Úvod: Šance úmrtí pacientů s akutními koronárními syndromy (AKS) v non-PCI (perkutánní koronární inter- vence) nemocnicích v České republice se mění v závislosti na celé řadě faktorů (věk, srdeční frekvence, systolický krevní tlak, kreatinin, Killipova třída, diagnóza, počet podaných doporučovaných léků a léčba ACE-inhibitory nebo sartanem). Cíle: Prezentujeme podrobný popis víceúrovňové logis- tické regrese aplikované při odvozování závěru popsaného v Úvodu, a to konkrétně srovnáváme víceúrovňovou logistickou regresi s logistickou regresí. Metody: Zmíněné klinické závěry byly odvozeny na základě dat z tříletého (7/2008-6/2011) registru akutních koronárních syndromů ALERT-CZ (Acute coronary syn- dromes – Longitudinal Evaluation of Real-life Treatment in non-PCI hospitals in the Czech Republic). Do registru přispívalo celkem 32 nemocnic. Počet pacientů s AKS (n=6013) v nemocnicích kolísal od 15 do 827. Výsledky: Test poměrem věrohodností ukázal, že ne- závislost léčebných výsledků napříč nemocnicemi nelze předpokládat (p<0,001, koeficient rozkladu rozptylu VPC=8,9%). Z tohoto důvodu jsme ke statistickému zhod- nocení dat vybrali víceúrovňovou logistickou regresi, a to konkrétně logistickou regresi se smíšenými efekty (iden- tita nemocnice byla náhodným efektem). Kalibrační vlast- nosti modelu byly velmi dobré (Hosmerův-Lemeshowův test, p=0,989). Celková diskriminační schopnost modelu byla 91,8 %. Závěry: Po zjištění určitých rozdílů mezi nemocnicemi bylo vhodné zohlednit příslušnost pacientů k různým nemocni- cím a použít víceúrovňovou logistickou regresi místo logis- tické regrese.

Methods: The above mentioned clinical findings have been derived on the basis of data from the three-year (7/2008-6/2011) registry of acute coronary syndromes ALERT-CZ (Acute coronary syndromes – Longitudinal Evaluation of Real-life Treatment in non-PCI hospitals in the Czech Republic). A total of 32 hospitals contributed into the registry. The number of patients with ACS (n=6013) in the hospi- tals varied from 15 to 827. Results: The likelihood ratio test showed that the in- dependence of medical outcomes across hospitals cannot be assumed (p<0.001, the variance partition coefficient VPC=8.9%). For this reason, we chose multilevel logis- tic regression to analyse data, specifically logistic mixed regression (the hospital identity was a random effect). The calibration properties of this model were very good (Hosmer-Lemeshow test, p=0.989). The total discriminant ability of the model was 91.8%. Conclusions: Considering some differences among hospi- tals, it was appropriate to take into account patient affi- liation to various hospitals and to use multilevel logistic regression instead of logistic regression.

Cardiology (MDMC) has been prepared by clinicians for clinical study in 2002. This model has been successfully implemented in an application for clinical data gathering. HL7 v3 Domain Analysis Model: Cardiology, Release 2 (HL7 DAM) has been published in HL7 September 2011 Ballot. Objectives: The objective of this paper is to compare these two data models. The main motivations for the comparison are nearly identical ways of development, and the same format of both specifications. Methods: HL7 DAM is much broader than EuroMISE MDMC. Thus I focus only on data elements present in MDMC but absent in HL7 DAM. Also different scales of elements present in both models are compared. Results: I have found 25 elements out of 181 elements defined in MDMC which are not contained in HL7 DAM. Conclusions: Results will be used for further discussion in HL7 Clinical Interoperability Council work group.
Cíl: Vytvořit podporu pro vkládání strukturovaných dat do elektronické zdravotnické dokumentace u poruch temporomandibulárního kloubu. Metody: Jsou popsány metody stukturování dat ve stomatologii a je představena interaktivní DentCross komponenta. Na několika případech je demonstrováno hlasem ovládané vkládání strukturovaných dat do zdravotnické dokumentace ve stomatologii. Dále je vysvětleno propojení této komponenty s elektronickým zdravotním záznamem MUDRLite. Výsledky: Představujeme použití aplikace DentVoice složené z elektronického zdravotnického záznamu MUDRLite a hlasem kontrolované interaktivní komponenty DentCross ke sběru stomatologických informací nutných pro léčbu poruch temporomandibulárního kloubu. Závěry: Aplikace DentVoice s komponentou DentCross prokázala praktické využití pro podporu léčení temporomandibulárních poruch.
Objectives: To identify support of structured data entry for an electronic health record application in temporomandibular joint disorders. Methods: The methods of structuring information in dentistry are described and the interactive DentCross component is introduced. A system of structured voice-supported data entry in electronic health record on several real cases in the field of dentistry is performed. The connection of this component to the MUDRLite electronic health record is described. Results: The use of DentVoice, an application which consists of the electronic health record MUDRLite and the voice-controlled interactive component DentCross, to collect dental information required by temporomandibular joint disorders is shown. Conclusions: The DentVoice application with the DentCross component showed the practical ability of the temporomandibular joint disorder treatment support.

