Stroke is a devastating cerebrovascular pathology with high morbidity and mortality. Inflammation plays a central role in the pathophysiology of stroke. Vagus nerve stimulation (VNS) is a promising immunomodulatory method that has shown positive effects in stroke treatment, including neuroprotection, anti-apoptosis, anti-inflammation, antioxidation, reduced infarct volume, improved neurological scores, and promotion of M2 microglial polarization. In this review, we summarize the current knowledge about the vagus nerve's immunomodulatory effects through the cholinergic anti-inflammatory pathway (CAP) and provide a comprehensive assessment of the available experimental literature focusing on the use of VNS in stroke treatment.
BACKGROUND: Brain injuries are the most common cause of death in productive age. Besides the extent of the injury, other systemic factors can also affect the outcome. Patients suffering from severe brain injury often experience extracranial inflammatory complications during the early period of treatment. Here, we investigate the changes in immunity in patients with brain injury. METHODS: 121 patients and 92 healthy controls were included in the research. Blood samples were collected on admission and analyzed by flow cytometry and biochemical methods. Multiple clusters of differentiation (CD) and antibody levels were investigated. The results were compared between patients and controls. In addition, results of two classes of severity (Glasgow Coma Scale, GCS, of 3-5 vs. 6-8) were also compared. RESULTS: Parameters of humoral immunity in patients immediately after admission were significantly lower than those from healthy donors, with the exception of IgE elevated as much as to resemble allergic reaction (p < 0.01). Of cellular parameters, only natural killer (NK) cluster CD56 + was elevated (p < 0.01). Extracranial infectious complications were more common in patients with GCS 3-5. CONCLUSIONS: Strong immune system disorders were observed in patients after severe brain injury, which may contribute to the worse outcome in such patients.

Úvod: Cílem naší studie je prezentovat výsledky léčby u pacientů s prodělaným aneuryzmatickým subarachnoidálním krvácením (SAK) po 3 měsících od ataky, kteří byli ošetřeni clippingem (NCH) nebo coilingem (EV). Materiál a metodika: Studie byla provedena na základě retrospektivní analýzy databáze pacientů s mozkovými aneuryzmaty, kteří byli léčeni mezi lety 2009 a 2019 ve FN Brno. Do studie bylo zařazeno 499 pacientů. Z těchto pacientů bylo 345 (69 %) ošetřeno NCH a 154 (31 %) bylo ošetřeno EV. Pacienty jsme rozdělili do 4 skupin: dle věku na skupiny 0–64 let a 65+ let a dále dle modality ošetření výduti NCH nebo EV. Hodnotili jsme vliv vstupního Hunt-Hessova skóre (HH) na výsledek ošetření dle věku a dané modality a také vztah mezi aktuálním věkem pacienta a výsledkem léčby po 3 měsících od ataky SAK, hodnoceným pomocí modifikované Rankinovy škály (modified Rankin scale; mRS), kde úspěšný výsledek léčby byl mRS 0–2 (soběstačný pacient). Výsledky: Z celkového počtu 499 pacientů bylo 345 (69 %) ošetřeno NCH a 154 (31 %) EV, přičemž průměrný věk NCH a EV ošetřených pacientů nebyl statisticky signifikantně rozdílný (p = 0,2216). U chirurgicky i endovaskulárně léčených pacientů ve všech věkových kategoriích jsme nezaznamenali statisticky signifikantní rozdíl ve skóre HH (p = 0,1664) ani v grafické závažnosti SAK dle Fischerova skóre (p = 0,5041). Uspokojivý výsledek léčby (mRS 0–2) po 3 měsících od ataky mělo ve skupině 65+ let po NCH 49 pacientů (52,88 %), u 65+ po EV 23 (60,61 %). Špatný výsledek léčby (mRS 3–6) po 3 měsících mělo 43 pacientů (47,12 %) ošetřených NCH a 15 (39,39 %) ošetřených EV, což je statisticky významné (p = 0,0002). Uspokojivý výsledek léčby (mRS 0–2) po 3 měsících s dobrým klinickým stavem při přijetí (HH 1–2) mělo ve skupině 0–64 let po NCH 136 pacientů (55 %) a u 65+ po NCH 28 (33 %). Špatný výsledek léčby (mRS 3–6) po 3 měsících při špatném počátečním klinickém stavu (HH 3–5) mělo 50 pacientů (20 %) ošetřených NCH ve skupině 0 -64 let a 39 (44 %) ošetřených NCH ve skupině 65+, což je statisticky významné (p = 0,0001). Závěr: Senioři (65+ let) na rozdíl od mladších pacientů po SAK trpí zvýšeným rizikem špatného neurologického stavu po 3 měsících od ataky, obzvláště jsou-li v těžkém klinickém stavu při přijetí. Nezanedbatelná část těchto pacientů však po ošetření výduti zůstává v dobrém klinickém stavu, proto je potřeba metodu ošetření a rozsah léčby posuzovat individuálně. U seniorů ošetřených endovaskulárně jsme pozorovali lepší výsledek léčby po 3 měsících ve srovnání s pacienty ošetřenými otevřenou operací.
