The biofilm-forming microbial species Candida parapsilosis and Staphylococcus epidermidis have been recently linked to serious infections associated with implanted medical devices. We studied microbial biofilms by high resolution scanning electron microscopy (SEM), which allowed us to visualize the biofilm structure, including the distribution of cells inside the extracellular matrix and the areas of surface adhesion. We compared classical SEM (chemically fixed samples) with cryogenic SEM, which employs physical sample preparation based on plunging the sample into various liquid cryogens, as well as high-pressure freezing (HPF). For imaging the biofilm interior, we applied the freeze-fracture technique. In this study, we show that the different means of sample preparation have a fundamental influence on the observed biofilm structure. We complemented the SEM observations with Raman spectroscopic analysis, which allowed us to assess the time-dependent chemical composition changes of the biofilm in vivo. We identified the individual spectral peaks of the biomolecules present in the biofilm and we employed principal component analysis (PCA) to follow the temporal development of the chemical composition.

Kazuistika popisuje případ 73letého pacienta, který byl indikován k implantaci totální endoprotézy pravé kyčle pro artrózu třetího stupně. Operace i pooperační průběh probíhal bez komplikací. V časném pooperačním období, již na rehabilitačním oddělení se objevily známky zánětu operační rány. Za aseptických podmínek byla provedena nekrektomie rány a následná resutura dehiscence v lokální anestezii. Operační rána i přes veškerou snahu dále neprospívala, a tak byl pacient indikován k resutuře operační rány v celkové anestezii. Během léčby byl proveden stěr z rány na kultivaci a citlivost s výsledkem Staphylococcus epidermidis. Dle výsledků kultivace a citlivosti byla změněna antibiotická léčba. Kontrolní stěry z rány po reoperaci byly již sterilní, taktéž i drény z rány, které byly poslány na mikrobiologii.
A case report describes the case of a 73-year-old patient who was indicated for a total right hip endoprosthesis by reason ofa grade 3 coxarthrosis. The operation and the postoperative course were without complications. There were signs of surgicalwound in the early postoperative period at the rehabilitation department. Under aseptic conditions, necrectomy of the woundand subsequent re-suturing of the dehiscence was performed in local anaesthesia. The surgical wound, despite all the efforts,did not progress well, so the patient was indicated for re-suturing of the surgical wound under general anaesthesia. During thetreatment, a wound swab was performed for cultivation and sensitivity, with Staphylococcus epidermidis as a result. Accordingto the cultivation results and sensitivity, the antibiotic treatment was changed. Control wound swabs after the reoperation werealready sterile, as well as the wound drains which were sent to a microbiology lab.

AIM: Raman spectroscopy is an analytical method with a broad range of applications across multiple scientific fields. We report on a possibility to differentiate between two important Gram-positive species commonly found in clinical material - Staphylococcus aureus and Staphylococcus epidermidis - using this rapid noninvasive technique. MATERIALS & METHODS: For this, we tested 87 strains, 41 of S. aureus and 46 of S. epidermidis, directly from colonies grown on a Mueller-Hinton agar plate using Raman spectroscopy. DISCUSSION & CONCLUSION: The method paves a way for separation of these two species even on high number of samples and therefore, it can be potentially used in clinical diagnostics.

Infekční komplikace TEP kolenního kloubu je závažná komplikace. V jejím řešení je s úspěchem používaná metoda dvoudobé reimplantace se zavedením statického nebo artikulovaného spaceru. Kazuistika popisuje případ starší pacientky po primární implantaci TEP kolenního kloubu pro gonartrózu, u které byl průběh příznivý až do pádu, kdy došlo k částečnému poškození m. quadriceps femoris a hemartrosu. Následně došlo postupně k uvolnění TEP v terénu opožděné infekce, 39 měsíců po primární implantaci byla provedena extrakce TEP, zaveden statický spacer. Z důvodu zhoršení zdravotního stavu již nebylo možné reimplantaci TEP v druhé době provést. Výsledek 5 let od zavedení spaceru je uspokojivý, koleno je klinicky stabilní, zkrat končetiny je 3 cm. Pacientka je spokojená, mobilní a soběstačná. Statický spacer je možnost definitivního řešení infekční komplikace TEP kolenního kloubu ve výjimečném případě, kdy celkový stav pacienta neumožní další reoperaci.
Total knee arthroplasty infection is a serious complication. The two- stage revision with the static or articulated spacer implantation is a successful treatment option. This case report describes one case of an older woman after primary total knee arthroplasty for gonarthrosis, in which the postoperative course was positive, until a fall with partial m. quadriceps femoris rupture with haemarthros. The prosthesis was loosed in the area of delayed infection at that point. 39 months after the primary implantation, the extraction of the prosthesis with the static spacer implantation was performed. Re-implantation was not possible in second stage due to the health condition deterioration. The result 5 years after the spacer implantation is acceptable. The knee is stable; the leg is shorter by 3cm. The patient is satisfied and mobile. The static spacer is the final option of total knee arthroplasty infection treatment in the rare cases when re-implantation is not possible due to the patient’s health condition.

