Východisko: Použitie indukčnej liečby v prípade transplantácie obličky u pacientov vo vysokom imunologickom riziku je všeobecne akceptované a odporúčané. Otázkou je použitie indukcie u pacientov v nízkom (resp. veľmi nízkom) imunologickom riziku. Cieľ: Cieľom štúdie bolo vytvorenie jednotného národného indukčného protokolu s ohľadom na imunologické riziko pacienta s použitím bodovacieho systému. Metodika: Ide o prospektnu multicentrickú štúdiu so zapojením všetkých štyroch transplantačných centier na Slovensku u pacientov po transplantácii obličky s 12-mesačným follow-up. Vo vzťahu k použitej indukcii sme sledovali výskyt oneskoreného nástupu funkcie štepu, rozvoj akútnej rejekcie štepu a funkciu štepu. Výsledky: Do štúdie bolo celkovo zaradených 152 príjemcov (muži – 66.4%). Pacienti boli rozdelení na základe aplikovanej indukcie do štyroch podskupín: bez indukcie (n = 19), indukcia s basiliximabom (n = 60), indukcia s antitymocytárnym globulínom (ATG) v kumulatívnej dávke 3.5 mg/kg telesnej hmotnosti (n = 42) a 6 mg/kg telesnej hmotnosti (n = 31). Dĺžka dialyzačného programu ≥ 3 roky a čas studenej ischémie ≥ 1080 minút boli identifikované (v univariantnej analýze) ako prediktory pre stratu štepu v prvom roku po transplantácii v basiliximabovej skupine. Najhoršie prežívanie pacienta (štatistiky) a najhoršie prežívanie štepu (numericky) bolo zaznamenané u pacientov bez indukcie. Záver: Na základe výsledkov pri použití jednotného národného indukčného protokolu v našej analýze sme vytvorili nový protokol s jednoznačným odporučením použitia indukčnej liečby u všetkých – aj u pacientov vo veľmi nízkom a nízkom imunologickom riziku. Pacientom v dialyzačnom programe ≥3 roky a časom studenej ischémie ≥18 hodín je v novom protokole pridelených viac bodov a následne odporučená indukcia s ATG.
Background: The use of induction therapy in case of kidney transplantation in patients at high immunological risk is generally accepted and recommended. The question is use of induction in patients with at low (or very low) immunological risk. Objectives: The aim of the study was to create a unified national induction protocol with regard to the patient‘s immunological risk using a scoring system. Methods: It is a prospective multicenter study involving all 4 transplant centers in Slovakia in patients after kidney transplantation with a 12-month follow-up. In relation to the induction used, we monitored the incidence of delayed graft function, the development of acute rejection and graft function. Results: The study included 152 recipients (men – 66.4%). Patients were divided into four subgroups based on applied induction: no induction (n = 19), induction with basiliximab (n = 60), induction with antithymocyte globulin (ATG) at a cumulative dose of 3.5 mg/kg body weight (n = 42) and 6 mg/kg body weight (n = 31). Dialysis program duration ≥ 3 years and cold ischemia time ≥ 1080 minutes were identified (in univariate analysis) as predictors for graft loss at one year after transplantation in the basiliximab group. The worst patient survival (statistics) and the worst graft survival (numerical) were recorded in patients without induction. Conclusions: Based on the results of using a unified national induction protocol in our analysis, we have created a new protocol with a clear recommendation for the use of induction treatment for everyone – including patients at very low and low immunological risk. Patients in the dialysis program ≥3 years and cold ischemia time ≥18 hours are assigned more points in the new protocol and induction with ATG is subsequently rocommended.

