Obezitu reprezentuje interval > 97 % v percentilovém grafu Body Mass Indexu Světové zdravotnické organizace diferencovaný dle pohlaví a věku adolescentů. Rozdíly v prevalenci obezity adolescentů ze všech SES kategorií rodin byly testovány pomocí χ2-testu. Míra vlivu korelátů obezity adolescentů v roce 2022 byla analyzována pomocí vícenásobné logistické regresní analýzy. Výsledky: Mezi lety 2018 a 2022 nebyl u dívek ani chlapců v žádné kategorii SES rodin zjištěn signifikantní přírůstek v prevalenci obezity.
Objective: The main objective of the study is to determine changes in the prevalence of obesity in a representative cohort of Czech adolescents between 2018 and 2022 and its current correlates with respect to the socioeconomic status (SES) of adolescents’ families. Methods: A nationally representative sample of 24535 adolescents aged 11–15 years (n = 11629/129062018/2022; boys: 50.4/50.6%2018/2022) was drawn from last two cycles of an online questionnaire survey conducted in 2018 and 2022. The SES of the families of the participating adolescents was categorised using the Family Affluence Scale. Obesity was represented by > 97% on the age-differentiated World Health Organization Percentile Body Mass Index charts differentiated by gender and age of adolescents. The chi-square tests (χ2) were performed to assess differences in prevalence of obesity in each gender and SES categories of adolescents between 2018 and 2022. Multiple logistic regression analysis was used to analyse the relationship between different correlates and obesity in adolescents in 2022 separately for boys and girls. Results: There was no significant increase in the prevalence of obesity among girls or boys in any SES family category between 2018 and 2022. Adolescents from low SES families had the significantly highest (p < 0.001) prevalence of obesity (11%BOYS and 5.8%GIRLS) compared to other SES categories of adolescents. Among adolescents from low SES families, individuals who engaged in daily moderate to vigorous physical activity (PA) (p < 0.005) or 3 times per week vigorous PA (p < 0.05) had significantly lower odds of obesity than their less active peers. Skipping breakfast significantly (p < 0.05) increased the odds of obesity, but only in adolescents from low SES families. Shorter screen time significantly reduced (p < 0.05) the odds of obesity in all SES categories of adolescents. Conclusions: Obesity, because of long-term positive energy balance mediated by imbalanced behaviour, is most pronounced in adolescents from low SES families. It was confirmed that significantly lower odds of obesity in adolescents from low SES families are associated with regular practice of recommended PA, shorter ST duration and not skipping regular breakfast.

Cíl: Hlavním cílem studie je pomocí objektivních přístrojů zhodnotit 24hodinové pohybové chování (tj. spánek, sedavé chování a pohybovou aktivitu) u českých rodin s dětmi ve věku 3–8 a identifikovat faktory snižující riziko výskytu nadměrné tělesné hmotnosti u těchto dětí. Metodika: Výzkumný soubor tvoří reprezentativní vzorek 425 rodin z městských a venkovských obcí z regionů Čech, Moravy a Slezska. Všichni členové rodiny dokončili sedmidenní monitorování 24hodinového pohybového chování pomocí akcelerometrů nošených na zápěstí. Antropometrické údaje spolu s potenciálními faktory souvisejícími s tělesnou hmotností byly zjišťovány prostřednictvím dotazníku a rodinného deníku vyplňovaného rodiči. Z-skóre body mass indexu a kategorizace podle percentilových grafů Světové zdravotnické organizace diferencovaný dle věku a pohlaví dítěte byly použity pro stanovení nadváhy a obezity. Faktory snižující míru výskytu nadměrné tělesné hmotnosti byly analyzovány pomocí binární logistické regresní analýzy. Výsledky: Děti s nadměrným sezením v nižší míře splňují doporučení pro spánek a pohybovou aktivitu a méně často společně snídají s rodiči než děti s kratší dobou denního sezení. Mezi výskytem nadměrné tělesné hmotnosti u 3–8letých dívek (19,2 %) a chlapců (18,7 %) nenacházíme statisticky významné odlišnosti, na rozdíl od rodičů, kde ve výskytu obezity (14,4 %OTCOVÉ a 9,5 %MATKY) i nadváhy (46,5 %OTCOVÉ a 20,1 %MATKY) otcové signifikantně (p < 0,05) převyšují matky. Výskyt obezity u rodičů významně (p < 0,05) zvyšuje šanci výskytu nadměrné tělesné hmotnosti u dětí, zatímco kratší doba sedavého chování dětí snižuje riziko výskytu jejich nadměrné tělesné hmotnosti. Závěry: Nadměrné sezení dítěte a obezita rodičů zvyšuje riziko jeho nadváhy a obezity, a to bez ohledu na socioekonomický status rodin nebo pohlaví dětí.
