PURPOSE OF THE STUDY In the last number of years, the anatomic hemiarthoplasty has gradually been pushed out of clinical practice by modern reverse shoulder arthroplasty (RSA) designs. This is due to the clear excellent functional results of RSAs in a wide spectrum of indication criteria. Nevertheless, RSAs have several possible complications that cannot occur in an anatomic hemiarthroplasty. In anatomic hemiarthoplasty, the importance of correct indication criteria and observing correct operative technique including soft tissue reconstruction is much more important than in RSA. Furthermore, there is a clear recent trend of increased use of humeral components fixed only in the proximal metaphyseal cancellous bone. Our aim was to summarise our six-year experience with the SMR® Stemless (LimaCorporate, Italy) system which is one of the most modern ones. MATERIAL AND METHODS Twenty cases of SMR® Stemless anatomic shoulder hemiarthroplasty performed between 2016 and 2021 were included in the study. All patients were followed up prospectively. The function was evaluated preoperatively and at the last follow-up. We evaluated the range of active elevation, classic Constant Score (CS) and pain level according to the visual analogue scale (VAS). Statistical evaluation was performed by using basic statistical methods and the statistical significance of the results was assessed with a paired t-test. Level of statistical significance was set at p= 0.01. RESULTS The mean follow-up in our cohort was 3.01 years (range 0.32-5.69, Median 2.82, SD 1.56) All cases were indicated for surgery due to primary osteoarthritis with a limitation of movement and pain. The mean postoperative CS was 85.7 (range 70-96, Median 86, SD 6.83). The mean active elevation postoperatively was 143° (range 100-170°, Median 150°, SD 20.76). Mean postoperative pain according to VAS was 1.05 (range 0-4, Median 1, SD 1.02). The mean preoperative elevation was 60.5° (range 30-100°, Median 65°, SD 18.83). After surgery the mean elevation increased to 143° (range 100-170°, Median 150°, SD 20.76). Statistical evaluation showed a statistically significant increase in the CS (41.7 preoperatively to 85.7 postoperatively), range of active elevation (60.5° preoperatively to 143° postoperatively) and a statistically significant decrease in pain (VAS 6.95 preoperatively to 1.05 postoperatively). We observed no cases of failure or loosening of the implant. A statistically significant increase in post-operative range of motion was demonstrated. DISCUSSION Most modern shoulder arthroplasty designs now include implants allowing for proximal humerus metaphyseal fixation in hemiarthroplasty and even RSA designs. The advantage of metaphyseal fixation without the use of a longer stem is clear. Notably, treatment of periprosthetic humeral fractures is simpler, extraction of the implant for any reason is easier and the preoperative anatomic position of the humeral head can be respected. As with any anatomic shoulder arthroplasty, the functional result is dependent on correct indication criteria, precise surgical technique, correct humeral head position and soft tissue reconstruction - primarily the rotator cuff. CONCLUSIONS Between 2016 and 2021, we performed 20 SMR® stemless shoulder hemiarthroplasties for primary osteoarthritis. The mean follow up was 3 years. The shoulder function improved significantly post-operatively in all patients. There were no cases of implant loosening or failure. Radiographic evaluation showed no implant loosening or change in implant position in the humeral metaphysis. PURPOSE OF THE STUDY Reverse total shoulder arthroplasty (RSA) can be considered a proven method for the treatment of the most severe shoulder joint affections. In this study, we present 14-year experiences of the authors with the LimaCorporate SMR® RSA system in 496 cases. MATERIAL AND METHODS Included in the study are 496 RSAs performed between 2007 and 2020. We successfully followed up 368 shoulders in 358 patients. This was a prospective study with function being evaluated preoperatively and at the last follow-up in 2020. We evaluated the range of motion in active elevation and the classic Constant Score (CS). According to this score, the pain level was also evaluated. Standard statistical methods were used with a paired t-test used for comparisons of values. RESULTS The mean follow-up in our group was 5.5 years (0.7-13.6, SD 3.22, median 4.96). Indications were: primary osteoarthritis (84), acute trauma (69), posttraumatic sequelae (79), cuff tear arthropathy (37), RA (29), chronic dislocations (18), final treatment of infectious complications (7), avascular necrosis (6), tumours (4) and TSA revisions (9). The mean post-operative CS of all patients was 71.9 (2-94, SD 11.26, median 73). The mean post-operative active elevation was 127.35° (30°-180°, SD 28.36, median 130°). The mean pain level at final follow-up was 0.65 (0-3, SD 0.65, median 1). There was a statistically significant improvement In the CS (26.9 to 71) and the final achieved elevation (48.5° to 127.35°) in all groups except acute traumas for obvious reasons. A significant decrease in pain (2.8 to 0.65) was observed in all groups. We saw no implant failures or UHMWPE component wear. DISCUSSION We compared our results with those published by other authors. When considering the functional outcomes, our results are comparable with those published previously. Compared to other studies, an interesting result is the low incidence of scapular notching. In our cohort of patients, it was only present in 10 (2.7%) cases. We attribute this to the design of the SMR® implant which meets the main criterium of a modern shoulder arthroplasty