The development of acute respiratory distress syndrome (ARDS) is known to be independently attributable to aspiration-induced lung injury. Mechanical ventilation as a high pressure/volume support to maintain sufficient oxygenation of a patient could initiate ventilator-induced lung injury (VILI) and thus contribute to lung damage. Although these phenomena are rare in the clinic, they could serve as the severe experimental model of alveolar-capillary membrane deterioration. Lung collapse, diffuse inflammation, alveolar epithelial and endothelial damage, leakage of fluid into the alveoli, and subsequent inactivation of pulmonary surfactant, leading to respiratory failure. Therefore, exogenous surfactant could be considered as a therapy to restore lung function in experimental ARDS. This study aimed to investigate the effect of modified porcine surfactant in animal model of severe ARDS (P/F ratio =13.3 kPa) induced by intratracheal instillation of hydrochloric acid (HCl, 3 ml/kg, pH 1.25) followed by VILI (V(T) 20 ml/kg). Adult rabbits were divided into three groups: untreated ARDS, model treated with a bolus of poractant alfa (Curosurf®, 2.5 ml/kg, 80 mg phospholipids/ml), and healthy ventilated animals (saline), which were oxygen-ventilated for an additional 4 h. The lung function parameters, histological appearance, degree of lung edema and levels of inflammatory and oxidative markers in plasma were evaluated. Whereas surfactant therapy with poractant alfa improved lung function, attenuated inflammation and lung edema, and partially regenerated significant changes in lung architecture compared to untreated controls. This study indicates a potential of exogenous surfactant preparation in the treatment of experimental ARDS.

BACKGROUND: Since December 2019, SARS-CoV-2 virus has infected millions of people worldwide. In patients with COVID-19 pneumonia in need of oxygen therapy or mechanical ventilation, dexamethasone 6 mg per day is currently recommended. However, the dose of 6 mg of dexamethasone is currently being reappraised and may miss important therapeutic potential or may prevent potential deleterious effects of higher doses of corticosteroids. METHODS: REMED is a prospective, open-label, randomised controlled trial testing the superiority of dexamethasone 20 mg (dexamethasone 20 mg on days 1-5, followed by dexamethasone 10 mg on days 6-10) vs 6 mg administered once daily intravenously for 10 days in adult patients with moderate or severe ARDS due to confirmed COVID-19. Three hundred participants will be enrolled and followed up for 360 days after randomization. Patients will be randomised in a 1:1 ratio into one of the two treatment arms. The following stratification factors will be applied: age, Charlson Comorbidity Index, CRP levels and trial centre. The primary endpoint is the number of ventilator-free days (VFDs) at 28 days after randomisation. The secondary endpoints are mortality from any cause at 60 days after randomisation; dynamics of the inflammatory marker, change in WHO Clinical Progression Scale at day 14; and adverse events related to corticosteroids and independence at 90 days after randomisation assessed by the Barthel Index. The long-term outcomes of this study are to assess long-term consequences on mortality and quality of life at 180 and 360 days. The study will be conducted in the intensive care units (ICUs) of ten university hospitals in the Czech Republic. DISCUSSION: We aim to compare two different doses of dexamethasone in patients with moderate to severe ARDS undergoing mechanical ventilation regarding efficacy and safety. U části pacientů po těžkém průběhu onemocnění covidem-19 nedochází ke spontánní resorpci (po) zánětlivých změn a je zde riziko rozvoje organizující se pneumonie a fibrotizace plicního parenchymu. Přes nedostatek důkazů o jejich efektivitě v této indikaci je použití glukokortikoidů v léčbě plicního postižení v rámci post-covidového syndromu běžnou praxí. Studií zabývajících se jejich použitím v pozdní fázi onemocnění je bohužel málo. Na základě dostupných prací se zdá, že v případě neregredujících infiltrátů s větším rozsahem než 15 % plicního parenchymu a za současné nepřítomnosti kontraindikace kortikoterapie je jejich použití opodstatněné. Článek shrnuje význam kortikoterapie v léčbě a prevenci plicního post-covidového postižení.
Prolonged resorption occurs in the small number of patients with severe course of COVID-19. There is the risk of development of organizing pneumonia and subsequent fibrotization of lung parenchyma. The use of glucocorticoids in the treatment of post-COVID lung involvement is common practice, despite the lack of evidence regarding their efficacy in this indication. The number of studies dealing with their use in late phase of the disease is unfortunately small. According to current scarce evidence their use may be justified in case of 15% or larger lung involvement without spontaneous resolution. Of course, the absence of contraindications for their use is required. The article focuses on the position of glucocorticoids in the prevention and therapy of lung involvement in post-COVID syndrome patients.