Objective: The main objective of the study is to use objective instruments to assess the 24-hour movement behaviour (i.e., sleep, sedentary behaviour and physical activity) of Czech families with 3–8-year-old children and to identify factors that reduce the risk of excess body weight in these children. Methods: The set consists of a representative sample of 425 families from urban and rural municipalities in the regions of Bohemia, Moravia and Silesia. All family members completed 7 days of 24-hour monitoring of movement behaviour using wrist-worn accelerometers. The anthropometric data along with potential weight-related factors were collected via questionnaire and a family completed by parents. Body mass index z-scores and categorization by World Health Organization percentile charts differentiated by age and gender of the child were used to determine overweight and obesity. Binary logistic regression analysis was used to analyse factors reducing the risk of prevalence of childhood excess body weight. Results: Children with excessive sitting were less likely to meet sleep and physical activity recommendations and were less likely to have breakfast with their parents than children with shorter daily sedentary time. There were no statistically significant differences between the prevalence of excess body weight in 3–8-year-old girls (19.2%) and boys (18.7%), contrary to parents, where fathers significantly (p < 0.05) outnumbered mothers in the prevalence of obesity (14.4%FATHER and 9.5%MOTHER) and overweight (46.5%FATHER and 20.1%MOTHER). The prevalence of obesity in parents significantly (p < 0.05) increased the risk of excess body weight in children, while shorter duration of sedentary behaviour significantly (p < 0.05) decreases the risk of excess body weight in children. Conclusions: A child’s excessive sitting and parental obesity increases their risk of being overweight and obese, regardless of the socioeconomic status of the families or the gender of the children.

Background: The influence of parents on physical activity (PA) and body weight of their children is confirmed by scientific studies. However, it is not known whether only children or those with siblings have a higher level of PA and healthier body weight. Objective: The aims of this study are (i) to assess whether there are any differences in the achievement of the daily step count (SC) recommendation and in the prevalence of overweight/obesity between only children and children from families with multiple children, and (ii) to investigate whether the achievement of the SC recommendation by children/parents and parental overweight/obesity is associated with childhood obesity. Methods: The analysis included 566 families (10.6%/89.4% with single child/two or more children) with complete data on weight status and PA monitored with a Yamax pedometer during regular school/work between 2013-2019. The daily SC recommendation amounted to ≥ 13,000/11,000 SC for 5-12-year-old sons/daughters and ≥ 10,000 SC for 12-16-year-old adolescents and adults. The chi-square test was used to compare the achievement of the daily SC recommendation and the prevalence of overweight/obesity. Binary logistic regression analyses were used to investigate whether the achievement of the daily SC recommendation by children/parents and parental overweight/obesity were associated with obesity in their offspring. Results: A significantly higher proportion of children with siblings achieved the recommended daily SC (51.8% vs. 31.7%, p = .003) and showed a lower prevalence of overweight (16.2% vs. 20.0%, p = .041) and obesity (7.1% vs. 20.0%, p < .001) compared with only children. The achievement of the daily SC recommendation in children significantly (p < .05) decreased the odds ratio of obesity (odds ratios 0.22-0.34, 95% confidence intervals 0.07-0.85). Conclusions: Children from families with siblings had higher physical activity and healthier body weight than those from single-child families. However, reaching the recommended daily SC reduces the likelihood of childhood obesity, regardless of the presence of a sibling in the family.
