Terapie je založena na přesném odlišení organické a funkční chronické obstipace. Léčebná opatření jsou stále založena hlavně na empirických zkušenostech. Terapie je dlouhodobá a obtížná. Nedosažení trvalého léčebného efektu a recidivy jsou v běžné praxi způsobeny hlavně nepřesnou charakteristikou obstipace a nedostatečnou pozorností věnovanou léčbě. Terapie je komplexní a zahrnuje pečlivé vstupní poučení dítěte a rodiny, pravidelné kontroly, režimová opatření, odstranění akumulace stolice, změnu konzistence stolice a navození defekačního reflexu.
