PURPOSE OF THE STUDY Population aging is connected with an increased incidence of chronic diseases. A common related problem is chronic skin ulcers, which, while not life-threatening, can significantly decrease the quality of the patient's life. The present study aims to evaluate new materials and methods to improve and accelerate the treatment of leg ulcers. MATERIAL AND METHODS Twenty-five patients with chronic ulcers treated using autologous growth factors applied on a nanofiber carrier were included in the cohort. The control group consisted of 15 patients treated using standard moist wound therapy. The surface area of the ulcer was measured on the 0th, 14th, 28th, 56th, 84th, 112th, 140th, 140th, and 168th day of treatment. Ulcer depth was measured on the 0th, 5th, 28th, 84th, and 168th day of treatment. Results were statistically processed and evaluated. RESULTS During the study, the defect area decreased in both the control and experimental group. Statistically significantly better results were observed in the experimental group relative to the progress of ulcer depth. The experimental group also had more healed ulcers. DISCUSSION Moistness is necessary for chronic wounds to heal; it is needed to ensure optimal cell growth, angiogenesis, and fibrinolysis. Wounds can be treated using non-active dressings with high absorption qualities; however, these do not guarantee optimal conditions for healing, or wounds can be treated with an interactive dressing that interacts with the wound surface. The third option for treatment is the use of bioactive materials that adhere to the wound and participate directly in the individual stages of healing. CONCLUSIONS The study found that autologous growth factors had statistically significant effects on the treatment of chronic ulcers. The authors believe that this method can accelerate the healing of primary post-injury or secondary postoperative wounds of lower leg soft tissues. Optimalizace péče o chronické rány je neustále předmětem výzkumu. Pochopení fyziologického hojení ran, neustálý vývoj nových krycích materiálů a správné dodržování pravidel antisepse vede k novým přístupům v péči o chronické rány. Prvořadým cílem při péči o chronickou ránu je její rychlé uzavření funkčně a esteticky vyhovující jizvou. Základní léčba u primárně se hojících ran je jednoduchá. Řada problémů však nastává u ran chronických, zejména u starších pacientů a pacientů s přidruženými komorbiditami. Je potřeba udržovat ránu v podmínkách zaručujících její fyziologické hojení. U každé rány je třeba řádně posoudit její stav, pečlivě ránu očistit a dále ji ošetřovat individuálně dle aktuální fáze hojení. Mnoho dříve používaných terapeutických postupů je již zastaralých. V současnosti máme řadu moderních krycích materiálů včetně bioaktivních látek, jež je třeba při léčbě chronických ran využívat. Nové poznatky v molekulární a buněčné biologii zlepšily naše znalosti o fyziologickém hojení ran a přístup k ošetřování ran.
Optimization of chronic wound treatment has been constantly a subject of research. Understanding the physiological healing of wounds, permanent development of new dressing materials and correct observance of antiseptic guidelines have led to new approaches in the care of chronic wounds. The primary objective in treatment and care of chronic wounds is rapid wound closure with a functional and aesthetically acceptable scar. Though primary healing wounds is simple, many problems occur in chronic wounds, especially in the elderly and in patients with associated comorbidities. The wound should be maintained in conditions ensuring physiological healing. The condition of each wound must be properly evaluated; the wound must be carefully cleaned and then treated individually according to the current stage of healing. Many of the formerly used therapeutic procedures are outdated today. At present we have numerous modern materials including bioactive dressings which should be used in treatment of chronic wounds. New findings in molecular and cellular biology have improved our knowledge of the physiological healing of wounds and the approach to wound treatment.

Kazuistika popisuje případ invaginace, ke které došlo u 67letého nemocného na podkladě submukózního lipomu tenkého střeva. Nemocný byl přijat na chirurgické oddělení s třídenní anamnézou bolestí břicha, zvracení, podle provedeného CT vyšetření suspekce na invaginaci tenké kličky, pro kterou nemocný následně operován. Provedena resekce invaginované kličky, anastomóza end to end. Histologicky nalezen submukózní lipom.
The casuistry describes the invagination that occurred to a 67-years old patient due to the small intestine lipom. The patient with the three-day abdominal pain and vomiting was admitted to the department of surgery. On the basis of the CT examination the patient underwent an operation for the invagination of the small inkotine. The resection of the invaginated loop, the anastomosis end to end were performed. Histologicaly the submucosus lipom was found.
