PURPOSE: To evaluate the contribution of F-18 FDG-PET/MRI in the search for the etiology of the inflammation of unknown origin (IUO) and fever of unknown origin (FUO). MATERIAL AND METHODS: The study included 104 patients who underwent F-18 FDG-PET/MRI for IUO or FUO. The sensitivity, specificity, predictive values of the PET/MRI findings in relation to the final diagnosis of IUO/FUO were evaluated. A five-point Likert scale was used to semiquantitatively assess the probability of the cause of IUO/FUO based on PET/MRI finding. Furthermore, clinical (fever, arthralgia, weight loss, night sweats, age) and laboratory (C-reactive protein, leukocytes) parameters were monitored and compared with the true positivity rate of PET/MRI. RESULTS: In patients with definitively identified etiology of FUO and IUO, FDG-PET/MRI achieved a sensitivity of 96 %, specificity of 82 %, and positive and negative predictive values of 92 and 90 %. The cause of the IUO was determined in 71 patients (68.3 %). In 33 (31.7 %) patients, the etiology of IUO/FUO remained unknown, while in 25 (75.8 %) of them the symptoms resolved spontaneously and in 8 (24.2 %) patients they persisted without explanation even after 12 months of the follow-up. The most significant parameter in relation to subsequent PET/MRI finding was increased level of CRP, which was present in 96 % of true positive PET/MRI and normal CRP level was present in 56 % of true negative PET/MRI. CONCLUSION: Based on this study, FDG-PET/MRI is a suitable alternative for the investigation of IUO/FUO, this imaging technique has a very high sensitivity and negative predictive value.

Divertikulitida ilea je zánětlivá komplikace divertikulózy. Jedná se o málo častou příčinu náhlé příhody břišní, která může mít velmi závažný průběh, vedoucí například ke střevní perforaci či krvácení. Nález na zobrazovacích metodách je velmi často negativní a pravá příčina stavu je odhalena až peroperačně. V této kazuistice prezentujeme případ perforované divertikulitidy ilea u pacienta s oboustrannou plicní embolií. Ta byla hlavním důvodem konzervativního postupu v první době. Po zaléčení plicní embolie byl při další atace s odstupem proveden resekční výkon postiženého segmentu střeva.
Diverticulitis of the ileum is an inflammatory complication of diverticulosis. It is an uncommon cause of acute abdomen that can have a very serious course, leading, for example, to intestinal perforation or bleeding. Imaging findings are very often negative and the true cause of the condition is only revealed peroperatively. In this case report, we present a case of perforated ileal diverticulitis in a patient with bilateral pulmonary embolism. This was the main reason for conservative management in the first period of time. After resolution of the pulmonary embolism, resection of the affected bowel segment was performed at the time of the next attack.
Magnetická rezonance je v současnosti klíčovým vyšetřením u tumorů rekta přecházejících svalovou vrstvu stěny. Důvodem je přesný TNM staging a volba správného léčebného postupu. Využití PET/MR v sobě kombinuje možnost nejen přesného vyšetření rekta a provedení T stagingu, ale díky PET části i zpřesnění hodnocení vlastní tumorózní léze a okolních uzlin, a rovněž stanovení vzdálených metastáz. Při využití hepatospecifické kontrastní látky i možnost přesného zhodnocení ložisek v parenchymu jater. Všechna nutná vyšetření ke stanovení TNM klasifikace jsou tak provedena v jeden den během jednoho vyšetření. Tento přístup je komfortní jednak z pohledu pacienta, ale ušetří tak i vyšetřovací čas na ostatních modalitách a v neposlední řadě v konečném důsledku přinese i snížení celkových nákladů na diagnostiku.
Magnetic resonance is currently a key examination for rectal tumors invading through muscularis propria. The reason is accurate TNM staging and selection of appropriate treatment. The use of PET/MRI combines the possibility not only to examine the rectum and performe T staging, but it can help delineate extent of tumor better and also better characterize small lymph nodes and improve detection of distant metastases. The use of hepatospeific contrast agent improves detection and specification of the focal liver lesion. This aproach called one - stop - shop ensures to determine TNM classification in one examination. Moreover, this approach is beneficial from the patient ́s point of view and it also saves examination times on other modalities and ultimately reduces the overall costs of diagnosis.