Cíl: Hlavním cílem studie je zjistit vztahy mezi nadváhou/obezitou dětí a objektivně monitorovanou pohybovou aktivitou (dále PA), aktivní účastí ve volnočasové organizované PA, zábavním „screen time“ (dále ST) a úrovní tělesné hmotností jejich rodičů u reprezentativního souboru 1101 českých rodin. Metodika: Analýzy zahrnovaly 1101 párů rodič-dítě (648 matka-dítě a 453 otec-dítě) s kompletními údaji o úrovni tělesné hmotnosti, zábavním ST, PA a účasti v organizované volnočasové PA monitorované pomocí krokoměru Yamax během běžného školního/pracovního týdne na jaře a podzim mezi lety 2013–2019. Logistická regresní analýza byla použita pro kvantifikování poměru šancí dětské nadváhy/obezity a indikátorů životního stylu rodičů (nadváha/obezita, ≥ 10 000 kroků za den, ≤ 2 hodiny zábavního ST denně, aktivní účast (≥ 1krát za týden) v organizované volnočasové PA). Výsledky: Matčina nadměrná tělesná hmotnost významně (p < 0,001) více než 2,5krát zvyšuje šanci nadváhy/obezity u svých potomků, zatímco aktivní účast otců v organizované volnočasové PA a dosažení 10 000 kroků za den významně (p < 0,05) snižuje šanci výskytu nadváhy/obezity u svých dětí. Závěry: Kumulativní efekt rodičovského aktivního zapojení do organizované volnočasové PA a vlastního dosažení 10 000 kroků za den je vzhledem k rodinným vazbám přirozeným prostředkem prevence vzniku nadváhy/obezity jejich potomků.
Objective: The main objective of this study was to identify the relationship between overweight/obesity in children and objectively monitored physical activity (PA), active participation in organized leisure-time PA, entertaining screen time (ST), and the level of body weight in their parents in a representative group of 1,101 Czech families. Methods: The analysis included 1,101 parent-child pairs (648 mother-child and 453 father-child) with complete body weight status data, entertainment ST, PA, and participation in organized leisure-time PA monitored using a Yamax pedometer during regular school/work week during spring and autumn between 2013 and 2019. Logistic regression analysis was used to quantify the odds ratio of childhood overweight/obesity and parents’ lifestyle indicators (overweight/obesity, ≥ 10,000 steps a day, ≤ 2 hours of entertainment ST daily, active participation (≥ 1 a week) in organized leisure-time PA). Results: Mothers’ excessive body weight significantly (more than 2.5 times p < 0.001) increased the odds ratio of overweight/obesity in their offspring, while active participation of fathers in organized leisure-time PA and achieving 10,000 steps a day significantly (p < 0.05) reduced the odds ratio of overweight/obesity in their children. Conclusions: The cumulative effect of parental active involvement in organized leisure-time PA and achieving 10,000 steps a day is a natural means of preventing overweight/obesity in their offspring due to family ties.

