Míšní šok nastává náhlým přerušením descendentních supraspinálních drah. Termín míšní šok tedy popisuje stav, kdy dojde většinou v důsledku traumatu, ischemie, hemoragie či zánětu k náhlému výpadku nervových funkcí pod úrovní akutní míšní léze. Klinicky dominuje dočasná ztráta nebo pokles míšní reflexní aktivity pod úrovní léze, hypotonie a porucha motorických, senzitivních a autonomních funkcí. Pro objasnění mechanizmu vzniku míšního šoku a následného návratu reflexů až rozvoje hyperreflexie je zásadní pochopení změn, ke kterým dochází v oblasti motoneuronu pod úrovní míšní léze. Tyto změny probíhají ve třech hlavních fázích. Ke snížení reflexní aktivity dochází v důsledku poklesu excitability míšních motoneuronů. Obnovení reflexní aktivity a případně volního pohybu je ve fázi návratu reflexů připisováno denervační hypersenzitivitě, ve fázi hyperreflexie pak růstu nových synapsí. Motoneuron pod úrovní míšní léze se v důsledku popsaných změn dostává dominantně buď pod volní, nebo reflexní kontrolu. Výsledkem je rozvoj různě závažné formy spasticity a u nekompletních míšních lézí návrat volní hybnosti v závislosti na rozsahu míšního poškození.
Spinal shock occurs as a consequence of an abrupt interruption of descending supraspinal pathways. The term spinal shock describes a phenomenon when a trauma, ischemia, hemorrhage or inflammation results in sudden loss of neurological functions below the level of the injury. Clinically, transient loss or decrease of reflex activity below the level of the injury, hypotonia and disturbance of motor, sensory and autonomic functions predominate. In order to clarify the mechanism behind the spinal shock and subsequent return of reflex activity or even hyperreflexia, we need to understand the changes taking place in motor neurons below the level of the injury. These changes consist of three main phases. Reduction of reflex activity results from decreased excitability of spinal motor neurons. Resumption of reflex activity or voluntary movement is attributed to denervation hypersensitivity and hyperreflexia occurs due to new synaptic growth. Consequently, the motor neuron below the level of injury is predominantly under either voluntary or reflex control. As a result, various levels of spasticity develop or, in case of an incomplete lesion, voluntary movement might be recovered, depending on the extent of spinal cord injury. The recently proposed adaptor protein 4 (AP-4) deficiency syndrome comprises a group of congenital neurological disorders characterized by severe intellectual disability (ID), delayed or absent speech, hereditary spastic paraplegia, and growth retardation. AP-4 is a heterotetrameric protein complex with important functions in vesicle trafficking. Mutations in genes affecting different subunits of AP-4, including AP4B1, AP4E1, AP4S1, and AP4M1, have been reported in patients with the AP-4 deficiency phenotype. We describe two siblings from a non-consanguineous couple who presented with severe ID, absent speech, microcephaly, growth retardation, and progressive spastic tetraplegia. Whole-exome sequencing in the two patients identified the novel homozygous 2-bp deletion c.1160_1161delCA (p.(Thr387Argfs*30)) in AP4B1. Sanger sequencing confirmed the mutation in the siblings and revealed it in the heterozygous state in both parents. The AP4B1-associated phenotype has previously been assigned to spastic paraplegia-47. Identification of a novel AP4B1 alteration in two patients with clinical manifestations highly similar to other individuals with mutations affecting one of the four AP-4 subunits further supports the observation that loss of AP-4 assembly or functionality underlies the common clinical features in these patients and underscores the existence of the clinically recognizable AP-4 deficiency syndrome.

STUDY DESIGN: Case report. CONTEXT: In patients with a complete spinal cord injury (SCI) above T6 level, autonomic dysreflexia (AD) can be the only alerting sign of complications below the level of injury. A case report is presented of a patient with tetraplegia who progressively developed an AD syndrome after falling from a wheelchair. Initially, he was treated for symptomatic urinary tract infection and only later an unstable pelvic fracture was detected. FINDINGS: A patient with chronic tetraplegia fell from his wheelchair while intoxicated. After the fall, he showed no signs of injury. Two days later, AD symptoms appeared while lying on his side or sitting. He presented to his community-based physician and received antibiotics for concomitant urinary tract infection. Because of persisting complaints, the patient was referred to the Spinal Cord Unit. Once the history of falling from the wheelchair was made known, symptoms of AD were suggestive of an internal injury. Radiography showed pelvic fracture and conservative treatment was administered. The fractures healed in 3 months, the therapeutic bed rest regimen was relaxed without trigerring AD symptoms. CONCLUSION: AD symptoms can suggest complications below the SCI level. Awareness and recognition of these symptoms are of utmost importance. However, etiology may be misleading. The role of a community-based general physician is to recognize the right moment when the need of a specialized assessment in a Spinal Cord Unit arises. In addition, patients should also be well informed about the most common possible complications from specialized SCI centers.

Do té doby zcela zdravý, 32letý muž byl nalezen tetraplegický a nedýchající na místě pádu z korby nákladního automobilu. Po zabezpečeném transportu na příjmový úsek nemocnice byla stanovena diagnóza tříúlomkové zlomeniny atlasu Jeffersonova typu společně se zlomeninou zubu čepovce II. typu. Doplněné vyšetření magnetickou rezonancí zobrazilo míšní otok v rozsahu C0–C3. Vzhledem k nepříznivé oběhové situaci pacienta jsme k chirurgické stabilizaci předním přístupem mohli přikročit až třetí poúrazový den. Stabilizace spočívala v kombinaci přímé osteosyntézy zubu čepovce a v anterolaterální transartikulární déze C2–C1. V průběhu pooperační pasivní rehabilitace pacient začal 10. den po traumatu interferovat s umělou plicní ventilací a elevoval ramena. Ústup těžkého neurologického deficitu však pokračoval i nadále, takže po třech týdnech jsme měli možnost konstatovat suficientní spontánní dýchání a polykání. Regrese tetraplegie pacientovi postupně umožnila aktivní rehabilitaci horních končetin a nácvik chůze s výstupní samostatnou chůzí za lehkého patologického stereotypu. Tento mimořádně pozitivní klinický výsledek je argumentem pro aktivní terapeutický přístup včetně chirurgické intervence, a to i v případě pacientů se vstupně značně nepříznivými prognostickými ukazateli klinického stavu.
Otherwise healthy 32-year-old man was found quadriplegic and without any signs of breathing at a site of his fall from a body of a truck. The diagnosis of three fragment Jefferson's type fracture of atlas together with incidental type II fracture of the dens of C2 was made at an emergency unit were the mechanically ventilated patient was transported. MRI subsequently revealed C0–C3 spinal cord oedema. Surgical stabilization of the fracture of both uppermost cervical vertebras was postponed to the third day after the trauma due to the patient's haemodynamic instability. The circulatory system instability was also the reason for selecting the anterior approach. A combination of anterolateral transarticular screws together with a single screw for direct osteosynthesis of the dens stabilized the C1–C2 complex. At day 10 of postsurgical passive physiotherapy, the patient began to interfere with the mechanical ventilation. Gradual reversal of neurological deficit continued, so that three weeks after the trauma, the patient developed a sufficient spontaneous breathing. Improving paraparesis allowed him to walk. The patient, walking just with a pathological gait stereotype, continued his functional treatment at a specialized rehabilitation institute eight weeks after the accident. This extremely positive outcome supports the use of a proactive and early surgical approach even in patients with complete spinal cord injury at the upper cervical levels.