Cíl: Zhodnocení počtu a charakteru patologic- kých nálezů na štítné žláze při PET/MR vyšetřeních provedených z důvodu jiné indikace než onemocnění štítné žlázy. Metodika: Hodnotili jsme soubor 88 po sobě jdoucích pacientů (ve věku 17–83 let, z toho 37 mužů a 51 žen), kterým bylo v období říjen 2019 – březen 2020 na našem pracovišti provedeno PET/MR celého těla či jen krku a hrudníku dle standardizovaného protokolu a u kterých byl na štítné žláze popsán jakýkoliv patologický nález. Procentuálně byl vyjádřen počet nálezů z celkového počtu provedených PET/MR vyšetření. Nálezy na PET/MR vyšetření byly rozděleny do pěti skupin a byl vyjádřen absolutní a procentuální počet pacientů ve skupinách. Dále bylo hodnoceno, zda jsou pacienti v daných skupinách endokrinologicky došetřovaní a zda je nález stacionární či v parciální regresi. Výsledky: 88 pacientů zařazených do studie, tedy 15 % ze všech provedených vyšetření (579). Počty pacientů v jednotlivých skupinách: ložisko bez aktivity 35, aktivita ve štítné žláze bez morfologického korelátu 19, nespecifické nálezy 15, ložisko s aktivitou 13, ložisko bez aktivity s jiným okrskem vyšší aktivity ve štítné žláze 6. Nejvyšší počet endokrinologicky došetřovaných pacientů byl ve skupině ložisko bez aktivity – 8, nejmenší ve skupině nespecifické nálezy – 2. Stacionární nález či parciální regrese nejvíce zastoupen ve skupině ložisko bez aktivity – 18, nejméně ve skupině nespecifické nálezy – 3. Závěr: Během PET/MR vyšetření můžeme ve štítné žláze nalézt různé abnormality, vyšší pozornost si zaslouží ložiska s metabolickou aktivitou. Důležité je řádné endokrinologické došetření pacientů.
Aim: Evaluation of the number and character of pathological findings of the thyroid gland during PET/MRI examinations performed for an indication other than thyroid diseases. Methods: We evaluated a cohort of 88 consecutive patients (17–83 years, men 37, women 51) who underwent PET/MRI of the whole body or just the neck and chest in the period October 2019 – March 2020 with the local guidelines in the University Hospital Brno and in which any pathological finding was described in the thyroid gland. Findings were divided into five groups. Furthermore, it was evaluated whether the patients in the given groups are being monitored and whether the findings are stationary or in partially regression. Results: 88 patients in the study is 15% of all performed examinations (579). Number of patients in groups: morphologically visible lesion without activity 35, activity in the thyroid gland without a morphological correlate 19, non-specific findings 15, morphologically visible lesion with activity 13, morphologically visible lesion without activity with another area of higher activity in the thyroid gland 6. The highest number of monitored patients was in the group lesion without activity – 8, the smallest in the non-specific findings group – 2. Stationary finding or partial regression is most represented in the group of lesion without activity – 18, least in the group of non-specific findings – 3. Conclusion: Various abnormalities can be found during a PET/MRI examination in the thyroid gland, lesions with higher metabolic activity deserve greater attention.