system: modularity of both glenoid components. The glenospheres are available in 36, 40 and 44mm sizes with standard and distalised options. Furthermore, the system also contains the Axioma® revision glenoid component. Humeral stems are available in a range of sizes and lengths including revision stems. The humeral body is also available in two sizes. The short variant is optimal for revision with hemiarthroplasty to reverse conversion. CONCLUSIONS During a period of 14 years, we performed 496 implantations of RSAs using the SMR® system. We evaluated 368 cases with an average follow-up of 5.5 years. During long-term follow-up we experienced no implant failures or complications indicative of any constructional insufficiencies. We saw no signs of UHMWPE liner wear. The SMR® system allows for treatment of even the most complex shoulder affections due to its high modularity. Úvod: Aponeurosis plantaris je důležitou strukturou v oblasti předonoží. Prostorové uspořádání jednotlivých jejích částí je důležité pro udržování stability nohy. V dostupné anatomické literatuře je zejména v grafickém znázornění množství chyb a nepřesností. V našem článku jsme se rozhodli poukázat na některé důležité aspekty týkající se anatomie plantární aponeurózy a z nich vyplývající biomechaniky. Materiál a metoda: Ve studii bylo použito celkem 20 preparátů prvních metatarzofalangových skloubení 12 kadáverů v Anatomickém ústavu 1. lékařské fakulty Univerzity Karlovy v Praze. Do studie byly zařazeny pouze končetiny, které nevykazovaly deformitu předonoží a proběhlé operace. Zvláštní pozornost jsme pak věnovali uspořádání distální části plantární aponeurózy, sagitálním septům, která se upínají do plantárních plotének (fibrocartilago art. metatarsophalangeae) jednotlivých metatarzofalangových skloubení. Výsledky: Ve studii bylo vyhodnoceno 20 plantárních aponeuróz u 12 kadáverů. V souboru preparátů bylo 8 žen a 4 muži. U všech preparátů byly vždy přítomny a plně vyvinuty pruhy centrální části, mediální i laterální části. Ve všech případech byly vyvinuty distální pruhy centrální části v podkoží stáčející se do meziprstních prostor. Pruhy distální části byly značně variabilní a jejich okraje se vytrácely do podkoží okolní tkáně. Průměrná šíře byla 7 mm (2–13 mm). Sumací jednotlivých šířek a směrů těchto distálních pruhů vznikla rekonstrukce a schéma, které jsou součástí této publikace. Diskuze: Komplex aponeurosis plantaris a na ní napojených struktur tvoří důležitou funkční jednotku, která je základem pro fungování stabilizačního mechanismu nohy, takzvaného navijákového (windlass) mechanismu. Mnoho autorů se ve svých pracích zabývá úlohou plantární aponeurózy při rozvoji některých patologií prvního paprsku nohy. Napříč literaturou pak panuje shoda ohledně důležitosti obnovení fyziologických poměrů při chirurgické léčbě nejčastějších deformit prvního paprsku, jako jsou hallux valgus a hallux rigidus. Distální část plantární aponeurózy tvoří prostorovou síť, která slouží nejenom pro udržování správného postavení skeletu nohy, ale rovněž pro ukotvení nášlapných polštářů. Při poruchách funkce aponeurosis plantaris vznikají deformity nohy typu hallux valgus anebo hallux rigidus.
Introduction: Aponeurosis plantaris is an important structure in the forefoot. The spatial arrangement of its parts is important in maintaining the stability of the foot. The anatomical literature available often contains many number of errors and inaccuracies, mainly in the graphical representation. In this article, we decided to highlight some important aspects relating to the anatomy of plantar aponeurosis and biomechanics resulting from them. Materials and methods: In this study, a total of 20 specimens of metatarsophalangeal articulation in 12 cadavers at the Department of Anatomy, 1st Medical Faculty of Charles University in Prague. The study included only limbs that did not show deformity of the forefoot and or any surgery to it. Special attention was then devoted to the arrangement of the distal part of the plantar aponeurosis, sagittal septum that was clamped into the plantar discs (fibrocartilage art. Metatarsophalangeae) and each individual metatarsophalangeal articulation. Results: The study evaluated 20 plantar aponeurosis in 12 cadavers. The samples came from 8 women and 4 men. All samples always had present and fully developed streaks in the central portion, medial and lateral parts. In all cases, distal stripes on the central part in the dermis were developed, curving into the interdigital spaces. Stripes on the distal part were highly variable and their edges faded into the hypodermic tissue. The average width was 7 mm (2-13 mm). By summarising the various widths and orientations of the distal bands a reconstruction and a scheme were constructed, which are included in this publication. Discussion: The complex aponeurosis plantaris and its associated structures form an important functional unit, which is the basis for the functioning of the stabilization mechanism of the legs, the so-called windlass mechanism. Many authors in their papers deal with the role of plantar aponeurosis in the development of some pathologies of the first ray of the foot. Across the literature, there is a consensus on the importance of restoring physiological conditions during surgical treatment of the most common deformities of the first ray, such as hallux valgus and hallux rigidus. The distal part of the plantar aponeurosis creates a spatial network, which serves not only to maintain the correct position of the bones of the legs, but also as an anchor for the sole of the foot. Malfunctions to the aponeurosis plantaris lead to foot deformities such as hallux valgus or hallux rigidus.