Lidský koronavirus SARS‑CoV- 2 (Severe Acute Respiratory Coronavirus 2) představuje, kvůli schopnosti způsobovat postižení plic u nezanedbatelného procenta postižených, zásadní zdravotnický a potažmo společenský problém. Od propuknutí pandemie koncem roku 2019 byla testována řada léčiv ovlivňujících průběh onemocnění. Až na několik případů však nebyl jejich pozitivní účinek prokázán, navíc v dostatečně kvalitních studiích. Jednou z takových výjimek jsou kortikoidy aplikované u nemocných s těžším stupněm oxygenační poruchy. Kortikoidy mají potenciál příznivě ovlivňovat plicní poškození a reparaci plicních funkcí díky modulaci zánětlivé odpovědi zprostředkované přes glukokortikoidní receptory. Jejich účinek byl potvrzen v několika velkých randomizovaných studiích a v současnosti jsou brány jako nedílná součást komplexní péče o pacienty s pneumonií vyvolanou virem SARS‑CoV- 2, kteří vyžadují oxygenační podporu či umělou plicní ventilaci. Existuje však značná nejistota ohledně optimální dávky kortikoidů napříč takto širokým spektrem pacientů. Data z dřívějších studií u pacientů se syndromem akutní respirační tísně (ARDS) nekoronavirové etiologie svědčí spíše pro vyšší dávky kortikoidů, než byly užity v pracích u pacientů s nemocí vyvolanou SARS‑CoV- 2 (covid-19, coronavirus disease 2019). Stejně tak není jasné optimální načasování a volba konkrétního přípravku. Na tyto otázky by mohla v dohledné budoucnosti pomoci odpovědět právě probíhající klinická hodnocení.
Human coronavirus SARS-CoV-2 (Severe acute respiratory coronavirus 2), due to its ability to cause lung injury in significant percentage of patients, represents great medical and even social problem. Since the outbreak of the pandemic at the end of 2019, many new therapeutics have been tested. Unfortunatelly, real benefit in high quality studies was confirmed in just a few of them. One of these exceptions is represented by corticosteroids used in patients with advanced grade of oxygenation failure. Corticosteroids have favorable influence on lung injury and potentially facilitate reparation of pulmonary functions by modulating inflammatory response mediated through glucocorticoid receptors. Their benefit was confirmed in several randomized trials and they actually represent an integral part of complex care in those patients with pneumonia caused by SARS-CoV-2, who are in need of oxygenotherapy or mechanical ventilation. Nevertheless, uncertainty exists regarding their optimal dose in such a broad spectrum of patients. Data from former studies performed in non coronavirus patients with Acute Respiratory Distress Syndrome (ARDS) favor rather larger doses of steroids than those tested in covid-19 patients. Also optimal timing and choice of specific corticosteroid remains unknown. Ongoing clinical trials could provide answers to these relevant questions.
The FDA-approved drugs raloxifene and bazedoxifene could be among the best candidates to prevent mortality in severe COVID-19 patients. Raloxifene and bazedoxifene inhibit IL-6 signaling at therapeutic doses, suggesting they have the potential to prevent the cytokine storm, ARDS and mortality in severe COVID-19 patients, as is being shown with humanized antibodies blocking IL-6 signaling. In addition, raloxifene and bazedoxifene are selective estrogen receptor modulators with strong antiviral activity.