RATIONALE AND OBJECTIVES: Although the gold standard in predicting future progression from clinically isolated syndrome (CIS) to clinically definite multiple sclerosis (CDMS) consists in the McDonald criteria, efforts are being made to employ various advanced MRI techniques for predicting clinical progression. This study's main aim was to evaluate the predictive power of diffusion tensor imaging (DTI) of the brain and brain volumetry to distinguish between patients having CIS with future progression to CDMS from those without progression during the following 2 years and to compare those parameters with conventional MRI evaluation. MATERIALS AND METHODS: All participants underwent an MRI scan of the brain. DTI and volumetric data were processed and various parameters were compared between the study groups. RESULTS: We found significant differences between the subgroups of patients differing by future progression to CDMS in most of those DTI and volumetric parameters measured. Fractional anisotropy of water diffusion proved to be the strongest predictor of clinical conversion among all parameters evaluated, demonstrating also higher specificity compared to evaluation of conventional MRI images according to McDonald criteria. CONCLUSION: Conclusion: Our results provide evidence that the evaluation of DTI parameters together with brain volumetry in patients with early-stage CIS may be useful in predicting conversion to CDMS within the following 2 years of the disease course.

BACKGROUND AND PURPOSE: This study's aim was to investigate diffusion properties of the cervical spinal cord in patients with clinically isolated syndrome (CIS) through analysis of diffusion tensor imaging (DTI) data and thereby to assess the capacity of this technique for predicting the progression of CIS to clinically definite multiple sclerosis (CDMS). METHODS: The study groups were comprised of 47 patients with CIS (15 of them with progression to CDMS within 2 years of follow-up) and 57 asymptomatic controls. All patients and controls had undergone magnetic resonance imaging (MRI) of the cervical spine including DTI and brain MRI. Methodological approaches included histogram analysis of the cervical cord's diffusion parameters and evaluation of T2 hyperintense lesions of the spinal cord and brain. All parameters were compared between the study groups. Sensitivity and specificity calculations were then performed with a view to predicting conversion to CDMS. RESULTS: The patient subgroups defined by progression to CDMS differed significantly in values of fractional anisotropy (FA) kurtosis measured within white matter (WM) and normal-appearing WM (NAWM). The same parameters also differed significantly when patients with progression to CDMS were compared to healthy controls. Receiver operating characteristic (ROC) analysis revealed sensitivity and specificity of FA kurtosis of WM and NAWM of 93% and 72%, respectively, in terms of predicting CIS to CDMS progression. CONCLUSION: This study presents evidence that histogram analysis of diffusion parameters of the cervical spinal cord in patients with CIS may be helpful in predicting conversion to CDMS.

Cíl: Zhodnotit přínos fúzního 18F-FDG-PET/ MR při pátraní po etiologii horečky nejasné etiologie (FUO). Metodika: Do studie bylo zařazeno 57 po sobě jdoucích pacientů v období 7/2016 až 12/2019, kteří podstoupili 18F-FDG-PET/MR (Signa, GE) pro FUO. Indikační kritéria byla oproti definici rozšířena na všechny pacienty s nejasnou teplotou nad 37 °C. Byla hodnocena senzitivita, specificita, prediktivní hodnoty a AUC nálezu na PET/MR ve vztahu k etiologii FUO. K semikvantitativnímu hodnocení pravděpodobnosti příčiny FUO na základě nálezu na PET/MR byla použita pětibodová Likertova škála (Likert 1 - nález na PET/MR není příčinou FUO, Likert 5 - nález na PET/MR je jistou příčinou FUO). Jako pozitivní PET/ MR byl hodnocen nález Likert 3 až 5. Dále byly sledovány klinické (horečka, artralgie, úbytek hmotnosti, noční pocení) a laboratorní (CRP, leukocyty) parametry, které byly korelovány s výtěžností PET/MR. Výsledky: 18F-FDG-PET/MR dosahoval při určení etiologie FUO senzitivity 93 %, specificity 65 %, pozitivní a negativní prediktivní hodnoty 78 a 87 %, AUC 0, 82. Příčina horečky nejasné etiologie byla stanovena u 27 pacientů (57 %), z nichž 17 (36 %) mělo neinfekční zánětlivé onemocnění, 8 (17 %) infekční zánětlivé onemocnění a dva (4 %) novotvar. U 20 (43 %) pacientů zůstala FUO bez zjištěné etiologie, přičemž u 14 (30 %) z nich příznaky spontánně odezněly a u šesti (13 %) pacientů přetrvávaly bez vysvětlení i po roce sledování. Zvýšená hladina CRP v době 18F-FDG-PET/ MR vyšetření vykazovala až 100% senzitivitu a 100% negativní prediktivní hodnotu pro správně pozitivní PET/MR, specificita dosahovala pouze 55 %. Závěr: 18F-FDG-PET/MR je na základě této pilotní studie vhodnou alternativou 18F-FDG-PET/ CT při pátrání po etiologii horečky nejasné etiologie.

