with short term use of topical estrogens. As these symptoms require highly complex management, multidisciplinary approach is recommended.

STUDY QUESTION: What is the optimal management of women with premature ovarian insufficiency (POI) based on the best available evidence in the literature? SUMMARY ANSWER: The guideline development group (GDG) formulated 99 recommendations answering 31 key questions on the diagnosis and treatment of women with POI. WHAT IS KNOWN ALREADY: NA. STUDY DESIGN, SIZE, DURATION: This guideline was produced by a multidisciplinary group of experts in the field using the methodology of the Manual for ESHRE Guideline Development, including a thorough systematic search of the literature, quality assessment of the included papers up to September 2014 and consensus within the guideline group on all recommendations. The GDG included a patient representative to ensure input from women with POI. After finalization of the draft, the European Society for Human Reproduction and Embryology (ESHRE) members and professional organizations were asked to review the guideline. PARTICIPANTS/MATERIALS, SETTING, METHODS: NA. MAIN RESULTS AND THE ROLE OF CHANCE: The guideline provides 17 recommendations on diagnosis and assessment of POI and 46 recommendations on the different sequelae of POI and their consequences for monitoring and treatment. Furthermore, 24 recommendations were formulated on hormone replacement therapy in women with POI, and two on alternative and complementary treatment. A chapter on puberty induction resulted in five recommendations. LIMITATIONS, REASONS FOR CAUTION: The main limitation of the guideline is that, due to the lack of data, many of the recommendations are based on expert opinion or indirect evidence from studies on post-menopausal women or women with Turner Syndrome. WIDER IMPLICATIONS OF THE FINDINGS: Despite the limitations, the guideline group is confident that this document will be able to guide health care professionals in providing the best practice for managing women with POI given current evidence. Furthermore, the guideline group has formulated research recommendations on the gaps in knowledge identified in the literature searches, in an attempt to stimulate research on the key issues in POI. STUDY FUNDING/COMPETING INTERESTS: The guideline was developed and funded by ESHRE, covering expenses associated with the guideline meetings, with the literature searches and with the implementation of the guideline.
