PURPOSE: Cubital tunnel syndrome is a well-described entity with many reported etiologies and anatomical compression sites. Accessory ossicles of either traumatic or congenital origin might occur around the elbow joint. Only one case reporting such ossicles compressing the ulnar nerve exists in previous literature. We aim to present this entity with a detailed description of the patient history and treatment. CASE REPORT: We report a case of 30-year-old female presenting with classical signs of cubital tunnel syndrome-positive Wartenberg's and Froment's signs, hypoesthesia in the fourth and fifth finger with decreased finger duction strength but without gross hypotrophy of interosseous and hypothenar muscles. Tinel's sign was positive over the ulnar sulcus and an accessory ossicle was found on the elbow radiograph within the ulnar sulcus. The first signs of calcification in this patient were reported 6 years prior in a follow-up after the dislocation of her elbow joint following a bike accident. The EMG confirmed ulnar nerve neuropathy in the elbow area. The ossicle was extirpated, the ulnar nerve was decompressed in the ulnar sulcus in a standard manner and the symptoms quickly resolved. The patient has been regularly visiting our outpatient clinic for the next 12 years without any complaints considering her elbow and the ulnar nerve. CONCLUSION: This is a rare case of cubital tunnel syndrome caused by an accessory ossicle of traumatic origin. Simple bone extirpation with ulnar nerve release followed by anterior subcutaneous transposition is the recommended method of treatment. No report of congenital accessory bones causing ulnar nerve compression in the elbow exists in the literature.
Práce se zabývá profesionálními periferními neuropatiemi typu úžinových syndromů na horních končetinách a posuzováním invalidity. Popisuje dosavadní poznatky o syndromu karpálního a kubitálního tunelu, jejich etiologii, klinický obraz, vyšetřovací metody a léčbu. Jsou též uvedena statistická data o počtu uznaných nemocí z povolání, o invaliditách a kazuistiky.
The work deals with professional peripheral neuropathies such as strait syndromes in the upper limbs and disability assessment. It describes the current knowledge about the carpal and cubital tunnel syndrome, their etiology, clinical picture, examination methods and treatment. Statistical data on the number of recognized occupational diseases, disabilities and case reports are also provided.

Cíl: Cílem práce bylo analyzovat výsledky endoskopicky asistované dekomprese n. ulnaris v loketní krajině u většího souboru pacientů se syndromem ulnárního sulku. Materiál a metodika: Byla provedena analýza 204 pacientů s diagnózou syndromu ulnárního sulku, u kterých jsme provedli 213 endoskopicky asistovaných dekompresí n. ulnaris v loketní krajině v období od května 2007 do prosince 2016. Soubor byl nejprve sledován prospektivně po dobu 3 měsíců (70 % pacientů), další část výsledků byla získána retrospektivně s delším odstupem od operace na základě námi vytvořeného dotazníku vyplněného 55 % pacientů. Výsledky: Ze 149 prospektivně sledovaných případů (141 pacientů) bylo popsáno zlepšení či dokonce výrazné zlepšení u 90 % z nich. Jako stacionární hodnotilo obtíže 9 % a zhoršení popsalo 1 % operovaných. Celkem 137 pacientů bylo po výkonu vyšetřeno elektromyograficky. U 89 % z nich byl pooperační elektrofyziologický nález hodnocen jako zlepšený, u 10 % jako stacionární a u 1 % (n = 2) došlo ke zhoršení. Obě tato zhoršení byla přechodná a při dalším elektromyografickém vyšetření s odstupem 10, resp. 15 měsíců od operace bylo prokázáno zlepšení bez nutnosti operační revize, a to i vzhledem ke zlepšenému klinickému stavu. Vyplněný dotazník byl získán ve 121 případech (115 pacientů) s celkovým follow-up 14–129 měsíců, v průměru 60 měsíců. Celkové zlepšení operované ruky popsalo 92 % respondentů, spokojenost s účinkem a celkovým průběhem výkonu a rekonvalescence vyjádřilo 89,5 %. Nebyl prokázán statisticky signifikantní vztah mezi předoperační tíží nálezu (skóre dle McGowana či elektrofyziologický nález) a pooperační spokojeností pacienta. Byl naopak jasně prokázán vztah mezi spokojeností pacienta po operaci a subjektivním návratem citlivosti nebo zlepšením síly úchopu. Závěr: Náš soubor pacientů potvrzuje, že endoskopicky asistovaná dekomprese n. ulnaris v loketní krajině představuje bezpečnou a spolehlivou alternativu v chirurgické léčbě syndromu ulnárního sulku. Při správné indikaci vede tento výkon u naprosté většiny pacientů, vč. těch s pokročilou lézí nervu před operací, k dlouhodobému zlepšení obtíží spojených s tímto úžinovým syndromem.
