BACKGROUND: Ethics is an inherent part of psychotherapy that protects the interests and rights of all parties engaged in the therapeutic relationship. This article focuses on ethical issues and dilemmas that may arise when using schema therapy. METHOD: We created a narrative review by searching the databases PubMed, Web of Science, and Scopus with the keywords "psychotherapy", "schema therapy", "therapeutic relationship", "ethics", "ethical questions", and "ethical dilemmas". In addition, we focused on the clinical experience of therapists, training instructors, and supervisors. RESULTS: Ethical psychotherapy requires adherence to ethical codes and standards. Among the most important ethical principles are confidentiality, informed consent, boundaries of the therapeutic relationship, and dual relationships. Understanding transference, countertransference, and one's modes and schemas is essential to ethical reflection in schema therapy. The article presents examples of ethical dilemmas in schema therapy and suggests possible solutions. At the same time, we point out the need for further research in this field. CONCLUSION: Similarly to other psychotherapeutic approaches, one of the schema therapist's core competencies is following the profession's ethical principles and productively finding solutions to the occasional ethical dilemmas. Ethics is a prominent part of all psychotherapeutic sessions. Still, it becomes even more central when working with challenging issues such as personality disorders that schema therapy routinely treats. More research on the topic is needed.

Self-awareness can be characterised as impartial, non-judgmental thoughtful attention towards the self. Self-reflection in therapy is when a therapist reviews their experiences, thoughts, and behaviours concerning therapy and changes them as needed to enhance the therapeutic process. Therapists with good quality self-reflection can make more effective and ethical decisions, differentiate their own needs from clients', understand transference and countertransference, and consider the optimal response during a session. Practising the CBT approach and reflecting on one's own experiences can be essential for successful therapeutic development. Furthermore, self-reflection forms the basis of a fruitful therapeutic relationship and the therapist's self-confidence and sense of competence.

Dysfunctional patterns, beliefs, and assumptions that affect a patient's perception of other people often affect their perceptions and behaviours towards the therapist. This tendency has been traditionally called transference for its psychoanalytical roots and presents an important factor to monitor and process. In supervision, it is important to put the patient's transference in the context of the conceptualization of the case. Countertransference occurs when the therapist responds complementary to the patient's transference based on their own dysfunctional beliefs or assumptions. Transference and countertransference provide useful insights into the inner world of the patient, therapist, and supervisor. Guided discovery is one of the most common approaches used by a supervisor and a supervisee to map all types and directions of transference and countertransference. Other options to map transference and countertransference are imagery and role-playing techniques.

Psychoanalytická psychoterapie dětí a adolescentů je významnou terapeutickou modalitou, která prošla v oblasti teorie a praxe podobným vývojem jako psychoanalýza. Autoři připomínají původní myšlenky A. Freudové, M. Kleinové a dalších psychoanalytiků, kteří dětskou psychoterapii ovlivnili. Zabývají se také současnými tématy, jako je například otázka indikace dětských pacientů pro psychoanalytickou psychoterapii, aspekty terapeutického procesu, úloha rodičů, problémy protipřenosu terapeuta.
Psychoanalytic psychotherapy in children and adolescents is an important therapeutic modality with a similar development as psychoanalysis in the area of theory and practice. Authors mention original ideas of A. Freud, M. Klein and other psychoanalysts who influenced child psychotherapy. They address present topics too, such as the matter of the indication of child patients for psychotherapy, the aspects of the therapeutic proces, the role of parents and the problems of the countertransference of the therapist.
V kauzistickej štúdii autorka prináša svoje skúsenosti zo psychoterapeutickej liečby klienta prostredníctvom použitia metódy katatýmne-imaginatívnej psychoterapie (KIP). Zameriava sa pri práci s klientom na fenomény prenosu, protiprenosu a odporu a možnosti ich analýzy a potenciál využitia v procese KIP. Autorka objasňuje motiváciu k použitiu KIP metódy. Na obrázkoch z imaginácií klienta ukazuje priebeh terapie aj prekonávanie odporu klienta k zmene. Kazuistika prináša tiež informácie o klientovi, prvý kontakt s ním, anamnestické údaje a diagnostickú úvahu. V závere hodnotí dosiahnuté výsledky terapeutickej liečby.
In this case study the author is introducing her own experience with psychotherapy treatment of the client by using the method of Guided Affective Imagery. The process of psychotherapy treatment is focused on transference, countertransference and resistance, possibilities of their analysis and potential of their use in the process of Guided Affective Imagery. Author also explains the motivation to use Guided Affective Imagery. In pictures from client's imaginations, author shows the therapy development and overcoming of client's resistance to change. This casuistic study also provides information about the client, first psychotherapeutic contact with him, anamnestic data and diagnostic considerations. In the conclusion, author evaluates the results of therapeutic treatment.