Objective: The main objective of the study is to analyze time trends in obesity of the Czech adolescents between 2002 and 2018 with regard to socioeconomic status (SES) of adolescent families and to find SES-mediated correlates of adolescents' obesity. Methods: A nationally representative sample of 29,879 adolescents (49.6% boys) aged 10.5–16.5 years was drawn from the Health Behaviour in Schoolaged Children cross-sectional, self-reported questionnaire surveys conducted in 2002, 2006, 2010, 2014, and 2018 in the Czech Republic. Using the Family Affluence Scale, the SES of respondents' families was calculated as low, medium and high. Obesity was represented by > 97% on the age-differentiated World Health Organization Percentile Body Mass Index charts. The chi-square tests were performed to assess differences in prevalence of obesity in each gender and SES categories of adolescents between 2002 and 2018. Multiple logistic regression analysis was used for examining the correlates of adolescents' obesity in 2018 data collection. Results: Across the quadrennial surveys 2002–2018, we observed a clear increase in prevalence of obesity in all SES categories of adolescents, which was most striking (p < 0.05) in boys (+7.5 percent points (p.p.): 4.5%2002 → 12.0%2018) and girls (+2.4 p.p.: 2.7%2002 → 5.1%2018) of low SES. Comparing all the survey cycles, the highest prevalence of obesity was evident in the low SES adolescents in 2018 both in girls and boys. The lower odds of obesity were significantly (p < 0.05) associated with regular vigorous physical activity, participation in organized sport, and daily consumption of sweets irrespective of SES of adolescent families. However, the lowest proportion of participants in organized sports and adolescents engaged in vigorous physical activity regularly is in the low SES category. Conclusions: An unreasonable increase in prevalence of obesity in adolescents with low SES suggests that national health- -related and sports programs have the least impact on adolescents from low SES families. The findings are highly relevant to policy makers involved in obesity prevention programs and highlight the need for more accurate targeting of interventions on children and adolescents with low SES background.

V rodinách s dětmi s nadváhou/obezitou ovšem významně zvyšuje šanci dětí dosáhnout denního doporučovaného PK v pracovních dnech pouze matčino dosažení doporučovaného denního PK (ORpracovní dny/víkendy = 3,58/12,48) a nenadměrný ST (p < 0,001).
Objective: The main objective of the study was to reveal which lifestyle-related indicators of families (step count (SC), screen time (ST), weight status and active participation in organized leisure-time physical activity (OPA)) help normal weight children and overweight/obese children achieve the daily SC recommendations. Methods: A nationally representative sample comprised 834 families including 1,564 parent-child dyads. The participants wore the Yamax pedometer for at least eight hours a day on at least four weekdays and both weekend days, and completed a family log book (anthropometric parameters, SC, and ST). Logistic regression analyses (OR) were used to investigate whether parents’ meeting the daily SC recommendation (10,000 SC/day), non-excessive ST (< 2 h/day), weight status, and participation in OPA were associated with children’s achievement of their daily SC (11,500 SC/day for pre-schoolers and 11,000/13,000 SC/day for school-aged girl/boys), separately for weekdays and weekends. Results: In families with normal weight children, parents’ SC was significantly associated with their offsprings’ odds to achieve the daily SC recommendation (mothers in the model: ORweekdays/weekends = 3.02/6.03, fathers in the model: ORweekends = 3.91, p < 0.001) regardless of their age category, or gender. In families with overweight/obese children, only the mother’s achievement of the SC recommendations (ORweekdays/weekends = 3.58/12.48) and non-excessive ST significantly (p < 0.001) increased the odds of their children for reaching the daily SC recommendation. Children’s participation in OPA increased the odds of all children achieving the daily SC recommendations on weekdays (OR = 2.02–3.16), however, for overweight/obese children this remained non-significant. Conclusions: The mother’s physical activity (achievement of at least 10,000 SC/day) is significantly associated with the achievement of recommended daily SC in overweight/obese preschool and school-aged children. Physical activity enhancement programmes for overweight/obese children cannot rely solely on the active participation of children in OPA, they also need to take other family-based physical activity, especially at weekends, into account.

