Článek vznikl na základě dotazů rodičů pečujících o děti s poruchou autistického spektra. Nejčastějšími otázkami rodičů byla problematika právní ochrany dětí, a to i po dosažení jejich dospělosti, podmínky omezení svéprávnosti, zákonné zastoupení nezletilých a způsoby, jak mohou rodiče své děti v jejich dospívání i dospělosti podporovat při čerpání zdravotních a sociálních služeb.
This article is based on questions from parents caring for children with an autistic spectrum disorder. The most frequent questions asked by parents were about the legal protection of their children, even after they reach adulthood, the conditions for limiting their legal capacity, the legal representation of minors, and ways in which parents can support their children in their adolescence and adulthood in accessing health and social services.

Úvod: Zdravotní péče je inherentně spojena s rizikem poškození zdraví pacientů. Především pochybení spojené s farmakoterapií je častým typem hlášených nežádoucích událostí. Analýzou kořenových příčin medikačních chyb lze navrhnout efektivní preventivně nápravná opatření snižující pravděpodobnost jejich výskytu. Cílem studie bylo identifikovat důvody pochybení při podávání léčiv sestrami, popsat bariéry v jejich hlášení a přinést odhadovaný počet skutečně nahlášených pochybení. Metodika: Design empirického šetření byl založen na kvantitativní metodě, prostřednictvím standardizovaného dotaz- níku Medication Administration Error Survey (MAE survey). Výzkumné studie se zúčastnilo 112 sester ze čtyř nemocnic Jihočeského kraje. Výsledky: Mezi rizikové faktory, které zvyšují pravděpodobnost pochybení při podávání léčiv sestrami, patří podobnost názvů (3,7 ± 1,3) a balení léků (3,9 ± 1,5), časté změny medikací (3,2 ± 1,5), nečitelnost (3,1 ± 1,6) a nejasnost lékařských záznamů (2,6 ± 1,5). Jen část těchto pochybení je sestrami nahlášeno (16 % až 21 %). Důvodem nízké motivace sester hlá- sit pochybení při podávání léčiv je strach z obviňování za zhoršení zdravotního stavu pacienta (3,3 ± 1,7), strach z reakce lékaře na medikační pochybení (2,6 ± 1,4) nebo represivní odpověď vedení nemocnice na hlášení pochybení (2,9 ± 1,5). Závěr: Mezi opatření snižující pravděpodobnost výskytu pochybení při podávání léčiv sestrami patří budování netrestají- cího systému hlášení nežádoucích událostí, zavedení elektronické preskripce léčiv, podpora otevřené týmové komunikace, zapojení klinických farmaceutů do procesu farmakoterapie a pravidelné komplexní školení ošetřovatelského personálu.
Introduction: Healthcare is inherently associated with a risk to patient health. One risk is associated with medication-related errors, which are commonly reported adverse events. By analyzing the root causes of medication errors, effective preventive measures can be proposed to reduce their likelihood. This study aimed to identify the reasons of medication administration errors, determine the number of medication administration errors reported, and describe the barriers hindering reporting. Methodology: The study used a standardized Questionnaire Medication Administration Error Survey (MAE survey) that was quantitatively analyzed. The study involved 112 nurses from four hospitals in the South Bohemian Region. Results: Risk factors that increase the likelihood of medication administration errors include similarity of drug names (3.7 ± 1.3) and packaging (3.9 ± 1.5), frequent prescription changes for patients (3.2 ± 1.5), illegibility of written prescriptions (3.1 ± 1.6), a lack of clarity of medical records (2.6 ± 1.5). Only a proportion of medication administration errors are reported by nurses (16% to 21%). The reluctance of nurses to report medication administration errors is linked to fear of being blamed for the deterioration of the patient's health (3.3 ± 1.7), fear of the doctor's reaction to a medication administration error (2.6 ± 1.4), and repressive responses from hospital management to reported misconduct (2.9 ± 1.5). Conclusion: Measures to reduce the likelihood of medication administration errors include building a non-punitive system for reporting adverse events and medication errors, introducing electronic prescription systems, promoting open communication within the team, involving clinical pharmacists in the pharmacotherapy process, and regular comprehensive training of nursing staff.
