OBJECTIVES: An enhanced worldwide application of platinum group elements (PGE), in particular platinum, has been observed during recent decades. An increased concentration of PGE was determined in collected samples of great amount of aqueous ecosystems.The aim was to compare phytotoxic effect of platinum (PtCl4) by performing two different bioassays on green algae Pseudokirchneriella subcapitata and macrophyte duckweed, Lemna minor. MEDTHODS: The algal experiment (Pseudokichneriella subcapitata) followed OECD 201, the concentration row for PtCl4 was: 0.05; 0.01; 0.25; 0.5; 1 µM. The duckweed (Lemna minor) experiment was conducted according to OECD 221, employed PtCl4 concentrations were: 5; 10; 25; 50; 100 µM. Plants were cultivated as a microbiotest, using micro-volumes. RESULTS: The results of the algal test showed significant growth inhibition of the final biomass. The values of 72hEC5(µ), 72hEC10(µ), 72hEC20(µ) counted on a basis of average specific growth rate (µ) were 0.31 µM, 0.58 µM and 1.12 µM of PtCl4, respectively. The values, obtained on a basis of the area under the growth curves (A), were 0.04 µM (72hEC5(A)), 0.24 µM (72hEC10(A)) and 0.64 µM (72hEC20(A)). The experiment with duckweed showed 50% of growth inhibition and the values of 168hEC50(µ) were 19.55 µM and 168hEC50(A) 13.63 µM of PtCl4. CONCLUSION: The fronds of duckweed showed strong adverse effect of platinum influence (chlorosis, necrosis). The algal test and the estimation of 72hEC5(A) appears to be the most sensitive.

The introduction of the method of multi-locus sequence typing for Streptococcus agalactiae shall render possible the comparison of Czech hyper-virulent clones with those from abroad. Knowledge of the distribution of genes of the potential factors of virulence in invasive and non-invasive Streptococcus agalactiae strains is essential for the identification of the mechanisms of pathogenesis. In view of the significance of surface proteins in S. agalactiae for vaccination, there is needed their diagnosticsof high quality that have not been rendered possible by the hitherto mode of detection. The detection of genes coding surface protein markers shall contribute to the optimization of the composition of the vaccine against S. agalactiae.
Výsledky nově zavedené metody multilokusové sekvenační typizace pro kmeny Streptococcus agalactiae umožní porovnání výskytu hypervirulentních klonů v ČR a v zahraničí. Porovnání distribuce genů potenciálních faktorů virulence u invazivních a neinvazivních kmenů Streptococcus agalactiae přispěje k poznání mechanismů patogeneze. Dosavadní způsob detekce povrchových proteinů Streptococcus agalactiae není dostatečný. Nový způsob jejich detekce metodou PCR je přesnější. Detekce genů povrchových proteinovýchmarkerů přispěje k optimalizaci složení vakciny proti Streptococcus agalactiae.

V článku jsou prezentovány první výsledky charakterizace 77 českých novorozeneckých kmenů Streptococcus agalactiae metodou multilokusové sekvenační typizace. Cílem této studie bylo ověřit, zda české izoláty patří k potenciálně invazivnímu klonálnímu komplexu CC17, a dále analyzovat vztahy mezi sérotypy a sekvenčními typy (ST). Dvanáct zjištených ST bylo přiřazeno k 5 klonálním komplexům. U většiny sérotypů našeho souboru byla zjištěna variabilita sekvenčních typů. Potenciálně invazivní ST17 byl jediným sekvenčním typem striktně vázaným na sérotyp III. ST17, ST23 a ST19 dohromady tvořily 83 % invazivních kmenů. „Hyperinvazivní“ ST17 byl zachycen u 85 % (6 ze 7) izolátů z mozkomíšního moku, ale pouze u 32 % (7 z 22) kmenů S. agalactiae izolovaných z hemokultur.
The first multilocus sequence typing results of Czech neonatal Streptococcus agalactiae isolates are presented in this paper. The aims of the study were to prove if the Czech isolates belong to potential invasive clonal complex CC17 and to further analyse serotype – sequence type relationships. Twelve sequence types detected among 77 isolates were assigned to 5 clonal complexes. Sequence type variability was found for most of our collection serotypes. As many as 83 % of the invasive isolates were covered by as few as 3 sequence types: S17, ST23 and ST19. ST17 was the only sequence type strictly tied to serotype III. “Hyperinvasive” ST17 was identified in 85 % of the cerebrospinal fluid isolates (6 out of 7), but in 32 % of the blood isolates only.
An unusual emm53, T-28/T-non-typeable, iMLS(B) phenotype clone represented a substantial proportion (28.6%) of invasive erythromycin-resistant group A streptococcus (GAS) isolates in the Czech Republic during 2003. Clonal analysis of emm53 isolates between 2001 and 2004 revealed four pulsed-field gel electrophoresis (PFGE) patterns and two emm subtypes. Isolates produced identical PFGE patterns regardless of their invasiveness and/or tetracycline resistance. Multilocus sequence typing classified all isolates as ST340. An ST5 (emm83) isolate, a potential ancestor of ST340, was isolated in the Czech Republic from an impetigo patient in 1988. The Czech emm53/ST340 isolates shared only three of seven alleles with the original Lancefield emm53/ST11 isolate.

