Onemocnění covid-19 je spojováno nejen s příznaky akutní fáze, ale u části pacientů některé příznaky přetrvávají i po jejím překonání, mluvíme o tzv. post-covid syndromu. Cílem naší práce bylo identifikovat faktory ovlivňující míru úzkosti u pacientů s tímto syndromem. Celkově byla získána data od 404 pacientů, míra úzkosti byla hodnocena pomocí Beckova inventáře úzkosti (BAI). Soubor obsahoval 143 mužů a 261 žen, ve věkovém rozmezí 19-90 let. Průměrná hodnota BAI byla 13,2 bodu. Nejčastěji pacienti prodělali lehký průběh infekce covid-19. Dle našich výsledků nemá tíže samotné infekce covid-19 a přítomnost komorbidního somatického onemocnění vliv na hodnotu BAI; co se naopak ukázalo být významným faktorem, je hospitalizace. Hospitalizovaní pacienti měli statisticky významně nižší míru úzkosti než ti, kteří toto onemocnění prodělali v domácích podmínkách. Pacienti s patologickou mírou úzkosti byli signifikantně mladší a při srovnání pohlaví byly signifikantně více úzkostné ženy. Souvislost mezi přítomností plicních post-zánětlivých změn na RTG hrudníku a mírou úzkosti nebyla prokázána, byl ale zjištěn statisticky významný trend.
The COVID-19 is associated not only with symptoms of the acute phase, but in some patients some symptoms persist even after overcoming it, we are talking about the so-called post-COVID syndrome. The aim of our work was to identify factors influencing the level of anxiety in patients with this syndrome. In total, data were obtained from 404 patients, anxiety levels were assessed using the Beck Anxiety Inventory (BAI). The file contained 143 men and 261 women, in the age range of 19-90 years. The average BAI value was 13.2 Most often, patients experienced a mild course of covid-19 infection. According to our results, the severity of the COVID-19 infection itself and the presence of comorbid somatic disease have not effect on the BAI value, on the contrary, hospitalization turned out to be a significant factor. Hospitalized patients had a statistically significantly lower level of anxiety than those who experienced this disease at home. Patients with a pathological level of anxiety were significantly younger and, when comparing gender, there were significantly more anxiety in women. The connection between the presence of pulmonary post-inflammatory changes on the chest X-ray and the degree of anxiety was not proven, but a statistically significant trend was found.

BACKGROUND: Both genetic and environmental factors are important in etiology of mental disorders. Calculating polyenviromic risk/protective scores provides an updated perspective in research on the environmental causes of psychiatric disorders. We aimed to compare environmental risk and protective factors in patients with psychosis or a mood disorder (PSYCH+MOOD) and those with an anxiety disorder (ANX). METHODS: We administered the internationally accepted questionnaire from the EUropean Network of National Schizophrenia Networks Studying Gene-Environment Interactions (EU-GEI) study, enriched with mood and anxiety disorder-relevant measures, to patients at two large university hospitals in the Czech Republic. RESULTS: Ninety-four PSYCH+MOOD patients (average age 42.5 years; 46 males) and 52 ANX patients (average age 47.2 years; 17 males) participated. Neither polyenviromic risk score nor polyenviromic protective score differed significantly between PSYCH+MOOD and ANX groups (p = 0.149; p = 0.466, respectively). CONCLUSION: Scientific validity of the polyenviromic risk/protective score construct must still be demonstrated in large psychiatric samples, ideally in prospective studies. Nevertheless, researchers have already started to investigate environmental factors in the etiology of mental disorders in their complexity, similarly to polygenic risk scores.