Zavedení umělých kloubních náhrad představuje pro miliony lidí na celém světě převratnou metodu léčby závažných kloubních postižení. Ročně narůstá počet primoimplantací a rostou nároky na životnost kloubních náhrad. S tím se ale také zvyšují nároky na odolnost artikulačních povrchů vůči opotřebení. Nejužívanějším materiálem pro jejich výrobu je stále vysokomolekulární polyetylén (UHMWPE – ultra-high molecular weight polyethylene). Ten byl zaveden do klinické praxe již v 60. letech 20. století. Fyzikálně chemické vlastnosti UHMWPE jsou předmětem mnoha výzkumů, které postupně vedou k jeho vyšší odolnosti vůči otěru při zachování stability vůči oxidativní degradaci.
The introduction of artificial joint replacement constitutes a breakthrough method of treatment for severe joint disease for millions of people worldwide. Annual increase in the number of primary replacement and also increasing demands on the longevity of joint replacements are leading to increased demands on wear resistance of articular surface. Ultra-high molecular weight polyethylene (UHMWPE) is still most commonly used material for the production of articular surface. It was introduced into clinical practice in the 60s of the 20th century. Physical-chemical properties of UHMWPE are subject of many studies. These lead gradually to its improvement in terms of higher wear resistance while maintaining the stability against oxidative degradation. The main objective of this review is to summarize the basic properties of high-molecular weight polyethylene which are important for its use in orthopaedic practice and to explain the possibilities of its modification and sterilization. Knowledge of the latest trends about this material helps to orthopaedic surgeons get oriented in the issues and then to choose for their patients implants with the highest implant longevity.

Rehabilitace po aloplastice ramenního kloubu je proces, který započíná časně po výkonu a musí pokračovat dlouhodobě. Klíčovým faktorem je správně sestavený a odborně vedený program, který primárně musí definovat sám operatér. Je neoddělitelnou součástí celé léčby kloubního postižení a je podmínkou pro dosažení dobrého výsledku. Postup rehabilitace u anatomické náhrady v různých indikacích i reverzní náhrady se mírně odlišuje, zejména v rozvržení fáze aktivního cvičení. Fyzioterapeut, který provádí jednotlivá cvičení, musí být vyškolený na tuto problematiku. Chyby v intenzitě a detailech mohou vést k luxaci reverzní TEP, vzniku nestability či selhání rekonstrukce rotátorové manžety. Po celou dobu cvičení je nezbytné klást důraz na správný pohybový stereotyp při provádění každého cviku a kontrolovat jej.
Rehabilitation after total shoulder arthroplasty is a process that starts early after surgery and must continue in the long term. A properly designed rehabilitation programme with expert oversight is a key factor, which has to be primarily defined by the surgeon who performed the arthroplasty. It is an integral part of the overall treatment of joint damage and it is a prerequisite for achieving a good result. The process of rehabilitation in cases of using a prosthetic joint differs for various indications and for reverse prosthetic, especially in how active exercise is scheduled. The physiotherapist who performs each exercise must be specifically trained in this area. Errors in intensity and detail of the technique of the exercises can lead to dislocation of the reverse implant, to instability or failure of the reconstruction of the rotator cuff. Throughout the exercises, it is necessary to emphasize control and healthy movement patterns in the implementation of each exercise.

Indikacemi aloplastiky ramenního kloubu jsou chronická postižení glenohumerálního kloubu, stavy bolestivého rozsáhlého poškození rotátorové manžety, traumatická postižení proximálního humeru a onkologické afekce. Při indikaci je nutná individuální rozvaha o pohybových potřebách pacienta. Podle toho pak je volen i design a typ implantátu. Výsledky tuto metodu mezi zavedené a ověřené metody léčby.
Indications arthroplasty of the shoulder joint are chronic disability glenohumeral joint, a painful condition of extensive damage to the rotator cuff, traumatic involvement of the proximal humerus and oncological affections. When indication is required individual balance sheet on physical needs of the patient. Accordingly, it is chosen as the design and type of implant. The results of shoulder arthroplasty demonstrate that it is a proven method of treatment.

Anatomická hemiartroplastika ramenního kloubu je výkon, u kterého je funkční výsledek velmi závislý na správné indikaci, operační technice a následné rehabilitaci. Operační technika se odlišuje u chronických degenerativních onemocnění ramena a u traumatických stavů. V těchto případech je zásadní optimální rekonstrukce a refixace velkého a malého hrbolu humeru s neporušenými úpony rotátorové manžety.
Anatomical hemiarthroplasty of the shoulder joint is a procedure for which the functional outcome highly depends on correct indication, surgical technique and subsequent rehabilitation. Surgical technique differs for chronic degenerative diseases and traumatic conditions of the shoulders. In these cases optimal reconstruction and fixation of the fixation of the greater and lesser tuberosity of the humerus with intact rotator cuff insertions is essential.
