Cíl práce: Kmeny Staphylococcus aureus patří mezi obávané bakteriální původce nozokomiálních i komunitních infekcí, a to hlavně tehdy, jsou-li rezistentní k antibiotikům, či mají-li významné faktory patogenity, nebo jsou schopny tvořit odolný biofilm. Hledání nových terapeutik, účinných na takové kmeny, je jednou z hlavních priorit medicíny a vědeckého výzkumu. Cílem studie bylo popsat výskyt S. aureus a podíl kmenů rezistentních k methicilinu (MRSA) diagnostikovaných v laboratořích Mikrobiologického ústavu Lékařské fakulty Masarykovy univerzity (LF MU) a Fakultní nemocnice (FN) u sv. Anny v Brně v letech 2011–2018. U vybraných kmenů S. aureus byla studována schopnost tvorby biofilmu, testována citlivost k antibiotikům a k fágovému přípravku Stafal®. Podmínkou bylo navrhnout metodu vhodnou pro jednoduché rutinní testování citlivosti bakterií k fágům. Materiál a metody: U 867 klinických izolátů S. aureus a u 132 kmenů náležících k dalším druhům rodu Staphylococcus (izolovaných v letech 2011–2017) byla testována citlivost k fágovému preparátu Stafal® za použití metody dvouvrstevného agaru. U všech kmenů S. aureus byla testována schopnost tvorby biofilmu modifikovanou Christensenovou metodou využívající mikrotitrační destičky a citlivost k protistafylokokovým antibiotikům (Diskovým difuzním testem). U 95 kmenů S. aureus byla metoda dvouvrstevného agaru (MDA) porovnána s námi navrženou metodou, která je založena na úbytku optické denzity (MOZ) bakteriální suspenze. Výsledky: Ve sledovaném období bylo v laboratořích LF MU a FN u sv. Anny v Brně diagnostikováno průměrně 2 900 kmenů S. aureus ročně. V hemokulturách a venózních katetrech s nálezem S. aureus byl podíl kmenů MRSA 8,8–15,2 %. Bylo potvrzeno, že kmeny S. aureus izolované z venózních katetrů a hemokultur byly silnějšími producenty biofilmu než kmeny z jiných klinických materiálů. Kmeny MRSA produkovaly biofilm silněji než kmeny methicilin citlivé (MSSA). K preparátu Stafal® bylo citlivých 90,4 % kmenů S. aureus. V souboru MRSA to bylo dokonce 99,0 % citlivých kmenů. Nebyla prokázána souvislost mezi citlivostí ke Stafalu® a schopností tvořit biofilm. Přesto, že jde o preparát zaměřený na S. aureus, byla určitá citlivost (26,5 %) prokázána i u dalších druhů stafylokoků. Nová jednoduchá metoda navržená pro rutinní testování citlivosti k fágovým preparátům pomocí MOZ byla srovnatelně citlivá a spolehlivá jako běžně používaná metoda MDA, kromě jednoduchosti a časové nenáročnosti přináší výhodu také v tom, že po prodloužení kultivace suspenze a frekvence měření můžeme sledovat i dynamiku působení fágů. Závěry: Ukázalo se, že podíl diagnostikovaných kmenů MRSA odpovídá výskytu kmenů v celé České republice a že zjištění týkající se produkce biofilmu jsou v souladu s dosavadními vědeckými poznatky. Hostitelské spektrum přípravku Stafal® je relativně široké a zahrnuje většinu kmenů S. aureus i některé koaguláza-negativní stafylokoky. Nejvyšší účinnost vykazuje na kmeny MRSA s další přidruženou rezistencí k antibiotikům (99,4 %). Na základě testování 867 kmenů S. aureus a 132 jiných druhů stafylokoků in vitro bylo prokázáno, že fágový preparát Stafal® je vhodným kandidátem pro léčbu stafylokokových infekcí i tam, kde klasická antibiotika selhávají. Navíc byla navržena jednoduchá metoda, jak rutinně testovat citlivost k fágům u klinických bakteriálních izolátů, což je nezbytným předpokladem terapeutické aplikace.