Schizofrenie je závažná dlouhodobá psychotická porucha vyznačující se pozitivními i negativními symptomy, závažnými poruchami chování a deficitem kognitivních funkcí. Příčina této poruchy není zcela jasná, ale předpokládá se, že je multifaktoriální a zahrnuje jak dědičné, tak vnější faktory. Vzhledem k tomu, že lidský mozek reguluje veškeré chování, zaměřily se studie schizofrenie na identifikaci změn v neurobiologii a biochemii mozku. Mozek je orgánem nejvíce bohatým na lipidy (lipidy tvoří přibližně 50 % suché hmotnosti mozku). Výsledky četných studií prokázaly významný pokles polynenasycených mastných kyselin (PUFA), zejména kyseliny dokosahexaenové (DHA) v periferní krvi a také v mozku pacientů se schizofrenií v různých vývojových fázích poruchy. Nedostatek PUFA je spojován s psychotickými symptomy a kognitivními deficity u schizofrenie. Tato zjištění vedla k řadě klinických studií zkoumajících, zda by suplementace PUFA ve stravě mohla zlepšit průběh onemocnění u pacientů se schizofrenií. Výsledky jsou nekonzistentní, což lze přičíst zejména heterogenitě sledovaných pacientů. Zda se však, že pacienti s nízkými hladinami PUFA premorbidní, prodromálně nebo v časných stadiích schizofrenie by mohli mít z léčby PUFA prospěch. Každopádně je potřeba více kontrolovaných klinických studií.
Schizophrenia is a serious long-term psychotic disorder marked by positive and negative symptoms, severe behavioral problems and cognitive function deficits. The cause of this disorder is not completely clear, but is suggested to be multifactorial, involving both inherited and environmental factors. Since human brain regulates all behaviour, studies have focused on identifying changes in neurobiology and biochemistry of brain in schizophrenia. Brain is the most lipid rich organ (approximately 50 % of brain dry weight). Results from numerous studies have shown significant decreases of PUFAs, in particular, DHA as well as brain of schizophrenia patients at different developmental phases of the disorder. PUFA deficiency has been associated to psychotic symptoms and cognitive deficits in schizophrenia. These findings have led to a number of clinical trials examining whether dietary PUFA supplementation could improve the course of illness in patients with schizophrenia. Results are inconsistent. Some report beneficial whereas others show not effective. The discrepancy can be attributed to the heterogeneity of patient population. In this review, results from recent experimental and clinical studies, which focus on illustrating the role of PUFAs in the development of schizophrenia were examined. It suggests that patients with low PUFA levels before onset of disease, prodromally, or in the early stages of schizophrenia might benefit from lipid treatments, but more controlled clinical trials are warranted.

The textbook consists of three sections. The first offers a general overview of factors important to the etiology of mental disorders, particularly the genetic, epigenetic, environmental, and microbiomatic elements. The next part examines over two dozen significant mental disorders and details specific factors in their etiology. Finally, suggestions are made for further research in and development of the field. While primarily intended for medical students studying in English, the textbook's clear and straightforward presentation should make it useful to specialists in the fields of psychiatry and neuroscience, as well.

Background: Psychotic symptoms in BPD are not uncommon, and they are diverse and phenomenologically similar to those in schizophrenia spectrum disorders. Despite their prevalence in BPD patients, knowledge about the characteristics and severity of hallucinations is limited, especially in modalities other than auditory. Aim: This review summarises the causes, phenomenology, severity, and treatment options of hallucinations and other psychotic symptoms in BPD. Methods: The PubMed database was used with the following key terms: "borderline personality disorder" and 'hallucinations' and "psychotic symptoms". Articles were selected between January 1990 and May 2021. The primary keyword search yielded a total of 545 papers, of which 102 articles met the inclusion criteria and were fully screened. Papers from the primary source reference lists were also screened, assessed for eligibility, and then added to the primary documents where appropriate (n = 143). After the relevance assessment, 102 papers were included in the review. We included adult and adolescent studies to gather more recent reviews on this topic. Results: Hallucinations are significantly prevalent in BPD, mainly auditory, similar to schizophrenia spectrum disorders. The relationship between hallucinations and depression, anxiety, suicidality, schizotypy, and loneliness in BPD has been discovered but requires more research. Studies for treatment options for hallucinations in BPD are lacking. Conclusion: Recognition of psychotic symptoms in patients with BPD as distinguished psychopathological phenomena instead of diminishing and overlooking them is essential in the clinical assessment and can be useful in predicting complications during treatment. More focused research in this area is needed.
