Terapie srdečního selhání je komplexní zahrnuje jak režimová opatření, tak farmakologické a nefarmakologické postupy. Moderní léčba vedla k prodloužení přežití nemocných a zlepšení kvality jejich života. I přesto zůstává prognóza nemocných se srdečním selháním závažná. Základem farmakologické léčby je inhibice patologicky zvýšené aktivity sympatoadrenergního systému (β-blokátory) a systému renin-angiotenzin-aldosteron (inhibitory angiotenzin konvertujícího enzymu/blokátory receptorů AT1 pro angiotenzin II a antagonisté mineralokortikoidních receptorů). Zcela novou lékovou skupinou v léčbě srdečního selhání s pozitivním přínosem na mortalitu jsou duální inhibitory angiotenzinových receptorů a neprilysinu (sakubitril/valsartan). Velkým příslibem v léčbě srdečního selhání jsou inhibitory SGLT2. Současná strategie léčby je založena na doporučeních Evropské kardiologické společnosti z roku 2016 a konsenzu expertů Asociace srdečního selhání Evropské kardiologické společnosti z roku 2019.
Therapy of heart failure is complex, it contains lifestyle changes, pharmacological and non-pharmacological approaches. Modern treatment reduces mortality and improves quality of life. Nevertheless, the prognosis of patients with heart failure remains severe. Pharmacological therapy is based on the inhibition of the pathologically enhanced activity of the sympatoadrenergic system (betablockers) and the renin-angiotensin-aldosterone system (angiotensin-converting enzyme inhibitors/AT1 receptor blockers for angiotensin II and mineralocorticoid receptor antagonists). Angiotensin receptor – neprilysin inhibitors (sacubitril/valsartan) are a new drug class in the treatment of heart failure with a positive mortality benefit. SGLT2 inhibitors are a great promise in the treatment of heart failure. The current treatment strategy is based on the recommendations of the 2016 European Society of Cardiology and the consensus of the European Heart Society Association of 2019 experts.

Akutní koronární syndrom jako akutní forma ischemické choroby srdeční je významnou příčinou morbidity a mortality pacientů. Jeho nejefektivnějším způsobem léčby je perkutánní koronární intervence (PCI) rutinně využívající angiografické zobrazení. V současnosti nejpřesnější zhodnocení morfologie koronárních tepen pak poskytuje optická koherentní tomografie (OCT), intravaskulární zobrazovací metoda. Hlavní využití OCT se v posledních letech posouvalo od vědecké báze k téměř rutinní praxi, a to především u pacientů s akutním koronárním syndromem. Optická koherentní tomografie je s několika výjimkami nepřekonatelnou zobrazovací metodou co se týče obrazového rozlišení a množství získaných informací. K dispozici jsou již první data naznačující profit pacientů z použití OCT v intervenční léčbě pacientů s akutním infarktem myokardu. Práce poskytuje přehled o využití OCT především u pacientů s AKS.
Acute coronary syndrome as an acute form of coronary artery disease is one of the most important causes of patient morbidity and mortality. Its most effective treatment method is percutaneous coronary intervention (PCI) routinely using angiographic imaging. Currently, the most accurate assessment of coronary artery morphology is provided by optical coherence tomography (OCT), an intravascular imaging technique. The main application of OCT has been shifting from mostly scientific use to almost routine practice in recent years, particularly in patients with acute coronary syndrome. Optical coherence tomography is, except for a few limitations, one of unbeatable imaging methods with respect to image resolution and the amount of information obtained. First data have been available suggesting a patient benefit from the use of OCT in interventional treatment of patients with acute myocardial infarction. The paper provides an overview of the use of OCT, especially in patients with acute coronary syndrome.

