UNLABELLED: Backround. The use of artificial vascular grafts (arteriovenous graft, AVG) is indicated in patients in hemodialysis programs if the subcutaneous venous bed is exhausted or unsuitable for arteriovenous fistula (AVF) creation. The native fistula should be the hemodialysis access of first choice: AVF has better results in terms of function and potential complications. However, the use of AVG is necessary in some patients. In these patients, extensive clinical examination, color duplex sonography and angiography should be performed prior to indication. The technique of graft implantation requires respect for geometric relations for the graft anastomoses to minimize the formation of intimal hyperplasia mainly on the venous anastomosis. The main complications of AVG are stenosis on the venous anastomosis (VAG), causing closure of graft and graft infection. The cumulative function of AVG is 59-90% in the first year and 50-82% in the second year. Arteriovenous graft stenosis leading to thrombosis is a major cause of complications in patients undergoing hemodialysis. The purpose of this review is to summarise current knowledge of the diagnostics and treatment of graft thrombosis and discuss the issue in combination with relevant publications via Pubmed database. CONCLUSION: The most frequent cause of failure of AVG for hemodialysis is stenosis and closure by VAG. AVG closure can be addressed surgically, endovascularly (amenable to thrombectomy by radiological or surgical means) and by hybrid performance.

Pro přiznání profesionality poškození nervus ulnaris v oblasti lokte je rozhodující Stanovení nejméně střední tíže postižení a splnění pracovních podmínek uvedených v seznamu nemocí z povolání. U 44 členného retrospektivně hodnoceného souboru osob s profesionální lézí nervus ulnaris považovali autoři za EMG kritérium poruchy střední tíže zpomalení rychlosti vedení motorickými vlákny nervus ulnaris kolem lokte na 40 m/s, za dolní hranici normy byla pokládána rychlost 50 m/s. U 20 osob bylo onemocnění přiznáno po průměrné délce práce 15,8 let jako onemocnění z přetěžování horních končetin, přičemž nejčastěji byly postiženy elektromontážní dělnice. U 24 osob bylo onemocnění přiznáno po průměrných 23,9 letech práce s vibrujícími nástroji, nejčastěji onemocněli brusiči kovů. Je konstatována častá koincidence profesionální leze nervus ulnaris v oblasti lokte s jinými onemocněními pohybového aparátu a nervů horních končetin, u 70,4 % souboru byl diagnostikován současně syndrom karpálního tunelu, u poloviny postižených byl zjištěn cervikobrachiální syndrom.
The determination of at least moderate severity of affection and fulfilling working conditions given in the list of occupational diseases are decisive for accepting an occupational damage to the nervus ulnaris in the elbow area. In a retrospectively evaluated set of 44 persons with an occupational lesion of the nervus ulnaris an EMG criterion of decreased conduction velocity through the nervus ulnaris fibres around the elbow below 40 m/s has been considered as disturbance of moderate severity, the velocity of 50 m/s was thought the lower limit of normal values. In 20 patients, the disease was accepted after the average job duration of 15.8 years as a disorder due to overloading upper extremities, female electricians were affected most often. In 24 subjects the disease was accepted after the average of 23.9 years of work with vibrating tools, metal cutters suffered from the disorder most often. Frequent coincidence of an occupational lesion of the nervus ulnaris in the elbow area with other disorders of the locomotor apparatus and nerves of upper limbs has been stated. The carpal tunnel syndrome was diagnosed simultaneously in 70.4 % of the set, cervicobrachial syndrome was revealed in one-half of the affected.
The aim of the study was to determine the prevalence of risk factors for cardiovascular diseases among physicians at a teaching hospital. In total, 203 men and 167 women were included in the study. The participants filled in a questionnaire; their height, weight, blood pressure, serum cholesterol and glucose levels were added. 19.2 % males and 13.8 % females were smokers, hypertension was diagnosed in 10 % of males and in 6.6 % of females, 52.2 % males and 17.4 % females were overweight, 37 % males and 43.1 % females had hypercholesterolemia. The above findings suggest that Czech physicians have more favourable values of all the studied cardiovascular diseases risk factors than the general Czech population. However, Czech physicians smoke more than those in other countries and their level of cardiovascular diseases risk factors is unsatisfactory and calls for further intensive prevention. Preliminary outcomes of the study repeated after two years show no positive trends as well as physicians' low willingness to actively participate in lowering cardiovascular diseases risk factors.

Stanovit riziko kardiovaskulárních onemocnění (KVO) v rámci preventivních prohlídek rizikových zaměstnanců je jednoduché a lze je doporučit k širšímu použití. V rámci projektu bylo vyšetřeno 2059 zaměstnanců (329 mužů a 1730 žen) průměrného věku 36,1 roku (36,9 a 36,0). Riziko KVO 10-20 % mělo 139 zaměstnanců (6,8 %) (47 mužů a 92 žen) a riziko 20-40 % 20 zaměstnanců (0,9 %) (17 mužů a 3 ženy). Riziko v 60 letech věku 10-20%mělo 715 zaměstnanců (34,7 %) (198 mužů a 517 žen) a riziko 20-40 % 101 zaměstnanců (4,9 %) (69 mužů a 32 žen). Ke stanovení rizika lze použít jednoduchý počítačový program, který je dostupný na našich stránkách www.vasezdravesrdce.cz. V rámci prevence KVO u rodinných příslušníků pacientů s KVO onemocněním bylo vyšetřeno 55 rodinných příslušníků (19 mužů a 36 žen) průměrného věku 42 roky (42,8 a 41,6). U 7 příbuzných (7 mužů) průměrného věku 56,8 roku bylo zjištěno KVO riziko nad 20 % v 60 letech. Je nutné dále pokračovat v aktivním vyhledávání rizikových pacientů. K provádění prevence KVO je nutno spojit obě dostupné strategie, jak lékařskou, tak i populační.
It is simple to determine the risk of cardiovascular diseases (CVD) within the framework of preventive examinations of employees under risk, and it can be recommended for broader application. Within the framework of the project there have been examined 2059 employees (329 males, 1730 females), their mean age being 36.1 years (36.9 and 36.0, respectively). A 10-20% risk of CVD was found in 139 (6.8%) employees (47 males, 92 females) and a 20-40% risk in 20 (0.9%) employees (17 males, 3 females). The risk at 60 years of age was 10-20% in 715 (34.7%) employees (198 males, 517 females) and 20-40% in 101 (4.9%) employees (69 males, 32 females). For establishing the risk there can be applied a simple computer program which is available on our web site www.vasezdravesrdce.cz.Within the framework of CVD prevention in family members of patients suffering CVD there have been examined 55 family members (19 males and 36 females),mean age 42.0 years (42.8 and 41.6, respectively). There has been found an over 20% risk of CVD at 60 years of age in 7 relatives (7 males), mean age 56.8 years. It is necessary to continue in searching for patients under risk. For conducting CVD prevention both available strategies, the medical as well as the population one, should be combined.