Introduction: The aim of our study is to present the treatment outcomes of patients with aneurysmal subarachnoid hemorrhage (SAH) 3 months after the attack who were treated with clipping (MS) or coiling (EV). Materials and methods: The study was based on a retrospective analysis of a database of patients with cerebral aneurysms who were treated at the University Hospital Brno between 2009 and 2019. A total of 499 patients were included in the study. Of these patients, 345 (69%) were treated with surgical MS and 154 (31%) were treated with EV. Patients were divided into 4 groups: according to the age into 0–64 years and 65+ years group, and according to the modality of treatment of aneurysm by MS or EV. We evaluated the effect of the initial Hunt-Hess (HH) score on the treatment outcome according to age and modality and also the relationship between the current age of the patient and the treatment outcome at 3 months after the SAH attack, as assessed by a modified Rankin scale (mRS), where the successful treatment outcome was mRS 0–2 (self-sufficient patient), and the effect of the initial Hunt-Hess score (HH) on the treatment outcome was based on age and modality. Results: Of the 499 patients, 345 (69%) were treated with MS and 154 (31%) with EV, and the average age of MS and EV treated patients was not statistically significantly different (P = 0.2216). In both surgically and endovascularly treated patients in all age categories; we did not observe a statistically significant difference in HH score (P = 0.1664) or in graphical severity of SAH according to the Fischer score (P = 0.5041). Satisfactory treatment outcome (mRS 0–2) at 3 months after the attack was found in 49 (52.88%) in the 65+ years group after MS and 23 (60.61%) in the 65+ years group after EV. Poor treatment outcome (mRS 3–6) at 3 months was found in 43 (47.12%) in MS treated and 15 (39.39%) in EV treated patients, which was statistically significant (P = 0.0002). Satisfactory treatment outcome (mRS 0–2) at 3 months with good clinical status on admission (HH 1–2) was seen in 136 (55%) patients in the 0–64 years group after MS and 28 (33%) in the 65+ years group after MS. Poor treatment outcome (mRS 3–6) after 3 months with poor initial clinical condition (HH 3–5) had 50 (20%) patients treated with MS in the 0 -64 years group and 39 (44%) treated with MS in the 65+ years group, which is statistically significant (P = 0.0001). Conclusion: Elderly (65+ years) after SAH are at a higher risk of poor neurological status 3 months after the attack compared to younger patients, especially if they are in a severe clinical condition on admission. However, a significant proportion of these patients remain in a good clinical condition after treatment of an aneurysm, so the method of treatment and extent of treatment needs to be assessed individually. In elderly patients treated endovascularly, we observed a better treatment outcome at 3 months compared with patients treated by open surgery.

The production of free radicals is inevitably associated with metabolism and other enzymatic processes. Under physiological conditions, however, free radicals are effectively eliminated by numerous antioxidant mechanisms. Oxidative stress occurs due to an imbalance between the production and elimination of free radicals under pathological conditions. Oxidative stress is also associated with ageing. The brain is prone to oxidative damage because of its high metabolic activity and high vulnerability to ischemic damage. Oxidative stress, thus, plays a major role in the pathophysiology of both acute and chronic pathologies in the brain, such as stroke, traumatic brain injury or neurodegenerative diseases. The goal of this article is to summarize the basic concepts of oxidative stress and its significance in brain pathologies, as well as to discuss treatment strategies for dealing with oxidative stress in stroke.