Metodika: Pro průřezovou studii byly vybrány reprezentativní vzorky českých adolescentů (9382 dívek a 8868 chlapců ve věku 10,5–16,5 let) z mezinárodní studie „Health Behaviour in School-aged Children“ (dále HBSC) realizované v letech 2002, 2006, 2010 a 2014 pomocí standardizovaného dotazníku a v souladu s pravidly závazného výzkumného protokolu. V roce 2014 byla prevalence nadváhy/obezity u dívek i chlapců z rodin s nízkým SES signifikantně (p < 0,01) vyšší než u dívek a chlapců z rodin s vysokým SES. Bez ohledu na SES mají nižší šanci výskytu nadváhy/obezity dívky než chlapci (p < 0,001), starší než 11letí adolescenti (p < 0,001) a adolescenti z roku 2002 než z roku 2014 (p < 0,05). Avšak pouze u adolescentů z rodin s vysokým SES platí, že riziko výskytu jejich na nadváhy/obezity signifikantně snižuje alespoň 60 minut denně PA střední až vysoké intenzity (OR = 0,69; 95% CI = 0,56–0,86; p < 0,01) a méně než 2 hodiny denního ST (OR = 0,61; 95% CI = 0,44 až 0,84; p < 0,01). Závěry: Vysoké zastoupení nadváhy/obezity u adolescentů z rodin s nízkým SES a současně zjišťovaná nízká úroveň jejich PA představují znepokojivá fakta zdůrazňující nutnost tvorby a realizace intervenčních programu cílených na snížení výskytu nadváhy/obezity právě u této ekonomicky znevýhodněné části populace.
Objective: The main objective of the study was to describe trends in overweight and obesity in Czech adolescents in relation with socioeconomic status (SES) of families between 2002 and 2014 and find out how correlates of overweight/obesity (gender, age, physical activity (PA) and screen time (ST)) vary among adolescents from families with a different SES. Methods: For the cross-sectional study, representative samples of Czech adolescents (9,382 girls and 8,868 boys aged 10.5 to 16.5 years) from the international Health Behavior in School-Aged Children (HBSC) study were selected in 2002, 2006, 2010 and 2014 using a standardized self-reported questionnaire and in accordance with the rules of the binding research protocol. The SES was derived from responses relating to family ownership and the number of cars, computers, bathrooms, dishwashers, their own children's room, and foreign family holidays. Overweight/obesity was calculated from the body weight and height according to the age-differentiated percentile body mass index charts. Trends in overweight/obesity prevalence were analysed by logistic regression analysis. Differences in overweight/obesity prevalence among individual SES categories of adolescents were detected using Chi-square. Results: The highest significant (p < 0.001) increase in overweight/obesity prevalence between 2002 and 2014 was revealed in boys from low (+9.3 p.p.: 19.7%2002 → 29%2014) and middle (+6.9 p.p.: 18%2002 → 24.9%2014) SES and girls from middle SES families (+5.1 p.p.: 6.5%2002 → 11.6%2014). However, among all girls (or boys), overweight/obesity was found more frequently in low SES categories than in high SES categories in all years of data collection (2002, 2006 and 2014). In 2014, the prevalence of obesity/obesity among girls and boys from low SES families was significantly (p < 0.01) higher than that of girls and boys from high SES families. Regardless of SES, there is a lower chance of overweight/obesity in girls than in boys (p < 0.001), older adolescents than those aged 11 years (p < 0.001) and in 2002 than in 2014 (p < 0.05). However, at least 60 minutes per day of moderate-to-vigorous PA (OR = 0.69, 95% CI = 0.56-0.86, p < 0.01) and less than 2 hours of daily screen time (OR = 0.61; 95% CI = 0.44-0.84; p < 0.01) are associated with significant reduction in risk of overweight/obesity only among adolescents from high SES families. Conclusions: High rates of overweight/obesity and poor level of daily moderate-to-vigorous PA among adolescents from low SES provide a disturbing evidence highlighting necessity of public health efforts to implement obesity-reduction interventions for this economically disadvantaged population.
Výsledky: V letech 2002–2014 došlo u všech váhových kategorií k významnému poklesu konzumace slazených nápojů (normální hmotnost OR: 0,47; CI = 0,42–0,52; nadváha OR: 0,47; CI = 0,36–0,63; obezita OR: 0,47; CI = 0,29–0,75). Navíc byly zaznamenány významné změny u denní konzumace sladkostí (OR: 0,81; CI = 0,74–0,90), ovoce (OR: 0,81; CI = 0,74–0,88) a snídání ve všedních dnech (OR: 1,28; CI = 1,17–1,39), ale statisticky významná změna byla pouze u adolescentů s normální hmotností. Konzumace zeleniny a snídání o víkendech zůstala v tomto období nezměněna.