Cílem výzkumné studie bylo identifikovat farmakologické a nefarmakologické rizikové faktory pádů pacientů v nemocnicích. Metodika: Pro výzkum byla zvolena multicentrická observační studie případů a kontrol. Subjektem výzkumu byli pacienti hospitalizovaní v letech 2017 až 2019 na 20 odděleních pěti nemocnic Jihočeského kraje. Celkem bylo hodnoceno 378 pacientů s pádem (skupina případů) a 1821 pacientů bez pádů (skupina kontrol). Výsledky: Mezi nejvíce rizikové diagnózy zvyšující pravděpodobnost pádu patřila demence OR 2,47 (95 %: CI 2,01-3,76) a cévní mozková příhoda OR 6,83 [95% CI: 3,47-13,45]. Lékové skupiny, které statisticky významně zvyšovaly riziko pádů, byly psycholeptika, antipsychotika, konkrétně tiaprid, a oftalmologika. Mezi statisticky významné nefarmakologické rizikové faktory pádu patřily: přítomnost pádu v anamnéze, zvýšené riziko pádu při příjmu pacienta k hospitalizaci, snížená pohyblivost pacienta, zmatenost a neklid pacienta. Naopak včasné zahájení rehabilitace bylo identifikováno jako ochranný faktor snižující riziko pádu. Závěr: Z výsledků studie vyplývá, že příčiny pádů mají multifaktoriální charakter. Je důležité, aby byl program prevence pádů pacientů v nemocnici komplexní, ale zároveň i vysoce individualizovaný. Toho lze dosáhnout multidisciplinární spoluprací zdravotnického týmu. Na prevenci pádů se podílí nejenom lékař, sestra, fyzioterapeut, ale v současné době roste i úloha klinického farmaceuta.
Goal: The goal of the study was to identify pharmacological and non-pharmacological fall risk factors in hospitalized patients. Methodology: A multicenter observational case study and monitoring was chosen for this purpose. The study involved patients hospitalized at 20 hospital units of the South Bohemian Region between 2017 and 2019. The total number included 378 patients with falls (case group) and 1,821 patients without falls (control group). Results: Dementia OR 2.47 [95%; CI 2.01-3.76] and stroke OR 6.83 [95%; CI: 3.47-13.45] were the most probable diagnoses to increase the fall risk. The medication groups that significantly increased the fall risk included psycholeptics, antipsychotics, particularly tiaprid, and ophthalmology drugs. The significant non-pharmacological risk factors included the presence of falls in the case history, an increased fall risk at the admission, patient`s reduced mobility, confusion and restlessness. On the other hand, an early physiotherapy was identified as a protective factor reducing the fall risk. Conclusion: The study shows that causes of falls have a multifactorial character. The plan for the prevention of hospitalized patients` falls needs to be complex and, at the same time, highly individualized. This can be achieved by multidisciplinary cooperation of the healthcare team. Besides doctors, nurses and physiotherapists, clinical pharmacists start to play an important role.

Cílem: studie bylo identifikovat a vyhodnotit rizika v procesu podávání léčiv sestrou.
The goal of the study was to identify and assess the risks in the process of medicament administration by nurses. Methodology: The empiric study was designed as a quantitative method using focus groups. 22 managers from 4 hospitals in South Bohemia took part in the focus groups. The process of medicament administration by nurses was analyzed using the FMEA method. Results: In the process of drug administration, 28 possible failure modes with 12 identifiable causes were identified. The mean value of the Risk Priority Number (RPN) was 97, the RPN was higher than 125 in six failure modes. They included the risk of medication errors, missed out drug administration, wrong ordering of oral drug administration, missed out comparison of the patient`s ID with the healthcare records, generic medication error made by a nurse, and missed out hand hygiene. The preventive measures minimalizing the risks associated with drug administration include the introduction of a complete system of electronic medicament prescription which is, at the same time, interconnected with the Positive List of Hospital Medicaments, the introduction of regular internal audits of keeping complete records on medicaments in the healthcare records.