A five-point Likert scale was used to semiquantitatively assess the probability of a cause of FUO based on PET/MRI finding (Likert 1 - PET/ MRI finding is not a cause of FUO, Likert 5 - PET/MRI finding is a certain cause of FUO). The Likert 3 to 5 finding was evaluated as positive PET/MRI. Furthermore, clinical (fever, arthralgia, weight loss, night sweats) and laboratory (CRP, leukocytes) parameters were monitored, which were correlated with the positivity rate of PET/MRI. Results: 18F-FDG-PET/MRI achieved a sensitivity of 93%, specificity 65%, positive and negative predictive values of 78 and 87%, AUC 0.82 when determining the etiology of FUO. The cause of fever of unclear origin was determined in 27 patients (57%), of whom 17 (36%) had non-infectious inflammatory disease, 8 (17%) infectious inflammatory disease and 2 (4%) neoplasm. In 20 (43%) patients, FUO remained without a known etiology, while in 14 (30%) of them the symptoms resolved spontaneously and in 6 (13%) patients they persisted without explanation even after one year of follow-up. The increased level of CRP at the time of 18F-FDG-PET/MRI examination showed up to 100% sensitivity and 100% negative predictive value for true positive PET/MRI, the specificity reached only 55%. Conclusion: Based on this pilot study, 18F-FDG-PET/MRI is a suitable alternative to 18F-FDG-PET/CT in the search for the etiology of the fever of unclear origin.
Cíl: Cílem této práce je představit klinicko-ekonomické aspekty stagingu karcinomu rekta pomocí 18F-FDG-PET/MR. Metodika: Pro staging nádorů rekta byl na našem pracovišti sestaven vyšetřovací protokol, který maximalizuje výtěžnost této zobrazovací metody za minimální čas. Protokol zahrnuje trupové PET/MR s pokrytím od báze lební přibližně do půli stehen, dynamické kontrastní vyšetření jater s aplikací hepato-specifické kontrastní látky, MR rekta a cílenou sekvenci na oblast plic. Pro srovnání ekonomic-koradiační situace jsme porovnali čtyři různé scénáře s využitím různých zobrazovacích metod, které mohou v praxi nastat při stagingu karcinomu rekta. U těchto modelových scénářů jsme porovnali celkovou radiační zátěž a celkové ekonomické náklady dle bodového systému. Výsledky: V období let 2018-2020 bylo na našem pracovišti provedeno 3950 PET/MR vyšetření, z čehož 867 (21,95 %) bylo provedeno PET/MR protokolem pro staging karcinomu rekta. Klinickým přínosem stagingu karcinomu rekta pomocí PET/MR je zrychlení diagnostického postupu. Staging karcinomu rekta pomocí 18F-FDG-PET/MR je 2. nejekonomičtější scénář, jehož radiační dávka je nejnižší. Závěr: PET/MR u tumorů rekta umožňuje vzhledem ke koncepci "one-stop-shop" detailní zhodnocení lokoregionálního nálezu i vzdálené diseminace při jednom vyšetření, urychluje celý diagnostický proces a snižuje radiační dávku u pacientů.