AIM: To report on patients who underwent surgical treatment of arteriovenous malformations (AVMs) at our institution. METHODS: This retrospective single-center case series enrolled the patients who underwent surgical treatment of pial AVM at the Department of Neurosurgery, University Hospital Brno, between 2005 and 2019. The data are summarized as descriptive statistics presenting basic characteristics in all the patients and in sex or age subgroups. RESULTS: Fifty patients were enrolled. The majority of AVMs were of Spetzler-Martin grade II (n=27; 54%), localized supratentorialy (n=43; 86%), and half of AVMs were ruptured. A total resection was performed in 48 patients (96%), and a good overall outcome was achieved in 44 patients (88%). Surgery-associated morbidity was 2%, and the mortality rate was 0% due to meticulous selection of patients for surgical treatment. CONCLUSION: Microsurgery is an appropriate method of treatment for S-M grade I-III pial AVMs. Microsurgery may be used to treat the majority of small-nidus AVMs with a low mortality and morbidity, when precisely planned and performed by an expert vascular team. The meticulous selection of patients for surgical treatment is crucial.
INTRODUCTION: Anterior communicating artery aneurysms (ACoAAs) are the most frequent intracranial aneurysms treated at neurosurgical departments with a vascular program. MATERIAL AND METHODS: We reviewed patients with ACoAAs in a single institution over ten years (2008-2017). The focus was on the final outcome; complications, age, and clinical condition with respect to modalities were analyzed. RESULTS: A total of 198 patients treated during this period was included in the study: 176 patients had a ruptured ACoAA and 22 had an unruptured ACoAA. Then, 127 (71%) were treated surgically and 51 (29%) by endovascular means. Out of the whole series, a good recovery occurred in 123 patients (62%), moderate disability in 11 (5.5%), severe disability in 19 (10%), vegetative state in 11 (5.5%), and death in 34 (17%). In the 157 patients (72.5%) with a subarachnoid hemorrhage (SAH), both modalities had a favorable outcome: 27.5% had an unfavorable outcome, 12% had complications in surgery versus 17.6% during endovascular treatment. No statistical difference in outcome, complications, and age was noted between modalities. Surgical treatment was more frequently adopted for patients in a better clinical condition (p ≤ 0.05). CONCLUSION: More than two thirds of the patients (72.5%) reached a favorable outcome. There was no difference in age between the treatment modalities. Risks of complications are present and specific for both modalities.

Cíl: Výhřez meziobratlové ploténky v bederní oblasti patří k nejčastějším indikacím operace páteře. Zlatým standardem chirurgické léčby výhřezu meziobratlové ploténky je otevřená diskektomie z transligamentózního přístupu. V poslední dekádě se čím dál více prosazuje endoskopická technika, která má srovnatelný efekt z hlediska výsledku léčby a navíc přináší řadu výhod pro operatéra i pacienta díky minimální invazivnosti endoskopické techniky. Neurochirurgická klinika FN Brno je prvním pracovištěm v ČR, které začalo získávat zkušenosti s tímto typem operací. V předkládaném článku přinášíme první zkušenosti a výsledky získané od roku 2017, kdy u nás byla endoskopická metoda léčby výhřezu meziobratlové ploténky zavedena. Metody: Do souboru jsou zařazování pacienti ve věkovém rozmezí 20– 70 let, u kterých je indikováno operační řešení výhřezu meziobratlové ploténky v etáži L4/ L5 nebo L5/ S1 a kteří dříve nepodstoupili žádnou operaci v inkriminovaných etážích. Dosud bylo do souboru zařazeno 15 pacientů, u kterých byla hodnocena míra intenzity bolesti charakteru dorzalgií a