Celiakie je velmi časté autoimunitní onemocnění vznikající u geneticky predisponovaných jedinců, kteří konzumují lepek. Celosvětová prevalence je udávána kolem 1 %. Většina případů však není diagnostikována. Klinické projevy jsou nesmírně pestré a postihují řadu orgánových systémů. Kromě klinického nálezu (často necharakteristického) je základem diagnostiky pozitivita sérologického vyšetření (autoprotilátky proti tkáňové transglutamináze) a histologické vyšetření tenkého střeva (biopsie). Článek pojednává o racionální diagnostice nemoci s důrazem na interpretační úskalí jednotlivých testů.
Celiac disease is a very common autoimmune disorder caused by the ingestion of dietary gluten products in genetically susceptible persons. Its global prevalence is estimated around 1 %. However, the most cases are not diagnosed. Clinical presentation is widely variable with the involvement of various human systems. Besides a clinical picture (that is often non characteristic), the diagnosis is based on positivity of serological testing (tissue transglutaminase autoantibodies) and histological evaluation of small intestinal mucosa. The article presents a rational diagnostic approach to celiac disease.

Aim: To investigate the utility of routine endoscopic distal duodenal biopsy in patients with Irritable Bowel Syndrome (IBS) for the diagnosis of Celiac Disease (CD). Method: This study was conducted at the outpatient Department of Gastroenterology at the Institute of Medicine, Tribhuvan University, Kathmandu, Nepal. One hundred patients with IBS selected by Rome III Criteria were prospectively evaluated from April 2015 through March 2016. Esophagogastroduodenoscopy (EGD) was performed on all patients, with four-quadrant distal duodenal biopsies obtained from each patient. The biopsy sections were studied for Marsh-type histology, a histology characteristic of CD. Patients with Marsh-type histology had Immunoglobulin-A anti-tissue transglutaminase (IgA anti tTG) serology ordered, were recommended to start a gluten-free diet (GFD), and were followed up in clinic in 3-6 weeks. The prevalence of CD (defined as Marsh histology grade I-III with serology positive responding to GFD) was calculated in patients with IBS as a whole and among IBS subtypes. Statistical analysis was done using SPSS version 19 software (Chicago, IL, USA). Results: Of the 100 IBS patients evaluated, 50 patients had abnormal duodenal histology. Among these 50 patients, 13 had Marsh-type histology, and the remaining 37 had non-specific duodenitis. Of the 13 patients with Marsh-type histology, 4 were seropositive for CD and 9 were seronegative. The overall prevalence of CD was 4%. All 4 seropositive and 2 out of 9 seronegative patients responded to GFD in 3 to 6 weeks. Conclusion: Routine endoscopic distal duodenal biopsy is useful for the diagnosis of CD in patients with IBS as it could also detect the disease in patients with negative celiac serology.