This protocol includes PET/MRI from skull base to mid-thigh, dynamic contrast enhanced MRI of liver with hepatospecific contrast agent, MRI of the rectum, sequence for lung evaluation. We compare 4 different scenarios using different imaging methods for rectal cancer staging. For each scenario we calculate total radiation exposure and economical costs. Results: From 2018 to 2020 our department performed 3950 PET/MRI examinations, 867 (21,95%) were performed by PET/MRI protocol for rectal cancer staging. The clinical benefit of PET/MRI in rectal cancer staging is the speeding-up of the diagnostic process. According to the proposed economic-radiation situations, the scenario using 18F-FDG-PET/MRI for rectal cancer staging was 2nd most economical with the lowest radiation exposure. Conclusion: PET/MRI for rectal cancer allows as a "one-stop-shop" imaging method detailed evaluation of locoregional finding and distant dissemination in one diagnostic setting, speeds up diagnostic process and reduces radiation exposure of patients.Aim: The aim of this paper is to present clinical-economic aspects of rectal cancer staging using 18F-FDG-PET/MRI. Magnetic resonance (MR) is a method very well suited to monitoring changes in hyaline cartilage and can be used to diagnose impairment in such cartilage. In indicated cases, local chondral changes are resolved through orthopaedic surgery – either using microfractures or by replacing the damaged site with an implant, in this work a biological CaRes 1S collagen type-I matrix implant. This project will focus on studying changes at the surgical site for two different treatment methods using multiparametric MR imaging. The study will be randomized and will include 20 patients whose chondral defects were treated with microfracture surgery and 20 patients with CaReS 1S implants. The results achieved in this study will help to improve understanding of changes occurring at the surgical site and, through a multiparametric evaluation, more precisely to estimate the prospects of individual surgery types and determine whether MR is a suitable predictive factor for the clinical outcome of surgery.
MR je velmi vhodná metoda pro sledování změn hyalinní chrupavky a lze ji využít k diagnostice jejího postižení. Lokální chondrální změny jsou v indikovaných případech řešeny ortopedickou operací - buď pomocí mikrofraktur nebo nahrazením poškozeného místa implantátem, v této práci biologickým implantátem CaRes 1S na bázi kolagen I. Projekt bude zaměřen na studium změn v místě operace dvěma odlišnými způsoby pomocí multiparametrického zobrazení magnetickou rezonancí. Studie bude randomizovaná a bude zahrnovat 20 pacientů s operačním řešením chondrálního defektu pomocí mikrofraktur, 20 pacientů s implantací CaReS 1S.Výsledky dosažené v této studii pomohou lépe porozumět změnám, které se odehrávají v místě operačního zákroku a díky multiparametrickému zhodnocení přesněji odhadnout perspektivu jednotlivých typů operace a zjistit, zda je magnetická rezonance vhodná jako prediktivní faktor klinického výsledku operace.

RATIONALE AND OBJECTIVES: This study's aims were to depict changes in cartilage quality after surgical intervention using magnetic resonance (MR) examination and in content of glycosaminoglycans chains (GAGs) after two types of surgeries - chondral defect treatment by microfractures and scaffold implantation in combination with microfractures. MATERIALS AND METHODS: Twenty-five patients were studied: 14 with implants, 11 with microfractures. MR examination was made before surgery and 6, 12, and 18 months thereafter. Qualitative changes in cartilage were observed by means of delayed gadolinium enhanced magnetic resonance imaging of cartilage sequence using Gd-DTPA2- and Gd-DOTA. In each examination, GAGs content was determined at three locations: the defect, its surroundings, and a non-load-bearing reference area. RESULTS: Measured indices showed no statistically significant differences in changes within the defect area when comparing the two treatment types at individual time points of 6, 12, and 18 months. In the case of microfracture treatment, more substantial decrease in GAGs concentration occurred at month 6, whereas the greatest decline occurred at month 12 when using an implant. Change in GAGs content and decline in cartilage quality were substantial also in the reference area and close surroundings. CONCLUSIONS: Hyaline cartilage behaves as a unified whole, and change in GAGs content was marked also in locations with no morphological damage. Over the monitored period, no statistically significant difference between treatment types was noted as measured by GAGs content in the defect or its close surroundings. dGEMRIC is suitable for monitoring cartilage quality even if use of Gd-DTPA2- is not possible, because comparable results were achieved using Gd-DOTA.
