BACKGROUND: Cleft lip with cleft palate (CLP) is a congenital condition that affects both the oral cavity and the lips. This study estimated the prevalence and mortality of CLP using surveillance data collected from birth defect registries around the world. METHODS: Data from 22 population- and hospital-based surveillance programs affiliated with the International Clearinghouse for Birth Defects Surveillance and Research (ICBDSR) in 18 countries on live births (LB), stillbirths (SB), and elective terminations of pregnancy for fetal anomaly (ETOPFA) for CLP from 1974 to 2014 were analyzed. Prevalence and survival (survival for LB only) estimates were calculated for total and subclassifications of CLP and by pregnancy outcome. RESULTS: The pooled prevalence of total CLP cases was 6.4 CLP per 10,000 births. The prevalence of CLP and all of the pregnancy outcomes varied across programs. Higher ETOPFA rates were recorded in most European programs compared to programs in other continents. In programs reporting low ETOPFA rates or where there was no ascertainment of ETOPFA, the rate of CLP among LB and SB was higher compared to those where ETOPFA rates were ascertained. Overall survival for total CLP was 91%. For isolated CLP, the survival was 97.7%. CLP associated with multiple congenital anomalies had an overall survival of 77.1%, and for CLP associated with genetic/chromosomal syndromes, overall survival was 40.9%. CONCLUSIONS: Total CLP prevalence reported in this study is lower than estimates from prior studies, with variation by pregnancy outcomes between programs. Survival was lower when CLP was associated with other congenital anomalies or syndromes compared to isolated CLP.

BACKGROUND: Choosing the optimal season for conception is a part of family planning since it can positively influence the pregnancy outcome. Changes in the monthly number of infants born with a birth defect can signal prenatal damage - death or malformation - related to a harmful seasonal factor. The aim of our paper was to search for possible seasonal differences in the numbers of new-borns with an orofacial cleft and thus for a period of conception that can increase the risk of orofacial cleft development. METHODS: Mean monthly numbers of live births in the Bohemia region of the Czech Republic during the years 1964-2000 were compared within a group of 5619 new-borns with various types of orofacial clefts and the control group derived from natality data on 3,080,891 new-borns. RESULTS: The control group exhibited regular seasonal variation in the monthly numbers of new-borns: significantly more babies born during March-May and fewer babies born during October-December. Similar natural seasonal variation was also found in the group of babies with an orofacial cleft. However, after subdividing the cleft group according to gender and cleft type, in comparison to controls, significant differences appeared in the number of new-born girls with cleft lip during January-March and in the number of boys born with cleft palate in April - May. CONCLUSIONS: We found significant differences from controls in the number of new-born girls with CL and boys with CP, whose dates of birth correspond to conception from April to August and to the estimated prenatal critical period for cleft formation from May to October. The latter period includes the warm season, when various injurious physical, chemical and biological factors may act on a pregnant woman. This finding should be considered in pregnancy planning. Future studies are necessary to investigate the putative injurious factors during the warm season that can influence pregnancy outcome.

BACKGROUND: Over the past 18 years, according to data from the Clinic of Plastic and Reconstructive Surgery, L. Pasteur University Hospital, there have been 493 cases of orofacial clefts (OC) reported in the area of Eastern Slovakia. The aim of this study was to map the occurrence of orofacial clefts reported in the area of Eastern Slovakia during the years 1996-2013. Also, we compared the occurrence of different types of clefts between the groups in relation to gender and ethnicity. METHODS AND RESULTS: The statistical analysis shows relationship between variables of location and gender and gender differences in the occurrence of various types of clefts. Moreover, in comparison with another study which analyzed the years 1985-2000 (1.29/10(3) live births), there was an increase in the incidence (1.42/10(3) live births) of OC in Eastern Slovakia. CONCLUSION: Our findings seem contradictive to similar studies which discuss ethnic differences in relation to OC. We recognize the relatively high occurrence of OC in Eastern Slovakia, and we link this phenomenon to several extrinsic factors, in particular socioeconomic status and embryotoxic factors.