radikulární symptomatologie pomocí Visual Analogue Score (VAS) a dále míra omezení v běžných činnostech pomocí Oswestry Disability Index (ODI). Údaje byly hodnoceny předoperačně a dále s odstupem 6 týdnů a 6 měsíců po operaci. Výsledky: Předoperační hodnoty VAS skóre byly u celého souboru signifikantně vyšší než hodnoty 6 týdnů a 6 měsíců po operaci, a to jak v případě dorzalgie, tak v případě radikulárních bolestí. Obdobné výsledky byly v podskupinách mužů a žen, kdy došlo u obou typů iritací ke značnému zlepšení 6 týdnů i 6 měsíců po operaci. U souboru jako celku byly signifikantně vyšší předoperační hodnoty ODI v porovnání s hodnotami 6 týdnů a 6 měsíců po operaci. V podskupině mužů nebyl prokázán signifikantní rozdíl mezi předoperačními hodnotami ODI a hodnotami 6 týdnů po operaci, zatímco 6 měsíců po operaci byly hodnoty výrazně nižší. V podskupině žen byly předoperační hodnoty ODI signifikantně vyšší než hodnoty 6 týdnů i 6 měsíců po operaci. Pooperačně nastalo u zkoumaného souboru značné zmírnění subjektivních obtíží, které ve většině případů odpovídalo normalizaci stavu. K výraznému zlepšení až k ústupu problémů došlo ve všech sledovaných parametrech, kterými byly dorzalgie, radikulární symptomatologie a omezení běžných denních aktivit v důsledku bolestí dolní části zad. Recidiva výhřezu disku se ve sledovaném období vyskytla v jednom případě. Závěr: Naše dosavadní zkušenosti s použitím endoskopické techniky k operaci výhřezu meziobratlové ploténky hodnotíme maximálně pozitivně a vzhledem k těmto zkušenostem i s odkazem na literární zdroje z jiných pracovišť plánujeme další rozvoj této operační metody.
Aim: Lumbar disc herniation is the most frequent indication for spinal surgery. Open discectomy is considered as a standard surgical procedure; however, the endoscopic technique has evolved recently as an alternative method of treatment. Compared to open discectomy, the endoscopic technique has a similar effect in terms of outcome and additionally, it is beneficial for both surgeon and patient, because the endoscopic technique is a minimally invasive procedure. Department of Neurosurgery in The University Hospital Brno is the first department in the Czech Republic in which endoscopic discectomy has been implemented. The aim of this article is to present the first results and experiences with this technique, which has been used in our department since 2017. Methods: So far, 15 patients (20– 70 years old) underwent endoscopic surgery for L4– 5 or L5– S1 herniation. Evaluation parameters were pain intensity (dorsalgia and radiculopathy) as ses sed by Visual Analogue Score (VAS) and limitations of com mon activities as ses sed by Oswestry Disability Index (ODI). The parameters were evaluated before surgery and after the surgery at the 6-week and 6-month time- -points. Results: In all study groups the VAS score (for both dorsalgia and radiculopathy) was signifi cantly higher before surgery compared to the 6-week and 6-month time-points. Similar results were found in male and female subgroups, and significant improvement was observed at both the 6-week and 6-month time-points. The ODI before surgery was significantly higher in all patients before surgery compared to the 6-week and 6-month time-points. In the male subgroup, there was no significant difference between ODI before surgery and the 6-week time-point, while ODI at the 6-month time-point was signifi cantly lower. In the female subgroup, ODI at both the 6-week and 6-month timepoints was signifi cantly lower than before surgery. Recur rent herniation had occurred in one case and was resolved by reoperation. Conclusion: In conclusion, endoscopic lumbar discectomy is a safe and effective option for lumbar disc herniation surgery.