Acute respiratory distress syndrome (ARDS) is characterized by acute hypoxemia, neutrophil-mediated inflammation, and lung edema formation. Whereas lung damage might be alleviated by nitric oxide (NO), goal of this study was to evaluate if intratracheal NO donor S-nitroso-N-acetylpenicillamine (SNAP) can positively influence the lung functions in experimental model of ARDS. New Zealand rabbits with respiratory failure induced by saline lavage (30 ml/kg, 9+/-3 times) were divided into: ARDS group without therapy, ARDS group treated with SNAP (7 mg/kg i.t.), and healthy Control group. During 5 h of ventilation, respiratory parameters (blood gases, ventilatory pressures) were estimated. After anesthetics overdosing, left lung was saline-lavaged and cell count, cell viability and protein content in bronchoalveolar lavage fluid (BALF) were measured. Right lung tissue was used for estimation of wet/dry weight ratio, concentration of NO metabolites, and histomorphological investigation. Repetitive lung lavage induced lung injury, worsened gas exchange, and damaged alveolar-capillary membrane. Administration of SNAP reduced cell count in BALF, lung edema formation, NO metabolites, and histopathological signs of injury, and improved respiratory parameters. Treatment with intratracheal SNAP alleviated lung injury and edema and improved lung functions in a saline-lavaged model of ARDS suggesting a potential of NO donors also for patients with ARDS.

Inflammation associated with acute respiratory distress syndrome (ARDS) can damage the alveolar epithelium and surfactant and worsen the respiratory failure. Glucocorticoids (GC) appear to be a rational therapeutic approach, but the effect is still unclear, especially for early administration and low-dose. In this study we compared two low doses of dexamethasone in early phase of surfactant-depleted model of acute respiratory distress syndrome (ARDS). In the study, lung-lavaged New Zealand rabbits with respiratory failure (PaO(2)<26.7 kPa in FiO(2) 1.0) were treated with intravenous dexamethasone (DEX): 0.5 mg/kg (DEX-0.5) and 1.0 mg/kg (DEX-1.0), or were untreated (ARDS). Animals without ARDS served as controls. Respiratory parameters, lung edema, leukocyte shifts, markers of inflammation and oxidative damage in the plasma and lung were evaluated. Both doses of DEX improved the lung function vs. untreated animals. DEX-1.0 had faster onset with significant improvement in gas exchange and ventilation efficiency vs. DEX-0.5. DEX-1.0 showed a trend to reduce lung neutrophils, local oxidative damage, and levels of TNFalpha, IL-6, IL-8 more effectively than DEX-0.5 vs. ARDS group. Both dosages of dexamethasone significantly improved the lung function and suppressed inflammation in early phase ARDS, while some additional enhancement was observed for higher dose (1 mg/kg) of DEX.

Inhalační trauma (IT) je definováno jako akutní postižení dýchacích cest způsobené inhalací produktů hoření nebo páry. I samotné inhalační trauma může být pro pacienta smrtelné, pokud se ovšem vyskytne v souvislosti s popálením, dramaticky zhoršuje prognózu. Vzhledem ke změně v demografických parametrech populace je každoročně zaznamenáván nárůst incidence termického a inhalačního traumatu u seniorů. V kazuistice prezentujeme případ 89letého muže s těžkým stupněm inhalačního traumatu a minoritní popáleninou. Průběh hospitalizace byl komplikován opakovaným rozvojem ventilátorové pneumonie. Bohužel i přes precizní mikrobiologickou surveillance a četné bronchoskopické výkony se nepodařilo pacienta zachránit.
Inhalation injury is defined as the acute airway infliction due to inhalation of combustion products. Inhalation injuiry itself might befatal for the patient and in combination with a burn leads to a dramatic reduction of prognosis. Due to a change in the demographicparameters of the population is annually recorded an increase in the incidence of thermal and inhalation trauma in the elderly.At this case we present a 89-year-old man with severe inhalation injury and minor burns. The course of hospitalization was complicatedby repeated development of ventilator-associated pneumonia. Unfortunately, despite precise microbiological surveillanceand numerous bronchoscopic procedures, we were unable to save the patient.