Aim: Staphylococcus aureus strains are the cause of frightening hospital and community infections, especially when they are resistant to antimicrobials, have important pathogenicity factors, or have biofilm production ability. Looking for novel therapeutic options which would be effective against such strains is one of the highest priorities of medicine and medical research. The study aim was to describe the occurrence of S. aureus strains and proportion of methicillin resistant strains (MRSA) detected in laboratories of the Microbiological Institute, Faculty of Medicine, Masaryk University (FM MU) and St. Anne's University Hospital, Brno in 2011-2018. Selected strains of S. aureus were tested for biofilm production ability and susceptibility to antimicrobials and Stafal®, a phage therapeutic agent. A prerequisite was to develop a simple routine method suitable for phage susceptibility testing of bacteria. Material and methods: Altogether 867 clinical isolates of S. aureus and 132 strains of other species of the genus Staphylococcus (isolated in 2011-2017) were tested for susceptibility to the phage therapy preparation Stafal® using the double-layer agar method. All strains of S. aureus were tested for biofilm production ability by the modified Christensen method with the use of titration microplates and for susceptibility to antistaphylococcal antibiotics by the disk diffusion test. For 95 S. aureus strains, the outcome of the double-layer agar method (DAM) was compared with that of our newly designed method (ODM) based on optical density decrease of the bacterial suspension. Results: During the study period, the laboratories of the Faculty of Medicine, Masaryk University (FM MU) and St. Anne's University Hospital, Brno detected 2900 strains of S. aureus per year on average. The proportion of MRSA among S. aureus isolates from blood culture and venous catheters ranged between 8.8-15.2 %. S. aureus strains recovered from venous catheters and blood culture were confirmed as stronger biofilm producers than those from other clinical specimens. MRSA strains showed higher biofilm production than methicillin susceptible strains (MSSA). As many as 90.4 % of S. aureus strains tested susceptible to the Stafal® preparation. Even a higher proportion, i.e. 99.0 %, of MRSA strains were Stafal® susceptible. No relationship was found between Stafal® susceptibility and biofilm production ability. Although Stafal® targets primarily S. aureus, some susceptibility (26.5 %) was also found for other staphylococcal species. A novel simple method designed for routine testing of susceptibility to phage therapy preparations based on optical density decrease was comparably sensitive and reliable as the commonly used double-layer agar method (DAM) and, in addition to being easy and rapid to perform, after prolonged suspension culture and at higher measurement frequency, it has an extra advantage of providing the possibility for monitoring also phage action dynamics. Conclusions: The proportion of MRSA strains detected in this study is comparable to that reported for the whole Czech Republic, and the biofilm production data are consistent with scientific evidence. The host range of the Stafal® preparation is relatively wide and covers most strains of S. aureus and some coagulase negative staphylococci. The highest efficiency of Stafal® (99.4 %) was observed against MRSA strains with multiple types of antibiotic resistance. In vitro testing of 867 strains of S. aureus and 132 other staphylococcal species has shown the phage therapy preparation Stafal® to be a suitable candidate therapeutic option for the treatment of staphylococcal infections, especially in case of failure of conventional antibiotic therapy. Moreover, a simple method for routine phage susceptibility testing of clinical bacterial isolates has been designed, which is an essential tool to be used in phage therapy.
Staphylococcus aureus may be a highly virulent human pathogen, especially when it is able to form a biofilm, and it is resistant to antibiotic. Infections caused by these bacteria significantly affect morbidity and mortality, primarily in hospitalized patients. Treatment becomes more expensive, more toxic, and prolonged. This is the reason why research on alternative therapies should be one of the main priorities of medicine and biotechnology. A promising alternative treatment approach is bacteriophage therapy. The effect of the anti-staphylococcal bacteriophage preparation Stafal® on biofilm reduction was assessed on nine S. aureus strains using both sonication with subsequent quantification of surviving cells on the catheter surface and evaluation of biofilm reduction in microtiter plates. It was demonstrated that the bacteriophages destroy planktonic cells very effectively. However, to destroy cells embedded in the biofilm effectively requires a concentration at least ten times higher than that provided by the commercial preparation. The catheter disc method (CDM) allowed easier comparison of the effect on planktonic cells and cells in a biofilm than the microtiter plate (MTP) method.