OBJECTIVE: To determine the incidence of nonsyndromic cleft lip and/or palate in the Czech Republic among infants born during 1994-2008 as well as to define the ratio per live births and sex ratios. Another aim was to determine whether there was any trend in the incidence in this time period. DESIGN: Retrospective study. SETTING: Prague Center for the Treatment of Congenital Facial Anomalies. MATERIAL AND METHODS: Data were collected from the National Birth Defects Register (Institute of Health Information and Statistics), the Czech Statistical Office and the Czech Health Statistics Yearbooks. The incidence, ratios per live births and sex differences were calculated. Possible trends in the data series were investigated. RESULTS: Totally 2417 infants with a cleft defect were found among 1 471 789 newborns in time period 1994-2008. The overall incidence was 1.64 per 1000 live births or 1 in 600 live births. The incidence of cleft lip was 0.39/1000, the incidence of cleft palate was 0.68/1000 and the incidence of cleft lip and palate was 0.57/1000. The ratio per live births was 1 in 2648 in cleft lip, 1 in 1801 in cleft lip and palate and 1 in 1505 in cleft palate. The incidence was much higher among male babies in cleft lip patients (male to female ratio 2.07) and in patients with cleft lip and palate (males to females ratio 1.85). The male to female ratio in cleft palate newborns was 0.92. No traceable trend was found in the incidence. CONCLUSIONS: In the Czech Republic, every year approximately 170 infants with cleft lip and/or palate were born, the incidence was 1.64 per 1000 live born infants during considered period. Males were affected more frequently with isolated cleft lip and cleft lip and palate, on the other hand, in cleft palate patients, there was a slight dominance of females. Further long-term studies are necessary in order to elucidate reasons of the oscillations in incidence of cleft anomalies, to identify possible teratogens and to give a starting indication for planning health service resource requirements for this group of affected infants (Tab. 3, Fig. 4, Ref. 51).

Cíl. Oronazální dutina se u člověka vyvíjí během 30.–60. dne embryonálního vývoje. V tomto období existují tři kritické periody, ve kterých tento proces může být narušen a důsledkem je vznik určitého druhu rozštěpu: rozštěp rtu (CL), rozštěp patra (CP) nebo nejtěžší postižení – celkový rozštěp (CLP). Posuzovali jsme, zda časné gestační krvácení může způsobovat nespecifické zhoršení stavu embrya, jehož důsledkem je vznik obličejového rozštěpu od rozštěpu rtu nebo patra až po nejzávažnější celkový rozštěp. Metodika. U skupiny dětských pacientů s orofaciálními rozštěpy bylo spektrum rozštěpů porovnáváno s gestačním krvácením v prvním trimestru udávaném matkou. Získaná data byla rovněž vztažena k pohlaví pacienta, dědičným faktorům a případným dalším malformacím. Výsledky. Ve skupině 2 524 matek, kterým se v České republice narodilo dítě s obličejovým rozštěpem v letech 1983–2009, mělo 253 (10 %) časné gestační krvácení. Ve skupině postižených dětí se u 497 (19,7 %) vyskytoval orofaciální rozštěp mezi příbuznými a 297 (11,8 %) bylo postiženo další vrozenou vadou. Ve skupině krvácejících matek při srovnání s matkami, které nekrvácely, byl výrazně vyšší počet dětí s CLP (p < 0,01) na úkor dětí s CP, jejichž počet se výrazně snížil (p < 0,01). Ve skupině dětí, kde se vyskytovaly rozštěpy mezi příbuznými, jsme nenalezli signifikantní rozdíly ve vztahu ke krvácení. Časné gestační krvácení bylo častější u matek dětí s dalšími malformacemi, ale tento rozdíl nebyl signifikantní (p = 0,112). Závěr. Předokládáme, že rozsah a tedy závažnost orofaciálního rozštěpu se může zvyšovat na základě hypoxie v důsledku gestačního krvácení.
Objective: The oronasal cavity in humans develops during embryonic day 30–60. There are three critical periods when this process can be affected, resulting in a specific type of orofacial cleft: cleft lip (CL), cleft palate (CP), or most serious, total cleft lip+palate (CLP). We assessed whether gestational bleeding during early pregnancy might act to produce a non-specific worsening of embryo status resulting in extension of the basic cleft type (CL or CP) into more serious CLP. Study design: In a group of the child patients with orofacial clefts, the cleft spectrum was correlated with first trimester gestational bleeding reported by the mother. Data were also related to the gender of patients, hereditary factors and additional malformations. Results: Among 2524 mothers who gave birth to babies with an orofacial cleft in the Czech Republic during 1983–2009, 253 (10.0%) had gestational bleeding. Among the children with an orofacial cleft, 497 (19.7%) had an orofacial cleft among relatives and 297 (11.8%) exhibited an additional congenital malformation. In comparison with mothers without bleeding, there was significant increase of children with CLP (p < 0.01) at the expense of children with CP, whose number significantly decreased (p < 0.01) in the bleeding mothers. In the group of children with clefts among relatives we did not find any significant change associated with bleeding. The maternal bleeding was more frequent in children with additional malformations, but this difference was not significant (p = 0.112). Conclusion: We hypothesize that size/extent and therefore seriousness of orofacial cleft might increase as a consequence of hypoxia resulting from gestational bleeding.