Ciele: Výmena dát v prostredí českého zdravotníctva je väčšinou založená na národných štandardoch. Tento článok popisuje využitie medzinárodných štandardov a nomenklatúr na skonštruovanie pilotnej platformy sémantickej interoperability (PSI), ktorá by slúžila na výmenu informácií medzi systémami elektronických zdravotných záznamov (EZZ) v českom zdravotníctve. Táto práca bola zastrešená národným výskumným projektom programu „Informačná spoločnosť“. Metódy: Na začiatku projektu boli formulované požiadavky, ktoré by mala PSI splňovať. Bolo analyzovaných niekoľko komunikačných štandardov (openEHR, HL7 v3, DICOM) a HL7 v3 bol zvolený pre naše riešenie na výmenu medicínskych záznamov. Do pilotného prostredia boli zahrnuté dva systémy: WinMedicalc 2000 a EZZ ADAMEKj. Výsledky: Na popis informačného obsahu oboch systémov boli vytvorené Lokálne Informačné Modely (LIM) založené na HL7. Koncepty z našich pôvodných informačným modelov boli namapované na kódovacie systémy podporované HL7 (LOINC, SNOMED CT and ICD-10) a výmena dát pomocou správ HL7 v3 bola naimplementovaná a otestovaná pomocou dotazov na pacientove administratívne údaje. Ako brána medzi lokálnymi systémami EZZ (sEZZ) a infraštruktúrou založenou na správach HL7 slúžil nami vyvinutý konfigurovateľný HL7 broker. Záver: Celonárodná implementácia PSI v plnej veľkosti založená na HL7 v3 by zahrňovala prijatie a preklad príslušných medzinárodných kódovacích systémov a nomenklatúr, vytvorenie implementačných postupov uľahčujúcich migráciu od národných štandardov po tie medzinárodné. Naša pilotná štúdia ukázala, že náš prístup je uskutočniteľný, ale úplná integrácia systému českého zdravotníctva do európskeho kontextu e-zdravia bude vyžadovať ešte veľké úsilie.
Objectives: The data interchange in the Czech healthcare environment is mostly based on national standards. This paper describes a utilization of international standards and nomenclatures for building a pilot semantic interoperability platform (SIP) that would serve to exchange information among electronic health record systems (EHR-Ss) in Czech healthcare. The work was performed by the national research project of the „Information Society" program. Methods: At the beginning of the project a set of requirements the SIP should meet was formulated. Several communication standards (openEHR, HL7 v3, DICOM) were analyzed and HL7 v3 was selected to exchange health records in our solution. Two systems were included in our pilot environment: WinMedicalc 2000 and ADAMEKj EHR. Results: HL7-based local information models were created to describe the information content of both systems. The concepts from our original information models were mapped to coding systems supported by HL7 (LOINC, SNOMED CT and ICD-10) and the data exchange via HL7 v3 messages was implemented and tested by querying patient administration data. As a gateway between local EHR systems and the HL7 message-based infrastructure, a configurable HL7 Broker was developed. Conclusions: A nationwide implementation of a full-scale SIP based on HL7 v3 would include adopting and translating appropriate international coding systems and nomenclatures, and developing implementation guidelines facilitating the migration from national standards to international ones. Our pilot study showed that our approach is feasible but it would demand a huge effort to fully integrate the Czech healthcare system into the European e-health context.

OBJECTIVES: To identify support of structured data entry for electronic health record application in forensic dentistry. METHODS: The methods of structuring information in dentistry are described and validation of structured data entry in electronic health records for forensic dentistry is performed on several real cases with the interactive DentCross component. The connection of this component to MUDR and MUDRLite electronic health records is described. RESULTS: The use of the electronic health record MUDRLite and the interactive DentCross component to collect dental information required by standardized Disaster Victim Identification Form by Interpol for possible victim identification is shown. CONCLUSIONS: The analysis of structured data entry for dentistry using the DentCross component connected to an electronic health record showed the practical ability of the DentCross component to deliver a real service to dental care and the ability to support the identification of a person in forensic dentistry.
