Mírná kognitivní porucha s sebou nese vysoké riziko vzniku demence. Existuje stále více důkazů, že kognitivní pokles má za následek zhoršení lokomočních parametrů. Ačkoli jsou obvykle spojeny s pozdějšími stadii demence, pokles výkonnosti chůze může být detekován mnohem dříve. Možným směrem v odhalení mírné kognitivní poruchy u osob po cévní mozkové příhodě je sledování lokomočních parametrů za podmínek dvojího úkolu.
Mild cognitive impairment entails a high risk of developing dementia. There is growing evidence that cognitive decline results in the deterioration of gait patterns. Though it is commonly associated with the later stages of dementia, a decline in gait performance may be detected much earlier. Observation in a dual task paradigm might improve the detection of mild cognitive impairment.

Cíl: Základem paradigmatu kognitivně-motorické interference je sounáležitost exekutivních a motorických funkcí. Podmínky běžného dne vedou k nutnosti souběžně plnit více úkolů najednou. Optimální funkční úprava po CMP by měla být účinná právě v kombinaci kognitivního a pohybového tréninku. Základní myšlenka neurorehabilitačního přístupu je tak spatřena především v potenciaci neuroplasticity. Doposud však nebyl prokázán zřejmý benefit kognitivně-motorické interference po CMP s ohledem na relativně nízkou kvantitu dostupných dat. Cílem systematického přehledu bylo potvrdit vliv kognitivně-motorické interference na zlepšení chůze a posturálního zajištění u pacientů po CMP. Metodika: Nejprve jsme formulovali výzkumnou otázku a specifikovali kritéria pro zařazení a vyloučení primárních studií do systematického přehledu. Vytvořili jsme systematické vyhledávací strategie a tříkrokové systematické vyhledávání publikovaných i nepublikovaných primárních studií v šesti databázích. Vyhledávali jsme experimentální a observační studie v rozmezí od 1. 1. 2003 do 31. 12. 2019 pouze v anglickém jazyce. Výsledky: Celkem jsme nalezli 58 studií a odstranili 20 duplikátů. Na základě dvoufázového hodnocení relevance a kritického hodnocení metodologické kvality studií pomocí standardizovaného hodnotícího nástroje Standard Quality Assessment Criteria for Evaluating a Primary Research Paper bylo zařazeno 5 metaanalýz a randomizovaných kontrolovaných studií. Závěr: Trénink kognitivně-motorické interference přináší benefit při lokomočních a posturálních strategiích u pacientů po CMP. Současný výzkum však ukázal i výraznou variabilitu v použití typu kognitivní zátěže.
Aim: The basis of the paradigm of cognitive-motor interference is the belonging of executive and motor functions. The conditions of a normal day lead to the need to perform several tasks simultaneously. Optimal functional adjustment after stroke should be effective in a combination of cognitive and movement training. The basic idea of the neurorehabilitation approach is primarily seen in the potentiation of neuroplasticity. However, so far, no obvious benefit of cognitive-motor interference has been demonstrated due to the relatively small amount of available data. The aim of the systematic review was to demonstrate the effect of cognitive-motor interference on the improvement of gait and postural stability in patients after stroke. Methods: First, we formulated a review question and specified criteria for the inclusion and exclusion of primary studies into a systematic review. We created a systematic search strategy and a three-step systematic search of published and unpublished primary studies in six databases. We searched for experimental and observational studies in the range from January 1, 2003 to December 31, 2019 in the English language only. Results: We found a total of 58 studies and removed 20 duplicates. Based on a two-phase evaluation of the relevance and critical evaluation of the methodological quality of the studies using the standardized evaluation tool Standard Quality Assessment Criteria for Evaluating a Primary Research Paper, 5 meta-analyses and randomized controlled trials were included. Conclusion: Cognitive-motor interference training brings benefits in locomotor and postural strategies in patients after stroke. However, current research has also shown a significant variability in the use of cognitive tasks.