Orofaciální rozštěpy se dělí na tři základní typy: rozštěp rtu (CL), rozštěp patra (CP) a rozštěp rtu i patra (CLP). Incidence jednotlivých typů rozštěpů v populaci a jejich relativní počet vykazuje specifické etnické a rasové rozdíly. Doposud však nemáme žádná data o incidenci a relativním počtu orofaciálních typů rozštěpů (CL, CLP, CP) u romské etnické skupiny. Cílem této studie bylo porovnat relativní počet jednotlivých typů rozštěpů mezi českou romskou a neromskou populací. Provedli jsme retrospektivní epidemiologickou studii u skupiny všech žijících pacientů s orofaciálním rozštěpem, kteří se narodili v České republice od roku 1964 do roku 2002. Pacienti s rozštěpem byli rozděleni do tří skupin: 5304 dětí, kde oba rodiče nejsou Romové (NN), 98 dětí, kde oba rodiče jsou Romové (GG), a 18 dětí, kde pouze jeden rodič byl Rom (NG). U skupiny NN pacientů byl nalezen relativní počet CP v 37,1 %, avšak u skupiny GG byl relativní počet pacientů s CP signifikantně redukován na 5,1% (P<0,01). Naopak relativní počet pacientů s CLP byl vyšší (P<0,01) u skupiny GG (62,2%) v porovnání se skupinou NN (39,2%). Tendence k poklesu relativního počtu CP a ke zvýšení relativního počtu CLP byla patrná také u NG skupiny pacientů, ale rozdíly nebyly signifikantní. Vyslovili jsme hypotézu, že pokles CP a vzestup CLP i CL u Romů může být způsoben genetickou predispozicí ke vzniku CL. Protože CP se vyvíjí během embryonálního vývoje později nežli CL, část rozštěpů patra postihne embrya s již přítomným rozštěpem rtu a vznikne CLP. Postnatální chybění pacientů s izolovaným CP by tak mohlo být vysvětleno jejich přesunem do skupiny pacientů s CLP.
Orofacial clefts are usually divided into three basic types: isolated cleft lip (CL), cleft lip and palate (CLP) and isolated cleft palate (CP). The incidence of specific cleft types in a population and their relative numbers show specific differences between ethnic groups and races. However, there are no available data about the incidence and relative numbers of orofacial cleft types (CL, CLP, CP) in the gypsy ethnic group. The aim of this study was to compare relative numbers of specific types of orofacial clefts between the Czech gypsy and non-gypsy populations.We conducted a retrospective epidemiological study using a set of all living patients with orofacial clefts born in the Czech Republic from 1964 until 2002. The cleft patients were subdivided into three groups: 5304 non-gypsy children, both parents of whom were non-gypsies (NN), 98 gypsy children, both parents of whom were gypsies (GG) and 18 children with one parent non-gypsy and one parent gypsy (NG). The relative number of isolated CP was 37.1% in NN children. However, the relative number of CP was significantly reduced to 5.1% (P<0.01) in the GG group. Conversely, the relative number of CLP was higher (P<0.01) in the GG group (62.2%) in comparison to the NN group (39.2%). The tendency to decrease in the relative number of CP and increase in the relative number of CLP was also apparent in the NG group, but not so well expressed.We hypothesize that the decrease in CP and increase in CLP and CL in gypsies might be caused by their genetic predisposition to CL. Since the CP originates later than CL during embryonic development, some CP arise in embryos with already existing CL giving rise to CLP. Consequently, the missing isolated CP might be hidden in the group of CLP patients postnatally.