Úvod: Riziko pádů pacientů v nemocnici je ovlivněno řadou rizikových faktorů. Mezi tyto rizikové faktory patří nízké hodnoty BMI, hypotenze, popřípadě patologické hematologické a biochemické krevní hodnoty. Cílem výzkumné studie bylo vyhodnotit vliv vybraných fyzikálních a laboratorních ukazatelů na výskyt pádů pacientů v nemocnici. Metodika: Pro výzkum byla zvolena studie případů a kontrol. Ve skupině případů bylo 222 pacientů s pádem během hospitalizace a ve skupině kontrol bylo 1076 pacientů s podobnými charakteristikami bez pádu během hospitalizace. Celkem bylo do studie zařazeno 1298 pacientů. Sběr dat trval od 1. ledna do 31. prosince 2017. Výsledky: U skupiny pacientů s pádem byl identifikován statisticky významně nižší průměrný index BMI (25,9 kg/m2) než u kontrolní skupiny pacientů bez pádu (27,3 kg/m2), p = 0,001. Odhad rizika (OR) při BMI pod 18,5 kg/m2 je 1,641 [95 %: CI 0,686–3,927]. OR při hodnotě systolického tlaku pod 110 mm Hg je 1,109 [95 %: CI 0,674–1,825]. U skupiny pacientů s pádem byla identifikována v průměru statisticky vyšší tepová frekvence (79,8 min) než u kontrolní skupiny pacientů bez pádu (77,9 min), p = 0,015. U skupiny pacientů s pádem byla identifikována statisticky významně vyšší hladina glykemie (7,1 mmol/l) než u kontrolní skupiny pacientů bez pádu (6,2 mmol/l), p = 0,001. Pacienti s pádem měli nižší průměrnou hodnotu hemoglobinu (120,8 g/l) než pacienti bez pádu (122,3 g/l). U skupiny pacientů s pádem byla potvrzena statisticky významně nižší hodnota K (4,2 mmol/l) a Cl (102,1 mmol/l) než u kontrolní skupiny pacientů bez pádu (K 4,7 mmol/l, p = 0,002) a (Cl 103,1 mmol/l, p = 0,026). Závěr: Z výsledků vyplývá, že monitoring BMI, krevního tlaku, hematologických a biochemických vyšetření krve je nedílnou součástí programu prevence pádů hospitalizovaných pacientů.
Introduction: The risk of patients' falls in hospitals is affected by a number of risk factors. The risk factors include low BMI values, hypotension or pathological haematological and biochemical blood values, respectively.The goal of the research study consisted in evaluating the effect of selected physical and laboratory indicators on the incidence of patients' falls in hospitals. Methods: The case-and-control study was chosen for the research. The case group included 222 patients with fall during hospitalization, and the control group included 1076 patients with similar characteristics within fall during hospitalization. The study included 1298 patients in total. The data collection lasted from 1. 1. to 31. 12. 2017. Results: The group of patients with fall showed a statistically significantly lower average BMI index (25.9 kg/m2) than the control group of patients without fall (27.3 kg/m2), p = 0.001. The risk estimate (RE) at BMI under 18.5 kg/m2 is 1.641 [95%: CI 0.686–3.927]. RE at systolic pressure value under 110 mm Hg is 1.109 [95%: CI 0.674–1.825]. The group of patients with fall showed, on average, a statistically higher heart rate (79.8 min) than the control group of patients without fall (77.9 min), p = 0.015. The group of patients with fall showed a statistically significantly higher glycaemia level (7.1 mmol/l) than the control group of patients without fall (6.2 mmol/l), p = 0.001. The patients with fall had a lower average value of haemoglobin (120.8 g/l) than the patients without fall (122.3 g/l). The patients with fall had statistically significantly lower values of K (4.2 mmol/l) and Cl (102.1 mmol/l) than the control group of patients without fall (K 4.7 mmol/l, p = 0.002) and (Cl 103.1 mmol/l, p = 0.026).Conclusion: The results show that the monitoring of BMI, blood pressure, haematological and biochemical blood examinations constitutes an integral part of the program of prevention of falls of hospitalized patients.

Úvod: Pády pacientů patří mezi časté nežádoucí události. Je důležitou úlohou managementu nemocnic zavést do praxe efektivní program prevence pádů pacientů, který je komplexní, multifaktoriální a zároveň vysoce individualizovaný. Cílem výzkumu bylo vyhodnotit účinnost zavedeného intervenčního programu prevence pádů hospitalizovaných pacientů ve vybraných nemocnicích Jihočeského kraje. Metodika: Jednalo se o epidemiologickou studii observačního a intervenčního typu. V roce 2018 byl na 16 odděleních čtyř nemocnic Jihočeského kraje zaveden intervenční program, jehož cílem bylo minimalizovat rizikové faktory pádů pacientů. Intervenční program byl zaveden u všech pacientů hospitalizovaných ve sledovaném období na vybraných pracovištích. V roce 2017 šlo o 24 379 pacientů a v roce 2018 o 25 773 pacientů. Následně byla sledována účinnost zavedených intervencí. Byl vyhodnocen rozdíl ve výskytu pádů v období 2017 před intervenčním programem a v roce 2018 v době realizace programu. Index pádů byl stanoven jako počet pádů na 1 000 pacientů a počet zranění z pádu na 1 000 lůžkodnů. Výsledky: Po zavedení preventivního intervenčního programu byl pokles pádů zaznamenán na odděleních následné a rehabilitační péče (z 39,12 na 30,9 pádů/1 000 pacientů a z 0,79 na 0,58 pádů se zraněním/1 000 lůžkodnů), na chirurgických odděleních (z 5,88 na 5,78 pádů/1 000 pacientů a z 0,98 na 0,59 pádů se zraněním/1 000 lůžkodnů) a oddělení psychiatrie (z 14,27 na 7,48 pádů/1 000 pacientů a z 0,58 na 0,23 pádů se zraněním/1 000 lůžkodnů). Naopak vyšší index pádů byl potvrzen na interních odděleních (z 8,54 na 10,4 pádů/1 000 pacientů a z 1,22 na 1,63 pádů se zraněním/1 000 lůžkodnů). Závěr: Monitoring pádů a kořenová analýza jejich příčin umožňuje managementu zdravotnických zařízení nastavit účinná nápravná a preventivní opatření snižující výskyt pádů a minimalizující jejich důsledky.
