BACKGROUND: Psychotherapy outcomes are typically measured in terms of symptom relief. However, this method might overlook important changes from clients' perspectives when they are asked to report on them. A more client-centred approach might bring a deeper understanding of psychotherapy outcomes. We aimed to evaluate the outcomes identified by clients within qualitative psychotherapy research. METHODS: The PsycArticles, PsycInfo, and MEDLINE Complete databases were searched for English language studies published until Nov 11, 2023. Additional studies were identified through references in the primary studies and previous meta-analyses or systematic reviews. Search terms were related to psychotherapy and counselling, clients' or patients' experiences, psychotherapy outcomes and changes, post-treatment perspectives, and types of qualitative methods. Qualitative studies on client-identified outcomes of individual psychotherapy were included. Findings related to clients' perceptions of psychotherapy outcomes were extracted (by ML and checked by TR and LT) and analysed (by all authors) using the descriptive-interpretative meta-analytic approach. All authors have personally experienced psychotherapy as clients. This study was pre-registered with PROSPERO (CRD42021277330). FINDINGS: We included 177 studies in the qualitative meta-analysis, from 24 countries, including descriptions from 2908 clients. Most of the studies were of good quality; they covered a wide range of therapeutic approaches and diagnoses. The descriptions of psychotherapy outcomes were classified into 60 meta-categories and grouped into ten clusters. These clusters related to clients' relational and social functioning; their emotional functioning; self-awareness, self-understanding, and more adaptive cognitive processing; behavioural functioning; developing their own resources; clients' attitudes towards themselves; generally embracing life; symptom and problem change; and more general wellbeing. The tenth cluster was outcomes that could not be clearly attributed to psychotherapy, which was considered outside the scope of this study. INTERPRETATION: The meta-analysis showed that clients value outcome dimensions beyond symptom reduction, such as deeper self-understanding, enhanced self-agency, and greater social engagement. By examining psychotherapy outcomes across various diagnoses and therapeutic approaches, we highlight limitations in traditional outcome measures, showing the need for more comprehensive, client-centred assessment tools and the value of incorporating qualitative methods into understanding dimensions of change. CONTEXT: The decolonial turn in psychology criticizes conventional Western psychotherapeutic frameworks and seeks to decolonize therapeutic practices by considering diverse cultural perspectives. Indigenous healing has been increasingly used in the psychotherapy of ethnic communities, but also in the psychotherapy of Western clients. The research questions of the present study were focused on how Western clients experience the therapies inspired by Indigenous healing. OBJECTIVE: The study aimed to explore the motivations, experiences, and mental health outcomes of participants of therapies inspired by Indigenous healing with a focus on issues related to intercultural transfer of therapeutic practice. DESIGN: Semi-structured qualitative interviews were used to gather narratives from 28 participants. Thematic analysis was used for data analysis. RESULTS: Participants reported the following long-lasting outcomes of Indigenous therapies: increased well-being, calm, satisfaction with life, better emotion regulation, coping with stress, behavioral control, problem solving, decision making as well as a more sensitive and deeper experience of reality. Conventional Western psychotherapy was perceived as conversation-based, while Indigenous therapy was perceived as experience-based and more complex. The integration of experiences from Indigenous therapies was more difficult in the Western sociocultural environment compared to participation in the place of their origin, within the community of Indigenous people. CONCLUSION: Indigenous healing has beneficial outcomes for Western clients, but the integration of these experiences is quite difficult and requires systematic psychotherapeutic support in the Western cultural environment.

OBJECTIVE: Dynamic systems theory and complexity theory (DST/CT) is a framework explaining how complex systems change and adapt over time. In psychotherapy, DST/CT can be used to understand how a person's mental and emotional state changes during therapy incorporating higher levels of complexity. This study aimed to systematically review the variability of DST/CT methods applied in psychotherapy research. METHODS: A primary studies search was conducted in the EBSCO and Web of Knowledge databases, extracting information about the analyzed DST/CT phenomena, employed mathematical methods to investigate these phenomena, descriptions of specified dynamic models, psychotherapy phenomena, and other information regarding studies with empirical data (e.g., measurement granularity). RESULTS: After screening 38,216 abstracts and 4,194 full texts, N = 41 studies published from 1990 to 2021 were identified. The employed methods typically included measures of dynamic complexity or chaoticity. Computational and simulation studies most often employed first-order ordinary differential equations and typically focused on describing the time evolution of client-therapist dyadic influences. Eligible studies with empirical data were usually based on case studies and focused on data with high time intensity of within-session dynamics. CONCLUSION: This review provides a descriptive synthesis of the current state of the proliferation of DST/CT methods in the psychotherapy research field.

Objective: This study aimed to develop an updated brief self-report post-session measure, suitable for collecting systematic feedback on clients' session reactions in the context of measurement-based care (MBC). Method: The Session Reactions Scale-3 (SRS-3; 33 items) was developed by extending and adjusting the Revised Session Reactions Scale. In Study 1, the psychometric properties of the SRS-3 were tested on N = 242 clients. In Study 2, a brief version of the SRS-3 (SRS-3-B; 15 items) was developed using a combination of conceptual, empirical, and pragmatic criteria. In Study 3, the psychometric properties of the SRS-3-B were tested on a new sample of N = 265 clients. Results: Exploratory factor analysis supported the use of the SRS-3-B as a two-factor (helpful reactions, hindering reactions) or unidimensional (overall session evaluation) instrument. The SRS-3-B was meaningfully related to another process measure (Individual Therapy Process Questionnaire) both on the item and factor levels. Conclusions: The SRS-3-B is a reliable process measure to elicit rich and clinically meaningful feedback from clients within the MBC context and as a research instrument to assess the helpful and hindering aspects of therapy sessions.