Aim: The aim of the study was to analyse results of endoscopically assisted decompression of the ulnar nerve in the cubital tunnel in a substantial number of patients with cubital tunnel syndrome. Materials and methods: We analysed a set of 204 patients with the diagnosis of cubital tunnel syndrome, in whom we performed 213 endoscopically assisted decompressions of the ulnar nerve in the cubital tunnel from May 2007 until December 2016. Firstly, the patients were followed for 3 months after surgery (70% of the patients) and secondly, we evaluated long-term results based on our own questionnaire completed by 55% of patients. Results: Out of 149 followed cases (141 patients), improvement or significant improvement was found in 90% of them. Nine percent of the patients described stationary symptoms and 1% felt worse. Postoperative electrophysiological examination was performed in 137 patients. Results were as follows: improvement in 89%, no change in 10%, and worsening in 1% (N = 2) of the cases. Both of these worsening’s were temporary and subsequent electromyography showed improvement after 10 and 15 months, respectively, without a need for revision surgery also due to the improved clinical condition. We obtained the completed survey from 115 patients (121 surgeries) with a follow-up of 14–129 months, 60 months on average. Overall improvement of the operated hand was expressed by 92% of patients, while 89.5% of them described a satisfactory effect of the procedure and its course. We failed to prove a statistically significant relationship between the severity of preoperative symptoms (McGowan score or electromyography finding) and the patient’s postoperative satisfaction. On the contrary, we found a strong relationship between the patient’s satisfaction after surgery and subjective return of hand sensitivity or improvement of grip strength. Conclusion: Our study proves that endoscopically assisted decompression of the ulnar nerve in cubital tunnel represents a safe and reliable alternative to other surgical procedures of ulnar nerve decompression. With the correct indication provided, the procedure leads to long-term improvement of the symptoms of cubital tunnel syndrome in vast majority of patients, including patients with a severe nerve lesion.
This case presents the utility of ultrasound examination in diagnostics, providing accurate therapy and follow-up of entrapment syndrome of the ulnar nerve, caused by heterotopic ossification. The heterotopic ossifications were in this case presumably linked to a long-term working with a vibration sander.

Některé případy poškození periferních nervů z přetěžování či vibrací mohou být za určitých okolností uznány jako nemoc z povolání (NZP). Rozhodnutí o uznání či neuznání NZP je v kompetenci věcně i místně příslušného "Střediska nemocí z povolán"“. V procesu uznání NZP připadá významné místo neurologickému konziliáři. V průběhu minulých 20 let tvořila onemocnění neurologického charakteru 29,6 % všech NZP. Přitom profesionální poškození z přetěžování či vibrací představovala 98,5 %. V prvé řadě se jedná o syndrom karpálního tunelu, méně často o lézi n. ulnaris (NU) v lokti a výjimečně o lézi NU v oblasti ruky – Guyonova kanálu. Na vzniku profesionálních poškození periferních nervů z fyzikálních příčin se podílí mechanický tlak, přetížení ruky, přenos vibrací na horní končetinu, chlad, nevýhodná poloha při práci a další faktory. Pro uznání profesionálního původu je nezbytné zjistit přítomnost odpovídajících klinických příznaků a nález na EMG vyšetření odpovídající nejméně středně těžké poruše. V práci jsou uvedeny anamnestické, klinické i EMG charakteristiky, včetně definice elektrofyziologických parametrů středního stupně postižení pro n. medianus i NU.
Some lesions of the peripheral nerves caused by physical factors can, under certain conditions, be acknowledged as an occupational disease. The decision whether it is an occupational disease or not is authorized by the "Centre of occupational diseases". Neurological consultants have a very important role in the process of acknowledgement of an occupational disease. During the last 20 years, disorders of neurological character represented 29.6% of all occupational diseases. At the same time, the lesions of the peripheral nerves due to overload or vibration constituted 98.5%. Carpal tunnel syndrome is most frequent, followed less often by an ulnar nerve lesion in the elbow, and rarely by an ulnar nerve lesion in the wrist and hand - the Guyon‘s channel. In the development of occupational compressive neuropathies, various mechanisms take place: mechanical pressure, hand overload, transmission of vibration on the upper extremity, cold, inappropriate position during work, and other factors. For acknowledgement of occupational origin of such a lesion, it is essential to determine clinical signs and symptoms, and EMG examination corresponding to at least the moderately severe lesion. Anamnestic, clinical, and EMG characteristics including electrophysiological parameters of a moderately severe lesion for the median and ulnar nerves were defined.