Introduction: Patient falls are frequent unwanted events. Hospital managements have the important task of implementing an efficient programme of patient fall prevention - which is complex, multifactorial and highly individualized. The goal of this research was to assess the efficiency of the implemented fall prevention programme in selected South-Bohemian hospitals. Methods: This is an epidemiological, observational and interventional study. In 2018, 16 departments in 4 South-Bohemian hospitals implemented the intervention programme - the aim of which was to minimize risk factors of patient falls. All patients were included in the programme during the monitored period at selected workplaces. In 2017, 24,379 patients were included, and in 2018 there were 25,773 patients. We then monitored the efficiency of the implemented interventions. We assessed the differences between the incidence of falls in 2017 before the implementation of the intervention programme and the incidence of falls in 2018 during the realization of the programme. The fall index was set to the number of falls of 1,000 patients and the number of fall injuries during 1,000 days in hospital. Results: After the implementation of the prevention programme, the decrease in the number of falls was recorded at the departments of subsequent and rehabilitation care (from 39.12 to 30.9 falls/1,000 patients and from 0.79 to 0.58 falls with injuries/1,000 hospital days), departments of surgery (from 5.88 to 5.78 falls/1,000 patients and from 0.98 to 0.59 falls with injuries/1,000 hospital days) and the department of psychiatry (from 14.27 to 7.48 falls/1,000 patients and from 0.58 to 0.23 falls with injuries/1,000 hospital days). On the contrary, a higher fall index of falls was confirmed at internal departments (from 8.54 to 10.4 falls/1,000 patients and from 1.22 to 1.63 falls with injuries/1,000 hospital days). Conclusions: The fall monitoring and the root analysis of their causes enable the managements of medical institutions to establish efficient remedial and preventative measures that decrease the incidence of falls and minimize their consequences.
Cíl: Vyhodnotit souvislost mezi vybranými sociodemografickými ukazateli seniorů ve věku 65+ a jejich úrovní zdravotní gramotnosti. Metodika: V rámci kvantitativního šetření bylo osloveno 326 seniorů ve věku 65+ s trvalým pobytem v Jihočeském kraji. Zdravotní gramotnost seniorů byla vyhodnocena pomocí standardizovaného dotazníku HLSQ-EU. Výsledky: Dostatečná zdravotní gramotnost byla zjištěna u 39,1 % seniorů, problematická zdravotní gramotnost u 37,8 % dotázaných a nedostatečná zdravotní gramotnost byla vyhodnocena u 23,1 % respondentů. Zdravotní gramotnost byla negativně ovlivněna věkem a pozitivně vzděláním respondentů. S příznivější socioekonomickou situací seniorů rostla i jejich úroveň zdravotní gramotnosti. Nebyla potvrzena souvislost mezi pohlavím nebo bydlištěm respondentů a úrovní jejich zdravotní gramotností. Závěr: Zdravotní gramotnost seniorů je ovlivněna řadou sociodemografických faktorů. Při plánování a realizaci programů na podporu veřejného zdraví je nutné přizpůsobit intervence možnostem a preferencím této cílové skupiny obyvatel.
Aim: The aim of this study was to assess the correlations among selected indicators in the elderly above 65 and their level of health literacy. Methodology: During this quantitative study, 326 seniors at the age above 65 years with a permanent address in the South Bohemian Region, CR, were addressed. The respondents´ health literacy was assessed using the standardized HLSQ-EU questionnaire. Results: Adequate, marginal and inadequate health literacy was identified in 39.1%, 37.8% and 23.1% of the respondents. Health literacy is negatively influenced by age and positively by education. Higher level of health literacy correlated with the respondents´ better socioeconomic situation. No correlation was confirmed between health literacy and sex or address. Conclusion: The elderly´s health literacy is influenced by a number of socio-demographic factors. When health promotion programs are planned and implemented, interventions need to be adapted to the preferences and possibilities of target groups.

