BACKGROUND: Psychotherapy outcomes are typically measured in terms of symptom relief. However, this method might overlook important changes from clients' perspectives when they are asked to report on them. A more client-centred approach might bring a deeper understanding of psychotherapy outcomes. We aimed to evaluate the outcomes identified by clients within qualitative psychotherapy research. METHODS: The PsycArticles, PsycInfo, and MEDLINE Complete databases were searched for English language studies published until Nov 11, 2023. Additional studies were identified through references in the primary studies and previous meta-analyses or systematic reviews. Search terms were related to psychotherapy and counselling, clients' or patients' experiences, psychotherapy outcomes and changes, post-treatment perspectives, and types of qualitative methods. Qualitative studies on client-identified outcomes of individual psychotherapy were included. Findings related to clients' perceptions of psychotherapy outcomes were extracted (by ML and checked by TR and LT) and analysed (by all authors) using the descriptive-interpretative meta-analytic approach. All authors have personally experienced psychotherapy as clients. This study was pre-registered with PROSPERO (CRD42021277330). FINDINGS: We included 177 studies in the qualitative meta-analysis, from 24 countries, including descriptions from 2908 clients. Most of the studies were of good quality; they covered a wide range of therapeutic approaches and diagnoses. The descriptions of psychotherapy outcomes were classified into 60 meta-categories and grouped into ten clusters. These clusters related to clients' relational and social functioning; their emotional functioning; self-awareness, self-understanding, and more adaptive cognitive processing; behavioural functioning; developing their own resources; clients' attitudes towards themselves; generally embracing life; symptom and problem change; and more general wellbeing. The tenth cluster was outcomes that could not be clearly attributed to psychotherapy, which was considered outside the scope of this study. INTERPRETATION: The meta-analysis showed that clients value outcome dimensions beyond symptom reduction, such as deeper self-understanding, enhanced self-agency, and greater social engagement. By examining psychotherapy outcomes across various diagnoses and therapeutic approaches, we highlight limitations in traditional outcome measures, showing the need for more comprehensive, client-centred assessment tools and the value of incorporating qualitative methods into understanding dimensions of change. OBJECTIVE: Understanding the client perspective is important for the provision of psychotherapy. The significant events paradigm, within which clients report on the most significant events of a therapy session immediately after the session, represents a useful means to explore clients' in-session experience. METHOD: The aim of this study was to investigate what types of client-identified impacts are reported in qualitative studies on helpful and hindering events in psychotherapy. Seventeen primary studies focusing on client-identified helpful and hindering events were identified through database searches and subsequent manual searches. The data were subjected to qualitative meta-analysis. RESULTS: The descriptions of event impacts were classified into 12 helpful (e.g., gaining a new perspective on the self; feeling heard, understood, and accepted; and feeling engaged in the therapeutic process) and eight hindering (e.g., lacking guidance from the therapist and feeling emotionally overwhelmed) impact meta-categories. CONCLUSION: The findings provide an update to a previous meta-analysis by identifying several new categories of helpful event impacts and by categorizing hindering event impacts.

OBJECTIVE: Client experience of psychotherapy is an important resource for our understanding of psychotherapy and deserves relevant attention in psychotherapy research. Emotion-focused therapy (EFT) is a relatively new adaptation of a humanistic therapy that has a tradition of giving a voice to the clients in therapy. Despite the number of qualitative studies looking at the experience of clients in EFT, there was no formal qualitative meta-analysis conducted synthesizing the existing qualitative research on the clients' experience of EFT. METHOD: A sample of 11 primary qualitative studies was selected through a systematic search of the literature. Primary studies were critically appraised, and data (findings) from them extracted and meta-analysed. RESULTS: All 11 studies featured experiences of helpful aspects of therapy, with difficult but helpful aspects reported in seven studies and unhelpful aspects reported in six studies. Most studies reported chair and experiential work and intense emotional work in EFT as helpful, with fewer reports and fewer clients finding them difficult but helpful or unhelpful. The multidimensional nature of the therapist and therapeutic relationship in EFT included emotional connection and support, validation and understanding and was commonly experienced as helpful to clients. Other client experiences reported included practical aspects of EFT such as session length, in-session outcomes such as clients' transformative experiences, and internal and external factors which were experienced by clients such as determination or reluctance to commit to therapy. CONCLUSIONS: Most clients experience EFT as an intense, challenging, but productive psychotherapy, but it appears a minority of clients experience aspects of EFT as challenging.