Cíl: Pro účely jednotného posuzování profesionální léze n. ulnaris v oblasti lokte (ulnar nerve entrapment; UNE) byla v roce 2011 vypracována metodika hodnocení za pomoci elektrofyziologických kritérií. Cílem práce bylo zjistit další vývoj vybraných EMG parametrů u UNE uznaných podle této metodiky jako nemoc z povolání v letech 2012- 016. Soubor a metodika: Byly zhodnoceny kontrolní EMG nálezy 55 osob (49 mužů) po 1 až 5 letech od uznání nemoci z povolání. Počet končetin postižených UNE činil 81. Hodnoceny byly změna rychlosti motorického vedení n. ulnaris přes loket (motor nerve conduction velocity of the ulnar nerve; MNCV-U) a změna amplitudy sumačního akčního potenciálu nad musculus abductor digiti minimi (compound muscle action potential over the abductor digiti minimi; CMAP ADM) oproti vstupnímu EMG vyšetření. Výsledky: V parametru MNCV-U bylo 54 % případů beze změny, 16 % bylo zhoršeno a u 30 % případů došlo ke zlepšení. Průměrná hodnota MNCV-U mezi vyšetřeními statisticky významně vzrostla. Amplituda CMAP ADM byla beze změny v 89 % případů, u 4 % postižení jevila zhoršení a pouze u 7 % případů nastalo zlepšení. Průměrná hodnota tohoto parametru se při kontrolním vyšetření statisticky významně nezměnila. Ani v jednom případě nedošlo k úplné normalizaci EMG nálezu. Závěr: Profesionální UNE je chronické onemocnění, jehož prognóza není ani po vyřazení z pracovního rizika uspokojivá.
Aim: For uniform assessment of occupational ulnar nerve entrapment at elbow (UNE) assessment methods using electrophysiological criteria were introduced in 2011. The study aimed at determining changes in selected EMG parameters in UNE recognized as occupational disease using the methods in 2012-2016. Patients and methods: In 55 participants (49 males), follow-up EMG at 1 to 5 years from recognition as occupational disease was assessed. Eighty-one arms were affected by UNE. Changes in the amplitude of motor nerve conduction velocity of the ulnar nerve across the elbow (MNCV-U) and in compound muscle action potential over the abductor digiti minimi (CMAP ADM) were compared with initial EMG. Results: The MNCV-U parameter remained unchanged in 54% of cases, deteriorated in 16% and improved in 30%. The mean MNCV-U increased significantly between the measurements. The CMAP ADM amplitude was unchanged in 89% of cases, worse in 4% and better only in 7%. Its mean value at follow-up was not statistically significantly different. There was no case of completely normalized EMG. Conclusion: Occupational UNE is a chronic condition with an unsatisfactory prognosis even after the occupational risk is removed.

BACKGROUND: Prospective randomized data is currently lacking which compares endoscopically assisted surgery with open surgical techniques in the treatment of cubital tunnel syndrome (CUTS). The aim of this study is to compare patient outcome in both techniques. METHOD: This prospective study comprised of 45 patients who, between October 2014 and February 2017, were randomly assigned to undergo either endoscopic or open surgery (22 and 23 patients respectively) for decompression of the ulnar nerve. Patients were followed up at 3 and 12 months postoperation. McGowan classification was used to determine the severity of symptoms. Surgical outcome was evaluated by Bishop classification. Pain levels were monitored according to gender from 0 to 10 days postoperation. Other factors investigated were chronic scar pain, working status, operation duration, and patient satisfaction regarding postoperative scarring and the procedure itself. RESULTS: Both methods are equally effective in the treatment of CUTS (Bishop score excellent or good 90% vs 96%). Postoperative pain is significant particularly in the first few days following surgery, but with no significant difference depending on procedure. In the open group, postoperative pain was significantly higher in women than in men; pain did not differ between the sexes in the endoscopic group. The tendency to lower levels of pain among endoscopically operated women in comparison with women in the open group was not statistically notable. Patients who underwent open decompression experienced notably higher levels of postoperative chronic scar pain. Although working status and satisfaction with the surgical outcome were the same in both groups, satisfaction with scarring was higher in the endoscopy group. Operation time was significantly longer by endoscopy. CONCLUSIONS: Both studied methods produced equal satisfactory outcomes in the treatment of CUTS. Endoscopy has the potential to minimize chronic scar pain and improve scarring esthetics, at the expense of longer operating time.
