Velký počet depresivních pacientů rezistentních na léčbu představuje klinický problém. Vedle farmakoterapie byly do léčby rezistentní deprese nedávno zavedeny dvě nové reverzibilní invazivní stimulační metody, vagová stimulace (VNS) a hluboká mozková stimulace (DBS). V přehledu přinášíme souhrn mechanizmu působení, neurobiologie a dostupných klinických dat o VNS a DBS u rezistentní deprese. Podstatou VNS je stimulace levého raménka nervus vagus v krční oblasti pomocí programovatelného stimulátoru. Účinnost VNS byla testována ve čtyřech klinických studiích s celkem 355 pacienty. VNS prokázala postupně narůstající zlepšení s plným účinkem po 6-12 měsících stimulace, který byl udržen až po následující 2 roky. Pacienti, kteří se zlepšili nejvíce, byli málo až středně rezistentní. Nicméně primární výsledky jediné kontrolované studie byly negativní. Princip DBS spočívá ve sterotaktické implantaci elektrod napajených pulzním generátorem do specifických oblastí mozku. Cílovými oblastmi pro depresi je subthalamické jádro, vnitřní globus pallidus, ventrální capsula interna/ventrální striatum, oblast subgenuální cinguly a nucleus accumbens. Antidepresivní účinek DBS byl zkoumán v kazuistických sériích s celkovým počtem 50 pacientů s rezistentní depresí. Stimulace různých oblastí mozku vedla k rychlé redukci příznaků deprese. Klinická data z aplikace VNS a DBS jsou povzbudiví Hlavním přínosem obou metod je inovativní přístup, který umožňuje přesné zacílení specifických oblastí mozku, jader a okruhů účastnících se v patofyziologii neuropsychiatrických poruch. Je však zapotřebí více dat z kontrolovaných studií a je také nutné vyloučit potenciální placebo efekt. Relativně vysoká incidence suicidálního jednání u DBS vede ke zvýšené opatrnosti. Pro zavedení do klinické praxe bude nutné identifikovat pacienty, kteří mohou z VNS a DBS profitovat.
High number of treatment-resistant patients with depression poses a clinical problem. In addition to pharmacotherapy, reversibl e invasive stimulation methods, vagus nerve stimulation (VNS) and deep brain stimulation (DBS), have been introduced into the man agement of treatment-resistant depression (TRD). The paper reviews principles, neurobiology, and available clinical evidence of VN S and DBS in TRD. The basis of VNS is a stimulation of the left cervical vagus nerve with a programmable neurostimulator. Efficacy of VNS was examined in four clinical trials with 355 patients. VNS demonstrated steadily increasing improvement with full benefit afte r 6–12 months, sustained up to 2 years. Patients who responded best had a low-to-moderate antidepressant resistance. However, the prim ary results of the only controlled trial were negative. DBS involves stereotactical implantation of electrodes powered by a pulse g enerator into the specific brain regions. For depression, targeted areas are subthalamic nucleus, internal globus pallidus, ventral inte rnal capsule/ ventral striatum, the subgenual cingulated region, and nucleus accumbens. Antidepressant effects of DBS were examined in case s eries with the total number of 50 TRD patients. Stimulation of different brain regions resulted in rapid reduction of depressive symp toms. The clinical data on the use of VNS and DBS in TRD are encouraging. The major contribution of the methods is a novel approach t hat allows for precise targeting of the specific brain areas, nuclei and circuits implicated in etiopathogenesis of neuropsychiatri c disorders. However, more controlled data are needed and potential impact of placebo effect has to be ruled out. Relatively high incidence of suicidal behavioral in DBS warrants caution. For clinical practice, patients who may benefit from VNS or DBS best should be identi fied.

Evokované potenciály (EP) jsou jednoduchou neinvazivní funkční metodou k vyšetření mnohých neurologických a psychiatrických onemocnění. Event related potentials (ERP) umožňují sledovat některé aspekty kognitivního zpracování informace u člověka. Nejznámější komponentou ERP je vlna P 300, která má nejen široký experimentálni, ale stále větší význam klinický. Parametry této odpovědi závisí zejména na schopnosti a rychlosti zpracování informace subjektem. Změny této vlny jsou pro některá onemocnění typické. Na téma evokovaných potenciálů bylo do současné doby publikováno velké množství studií. Článek se snaží obecně nastínit problematiku zejm. kognitivních evokovaných potenciálů.
Evoked potentials are simple non-invasive methods of functional examination of neurological and psychiatric disorders. ERP (event related potentials) allow monitoring of various aspects of cognitive processing in humans. The most frequently used component of ERP is wave P300; its utilization is not only experimental, but it also has a growing importance in clinical practice. Results depend mainly on two parameters, speed and ability of the subject to process information. Variations of the waves are illness-specific. Numerous studies investigating EP have been published. The aim of the paper is to provide a general overview of EP with special emphasis on cognitive evoked potentials.