Contemporary state: The falls of hospitalized patients are the most frequent adverse events in the hospital environment that prolong hospitalization and increase treatment costs. Goal: The goal of this study is to identify the causes and other factors causing falls regarding the selected group and recommend preventative and remedial measures. Materials and methods: It is a qualitative analysis of two casuistries of patients who fell during the hospitalization period. In both cases, we analyzed the root causes of falls (Root Cause Analysis). Results: In the first case, the cause of the fall was a sudden collapse of the patient, which resulted in the loss of balance. The collapse was caused by an infection, dehydration and the subsequent orthostatic hypotension. Other factors were the high-risk time (early morning) and insufficient bathroom lighting. In the second case, the fall was caused by the patient’s loss of balance during their morning hygiene regime. In both cases, the subsidiary factors were high age (80+), polymorbidity and pharmacotherapy. Conclusions: Falls, especially in elderly patients, are multifactorial. Preventative and remedial measures should be complex. In both cases, it is necessary to carry out the nursing plan with regards to the current changes in the health condition of hospitalized patients, and to share information on such changes among medical workers.

Cíl: Cílem výzkumu bylo verifikovat souvislost mezi vybranými rizikovými faktory pádů hospitalizovaných pacientů a okolnosti těchto pádů. Metodika: Výzkum probíhal kvantitativní metodou prostřednictvím retrospektivní analýzy pádů hospitalizovaných pacientů ve čtyřech nemocnicích Jihočeského kraje (Nemocnice České Budějovice, Nemocnice Tábor, Nemocnice Jindřichův Hradec a Nemocnice Písek). Subjektem výzkumu byli pacienti, kteří ve sledovaném období 2014 a 2015 upadli a jejich pád byl nahlášen. Analyzováno bylo 1101 případů pádů. Výsledky: Předcházející pád v anamnéze byl uveden v 54 % validních záznamů o pádu hospitalizovaných pacientů. S věkem pacientů, kteří upadli, statisticky významně rostla přítomnost pádu v jejich anamnéze, zvyšovalo se riziko pádu a rostla i závažnost zranění v důsledku pádu. Z retrospektivní analýzy pádů vyplynulo, že u pacientů, kteří byli v riziku pádu, nebo měli pád v anamnéze, byli zmatení či dezorientovaní, sestry statisticky významně častěji nastavovaly boční zábrany (postranice) na lůžku. Ve 20 % validních záznamů byla nezabezpečená (nevytažená) postranice uvedena jako zevní příčina pádů. Naopak ve 36,5 % validních záznamů bylo příčinou pádu uvedeno užití postranic po celé délce lůžka. Závěr: Při plánování preventivních intervencí snižujících riziko pádu by měli zdravotničtí pracovníci zvážit efektivnost vytažení obou postranic po celé délce lůžka. Vhodnou alternativou zabezpečení pacienta na lůžku se jeví snížení výšky lůžka, vytažení pouze jedné, popřípadě horní části postranice. Další možností jsou tzv. inteligentní lůžka, která jsou vybavena varovnými senzory pohybu.
The aim of research was to verify connection between chosen risk factors for fall incidents of hospitalised patients and also circumstances of these incidents. Methodology: Research was conducted using quantitative method using retrospective analysis of fall incidents of hospitalised patients in four South Bohemian hospitals (Nemocnice České Budějovice, Nemocnice Tábor, Nemocnice Jindřichův Hradec and Nemocnice Písek). Research subjects were patients who had an incident of falling during 2014 and 2015 that was later reported. 1101 cases of falling incidents were analysed. Results: In 54% of falling incident reports for hospitalised patients earlier falling incident was reported in the patient case history. With patient age falling incidents appeared increasingly with statistical significance, the risk of falling increased, as did the severity of injuries caused by the fall. The retrospective analysis showed that patients with risk of falling in their case file were disoriented or confused, nurses significantly more often raised the sidebars of their beds. In 20% of the incidents the unraised bar was stated as the cause of the falling incident, On the other hand, in 36.5% of falling incident reports the reason of the fall were the bars raised along the whole length of the bed. Conclusions: When planning preventive interventions leading to decrease of the risk of falling incidents nursing personnel should consider the effectiveness of raising the bars on both sides of the bed along its whole length. A suitable alternative seems instead lowering the height of the bed, raising only bar on one bedside, or only the upper one. Another possibility is the so-called intelligent beds equipped by warning sensors for movement