BACKGROUND: Anxiety is a common and disabling condition that significantly impacts quality of life. Subsyndromal anxiety (SSA) refers to anxiety symptoms that do not meet the full diagnostic criteria for an anxiety disorder but pose a risk for developing such disorders. We aimed to provide practical recommendations for the treatment of SSA in primary care settings. METHODS: A narrative review was conducted to identify strategies for recognizing and treating patients with SSA. RESULTS: The recommendations for treating SSA include lifestyle modifications such as exercise and stress reduction techniques, psychotherapy, and pharmacological treatments, including natural compounds like the lavender oil extract Silexan. Regular follow-up care is essential to monitor treatment response and address ongoing symptoms. Additionally, the use of the GAD-7 tool is recommended for accurately identifying patients with SSA. CONCLUSION: Implementing these recommendations in primary care can lead to effective treatment of SSA, preventing the development of more severe anxiety disorders. An integrative approach, combining lifestyle modifications, psychotherapy, and pharmacotherapy, including natural compounds, offers significant benefits for managing anxiety.

Introduction: Effective coping mechanisms and available support systems are essential to managing the disease and maintaining the mental health of women with breast cancer. It has been found that spirituality and religion can be an important supportive element in coping with cancer and its consequences. Aim: To analyse spiritual and religious (s/r) interventions provided by health care professionals to breast cancer patients, and their relationship to physical health, psychosocial, and spiritual outcomes. Methods: Integrative literature review of systematic literature reviews and/or meta-analysis was chosen. PubMed and Web of Science databases for the period 2013-2023 after entering the keywords "spiritual, religious, existential, positive psychology, mindfulness, interventions, breast cancer, cancer" in English were searched. Results: The review included 13 systematic reviews and/or meta-analyses (SRMA). Effect of mindfulness intervention was most frequently analysed. S/r interventions significantly associated with improvements in spiritual and existential well-being, quality of life and personal well-being, hope, optimism, cognitive functions and reductions of anxiety, depression, hopelessness, stress, and fatigue. Two SRMA found that s/r interventions were associated with improvements in cortisol levels, inflammatory cytokine activity, and lymphocyte function. Conclusion: The analysed studies showed that s/r interventions are associated with improved biological, psychosocial and spiritual outcomes, which supports the application of these interventions in clinical practice.

Záměrem sdělení je popsat stručný přehled soudobého vědeckého pojetí selektivního mutismu. Od první průkopnické práce doktora Kussmaula z roku 1877 a pozdější práce dětského psychiatra Tramera z roku 1934 doznalo povědomí o selektivním mutismu významných změn. Tato přehledová studie nabízí souhrn moderních vědeckých poznatků o SM. Hlavním cílem je upozornění na skutečnost, že současné studie a analýzy nabízí zařazení SM mezi neurovývojové poruchy, jelikož je selektivní mutismus heterogenní, komplexní a mnohostranná porucha. Zařazení mezi úzkostné poruchy se nejeví jako zcela přiléhavé. Text zároveň nabízí návrh subtypů pro SM, ze kterého vyplývá nutnost multimodální intervence. V popředí vědeckého zkoumání je nyní snaha odhalit vztah mezi úzkostí a selektivním mutismem: zda jde o etiologický mechanismus, či symptom. Vyjasnění této problematiky by pravděpodobně vedlo k reklasifikaci této relativně vzácné úzkostné poruchy.
The aim of this article is to describe a brief overview of the current scientific concept of selective mutism. Professional awareness of selective mutism has undergone significant changes since the first pioneering work of Dr. Kussmaul in 1877 and the later work of child psychiatrist Tramer in 1934. This review study provides a summary of the current scientific knowledge on selective mutism. The main goal is to draw attention to the fact that current studies and analyzes offer the classification of selective mutism among neurodevelopmental disorders, as selective mutism is a heterogeneous, complex and multifaceted disorder. Classification among anxiety disorders does not appear to be entirely appropriate. At the same time, the text offers a proposal of subtypes for selective mutism, which implies the necessity of a multimodal intervention. At the forefront of scientific research is now the effort to uncover the relationship between anxiety and selective mutism – whether it is an etiological mechanism or a symptom. Clarification of this issue would probably lead to the reclassification of this relatively rare anxiety disorder.

Selektivní mutismus je klasifikován jako méně častá úzkostná porucha. Tento typ poruchy je vysoce rušivý a limituje kvalitu života. Včasné odhalení a vhodně zvolená terapie je proto zásadní. Terapie by měla stát na dvou základních lidských potřebách: VZTAH a BEZPEČÍ. V současnosti se dominantně využívá terapie se zapojením více modalit. Vědecky je potvrzená účinnost zejména behaviorální terapie a využití „nesoustředěné“ komunikace (Oerbeck et al., 2014; Driessen et al., 2020). Multimodální přístup jsme v našem případě rozšířili o senzomotorickou arteterapeutickou metodu Práce v hliněném poli®. Jde o komplexní metodu zaměřenou na tělo a bazální smysly. Jde o tzv. „bottom-up“ terapii. Při jejím využití sledujeme na našem pracovišti významné zmírnění symptomatologie selektivního mutismu u dětí předškolního věku v krátkodobém horizontu. Práce v hliněném poli® by tedy mohla být dalším uznaným terapeutickým přístupem u dětí se selektivním mutismem