Urinary or ureteral catheter insertion remains one of the most common urological procedures, yet is considered a predisposing factor for urinary tract infection. Diverse bacterial consortia adhere to foreign body surfaces and create various difficult to treat biofilm structures. We analyzed 347 urinary catheter- and stent-related samples, treated with sonication, using both routine culture and broad-range 16S rDNA PCR followed by Denaturing Gradient Gel Electrophoresis and Sanger sequencing (PCR-DGGE-S). In 29 selected samples, 16S rRNA amplicon Illumina sequencing was performed. The results of all methods were compared. In 338 positive samples, from which 86.1% were polybacterial, 1,295 representatives of 153 unique OTUs were detected. Gram-positive microbes were found in 46.5 and 59.1% of catheter- and stent-related samples, respectively. PCR-DGGE-S was shown as a feasible method with higher overall specificity (95 vs. 85%, p < 0.01) though lower sensitivity (50 vs. 69%, p < 0.01) in comparison to standard culture. Molecular methods considerably widened a spectrum of microbes detected in biofilms, including the very prevalent emerging opportunistic pathogen Actinotignum schaalii. Using massive parallel sequencing as a reference method in selected specimens, culture combined with PCR-DGGE was shown to be an efficient and reliable tool for determining the composition of urinary catheter-related biofilms. This might be applicable particularly to immunocompromised patients, in whom catheter-colonizing bacteria may lead to severe infectious complications. For the first time, broad-range molecular detection sensitivity and specificity were evaluated in this setting. This study extends the knowledge of biofilm consortia composition by analyzing large urinary catheter and stent sample sets using both molecular and culture techniques, including the widest dataset of catheter-related samples characterized by 16S rRNA amplicon Illumina sequencing.

Identification and prevention of Staphylococcus aureus-caused infections may benefit from a fast and dependable method to distinguish between the methicillin-resistant (MRSA) and methicillin-susceptible (MSSA) S. aureus strains. The current methods involving polymerase chain reaction and/or other molecular tests are usually laborious and time-consuming. We describe here a fast and low-cost method employing capillary zone electrophoresis (CZE) to distinguish between MRSA and MSSA. The method makes use of a supercritical water-treated fused silica capillary, the inner surface of which has subsequently been modified with (3-glycidyloxypropyl)trimethoxysilane. With optimized proportions of suitable additives to the background electrolyte, a CZE separation of MRSA from MSSA may be completed within 12 min. The cells were baseline-resolved, and resolution was determined to be 3.61. The isoelectric points of MSSA and MRSA were found to be the same for both groups of these strains, pI = 3.4.
The effect of antibiotics on the microbial cells and concentration of antibiotics in the human body is essential for the effective use of antimicrobial therapy. The capillary isoelectric focusing is a suitable technique for the separation and the detection of bacteria, and amphoteric substances from nature. However, the determination of isoelectric points of ampholytic antibiotics by conventional techniques is time consuming. For this reason, capillary isoelectric focusing seems to be appropriate as a simple and reliable way for establishing them. The separation conditions for the capillary isoelectric focusing of selected ampholytic antibiotics with known isoelectric points and pK as, ampicillin (pI 4.9), ciprofloxacin (pI 7.4), ofloxacin (pI 7.1), tetracycline (pI 5.4), tigecycline (pI 9.7), and vancomycin (pI 8.1), were found and optimized in the suitable pH ranges pH 2.0-5.3, 2.0-9.6, and 9.0-10.4. The established values of isoelectric points correspond with those found in the literature except tigecycline. Its pI was not found in the literature. As an example of a possible procedure for direct detection of both ampholytic antibiotics and bacteria, Staphylococcus epidermidis, in the presence of culture media or whole human blood, was found. The changes of the bacterial cells after their treatment with tetracycline were confirmed by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry. Capillary isoelectric focusing allows the fast and simple determination of isoelectric points of relevant antibiotics, their quantification from the environment, as well as studying their effectiveness on microorganisms in biological samples.

