BACKGROUND: Sarcoidosis is a systemic inflammatory disease of unknown etiology, which can affect almost any organ. Cardiac involvement determines the prognosis of the affected individuals. Its prevalence in patients with extracardiac sarcoidosis with the absence of cardiac symptoms remains unclear. Cardiac magnetic resonance (CMR) provides excellent diagnostic accuracy in the detection of heart involvement by sarcoidosis. AIM: We sought to determine the prevalence of cardiac sarcoidosis in asymptomatic individuals with newly diagnosed extracardiac sarcoidosis using CMR. METHODS: We prospectively evaluated 55 consecutive patients including 23 women with newly diagnosed extracardiac sarcoidosis who underwent contrast-enhanced CMR and had no symptoms of heart disease. The mean (standard deviation) age of patients was 43 (11) years. The presence of myocardial late gadolinium enhancement (LGE) of non-ischemic etiology on CMR examination was considered diagnostic for cardiac sarcoidosis. RESULTS: In 3 (6%) patients, the LGE pattern consistent with cardiac sarcoidosis was detected. In all patients, preserved left ventricular systolic regional and global function was present, and in none of them, the elevation of blood biomarkers of myocardial injury or overload was found. CONCLUSIONS: Our study suggests that the prevalence of cardiac involvement in patients with newly diagnosed extracardiac sarcoidosis and no symptoms of heart disease is very low as assessed by CMR. However, CMR may be considered as part of routine evaluation of patients with extracardiac sarcoidosis due to its higher diagnostic yield in comparison with echocardiography and electrocardiography, respectively.

Sarkoidóza je systémové onemocnění neznámé etiologie, které je charakterizováno přítomností granulomatózního zánětu v postižených tkáních. Projevuje se nejčastěji nitrohrudním postižením s bilaterální hilovou lymfadenopatií a/nebo plicními infiltráty. Diagnózu sarkoidózy lze postavit na typickém klinickém a radiologickém obrazu, histologickém průkazu epiteloidních nekaseifikujících granulomů a současném vyloučení jiných onemocnění s obdobnými klinickými projevy a histologickými nálezy. Za jednoznačnou indikaci k léčbě se považují symptomatické formy (dušnost, kašel, bolesti na hrudníku) stadií II, III a IV. Z dalších indikací jsou to plicní hypertenze, perzistující hyperkalcemie a hyperkalciurie, postižení oka, srdce, neurosarkoidóza, deformující sarkoidóza kůže a jiné vážné orgánové změny. V mnoha případech se doporučuje s nasazením léčby vyčkat a sledovat aktivitu onemocnění a ovlivnění kvality života.
Sarcoidosis is a systemic disease of unknown etiology that is characterized the presence of granulomatous inflammation in the affected tissues. Manifests most often intrathoracic disorders with bilateral hilar lymphadenopathy and/or pulmonary infiltrates. The diagnosis of sarcoidosis can be based on a typical clinical and radiological picture, histological evidence of epithelioid non-caseinating granulomas and the concomitant exclusion of other diseases with similar clinical manifestations and histological findings are considered clear indications of treatment symptomatic forms (dyspnea, cough, chest pain) stages II, III and IV. Other indications are pulmonary hypertension, persistent hypercalcemia, hypercalciuria, eye disease, heart, neurosarcoidosis, deforming skin sarcoidosis and other serious organ changes. In many cases, it is recommended with the use of treatment, wait and monitor the activity of the disease and affect the quality of life.

BACKGROUND: Sarcoidosis is a systemic granulomatous disease affecting different organs including the heart. Myocardial strain analysis could potentially detect the early stages of cardiac dysfunction in sarcoidosis patients. The present study aims to assess the use of cardiac magnetic resonance (CMR) strain analysis using feature tracking (FT) in the detection of early cardiac involvement in asymptomatic patients with sarcoidosis. METHODS: One hundred and thirteen CMR studies of patients with sarcoidosis of the respiratory tract and/or extrapulmonary sarcoidosis without pre-existing known cardiovascular disease were included in the study and analysed using FT and compared to 22 age and gender-matched controls. Global longitudinal strain (GLS), global circumferential strain (GCS) and global radial strain (GRS) of the left ventricle (LV) were measured. RESULTS: The sarcoidosis patients did not significantly differ from the controls in basic demographic data and had normal global and regional systolic LV function-LV ejection fraction (EF) 66 ± 7% vs 65 ± 5% in the controls (p = NS). No statistically significant differences were found in all strain parameters between patients and controls: GLS (- 13.9 ± 3.1 vs. - 14.2 ± 2.5), GCS (- 23.4 ± 4.0 vs. - 22.2 ± 2.9) and GRS (53.4 ± 13.5 vs. 51.2 ± 13.6%) (p = NS). CONCLUSION: Patients with sarcoidosis of the respiratory tract and/or extrapulmonary sarcoidosis had normal myocardial deformation measured by CMR-FT derived global strain.