Základem Marshovy klasifikace celiakální sprue, či malabsorpčního syndromu v užším slova smyslu, je zvýšený počet intraepiteliálních lymfocytů, jakožto první a konstantní znak tohoto onemocnění. Teprve poté dochází ke strukturálním změnám sliznice, zejména architektury klků, krypt a enterocytů. Podle uvedené klasifikace odpovídá typ O normálnímu nálezu. Typ 1, infíltrativní, má více než 40 intraepiteliálních lymfocytů /100 enterocytů v jinak normální sliznici. Typ 2 má kromě zvýšeného počtu intraepiteliálních lymfocytů i prohloubené krypty. Typ 3, destruktivní, vykazuje zvýšený počet intraepiteliálních lymfocytů s postupnou atrofíí klků až po vyhlazenou sliznici a vzácný typ 4 má vyhlazenou sliznici, ale nezvýšený počet intraepiteliálních Ijonfocytů. Kombinací těchto biopticky postižitelných změn, anamnézy a klinického nálezu lze definovat šest klinicko-patologických stavů. Marshova klasifikace celiakální sprue je diagnosticky přínosná a měla by být aplikována ve všech případech diagnostiky malabsorpčního syndromu. Nedostatkem tohoto systému, který lze uplatnit na formol-parafinovém materiálu, je omezení diferenciální diagnostiky celiakální sprue, zejména stanovení reaktivity disacharidáz. Z tohoto hlediska by biopsie měla být doplněná histochemickým vyšetřením vzorku, zaměřeným na reaktivitu disacharidáz, zejména laktázy.
Marsh' classification of celiac disease or malabsorption syndrome sensu stricto is based on increased number of intraepithelial lymphocytes, as the first and constant sign/feature of the disease. Thereafter follow structural alterations of the mucous membrane, particularly of villous architecture, crypt height and enterocytes. According to the Marsh' classification, type 0 corresponds to normal mucous membrane, and type 1, the infiltrative type, is characterised by an increase in intraepithelial lymphocytes (lEL), which amount to more than 40 lymphocytes/100 enterocytes. Type 2 has normal villous architecture, an increase in lEL numbers and crypt hyperplasia. Type 3, the destructive type, is characterised by villous atrophy and may be divided into three subgroups depending on the degree of the atrophy - mild, marked, and total (flat mucosa). Combination of these histopathological findings, patient's history and clinical course may result in the definition of six „states' of celiac disease. Marsh' classification is helpful for an early diagnosis of the disease and should be applied in any case of the bioptical examination of the malabsorption syndrome. The disadvantage of this system, applied on formol-paraffin tissue sections, is impossibility of revealing disachaiidases (particularly lactase) deficiency. From this point of view, histochemical detection of disacharidases should accompany the bioptical examination of jejunal mucosa.
Celiakie je ve stáří diagnostikována zřídka. Studie naznačují, že prevalence celiakie je ve stáří vyšší. Základem diagnostiky celiakie je vyšetření protilátek proti tkáňové transglutamináze a protilátek proti endomysiu. Patologické hodnoty těchto testů indikují provedení gastroskopie a biopsie ze sliznice aborálního duodena a histologické vyšetření s použitím Marshovy klasifikace. Práce shrnuje prevalenci celiakie ve stáří, komorbidity, prodlení v diagnóze, benefit bezlepkové diety.
Celiac disease is poorly diagnosed in elderly. Studies have suggested that the prevalence of celiac disease in ederly is higher. The base of diagnosis celiac disease is an analysis of antibodies against tissue transglutaminase and antibodies against endomysium. A pathological value of the test indicates the performance of gastroscopy with a biopsy of the aboral duodenum mucosa and the histological examination using the Marsh classification. This review summarizes prevalence of celiac disease in elderly, comorbidity, delay in diagnosis, benefit of gluten- free diet.

Celiakie je v české populaci nedostatečně diagnostikována, většinou po dlouhém trvání nemoci. Nediagnostikovaná celiakie často vede k rozvoji osteoporózy, autoimunitních chorob i zhoubných onemocnění. Osoby s celiakií, která není diagnostikována, mají proti osobám séronegativním téměř čtyřnásobně větší úmrtnost. Zavedení cíleného screeningu je zásadním krokem v omezení negativních důsledků nepoznané nemoci. U osob indikovaných ke screeningu celiakie se provede kvantitativní vyšetření protilátek proti tkáňové transglutamináze a celkového IgA. Patologická hodnota indikuje provedení gastroskopie s biopsií sliznice aborální části duodena. Po histologickém vyšetření s využitím Marshovy klasifikace jsou dalšími možnými kroky vyšetření protilátek proti endomysiu, diferenciální diagnostika vylučující choroby napodobující celiakii, genetické vyšetření a potvrzení diagnózy pozitivní klinickou a sérologickou odezvou na bezlepkovou dietu.
Celiac disease is poorly diagnosed in Czech population, mostly after a long duration of the disease. Undiagnosed celiac disease often leads to the development of osteoporosis, autoimmune disorders and malignant diseases. People with celiac disease, where the disease hasn´t been diagnosed, have almost four times increased mortality compared to serum negative persons. The introduction of targeted screening is an essential step to limiting the negative consequences of unrecognized disease. The persons indicated for celiac disease screening a quantitative test for antibodies against tissue transglutaminase and total IgA is performed. A pathological value of the test indicates the performance of gastroscopy with a biopsy of the aboral duodenum mucosa. After the histological examination using the Marsh classification, the following steps can be taken: testing antibodies against endomysium; differential diagnosis of excluding any disease mimicking celiac disease; genetic testing and confirmation of diagnosis using a positive clinical and serological response to a gluten – free diet. histology, genetics.
