OBJECTIVES: The aim of this study is to propose an approach for developing trustworthy recommendations as part of urgent responses (1-2 week) in the clinical, public health, and health systems fields. STUDY DESIGN AND SETTING: We conducted a review of the literature, outlined a draft approach, refined the concept through iterative discussions, a workshop by the Grading of Recommendations Assessment, Development and Evaluation Rapid Guidelines project group, and obtained feedback from the larger Grading of Recommendations Assessment, Development and Evaluation working group. RESULTS: A request for developing recommendations within 2 week is the usual trigger for an urgent response. Although the approach builds on the general principles of trustworthy guideline development, we highlight the following steps: (1) assess the level of urgency; (2) assess feasibility; (3) set up the organizational logistics; (4) specify the question(s); (5) collect the information needed; (6) assess the adequacy of identified information; (7) develop the recommendations using one of the 4 potential approaches: adopt existing recommendations, adapt existing recommendations, develop new recommendations using existing adequate systematic review, or develop new recommendations using expert panel input; and (8) consider an updating plan. CONCLUSION: An urgent response for developing recommendations requires building a cohesive, skilled, and highly motivated multidisciplinary team with the necessary clinical, scientific, and methodological expertise; adapting to shifting needs; complying with the principles of transparency; and properly managing conflicts of interest.

BACKGROUND: Severe traumatic injury continues to present challenges to healthcare systems around the world, and post-traumatic bleeding remains a leading cause of potentially preventable death among injured patients. Now in its fifth edition, this document aims to provide guidance on the management of major bleeding and coagulopathy following traumatic injury and encourages adaptation of the guiding principles described here to individual institutional circumstances and resources. METHODS: The pan-European, multidisciplinary Task Force for Advanced Bleeding Care in Trauma was founded in 2004, and the current author group included representatives of six relevant European professional societies. The group applied a structured, evidence-based consensus approach to address scientific queries that served as the basis for each recommendation and supporting rationale. Expert opinion and current clinical practice were also considered, particularly in areas in which randomised clinical trials have not or cannot be performed. Existing recommendations were re-examined and revised based on scientific evidence that has emerged since the previous edition and observed shifts in clinical practice. New recommendations were formulated to reflect current clinical concerns and areas in which new research data have been generated. RESULTS: Advances in our understanding of the pathophysiology of post-traumatic coagulopathy have supported improved management strategies, including evidence that early, individualised goal-directed treatment improves the outcome of severely injured patients. The overall organisation of the current guideline has been designed to reflect the clinical decision-making process along the patient pathway in an approximate temporal sequence. Recommendations are grouped behind the rationale for key decision points, which are patient- or problem-oriented rather than related to specific treatment modalities. While these recommendations provide guidance for the diagnosis and treatment of major bleeding and coagulopathy, emerging evidence supports the author group's belief that the greatest outcome improvement can be achieved through education and the establishment of and adherence to local clinical management algorithms. CONCLUSIONS: A multidisciplinary approach and adherence to evidence-based guidance are key to improving patient outcomes. If incorporated into local practice, these clinical practice guidelines have the potential to ensure a uniform standard of care across Europe and beyond and better outcomes for the severely bleeding trauma patient.

Karcinom pankreatu představuje v České republice po kolorektálním karcinomu druhé nejčastější nádorové onemocnění trávicího traktu a šesté nejčastější nádorové onemocnění vůbec. Pětileté přežití pacientů všech stadií se dlouhodobě pohybuje pod 5 %. Očekává se, že za 10-15 let bude karcinom pankreatu z hlediska úmrtnosti na maligní nádor na prvním místě. Za poslední období se přežívání výrazněji nezměnilo, a to nehledě na zdokonalení zobrazovacích metod a preciznější chirurgický výkon. Karcinom pankreatu se vyvíjí relativně pomalu, přibližně v průběhu deseti let, a my jej léčíme až v jeho terminální fázi. Onemocnění je třeba chápat jako systémové, nikoli jako lokální a regionální. Cestou, jak v současné době a v blízké budoucnosti zlepšit péči o tyto nemocné, je zlepšení kvality doporučených postupů a jejich dodržování. Jedná se o problém nejen zdravotnický - organizační, ale i sociální a ekonomický.
Pancreatic carcinoma is the second most common cancer of the gastrointestinal tract in the Czech Republic after the colorectal cancer and the sixth most common cancer. The 5-year survival rate of all stages of the pancreatic carcinoma is below 5 %. It is expected that in 10-15 years, mortality of pancreatic carcinoma will take the first place among other malignancies. Over the last period, survival has not changed significantly despite the improvement of imaging methods and surgical performance becoming more precise. The pancreatic carcinoma develops relatively slowly, approximately over a decade, and we cure it until its terminal phase. Diseases should be understood as systemic, not local and regional. Improving the quality of guidelines and their compliance is the way to improve the care of these patients at present and in the near future. This is not only a health-organizational problem but also a social and economic problem.

Intravenózna trombolýza ako základná kauzálna liečba akútnej ischemickej cievnej mozgovej príhody je u pacientov liečených novými orálnymi antikoagulanciami za určitých okolností kontraindikovaná. Dabigatran ako jediný NOAK má svoje špecifické antidotum – monoklonálnu protilátku idarucizumab. Podanie antidota vyblokuje účinok antikoagulačnej terapie a umožní podanie kauzálnej liečby hyperakútneho štádia ischémie mozgu (trombolýza, mechanická trombektómia). Prezentujeme prípad 59-ročnej pacientky liečenej dabigatranom pre stav po ischemickej mozgovej príhode v minulosti s fibriláciou predsiení, ktorá utrpela recidívu, avšak pre antikoagulačnú terapiu bola trombolýza kontraindikovaná. Po podaní idarucizumabu sme boli schopní pacientke podať trombolytikum a vzhľadom na nález oklúzie veľkej cerebrálnej tepny následne aj mechanickú trombektómiu. Po uvedenej terapii sa stav pacientky zlepšil a pri NIHSS 2b bola prepustená do domácej starostlivosti.
Intravenous thrombolysis as the basic causal therapy for acute ischaemic stroke in patients treated with novel oral anticoagulantsis contraindicated under certain circumstances. Dabigatran, as the only novel oral anticoagulant, has its specific antidote– the monoclonal antibody idarucizumab. Delivery of the antidote blocks the effect of anticoagulant therapy and allows foradministration of causal therapy for hyperacute-stage cerebral ischaemia (thrombolysis, mechanical thrombectomy). We reporta case of a 59-year-old female patient treated with dabigatran for a post-ischemic stroke state with a history of atrial fibrillationwho had a recurrence, but thrombolysis was contraindicated because of anticoagulant therapy. Following the administration ofidarucizumab, we were able to give the patient a thrombolytic drug and, given the finding of occlusion of a large cerebral artery,subsequent mechanical thrombectomy. After providing this treatment, the patient’s condition improved and she could be dischargedto home care with an NIHSS of 2b.
