PURPOSE OF THE STUDY The study aimed to find out whether the higher rate of complications described in literature in the case of plate and intramedullary osteosynthesis of clavicle fractures is high enough to discredit one of these methods. MATERIAL AND METHODS In the period from July 2007 to March 2016, a total of 151 osteosyntheses of diaphyseal clavicle fractures were performed in 149 patients (106 men, 43 women). The plate as well as intramedullary techniques were used in this group of patients. The follow-up of 12 months was completed in 125 patients (91 men, 34 women). The age of patients ranged from 15 to 74 years. The postoperative rehabilitation with no load applied started immediately by elevating the arm up to 90 ° abduction for 3 weeks, which was followed by full range of motion exercises. The load was set based on the radiological finding. The number of complications, including the failure of osteosynthesis, was assessed. RESULTS Some kind of a whole range of complications (postoperative discomfort, infection, failure of osteosynthesis, non-union) occurred in 37 patients (29.6%), with osteosynthesis failing in six of them (4.8%), always within 6 months after the surgery. No later failure was reported. A statistically significant difference was observed only when comparing the patients discomfort for individual surgical techniques, with poorer results in case of intramedullary osteosynthesis. (p<0.001). DISCUSSION The dominance of "discomfort" in intramedullary fixation was caused by soft tissue irritation by the edge of material projecting thereto. Once it was removed, the results of both the methods in terms of the number of complications were comparable. In all the cases, either an incorrect indication of respective osteosynthesis techniques, or a technically poor surgical performance were identified as the likely causes of failure of the osteosynthesis. CONCLUSIONS The osteosynthesis always failed due to a wrong indication or technical errors in the execution of osteosynthesis. The intramedullary osteosynthesis is indicated in simple two-part fractures. The plate osteosynthesis can be applied to multiple fragment fractures. In preoperative planning, a suitable method shall be opted for based on the type of the fracture and the basic principles shall be adhered to during the surgery. Úvod: Ruptury bránice nejsou častými úrazy, mají však vzrůstající incidenci a lze říct, že jejich přítomnost bývá jistým indikátorem závažnosti poranění. Vyskytují se v naprosté většině u tupých poranění břicha a hrudníku a jsou součástí polytraumat či sdružených poranění. Kazuistika: V následujícím textu uvádíme tři kazuistiky námi léčených pacientů. U dvou byla ruptura bránice primárně ošetřena, u třetího pacienta byla diagnostikovaná ruptura bránice po 31 letech od úrazu obrazem ileózní náhlé příhody břišní. Diskuze: Ruptury bránice jsou poranění málo frekventní, často špatně diagnostikovatelná a i přehlédnuta, přesto ale svými důsledky mohou být zdrojem časných nebo pozdních komplikací. Nutné je na ruptury bránice myslet při tupých poranění břicha a hrudníku a u pádu z výše a cíleně po nich pátrat. Závěr: Včasná diagnóza ruptury bránice je rozhodující pro další management námi léčených pacientů.
Introduction: Ruptures of the diaphragm are not frequent injuries, but they have an increasing incidence and we can tell, that their presence is a certain indicator of the severity of the injury. Originate mostly from blunt abdominal and chest trauma, they are often part of polytrauma and multiple injuries. Case report: In the following text we present three case reports of patients treated by us. Two of the ruptures were treated immidiately after injury. The third patient was presented with rupture of the diaphragm 31 years after from the injury. Discussion: Diaphragm injuries are infrequent, often difficult to diagnose and even overlooked yet, but its consequences can be a source of early or late complications. It is necessary to think about the diaphragm injury in abdominal and chest trauma.

Biliární stenty představují široce užívanou alternativu léčby benigních biliárních obstrukcí, choledocholitiázy, biliárních fistul či leaku z ductus cysticus po nedostatečném ošetření jeho pahýlu cystiku během cholecystektomií. Zvláštní postavení zaujímají v paliativní léčbě nádorových striktur žlučových cest. Jako každá léčebná metoda má i tato své komplikace, nejčastěji obstrukce stentu s rozvojem ikteru, popřípadě cholangoitidy. Mezi méně časté komplikace patří proximální či distální migrace biliárních stentů. Autoři popisují formou kazuistiky jednu z možností komplikace vycestovalého biliárního stentu.
Biliary stents are widely used as an alternative treatment for benign biliary obstruction, choledocholithiasis, biliary fistulas or leak from the cystic duct after cholecystectomy. They occupy a special position in the palliative treatment of malignant biliary strictures. Like any other treatment method, this one also has its complications. Most often it is the development of stent obstruction and jaundice or cholangitis. Less common complications include proximal or distal biliary stent migration. The authors describe a case study of one of the possible complications of migrated biliary stent.

V souvislosti s nárůstem obliby posilování a snadné dostupnosti anabolických steroidů pozorujeme zvýšenou incidenci šlachových poranění v ne zcela typických lokalizacích. Kazuistika: Autor v práci popisuje léčení zraněného kulturisty pro rupturu šlachy velkého prsního svalu a v dalších časových obdobích také pro ruptury šlachy čtyřhlavého stehenního svalu a distálního úponu dvouhlavého pažního svalu. K ošetření ruptury šlachy m. pectoralis maior bylo zvoleno chirurgické řešení – reinzerce šlachy, přechodná imobilizace a postupná rehabilitace s dózovanou zátěží. Výsledkem byl plný návrat k běžným i sportovním aktivitám. Diskuse: Porovnává názory na úlohu steroidů při vzniku šlachových poranění a na způsob jejich ošetření. Na základě této zkušenosti autor, stejně jako řada dalších, dává přednost indikaci časného chirurgického léčení před postupem konzervativním.
Muscle building, gaining in popularity, and availability of anabolic steroids are connected with raised incidence of tendon injuries in uncommon locations. CASE REVIEW: The author describes a case of an injured body builder treated for the major pectoral muscle's tendon rupturing, and, later on, for the quadriceps tendon and distal bicipital tendon ruptures. The m. pectoralis major tendon rupture was managed surgically-including tendon reinsertion, temporary immobilization and gradual rehabilitation with dosed loading. The treatment resulted in full recovery and the patient was able to resume common and sport activities. DISCUSSION: The study assessed the role of steroids in development of tendon injuries, as well as their treatment methods. Based on his experience, the author, as well as other authors, favours indication for early surgical management to conservative treatment.