Ciele. Cieľom tejto kvalitatívne zameranej výskumnej štúdie bolo porozumenie subjektívneho prežívania budovania komunity a jeho popisu vo výpovediach účastníkov Community Building Workshop a Community Building Event (CBW/E) od východiskovej situácie rozhodnutia o účasti na workshope a životnej situácii v tom čase, cez najdôležitejšie momenty workshopu, až po prípadné zmeny u účastníkov v dôsledku tejto skúsenosti. Súbor a metódy. Účastníci z Veľkej Británie, Nemecka, USA a niekoľkí, ktorí neuviedli svoju národnosť, sa zúčastnili workshopov zorganizovaných vo Veľkej Británii a Nemecku a vyplnili sadu troch dotazníkov v anglickom jazyku tesne pred workshopom, tesne po workshope a tri mesiace po workshope. Analyzované boli údaje od 18 účastníkov. Hypotézy. Vzhľadom na to, že išlo o exploračne zameranú štúdiu, neboli vopred stanovené žiadne hypotézy. Analýza. Dáta boli analyzované prostredníctvom Konsenzuálneho kvalitatívneho výskumu (CQR), metódy komplexnej a dôkladnej kvalitatívnej analýzy, ktorá využíva konsenzuálny prístup. Výsledky. Participanti typicky prichádzali na workshop CBW/E s túžbou po osobnom raste, so záujmom o komunitu a jej úlohu v každodennom živote a tiež s nejakou aktuálne prežívanou osobnou záležitosťou. Jadro psychologického zážitku komunity tvorilo zdieľanie, ocenenie a autentickosť. U všetkých analyzovaných prípadoch (18) sa nachádzal pocit posilnenia (empowerment) ako dôsledok účasti na workshope CBW/E. Medzi ďalšie najčastejšie respondentmi výskumu uvádzané zmeny v dôsledku workshopu patrili väčšia schopnosť byť sám sebou, zvýšená sebaúčinnosť a sebaistota, dôvera a akceptácia, práca s vlastnými hranicami a aplikácia získaných poznatkov a zručností v živote. Toto učenie sa u väčšiny participantov výskumu spájalo s prežívaním emocionálneho distresu. Obmedzenia výzkumu. Hlavným obmedzením výskumu bolo použitie písomných, a nie verbálnych výpovedí účastníkov, čo znižuje bohatstvo dát, ktoré môžu byť potenciálne získané od participantov.
Objectives. The goal of this qualitatively oriented research study was to understand subjective experience of building community as found in the accounts of Community Building Workshop and Community Building Event (CBW/E) attendants. Subjects and setting. Attendants (n=18) from United Kingdom, Germany, United States and a few with not stated nationality, took part in workshops organized in United Kingdom and Germany. The data were collected by questionnaires prior, at the end, and three months after the workshop. The questionnaire captured participant’s experiences of the enrolment to the workshop, the most important events in the workshop, and the changes perceived as a result of their experience of community building. Hypotheses. Because of the explorative character of the research study, there was no hypothesis stated beforehand. Analysis.T he data were analysed using the Consensual Qualitative Research (CQR) a method of comprehensive and rigorous qualitative analysis that uses consensual approach to analysis. Results. Participants typically came to the workshop CBW/E with the desire for personal growth, with the interest in community and its role in their everyday life. Sharing, appreciation and being authentic were the core elements of psychological sense of community for the participants. In all analysed cases (18), there was a feeling of empowerment mentioned as a consequence of attendance of the workshop. The most frequently cited changes in attendants lives were greater ability to be authentic, increased self-efficiency, enhanced confidence, trust and acceptance, work with own boundaries and application of an acquired learning and skills in one’s life. The learning was in the majority of our research cases connected with experiencing of some emotional distress. Study limitations. The main limitation was the use of written rather than verbal accounts, which limits the richness of the data that can be gathered.

Dotazník CORE-OM (Clinical Outcomes in Routine Evaluation - Outcome Measure) je široko používaný sebaposudzovací nástroj merania, ktorý bol speciálně vyvinutý na meranie efektu psychologickej/psychiatrickej liečby. Proces jeho adaptácie na podmienky slovenskej klinickej praxe opisujeme v troch štúdiách. V štúdii I. sa zameriavame na overenie vnútornej konzistencie slovenského překladu dotazníka CORE-OM, overenie konštruktovej validity a schopnosti diskriminovat medzi klinickým a neklinickým výberom. V štúdii II., v ktorej sme použili revidovánu verziu slovenského překladu dotazníka, prinášame deskriptívne data z klinického a neklinického výběru, zameriavame sa na odhad test-retestovej reliability a výpočet kritérií štatisticky a klinicky signifikantnej změny. Studia III. opisuje proces konsenzuálnej tvorby finálnej verzie slovenského překladu CORE-OM, za účasti jedného z autorov originálnej verzie. Slovenská verzia CORE-OM preukázala dobré psychometrické vlastnosti, je vysoko vnútorne konzistentná a dosahuje dostatočnú test-retestovú reliabilitu, koreluje s referenčnými nástrojmi merania a dostatočne diskriminuje medzi klinickým a neklinickým výberom. Vypočítané kritérium statisticky signifikantnej změny (index spolahlivej změny RCI) má hodnotu 0,57. Cut-off skóre, ktoré je ukazovateľom klinicky signifikantnej změny, má pre celok CORE-OM hodnotu pre mužov 1,05 a pre ženy 1,12. Slovenská verzia dotazníka CORE-OM je připravená pre klinické používanie, jej ďalšie vlastnosti ohladom merania terapeutickej změny sa ďalej skúmajú.
CORE-OM (Clinical Outcomes in Routine Evaluation - Outcome Measure) is a widely used self-rating measure, developed specifically for measuring outcome of psychological/psychiatric treatment. The process of its adaptation for the use in Slovak context is described in three studies. Study I. presents internal consistency coefficients of the Slovak version of CORE-OM, an assessment of the construct validity of the instrument as well as the comparison of clinical and non-clinical samples. In Study II., where we used a revised Slovak translation of CORE-OM, we are presenting the descriptive data of clinical and non-clinical samples, the test-retest reliability and the criteria of the statistically and clinically significant change. Study III describes a process of establishing the final version of the Slovak CORE-OM in collaboration with one of the authors of the original version. The Slovak version of CORE-OM has good psychometric properties, is highly internally consistent, and has sufficient test-retest reliability. It discriminates well between clinical and non-clinical samples. The criterion of statistical significant change (reliable change index - RCI) for all items of CORE-OM is 0, 57. The cut-off score is 1,05 for males and 1,12 for females respectively. The Slovak version of CORE-OM is ready to be used widely in clinical practice; further research on its properties is being conducted.