Cíl práce: Periprotetické infekce jsou závažnou komplikací totálních endoprotéz (TEP) kyčelního a kolenního kloubu. Součástí komplexního terapeutického přístupu v rámci dvojdobé reimplantace je použití spacerů (dočasné kloubní náhrady) s obsahem antibiotik. Cílem této práce bylo zhodnotit bakteriologické nálezy u vybraných pacientů s periprotetickou infekcí, u kterých byl použit komerčně připravený spacer s gentamicinem. Materiál a metodika: V letech 2008–2012 byl použit u 24 pacientů při ošetření infekce kloubní náhrady kyčelního nebo kolenního kloubu – v rámci dvojdobé reimplantace – ready-made cementový spacer s gentamicinem. K mikrobiologickému vyšetření byly z první revize odeslány všechny komponenty endoprotézy a vzorky periprotetické tkáně, v druhé době pak spacer a vzorky okolní tkáně. U šesti pacientů se zavedeným spacerem kolenního kloubu byla měřena hladinu gentamicinu v kloubní tekutině. Pacienti byli následně pravidelně sledováni. Výsledky: Výsledkem bakteriologického vyšetření byl kultivační průkaz agens v 22 případech z 24 (92 %). Nejčastěji byly vykultivovány koaguláza-negativní stafylokoky, dále byl izolován Staphylococcus aureus, korynebakteria, anaerobní bakterie a Salmonella Enteritidis, citlivých ke gentamicinu bylo 19 z 25 prokázaných agens (76 %). Kultivační vyšetření spacerů bylo pozitivní u dvou pacientů, vykultivovány byly koaguláza-negativní stafylokoky, v obou případech rezistentní ke gentamicinu. Po dobu sledování (minimálně 2 roky) pacientů nedošlo u žádného z nich k rozvoji další periprotetické infekce. Závěr: Úspěšnost reimplantací TEP kyčelního a kolenního kloubu ve dvojdobém rytmu s užitím ready-made spacerů s gentamicinem byla v našem souboru 100 %, po dobu minimálně dvou let nedošlo v našem souboru pacientů k zaznamenání dalšího periprotetického infektu. Z hodnocení nálezů bakteriologického vyšetření našeho souboru vyplývá, že ready-made spacer s gentamicinem pokrývá pouze 76 % spektra původců. Řešením by bylo použití spaceru nasyceného kombinací vankomycinu a gentamicinu, které by zajistilo antimikrobiální působení vůči všem vykultivovaným agens.
Background: Periprosthetic infection is a serious complication in total hip and knee arthroplasty. The complex therapeutic approach within two-stage reimplantation includes the use of antibiotic-impregnated spacers (temporary joint replacements). The aim of this paper was to evaluate bacteriological findings in selected patients with periprosthetic infection in whom a ready-made gentamicin-impregnated spacer was used to treat the infection. Materials and methods: Between 2008 and 2012, a ready-made, gentamicin-impregnated cement spacer was used in 24 patients to treat periprosthetic hip or knee infection within two-stage reimplantation. All components of the prosthesis and periprosthetic tissue samples were sent for microbiological examination at the first revision surgery, while at the second revision surgery, the spacer and surrounding tissue samples were sent in. In six patients with an inserted knee spacer, the level of gentamicin in the joint fluid was measured. Subsequently, the patients were regularly monitored. Results: Twenty-two (92%) of 24 patients were bacteriologically positive by culture. The most commonly detected causative agents were coagulase-negative staphylococci. Other isolates were Staphylococcus aureus, Corynebacterium, anaerobic bacteria, and Salmonella serotype Enteritidis. Nineteen (76%) of 25 primary pathogens were gentamicin sensitive. Spacers from two patients were culture positive for coagulase-negative staphylococci that tested resistant to gentamicin. During the follow-up of at least two years, none of the patients developed another periprosthetic infection. Conclusion: The success rate of two-stage-reimplantation total hip and knee arthroplasty using ready-made, gentamicin-impregnated spacers was 100 % in our cohort of patients; no other periprosthetic infection was reported during the follow-up of at least two years. From the bacteriological results, it appears that the ready-made, gentamicin-impregnated spacer only covers 76 % of the range of the causative agents. The solution would be to use a spacer impregnated with a combination of vancomycin and gentamicin that would be effective against all cultured species.

