Lumbosacral transitional vertebra associated with sacral spina bifida occulta: a case report

Congenital malformations such as lumbosacral transitional vertebrae and spina bifida occulta constitute unrare anomalies and could affect the symptomatology of low back pain. A transitional vertebra is characterized by elongation of one or both transverse processes, leading to the appearance ofa sacralized fifth lumbar vertebra or a lumbarized first sacral vertebra. Furthermore, sacral spina bifida occulta is a developmental anomaly that corresponds to the incomplete closure of the vertebral column. In the present case report, we describe a case of a dried sacrum presenting a partially sacralized fifth lumbar vertebra and total spina bifida, extended from first to fifth sacral vertebra. A pseudoarthrosis is formed on the left side and the specimen could be incorporated in Castellvi's type IIa. Moreover, the incidence, morphology, clinical and surgical significance of these spinal malformations are discussed.

Exophytic intramedullary mature teratoma of the conus medullaris: case report and review of the literature

BACKGROUND: Intramedullary tumors affect a small but significant portion of patients with spinal tumors. Ependymomas and astrocytomas are the most common entities. The diagnosis of a mature teratoma is extremely rare, although not in the presence of associated developmental abnormalities. The medullary conus is the most common location. Such a case with caudal exophytic growth is presented here and the literature extensively reviewed. CASE PRESENTATION: Low back pain and muscle weakness led to diagnosis of a exophytic intramedullary lesion of the conus medullaris in a 52-year-old woman. After subtotal resection the symptomatology partially improved. Histopathological examination revealed mature teratoma. CONCLUSION: Literature review identified 68 cases of intramedullary teratomas. These should be considered in the differential diagnosis when an associated developmental abnormality is present. Subtotal resection is a valid alternative to radical tumor removal when neurological function is at risk. The prognosis of adult patients with intramedullary mature teratoma is excellent. Prenatal diagnosis of occipital dermal sinus associated with hemangioma using ultrasound and MRI : case report

OBJECTIVE: We report a case of prenatal diagnosis and postpartum management of a subcutaneous tumor without intracranial communication. METHODS: An occipital tumor without intracranial communication was found on ultrasound scan in the 21st week of pregnancy. Using MRI, the diagnosis was confirmed. Subcutaneous localization of the tumor was verified and communication with intracranial space excluded. RESULTS: The newborn was delivered spontaneously at term and underwent a successful surgical procedure 5 months postpartum. CONCLUSION: The prenatal diagnosis allowed differentiation between a communicating neural tube defect with poor prognosis, and a manageable extracranial subcutaneous tumor. The precise diagnosis of uncomplicated dermal sinus was possible only after the histological evaluation of the tumor confirmed occipital dermal sinus associated with hemangioma.