Lidská granulocytární anaplazmóza je emergentní zoonóza vyvolaná obligátní intracelulární bakterií Anaplasma phagocytophilum. A. phagocytophilum je v Evropě přenášena klíštětem obecným (Ixodes ricinus). Anaplazmóza je po lymeské borrelióze a evropské klíšťové encefalitidě třetím nejčastějším onemocněním přenášeným I. ricinus. Anaplazmóza se projevuje nespecifickými příznaky: únavou, teplotou, bolestmi hlavy, svalů a kloubů. Při těžším průběhu může být postižen zažívací nebo dýchací trakt. Průběh infekce je však většinou asymptomatický. Cílem naší práce bylo zjistit séroprevalenci A. phagocytophilum u pacientů se suspektní lymeskou borreliózou. Celkem bylo na protilátky proti A. phagocytophilum vyšetřeno 314 sér od pacientů s podezřením na lymeskou borreliózu. Ke stanovení protilátek ve třídách IgM a IgG byla použita metoda imunoblot. Pozitivita anaplazmových protilátek byla zjištěna u 34 vyšetřovaných (10,82 %). Protilátky třídy IgM byly pozitivní v 19 případech a protilátky třídy IgG v 10 případech. U pěti pacientů byla zjištěna pozitivita v obou třídách protilátek. Protilátky proti Borrelia burgdorferi sensu lato byly detekovány u 181 pacientů (57,64 %). Ko-séroprevalence A. phagocytophilum a B. burgdorferi s. l. byla zjištěna ve 26 případech (8,3 %). Nejvíce pozitivních vzorků na anaplazmové protilátky bylo u pacientů ve věkové kategorii 60–69 let. Naše výsledky ukazují, že nákaza A. phagocytophilum v České republice není vzácností a je na ni třeba pomýšlet v souvislosti s přisátím klíštěte.
Human granulocytic anaplasmosis (HGA) is an emerging tick-borne zoonotic disease caused by an obligate intracellular bacterium, Anaplasma phagocytophilum. In Europe, A. phagocytophilum is transmitted by Ixodes ricinus ticks. After Lyme borreliosis and European tick-borne encephalitis, HGA is the third most common tick-borne infection in the USA and Europe. The clinical symptoms of anaplasmosis are non-specific and include malaise, fever, headache, myalgia, and arthralgia. In more severe cases, the gastrointestinal or respiratory tract may be affected. However, most infections are asymptomatic. The aim of our study was to determine the seroprevalence of A. phagocytophilum in patients with suspected Lyme borreliosis. A total of 314 sera from patients with suspected Lyme borreliosis were screened for IgG and IgM antibodies against A. phagocytophilum. The immunoblot assay was used to detect the antibodies. Anti-A. phagocytophilum antibodies were detected in 34 patients, i.e. in 10.82 %. IgM antibodies were positive in 19 cases and IgG antibodies in 10 cases. Positivity to both IgM and IgG antibodies was revealed in five patients. Antibodies against Borrelia burgdorferi sensu lato were detected in 181 patients (57.64 %). Co-seroprevalence of Borrelia burgdorferi s. l. and A. phagocytophilum was found in 26 patients (8.3 %). Positivity for anti-A. phagocytophilum antibodies was most often seen in samples from the age group 60–69 years. Our results show that A. phagocytophilum infection is not uncommon in the Czech Republic and should be considered in patients with a history of a tick bite.

Antibiotics cure infections by influencing bacterial growth or viability. Antibiotics can be divided to two groups on the basis of their effect on microbial cells through two main mechanisms, which are either bactericidal or bacteriostatic. Bactericidal antibiotics kill the bacteria and bacteriostatic antibiotics suppress the growth of bacteria (keep them in the stationary phase of growth). One of many factors to predict a favorable clinical outcome of the potential action of antimicrobial chemicals may be provided using in vitro bactericidal/bacteriostatic data (e.g., minimum inhibitory concentrations-MICs). Consequently, MICs are used in clinical situations mainly to confirm resistance, and to determine the in vitro activities of new antimicrobials. We report on the combination of data obtained from MICs with information on microorganisms' "fingerprint" (e.g., DNA/RNA, and proteins) provided by Raman spectroscopy. Thus, we could follow mechanisms of the bacteriostatic versus bactericidal action simply by detecting the Raman bands corresponding to DNA. The Raman spectra of Staphylococcus epidermidis treated with clindamycin (a bacteriostatic agent) indeed show little effect on DNA which is in contrast with the action of ciprofloxacin (a bactericidal agent), where the Raman spectra show a decrease in strength of the signal assigned to DNA, suggesting DNA fragmentation.