Objective: The main aim of the present study was to evaluate trends in six eating behaviours of Czech adolescents in relation to their body weight between 2002–2014. The partial aim was to monitor the differences in eating behaviours between adolescents with normal body weight and obesity in 2002, 2006, 2010 and 2014. Methods: The data was obtained from the Czech HBSC (Health Behaviour in School-aged Children) survey in 2002, 2006, 2010 and 2014. Body weight category was determined by body mass index (BMI) and eating behaviours were obtained from a questionnaire filled by 11-, 13- and 15-year-old adolescents. Logistic regression (Enter method) (odds ratio – OR and 95% confidence interval – CI) was used to analyse changes in eating behaviours and the differences between body weight categories were calculated by chi-square test. Results: From 2002 to 2014, the data showed a significant decrease in consumption of soft drinks (normal body weight OR: 0.47; CI = 0.42–0.52; overweight OR: 0.47; CI = 0.36–0.63, and obesity OR: 0.47; CI = 0.29–0.75). In addition, the changes in trends were seen in daily consumption of sweets (OR: 0.81; CI = 0.74–0.90), fruit (OR: 0.81; CI = 0.74–0.88) and breakfast on weekdays (OR: 1.28; CI = 1.17–1.39), however, significant change was only in adolescents with normal body weight. There was no change in consumption of vegetable or breakfast on weekends. Lower prevalence was reported in consumption of sweets (all survey years), soft drinks (only in 2006) and breakfast on weekdays/weekends (only in 2006) in obese adolescents. Conclusions: Monitoring dietary trends in Czech adolescents in relation to body weight category is important for evaluation of current projects and development of new programmes targeted to decrease differences between body weight categories in adolescents.

OBJECTIVE: Due to the importance of surveillance of injuries and accidents in the Czech Republic, the purpose of this study was to report the temporal trends of injuries of Czech adolescents between 2002 and 2014. METHODS: Adolescents (N=20,038) from the Czech Republic, that took part in the 2002, 2006, 2010 and 2014 HBSC study, reported the frequency of medically attended injuries in the past 12 months. Repeated binary logistic regressions with different years as reference categories were performed. RESULTS: Less than half (44.7%) of all Czech adolescents reported they had experienced at least one medically attended injuries in the past 12 months, with boys experiencing more injuries than girls (p<0.001), and the injuries were more common in older adolescents. Fewer boys reported injury in 2014 when compared to 2002 (OR, CI: 0.81, 0.72-0.90), there was also a significant decrease in injuries among girls between 2014 and 2006 (OR, CI: 0.77, 0.69-0.86). CONCLUSIONS: The trend was not linear amongst boys. Along with improved safety promotion education, the rate of injuries decreased among adolescents between the years 2002 and 2014.

OBJECTIVE: School is a place where students spend most of their daytime hours. Previous studies indicate that the class climate significantly affects students in both positive and negative ways. The aim of our study is to describe the trends in the psychosocial school environment based on four surveyed years in the Czech Republic. METHODS: The trends in perception of school were assessed by the standardised self-reported HBSC questionnaire from data collected in 2002, 2006, 2010, and 2014. The overall sample included 8,530 girls and 8,087 boys. Data was analysed separately by gender and three age categories 11, 13, and 15 years. Trends were calculated using descriptive categories and percentages. To identify factors influencing school likeness we used binominal logistic regression. RESULTS: Results showed that school satisfaction declines with age both in boys and girls in each of four surveyed periods. Students who do not feel a high level of support from the teacher are more prone to negative perceptions of school. School duties in both genders were perceived to be greater and more stressful in 2014 than in 2002. CONCLUSIONS: Interventions to support the perception of psychosocial school environment via the educational system should, therefore, take the aforementioned variation into account.