Příznaky, se kterými se může setkat lékař prvního kontaktu, mohou být velmi nespecifické. Nejtypičtější potíže se objevují u akutní formy nemoci, tzv. Löfgrenova syndromu s typickou bilaterální hilovou lymfadenopatií zjišťovanou na skiagramu hrudníku, s negativním tuberkulinovým testem, s výsevem nodózního erytému na bércích, často provázeném oboustrannou artritidou talokrurálního skloubení. Löfgrenův syndrom má obvykle dobrou prognózu, ve většině případů nastává spontánní remise onemocnění. Při podezření na toto onemocnění je indikováno doplnění skiagramu hrudníku a při patologickém nálezu je vhodné odeslat nemocného k pneumologovi.
Sarcoidosis is a multisystem granulomatous disease of unknown aetiology that typically affects middle-aged and elderly individuals, most commonly women over 40 years of age. The symptoms that a primary care physician may encounter can be very non-specific. The most characteristic complaints occur in the acute form of the disease, Löfgren syndrome, presenting with a typical bilateral hilar lymphadenopathy detected on chest skiagram, a negative tuberculin test, and an eruption of erythema nodosum in the crural region, often accompanied by bilateral arthritis of the talocrural joint. Löfgren syndrome is usually associated with a good prognosis, with most cases developing spontaneous remission of the disease. When this disease is suspected, an additional chest skiagram is indicated and, in the case of a pathological finding, it is advisable to refer the patient to a pulmonologist.

Pozitronová emisní tomografie kombinovaná s výpočetní tomografií (PET/CT) je zobrazovací modalitou užívanou především v onkologické diagnostice. Nejčastěji používaným radiofarmakem je 2-deoxy-2-(18F)fluoro-D-glukóza (FDG). Vzhledem k nespecifické povaze své distribuce umožňuje užití i v diagnostice zánětu, kdy její zvýšená akumulace reflektuje nárůst metabolického obratu v aktivovaných buněčných elementech zprostředkujících zánětlivou reakci. V neperfuzní kardiologické diagnostice se FDG PET/CT uplatňuje především při detekci zánětlivých postižení srdečních kompartmentů, popř. implantabilních materiálů. Vzácně může přispět při lokalizaci neoplastického postižení srdce. V přehledovém článku jsou prezentovány aktuální literární poznatky a zkušenosti z klinické praxe. © 2020, ČKS.
The positron emission tomography combined with computer tomography (PET/CT) is a well established imag- ing modality in the oncological practice. The most frequently used radiopharmaceutical is 2-deoxy-2-(18F)fluoro-D-glucose (FDG). Due to the unspecific nature of its biodistribution high uptake of FDG reflects also increased metabolic turnover in the cells mediating inflammatory reaction. The most important role of FDG PET/CT in cardiac non-perfusion imaging is in the detection of the inflammation of the heart compartments, ev. implantable devices. The neoplastic diseases of the heart represent another (less frequent) possible role for PET imaging. A review of literature and clinical experiences are presented in this review article.

INTRODUCTION: Sarcoidosis is a systemic granulomatous disease affecting in particular the respiratory tract. Cardiac magnetic resonance (CMR), including a measurement of T1 relaxation time, could potentially detect early stadia of sarcoidosis of the heart. The study aims to assess T1 mapping in the detection of early cardiac involvement in asymptomatic patients with sarcoidosis. METHODS: One hundred and twenty patients with extracardiac sarcoidosis and without any heart disease history were included. One hundred and thirteen of them underwent a CMR examination. The mean time from the diagnosis of sarcoidosis was 0.8 (0.2-3.3) years. Cine images for the assessment of left ventricular (LV) functional parameters and pre- and post-contrast saturation method using adaptive recovery times for cardiac T1 mapping (SMART1 Map) and modified Look-Locker inversion recovery (MOLLI) images were acquired for the assessment of native T1 relaxation time and extracellular volume (ECV). The measured parameters were compared between sarcoidosis patients and 22 controls. RESULTS: The sarcoidosis patients had normal global and regional systolic LV function-LV ejection fraction 65 ± 5% versus 66 ± 7% (p NS). The mean native T1 relaxation times were not prolonged-1465 ± 93 ms versus 1480 ± 88 ms (p NS) measured by SMART1 Map and 1317 ± 60 ms versus 1313 ± 83 ms (p NS) measured using a MOLLI sequence. Similarly, the mean ECV values did not increase-16.9 ± 3.9% versus 17.9 ± 3.7% (p NS) measured by SMART1 Map and 30.9 ± 2.9% versus 31.6 ± 8.3% (p NS) measured using a MOLLI sequence. CONCLUSION: Myocardial native T1 relaxation times were not prolonged and ECV was not increased in asymptomatic patients with extracardiac sarcoidosis.