Biodegradabilní stenty (vstřebatelné stenty – bioresorbable vascular scaffolds, BVS) byly do klinické praxe uvedeny v roce 2011. Představují jednu z nejmodernějších, ale v současnosti také nejvíce kontroverzních technologií. Jsou vyrobeny z materiálů, k jejichž degradaci dochází po implantaci in vivo v řádu měsíců až let. Za hlavní pozitivum celého konceptu je považována prevence negativního ovlivnění cévní vazomotoriky stentovaného úseku. První generace BVS je však podle dostupných klinických dat spjata s vysokým rizikem pozdní trombózy ve stentu (ve srovnání s poslední generací potahovaných kovových stentů [DES]). Na základě těchto údajů je použití BVS v nedávných guidelines Evropské kardiologické společnosti (European Society of Cardiology, ESC) z roku 2018 doporučeno pouze v rámci klinických studií. Kazuistika demonstruje pacienta, který byl přibližně 12 měsíců po implantaci BVS přijat na naše pracoviště s akutním infarktem myokardu s elevacemi úseku ST (STEMI). Na jeho příkladu lze dobře ukázat několik příčin selhání konceptu BVS, mimo jiné nedodržení všech doporučení techniky implantace, které byly již v době implantace k dispozici.
Biodegradable vascular stents (or scaffolds [BVS]) were introduced into clinical use in 2011. These stents represent one of the most advanced technologies, however, the most controversial, too. The scaffolds are made from materials, that degrade over the period of months or years after the implantation. Prevention of cessation of the vessel vasomotoric is usually stressed out as the most positive aspect of the biodegradation. However, the first BVS generation presents with the high risk of the late and very late stent thrombosis (compared to last DES generation). Based on this data, the use of BVS is recommended only in clinical trials in recent ESC guidelines for myocardial revascularization. The case report describes patient, that was admitted to our cardiovascular center due to ST elevation myocardial infarction approximately 12 months after BVS implantation. The case demonstrated several reasons why the concept of BVS failed to prove its benefits, at least in the first generation.

AIMS: Transradial catheterization (TRC) is a dominant access site for coronary catheterization and percutaneous coronary interventions (PCI) in many centers. Previous studies reported higher intimal thickness of the radial artery (RA) wall in patients with a previous history of TRC. In this investigation the aim was to assess the intimal changes of RA using the optical coherence tomography (OCT) intravascular imaging in a serial manner. METHODS AND RESULTS: 100 patients with the diagnosis of non-ST-elevation myocardial infarction (nSTEMI) treated by PCI were enrolled (6 patients were excluded from this analysis because of occluded RA at follow-up [2 patients] and insufficient quality of OCT images [4 patients]). An 54mm long OCT run of the RA was performed immediately after the index PCI and repeated 9 months later. Volumetric analyses of the intimal layer and lumen changes were conducted. Median intimal volume at baseline versus 9 months was 33.9mm3 (19.0; 69.4) versus 39.0mm3 (21.7; 72.6) (p<0.001); and median arterial lumen volume was 356.3mm3 (227.8; 645.3) versus 304.7mm3 (186.1; 582.7) (p<0.001). There was no significant difference in the effect of any clinical factor on the RA volume changes. CONCLUSIONS: OCT volumetric analyses at baseline and 9 months showed a significant increase in the radial artery intimal layer volume and a decrease in lumen volume after transradial PCI. No significant factors affecting this process were identified.

BACKGROUND AND AIMS: Transradial catheterization is the predominant access site for coronary catheterization and percutaneous coronary interventions (PCI). Previous studies have reported a high incidence of radial artery (RA) injury. The aim of this investigation was to evaluate the incidence of RA injury using last generation optical coherence tomography (OCT) intravascular imaging in a serial manner. METHODS: 100 patients with a diagnosis of non-ST-elevation myocardial infarction (nSTEMI) treated by PCI were enrolled. OCT of RA was performed immediately after the index PCI. OCT was repeated 9 months later. RESULTS: There were 11 patients with RA injuries (11.0%) at baseline, including 3 patients with RA medial dissection and 8 patients with intimal tears. In the follow-up OCT data, the number of RA injuries was 10 (10.0%), including 7 patients with RA medial dissection and 3 patients with intimal tear. All injuries were clinically asymptomatic and there was no finding of vessel perforation. There was no significant difference between the baseline and follow-up procedure in terms of number of injuries. CONCLUSION: The study showed no significant difference between baseline and follow-up RA injury incidence. There was a higher risk of radial injury for repeated catheterization in women. The conclusion is that radial catheterization is a very safe procedure in terms of radial artery damage. This is evidenced by considerably fewer injuries compared to published studies. The use of the short radial sheath (7 cm in this study) is protective and reduces the incidence of radial injury.