Cíl: Cílem studie bylo prozkoumat změny v expresi pro-zánětlivého a pro-apoptotického cytokinu tumor nekrotizující faktor alfa (TNFα) a mikroRNA (miRNA), které se podílejí na jeho regulaci v časném období po subarachnoidálním krvácení (SAK). Soubor a metodika: Exprese miRNA (miR-125b, miR-146a, miR-346, miR-155, miR-15b) a mRNA (TNFα) byly stanoveny pomocí kvantitativní polymerázové řetězové reakce v reálném čase z mozkové tkáně experimentálních zvířat. Celkem 88 zvířat bylo rozděleno do skupin Sham (kontrolní operace bez indukce SAK), Lehké SAK, Těžké SAK, do časových intervalů 2, 4, 6 a 8 h (n = 7 ve skupině); 4 zvířata byla použita jako absolutní kontrola. Výsledky: Byly nalezeny statisticky významné rozdíly v expresi TNFα mezi skupinami Sham a Těžké SAK ve všech zkoumaných časových intervalech (p < 0,05), dále mezi skupinami Sham a Lehké SAK 4 h po indukci SAK (p < 0,05) a mezi skupinami Lehké SAK a Těžké SAK ve 2 a 6h časovém intervalu (p < 0,05). Dále byl pozorován významný rozdíl v expresi miRNA-15b mezi skupinami Sham a Těžké SAK 8 h po začátku SAK (p < 0,05). U dalších analyzovaných miRNA jsme v expresi nepozorovali žádné statisticky významné změny. Závěr: SAK bylo asociováno s časným nárůstem exprese TNFα a miR-15b, zejména u skupiny Těžké SAK. Navzdory komplexitě vzájemné regulace mezi cytokiny a mikroRNA, může informace o časné aktivaci zánětlivých/apoptotických mechanismů několik hodin po SAK přispět k lepšímu poznání patofyziologie SAK. Pochopení mechanismů vzájemné regulace proapoptotických markerů TNFα a miR-15b může přispět ke zlepšení terapie této závažné patologie.
Aim: The aim of the study was to investigate expression changes of pro-inflammatory and pro-apoptotic cytokine tumor necrosis factor alpha (TNFα) and microRNAs (miRNAs) involved in its regulation in early pathophysiological changes after subarachnoid haemorrhage (SAH). Materials and methods: MiRNAs (miR-125b, miR-146a, miR-346, miR-155, miR-15b) and mRNA (TNFα) expression were determined by quantitative real-time polymerase chain reaction in brain tissue samples. A total of 88 animals were divided to Sham (control surgery without induction of SAH), Mild SAH, Severe SAH groups in following time-points: 2, 4, 6 and 8 h (n = 7 per group); including 4 animals used as an absolute control. Results: We have found a statistically significant difference in TNFα expression between Sham and Severe SAH groups at all the time-points (p < 0.05), between Sham and Mild SAH groups 4 h after induction of SAH (p < 0.05) and between Mild and Severe SAH groups at 2 and 6 h time-points (p < 0.05). Furthermore, a significant difference in miR-15b expression between Sham and Severe SAH groups was observed 8 h after SAH (p < 0.05). All the other microRNAs have not been significantly changed. Conclusions: SAH was associated with an early increase in TNFα and miR-15b expression especially in Severe SAH group. Despite complex cross-regulation between cytokines and miRNA, any information about the activation of inflammation/apoptotic mechanisms within a few hours after SAH may improve our knowledge of SAH pathophysiology. Furthermore, it can lead to therapeutic improvement using a combination of both pro-apoptotic markers TNFα and miR-15b.

Úvod: Cílem výzkumu bylo stanovit epidemiologické faktory, které se podílejí na iniciaci syndromu karpálního tunelu (SKT), a určit, jak jednotlivé faktory souvisejí s mírou bolestivosti při SKT. Soubor a metodika: Výzkum byl prováděn v podobě dotazníkového šetření na Neurochirurgické klinice Fakultní nemocnice Brno. Účastnilo se jej celkem 93 pacientů, kteří na této klinice podstoupili operaci SKT v období od září 2015 do prosince téhož roku. Respondenti odpovídali na otázky mající vztah k epidemiologii a klinickým obtížím SKT. Výsledky: Vztah k rozvoji SKT byl prokázán u věku, pohlaví, vyšší tělesné hmotnosti, onemocnění štítné žlázy. Průměrná míra bolestivosti bez ohledu na existenci přítomných epidemiologických faktorů byla na stupni 6 vizuální analogové škály. Míra bolestivosti nebyla u našich pacientů v žádné závislosti na sledovaných epidemiologických faktorech, vyjma noční bolesti, která byla častěji přítomna u žen (p < 0,05). U žen bylo ve srovnání s muži také častější oslabení postižené ruky (p < 0,05). Závěr: S rozvojem vzniku SKT nepochybně souvisejí některé epidemiologické faktory, jako jsou věk, pohlaví, vyšší tělesná hmotnost, některá přidružená onemocnění nebo úrazy a manuální typ profese. Nepodařilo se však prokázat, že by kterýkoli z těchto faktorů zároveň znamenal horší subjektivní vnímání bolesti u SKT, vyjma noční bolesti, která v našem souboru byla častější u žen.