The NRL for Streptococci and Enterococci, the only laboratory skilled in S. pyogenes typing in the Czech Republic, intends to implement the genotyping method - emm typing to replace the labour- intensive and much less effective standard serotyping methodThis method will be suitable for genotype monitoring as a prerequisite for development of the new generation of type specific vaccine. This method will allow unambiguous identification of identical or diverse isolates. Emm typing will provide results comparable at an international level.
V NRL pro streptokoky a enterokoky, jediné laboratoři v České republice provádějící typizaci kmenů S.pyogenes, bude zavedena genetická metoda - emm typizace. Nahradí pracnou a z hlediska typovatelnosti kmenů málo efektivní standardní metodu sérologickou.Metoda umožní sledování genotypů nezbytných pro vytvoření typově specifické vakcíny nové generace. Jednoznačně prokáže identitu či rozdílnost izolátů při řešení epidemiologických otázek. Emm typizace umožní porovnávat výsledky v mezinárodním měřítku.

Tato práce shrnuje výsledky prvního roku aktivní surveillance závažných onemocnění vyvolanýchstreptokoky skupiny A probíhající v České republice v rámci projektu Strep-EURO, pro který bylapřijata mezinárodní definice těchto onemocnění. V období leden–prosinec 2003 bylo z 28 laboratoříČR zasláno do SZÚ 59 streptokoků skupiny A izolovaných z invazivních onemocnění, která odpovídaladefinici Strep-EURO. Zároveň byly poskytnuty klinicko-epidemiologické údaje a informaceo spádové populaci k dalšímu zpracování. Výpočtem byla pro rok 2003 stanovena nemocnost1,3/100 000 obyvatel a celková smrtnost 13,5 %. Mezi těmito 59 kmeny byl vysoce zastoupen typ emml,původně označovaný M1 (23,7 %). Kmeny rezistentní k erytromycinu, clindamycinu, resp. tetracyklinuse v tomto souboru podílely 24,1 %, 17,2 %, resp. 20,7 %. Zdánlivě nižší nemocnost a vyššísmrtnost závažných onemocnění způsobených streptokoky skupiny A zjištěná v České republicev letech 1994–1996 (0,4/100 000, resp. 46,5 %) v porovnání se současným stavem je důsledkem přijetímírnějších mezinárodních kritérií definice invazivního onemocnění pro současnou studii Strep-EURO.
Results obtained in the first year of active surveillance of serious diseases caused by groupA streptococci in the Czech Republic carried out within the Strep-EURO project are summarized.From January to December 2003, 28 Czech laboratories referred to the National Institute of PublicHealth 59 group A streptococcal strains isolated from invasive diseases meeting the Strep-EUROdefinition. At the same time, clinical-epidemiological data and information on the catchment areapopulation were provided. For 2003, the total morbidity and case fatality rates were calculated tobe 1.3/100 000 population and 13.5 %, respectively. A high proportion (23.7 %) of the 59 strains wereof type emm 1, initially labeled M1. As many as 24.1 %, 17.2 % and 20.7 % of the strains tested wereresistant to erythromycin, clindamycin and tetracycline, respectively. The seemingly lower morbidityrate and higher case fatality rate from serious diseases caused by group A streptococci reportedin the Czech Republic for 1994–1996, i.e. 0.4/100 000 and 46.5 %, respectively, in comparison with thelatest data can be explained by the use of softer international criteria for the Strep-EURO definitionof invasive disease.