Background: Gait disturbance accompanies many neurodegenerative diseases; it is characteristic for Parkinson's disease (PD). Treatment of advanced PD often includes deep brain stimulation (DBS) of the subthalamic nucleus. Regarding gait, previous studies have reported non-significant or conflicting results, possibly related to methodological limitations. Objective: The objective of this prospective study was to assess the effects of DBS on biomechanical parameters of gait in patients with PD. Methods: Twenty-one patients with advanced PD participated in this prospective study. Gait was examined in all patients using the Zebris FDM-T pressure-sensitive treadmill (Isny, Germany) before DBS implantation and after surgery immediately, further immediately after the start of neurostimulation, and 3 months after neurostimulator activation. We assessed spontaneous gait on a moving treadmill at different speeds. Step length, stance phase of both lower limbs, double-stance phase, and cadence were evaluated. Results: In this study, step length increased, allowing the cadence to decrease. Double-stance phase duration, that is, the most sensitive parameter of gait quality and unsteadiness, was reduced, in gait at a speed of 4.5 km/h and in the narrow-based gaits at 1 km/h (tandem gait), which demonstrates improvement. Conclusion: This study suggests positive effects of DBS treatment on gait in PD patients. Improvement was observed in several biomechanical parameters of gait.

Svalová únava je mnohostranný jev zahrnující periferní, centrální a také kognitivní faktory. V centrální úrovni dochází k rozvratu neurotransmiterové regulace, a tak proces motorického učení může výrazně prospět k opětovnému vyrovnání zatíženého systému. Díky zajištění variabilního projevu, umožňujeme motorickým vzorům se účinně přizpůsobit na specifické požadavky vnějšího a vnitřního prostředí, aniž by se změnil požadovaný cíl či záměr pohybu. Během našeho experimentu se snažíme objektivně zhodnotit vliv kognitivních funkcí na progresi či regresi svalové únavy. Z množství prototypových pohybů byla pro testování vybrána vysoce náročná pozice – statický dřep Wall Sit. Ten nás informuje o svalové zdatnosti v závislosti na čase a zároveň přináší poznatky o variabilitě zapojení svalových skupin dolních končetin během nástupu svalové únavy. Na základě tohoto dogmata vztahu mezi variabilitou a svalovou únavou potvrzujeme rozdíl na hladině statistické významnosti (p < 0,05), kdy došlo ke zlepšení výkonu z pohledu času za vlivu kognitivní zátěže. Výsledky experimentu tak potvrzují významný a zároveň pozitivní vliv externího zaměření pozornosti, kdy jsme potvrdili oddálení projevu svalové únavy.
Muscle fatigue represents wide-ranging phenomenon involving peripheral, central and also cognitive factors. At the central level, it leads to a collapse of neurotransmitter regulation and so the motor learning process can become significantly beneficial for restoration of the system under pressure. Because of ensuring variable behaviour, we enable the motor patterns to adapt effectively to specific requirements of an external and internal environment, without changing the objective or intention of the movement. In our experiment we try to evaluate objectively the impact of cognitive functions on progression and regression of muscle fatigue. To evaluate this issue adequately, a physically demanding posture – a squat Wall Sit, has been chosen from a range of prototype movements. Wall Sit provides information about muscle capability depending on time and also offers findings about variability of the engagement of lower limbs muscle group during the beginning of muscle fatigue. On the basis of the relation dogma between variability and muscle fatigue, we confirm the difference at the level of statistical significance (p < 0,05), when regarding the time during influence of cognitive load, the performance improved. The results of the experiment confirm a significant and at the same time positive impact of an external focus of attention, when we confirmed delay in the beginning of muscle fatigue.