Cieľom práce bolo analyzovať urologické komplikácie po transplantácii obličky na Urologickej klinike s Centrom pre transplantácie obličiek v Bratislave a poskytnúť racionálny návod na ich efektívne riešenie. Autori analyzovali komplikácie v súbore 1 353 pacientov po transplantácii obličky (z toho 95 obličiek odobratých od žijúcich darcov). Zamerali sme sa na výskyt, príčiny, diagnostiku a liečbu komplikácií, ktoré majú kauzálny vzťah k rekonštruovaným močovým cestám transplantovanej obličky, pretože práve tie sa pokladajú za najzávažnejšie chirurgické komplikácie po transplantácii obličky. Ich výskyt sa udáva v rozsahu 1–30% transplantácií, predstavujú asi 50% všetkých chirurgických komplikácií, môžu byť príčinou významnej morbidity i mortality, môžu viesť k oneskorenému nástupu funkcie aj ku strate transplantovanej obličky, môžu dokonca pacienta ohroziť na živote. Významný vplyv na výsledok transplantácie obličky a na vznik urologických komplikácií má chirurgická technika pri odbere a transplantácii obličky a pri rekonštrukcii močových ciest. Pri odbere obličky dochádza k prerušeniu všetkých nervových vlákien inervujúcich štruktúry obličky. Z hľadiska glomerulárnej i tubulárnej funkcie obličky je denervácia bezvýznamná a neovplyvňuje negatívne ani transportnú funkciu horných močových ciest. S denerváciou obličky však súvisí zmenená symptomatológia niektorých ochorení, resp. komplikácií po jej transplantácii. Transplantovaná oblička nebolí, preto pacienti nevyhľadávajú lekára včas a aj diagnostika komplikácií po transplantácii obličky je náročnejšia a často oneskorená. Formou prehľadového článku autori ponúkajú čitateľom časopisu Česká urologie svoje skúsenosti s diagnostikou a liečbou urologických komplikácií po transplantácii obličky. Algoritmy vyplývajú z vlastných klinických skúseností a vyjadrujú racionálny postup v diagnostike a liečbe urologických komplikácií po transplantácii obličky.
The aim of our work was to analyze urological complications after kidney transplantation at the Urology Department with the Center for Kidney Transplantation in Bratislava and to provide rational guidance for their effective solution. The authors analyzed complications in a group of 1353 renal transplant patients (of which 95 kidneys were retrieved from living donors). We focused on the occurrence, causes, diagnosis and treatment of complications that have a causal relationship to the reconstructed kidney transplant pathways because they are considered to be the most serious complications after kidney transplantation. They account for about 50% of all surgical complications, may cause significant morbidity and mortality, may lead to a delayed onset of the function as well as to the loss of the transplanted kidney. They may even endanger the patient's life. The outcome of kidney transplantation and of the occurrence of urological complications is significantly influenced by the surgical technique of kidney removal and transplantation and by the reconstruction of the urinary tract. During kidney removal, all nerves innervating the kidney are disrupted. In terms of glomerular and tubular function of the kidney, denervation is insignificant and does not adversely affect the transport role of the upper urinary tract. However, denervation of the kidney may change the symptomatology of some diseases or of the post-transplant complications. The transplanted kidney does not hurt, hence the patients do not seek help in time and the diagnostics of complications after kidney transplantation is more demanding and often delayed. In the form of a review article, the authors offer their experience with the diagnostics and treatment of urological complications after kidney transplantation to the readers of the Czech Urology magazine. The algorithms arise from the authors' own clinical experience in renal transplantation at the Department of Urology in Bratislava and express a rational approach to the diagnostics and treatment of urological complications following kidney transplantation.

Autosomal-dominant polycystic kidney disease (ADPKD) is an inherited disease that results in multiple kidney cysts, and it is a common cause of end-stage renal disease. Recent studies have shown that disease progression can be slowed by simultaneous disruption of the primary cilium and polycystins. The exact genetic mechanism of this process is still unknown. The aim of the present study was to characterize the mutation profile of ciliary signalling pathways in the renal epithelial cells of ADPKD patients. In our study, we performed an analysis of 110 genes encoding the components of Sonic Hedgehog, Hippo, Notch, Wnt and planar cell polarity signalling (PCP) by targeted next-generation sequencing. We analysed 10 formalin-fixed, paraffinembedded (FFPE) tissue samples of patients with ADPKD. We identified a unique mutation profile in each of the analysed ADPKD samples, which was characterized by the presence of pathogenic variants in eight to 11 genes involved in different signalling pathways. Despite the significant genetic heterogeneity of ADPKD, we detected five genes whose genetic variants affected most ADPKD samples. The pathogenic variants in NCOR2 and LRP2 genes were present in all analysed samples of ADPKD. In addition, eight out of 10 samples showed a pathogenic variant in the MAML2 and FAT4 genes, and six out of 10 samples in the CELSR1 gene. In our study, we identified the signalling molecules that may contribute to the cystogenesis and may represent potential targets for the development of new ADPKD treatments.