V článku jsou prezentovány první výsledky charakterizace 29 invazivních kmenů Streptococcuspyogenes, izolovaných v České republice v první polovině roku 2003, metodou multilokusové sekvenačnítypizace (MLST). U žádného z 16 přítomných emm typů nebyla mezi kmeny našeho souboruzjištěna variabilita sekvenčních typů (ST). První výsledky MLST ukazují, že populace kmenůzpůsobujících závažná onemocnění v České republice je odlišná od kmenů izolovaných v zahraničí.U sedmi kmenů byly popsány nové sekvenční typy: ST134, ST308, ST336, 5T340, obsahující novékombinace známých alel a ST341 se třemi dosud nepopsanými alelami (gki 91, murI 65 a yqiL 60),zjištěný u třech kmenů. Nově popsané sekvenční typy byly registrovány v celosvětové databáziMLST S. pyogenes.
First results of multilocus sequence typing (MLST) for characterization of 29 invasive Streptococcuspyogenes strains isolated in the Czech Republic in the first half of 2003 are presented. None of 16emm types detected among the study strains showed sequence type (ST) variability. The MLSTresults are indicative of differences between the strains causing serious diseases in the CzechRepublic and those isolated in other countries. In seven strains, four new STs with known alleles innew combinations, ST134, ST308, ST336, ST340, and one new ST with three as yet undescribed alleles(gki 91, murI 65 and yqiL 60), ST341, were described. These newly described STs were submitted tothe web-based reference MLST database for S. pyogenes.

Profylakticke nitrozilni podani penicilinu nebo ampicilinu nosickam Streptococcus agalactiae v prubehu porodu snizuje vyskyt casnych novorozeneckych invazivnich infekci zpusobenych timto druhem bakterie az o 50 %. V nasem souboru izolatu S. agalactiae od 265 novorozencu a 204 gravidnich zen nebyla prokazana rezistence ani smzena citlivost k temto antibiotikum. Alternativnimi antibiotiky pri precitlivelosti zen k penicilinu jsou erytromycin, a zejmena klindamycin pro vyhodnejsi farmakokineticke parametry. Rezistence k erytromycinu byla zjistena u 4,4 % izolatu od gravidnich zen a v temer dvojnasobne vyssi frekvenci u neinvazivnich (7,0 %) a invazivnich kmenu (8,5 %) izolovanych od novorozencu. Kmeny rezistentni k erytromycinu byly s vyjimkou jednoho kmene soucasne rezistentni take ke klindamycinu. Prevaha izolatu S. agalactiae rezistentnich k erytromycinu (65,5 %) pfislusela k serotypu V. Pri volbe profylaxe infekci zpusobenych S. agalactiae u novorozencu je treba dat vzdy prednost penicilinu nebo ampicilinu pro jejich bezvyhradnou ucinnost. Chybny vysledek laboratormlio vysetrem, ktery oznaci citlivy kmen S. agalactiae jako rezistentni k penicilinu, nebo neprovereni nadhodnocenych udaju gravidnich zen o jejich alergii k penicilinu zbytecne vedou k volbe alternativniho antibiotika. Je vsak treba zduraznit, ze ucinnost erytromycinu a klindamycinu nevi jista pro moznost vyskytu rezistentnich kmenu.
Prophylactic intravenous penicillin or ampicillin administration to carriers of Streptococcus agalactiae during childbirth reduces the incidence of early neonatal invasive infections caused by these bacteria by as much as 50%. In our group of isolates of S. agalactiae from 265 neonates and 204 pregnant women neither resistance nor reduced sensitivity to these antibiotics was proved. Alternative antibiotics in case of hypersensitivity of women to penicillin are erythromycin and in particular clindamycin because of more favourable pharmacokinetic parameters. Resistance to erythromycin was found in 4.4% isolates from pregnant women with an almost double frequency of non-invasive (7.0%) as compared with invasive strains (8.5%) isolated from neonates. Strains resistant to erythromycin were with the exception of one strain also resistant to clindamycin. The majority of isolates of S. agalactiae resistant to erythromycin (65.5%) belonged to serotype V. When selecting effective prophylaxis of infections caused by S. agalactiae in neonates always effective. Erroneous results of laboratory examination which describe a sensitive strain of S. agalactiae as penicillin resistant or omission of testing exaggerated data from pregnant women on their allergy to penicillin lead unnecessarily to selection of an alternative antibiotic. It must be however emphasized that the effectiveness of erythromycin and clindamycin is not certain because of the possible occurrence of resistant strains.
