OBJECTIVE: Patients with schizophrenia are at higher risk of cardiovascular (CVS) related mortality. Close attention is being paid to the clinical utility of readily available CVS markers. METHODS: A pilot one-year longitudinal study in inpatients with first-episode psychosis (FEP) was carried out to determine markers of inflammation and endothelial dysfunction (monocyte- and neutrophil-to-lymphocyte ratios) and basal blood pressure, pulse, and derived hemodynamic parameters (PP: pulse pressure; RPP: rate pressure product; and MAP: mean arterial pressure). RESULTS: After one year, PP and RPP increased, as did systolic blood pressure and heart rate. Systolic blood pressure, PP, total white blood cells, and neutrophils correlated with weight gain. After one year, correlations between monocyte-to-lymphocyte ratio and RPP and MAP were observed. CONCLUSION: Our study indicates worsening CVS health over the first year of treatment and emphasises the importance of early monitoring of CVS status using easily accessible parameters to prevent CVS-related mortality.Key pointsPatients with schizophrenia are at higher risk of cardiovascular mortality.The CVS risk could be evaluated using affordable, routinely available CVS markers such as monocyte- and neutrophil-to-lymphocyte ratios, blood pressure, and pulse together with the derived parameters.Our pilot study in first-episode psychosis patients indicates worsening of CVS health based on these parameters during the first year of treatment, the early monitoring of CVS status is highly relevant in clinical practice.

Výzkum psychóz, zvláště schizofrenie, je v posledních letech zaměřen na období před jejich propuknutím a těsně po něm. Zajímavým tématem výzkumu se v poslední době stává vliv stresu a dysregulace osy hypotalamus-hypofýza-kůra nadledvin, které zřejmě hrají významnou roli v etiopatogenezi schizofrenie. Výzkum se zaměřuje především na roli kortizolu a přitom se dochází k rozdílným výsledkům. Většina studií však svědčí pro zvýšenou bazální hladinu kortizolu u pacientů s první epizodou psychózy či osob s velmi vysokým rizikem rozvoje psychózy, ale na druhou stranu dynamická kortizolová odpověď na stres je snížená. Další hormon - aldosteron, který byl dlouho považován za mineralokortikoid bez jakékoli funkce v CNS, může dle recentních poznatků rovněž ovlivnit psychické funkce. Existují studie, které prokazují souvislost koncentrací aldosteronu s klinickým stavem u depresivních poruch - s jejich závažností, průběhem a výsledkem. V pilotní studii s pacienty trpícími schizofrenií byly výsledky odlišné než v případě depresivní poruchy - koncentrace aldosteronu byly vyšší u pacientů s delším trváním schizofrenie a nižší závažností psychopatologie, stejně jako poměr aldosteron / kortizol. Vzhledem k chybějícím poznatkům jsou nutné další studie k osvětlení role osy hypotalamus-hypofýza-kůra nadledvin a také kortizolu a aldosteronu jako potencionálních biomarkerů u psychotických poruch, zvláště schizofrenie.
Research on psychoses, especially schizophrenia, has been focusing on the period before and just after its onset. An interesting topic of research has recently become the influence of stress and dysregulation of the hypothalamic-pituitary-adrenal axis, which seems to play an important role in the etiopathogenesis of schizophrenia. The researchers are focused mainly on the role of cortisol, but the results are different. However, most studies suggest an increased basal cortisol level in patients with first episode of psychosis or people with a very high risk of developing psychosis, but on the other hand, a dynamic cortisol response to stress is reduced. Another hormone, aldosterone, which has been considered as mineralocorticoid without any function in the brain, appears to affect also psychological functions. There are studies showing the association of aldosterone levels related to clinical status (severity, course and outcome) in depressive disorder. In schizophrenia, there is a pilot study that has a different association with the severity of psychopathology of schizophrenia than in the case of depressive disorder, namely aldosterone serum levels were higher in patients with longer duration of schizophrenia and lower severity of psychopathology, as well as aldosterone / cortisol ratio. Because of lack of knowledge, further studies are needed to clarify the role of hypothalamic-pituitary-adrenal axis, cortisol and aldosterone as potential biomarkers in psychotic disorders, especially schizophrenia.

Článek shrnuje základní údaje o diagnostice a léčbě Huntingtonovy nemoci. Zdůrazněna je zejména neuropsychiatrická podstata tohoto onemocnění, u něhož se kromě motorických příznaků vyskytují i nejrůznější psychiatrické příznaky a kognitivní deficit. To je ilustrováno i přiloženou kazuistikou.
The article summarizes basic data about diagnostics and treatment of Huntington's disease. It emphasizes the neuropsychiatricbasis of the disease with not only motor symptoms but also with various psychiatric symptoms and cognitive deficit. These factsare confirmed with one case study.