UNLABELLED: An understanding of adherence ability is crucial in many areas, for example, in research on biofilms, evaluation of probiotics or in biotechnology. In all these analyses, the reproducible washing is very important in the prevention of false results. During washing, the force, direction of the flow, position of the pipette tip, number of washing cycles, type of washing solution and the way of removing the washing solution can be sources of inappropriate stress to attached cells. To overcome these problems, we here propose the use of high mass density solutions as flotation agents. As the density of bacteria is lower than that of the flotation solutions, nonattached or weakly attached bacteria are moved to the surface due to hydrostatic force. Caesium chloride, ammonium nitrate and sodium diatrizoate solutions, which are commonly used as FAs, were compared with a standard method of rinsing. Several concentrations of agents were used to investigate the optimal concentration and influence of hydrostatic pressure on adhered micro-organisms. We show that flotation is a rapid method for distinguishing between adhered and weakly attached or loosed cells with reproducible results. Due to its range of possible mass density concentration, the best FA was shown to be caesium chloride. SIGNIFICANCE AND IMPACT OF THE STUDY: This is the first study that suggests using flotation agents to separate planktonic from adhered bacteria. When a high-density solution is used, buoyancy of bacteria ensures their segregation in the solution. Flotation agents could be used instead of washing procedure, which is inaccurate and hardly reproducible. High-density flotation agents could be used for more precise evaluation of bacterial adherence in many assays, such as research of biofilms or evaluation of probiotics.
V práci je sumarizovaná história terapeutického využitia bakteriálnych imunomodulátorov. Sú prezentované výsledky hojenia chronických rán pri chronickej venóznej insuficiencii (CVI) s infekciou vyvolanou Staphylococcus aureus vrátane MRSA lokálnym prípravkom Stafal (imunopreparát – antistafylokokový fágový lyzát). Stafal je použiteľný tak v humánnej ako aj veterinárnej medicíne pri všetkých formách stafylokokových infekcií. Jeho úlohou je rozrušenie stafylokokových buniek v mieste prebiehajúcej infekcie. Antistafylokokový fágový lyzát Stafal predstavuje významný prostriedok pri komplexnej liečbe chronických foriem stafylokokových infekcií (hnisavé afekcie kože a podkožia, infekcie postihujúce hlboko uložené mäkké tkanivo). Významný je fakt, že zabraňuje možnému prechodu do septického stavu. Je tiež dôležitou súčasťou preventívnych opatrení, s cieľom zabrániť vzniku následných pyogénnych komplikácií po operatívnych zásahoch.
The paper summarizes the history of therapeutical use of bacterial immunomodulators. The results of chronic wounds healing in chronic venous insufficiency (CVI) with infection caused by Staphylococcus aureus including MRSA with local drug Stafal (immunopreparation - anti-staphylococcus phage lysate) are presented. Stafal may be used both in human and veterinary medicine in all types of staphylococci infections. Its task is to disintegrate staphylococci cells in the area of an active infection. Anti-staphylococcus phage lysate Stafal represents a significant tool in a complex treatment of a chronic form of staphylococci infections (purulent affections of skin and subcutis, infections affecting deep soft tissues). The fact that it prevents the potential progress into a septic condition is significant. It is also the important part of precautions aimed to prevent the development of subsequent pyogenous complications after surgical procedures