Introduction: The aim of this research was to determine the epidemiological factors involved in carpal tunnel syndrome (CTS) development and to determine how these factors contribute to pain symptoms associated with CTS. Patients and methods: A questionnaire survey was conducted at the Department of Neurosurgery, University Hospital Brno. A total number of 93 patients, who underwent surgery for CTS between September 2015 and December of the same year, were included in the study. The patients responded to the questions related to CTS epidemiology and clinical problems. Results: CTS development was associated with age, gender, higher body weight, and thyroid gland disease. The average pain level according to the visual analog scale was 6 irrespective of the presence of epidemiological factors. The severity of pain was not dependent on any of the epidemiological factors observed in our patients except for night pain which was more frequent in women (p < 0.05). There was also a more frequent weakening of the affected hand in women compared to men (p < 0.05). Conclusion: CTS is related to some epidemiological factors such as age, gender, higher body weight, some associated illnesses or injuries, and manual type of occupation. However, it could not be shown that any of these factors would also mean a worse subjective perception of pain, except for night pain which was more common in women.
BACKGROUND: Subarachnoid hemorrhage (SAH) is a severe condition associated with high mortality. Early brain injury (EBI) plays an important role in the pathophysiology of SAH, and inflammation is a major contributor to EBI. Inflammation is a widely studied topic in both experimental and clinical conditions; however, just a few clinical studies have focused primarily on the early inflammatory response after SAH, and detailed information about the association between the dynamics of early inflammatory response with main clinical characteristics is lacking. This study analyzes the early dynamics of inflammatory response after SAH and evaluates the possible associations between the markers of early inflammatory response and main clinical characteristics. PATIENTS AND METHODS: A total of 47 patients with a diagnosis of aneurysmal SAH within the last 24 hours were enrolled in the study. All treatments, including treatment of aneurysm (surgery/coiling) and implantation of a drainage system (external ventricular drainage/lumbar catheter), were conducted in the same way as in other patients with this diagnosis. Blood and cerebrospinal fluid (CSF) samples were collected three times a day for 4 days. The dynamics of proinflammatory cytokines were assessed, and associations between levels of the proinflammatory cytokines interleukin (IL)-6, IL-1β, or tumor necrosis factor (TNF)α and main clinical characteristics were evaluated using linear mixed-effect models. RESULTS: The CSF levels of IL-6 were massively increased initially after SAH (up to 72 hours) with an additional increase in later phases (after 72 hours), but there was high variability in IL-6 levels. A significant association was noted between the Glasgow Outcome Scale score and both overall levels of IL-6 (p = 0.0095) and their dynamics (p = 0.0208); the effect of the Hunt and Hess scale was borderline (p = 0.0887). No association was found between IL-6 levels and Fisher grade, modality of treatment (surgery, coiling, no treatment), and later development of cerebral vasospasm. Plasmatic levels of IL-6 increased slightly, but no significant association was found. The levels of IL-1β and TNFα were within the physiologic range in both CSF and plasma. CONCLUSIONS: Early dynamics of IL-6 in CSF are associated with a patient́s outcome. But it is difficult to use IL-6 alone for outcome prediction due to its high variability. The question is whether the dynamics of IL-6 could be used in combination with other early markers associated with brain injury. More detailed research is required to answer this question.