Post-stroke spasticity (PSS) is effectively treated with intramuscular botulinum toxin type A (BoNT-A), although the clinical improvement is likely mediated by changes at the central nervous system level. Using functional magnetic resonance imaging (fMRI) of the brain, this study aims to confirm and locate BoNT-A-related changes during motor imagery with the impaired hand in severe PSS. Temporary alterations in primary and secondary sensorimotor representation of the impaired upper limb were expected. Thirty chronic stroke patients with upper limb PSS undergoing comprehensive treatment including physiotherapy and indicated for BoNT treatment were investigated. A change in PSS of the upper limb was assessed with the modified Ashworth scale (MAS). fMRI and clinical assessments were performed before (W0) and 4 weeks (W4) and 11 weeks (W11) after BoNT-A application. fMRI data were acquired using 1.5-Tesla scanners during imagery of finger-thumb opposition sequences with the impaired hand. At the group level, we separately modeled (1) average activation at each time point with the MAS score and age at W0 as covariates; and (2) within-subject effect of BoNT-A and the effect of time since W0 as independent variables. Comprehensive treatment of PSS with BoNT-A significantly decreased PSS of the upper limb with a maximal effect at W4. Task-related fMRI prior to treatment (W0) showed extensive activation of bilateral frontoparietal sensorimotor cortical areas, bilateral cerebellum, and contralesional basal ganglia and thalamus. After BoNT-A application (W4), the activation extent decreased globally, mostly in the bilateral parietal cortices and cerebellum, but returned close to baseline at W11. The intra-subject contrast revealed a significant BoNT-A effect, manifesting as a transient decrease in the activation of the ipsilesional intraparietal sulcus and superior parietal lobule. We demonstrate that BoNT-A treatment of PSS of the upper limb is associated with transient changes in the ipsilesional posterior parietal cortex, possibly resulting from temporarily altered sensorimotor upper limb representations.

Cévní mozková příhoda (CMP) je jedna z hlavních příčin chronické disability populace v rozvinutých zemích. Obnova maximálně možné lokomoční samostatnosti, umožňující vyšší míru sociální komunikace (participace), patří k hlavním cílům neurorehabilitace nemocných po CMP. Optimální obnova samostatného krokového stereotypu nemocných po CMP je do značné míry také podmíněna volbou individuálně i časově přiměřené neurorehabilitační strategie. Její podstatou je maximálně možná multisenzorická stimulace neuroplasticity, hlavně cílenou pohybovou aktivitou. Tedy především vlastní chůzí, v přiměřené intenzitě a ve více variacích lokomočního tréninku a s ohledem na aktuální možnosti pacienta. Dnes se v neurorehabilitaci chůze nejčastěji využívají terapeutem vedená chůze, roboticky asistovaná chůze, chůze na chodícím pásu, dále chůze v odlehčení, ve virtuální realitě nebo s využitím přídatné senzorické zpětné vazby. Cílem přehledu je racionální argumentace efektivity neurorehabilitačních strategií určených k optimalizaci chůze u pacientů po CMP na podkladě rozsáhlých metaanalýz s ohledem na mechanizmy řízení chůze, na neurální podstatu funkční obnovy pohybu i na stávající rehabilitační strategie v obecném rámci.
Stroke is one of the leading causes of severe disability in western population. Regaining walking ability in patients after stroke to encourage their return to previous life activities, more independent living and related social participation are currently one of the key challenges in the rehabilitation. Functional gait improvement and optimization of gait mechanism are to a great extent dependent on choice of appropriate neurorehabilitation strategy. Basic essence of neurorehabilitation is neuroplasticity stimulation with respect to potentiation of multisensory processing. Multisensory afferent input is optimally provided by variable intensive task-specific training with respect to actual patient’s capabilities. The gait (as task-specific movement) should be practiced intensively and under variable conditions to restore the walking ability. Up-to-date for gait neurorehabilitation are mostly used: over ground gait training with therapist, robotic assisted gait training, treadmill gait training, and also gait training based on biofeedback or in virtual environment. Combination of more gait training modalities and conventional rehabilitation approaches seem to be the most beneficial neurorehabilitation strategy to reach the maximum of walking ability recovery in patients after stroke. The aim of this review is to focus on the basic principles of gait neurorehabilitation with respect to current state of knowledge regarding gait control mechanisms and neural plasticity accompanying motor recovery post stroke.
