Infuzní léčba ve smyslu tekutinové resuscitace je bezesporu jednou z nejčastějších terapeutických intervencí u akutně nemocných pacientů. I přes hromadící se data ukazující potenciálně negativní dopad neindikovaného podání tekutin je i nadále tento „tekutinový reflex“ běžnou praxí v prostředí urgentního příjmu. Jedním z důvodů je často velmi obtížná klinická interpretace volemie a hydratace, pokud pacient nevykazuje krajní poruchy tekutinové rovnováhy. Implementace ultrasonografie dolní duté žíly do vyšetření prvního kontaktu umožňuje relativně rychle a jednoduše odpovědět na základní klinickou otázku: „Potřebuje můj pacient tekutinu nebo dekongesci?“ Zdánlivá přímočarost této informace je ale vykoupena řadou limitací, které je nutné při hodnocení hemodynamiky akutních stavů znát.
Infusion therapy, in terms of fluid resuscitation, is without a doubt one of the most common therapeutic intervention in the management of an acutely ill patient. Despite the growing body of evidence showing potentially harmful effects of non-indicated fluid administration, this "fluid-reflex" remains a common practice in the Emergency department. One of the reasons for this is that fluid status assessment, based just on the clinical findings, is generally difficult if the patient does not have extreme fluid balance disturbances. Inferior Vena Cava ultrasound provides a relatively quick and simple answer to the basic clinical question: "Does my patient need fluids or decongestion?" However, this seemingly straightforward information is redeemed by several limitations which must be considered assessing the hemodynamics of medical emergencies.
Venózní tromboembolismus (TEN) je častou příčinou morbidity a mortality u hospitalizovaných nemocných. Ačkoli většinu pacientů lze bezpečně léčit antikoagulací, filtry dolní duté žíly (KF) představují u malé podskupiny pacientů důležitou alternativu k antikoagulaci. Po zavedení dočasných KF počet implantací má stoupající trend. Indikace pro zavedení KF se liberalizovaly i přes omezené důkazy podporující tuto praxi. Navíc KF jsou spojeny s řadou závažných komplikací. V tomto článku jsme komplexně zhodnotili celou problematiku KF, včetně indikací, techniky umístění a komplikací.
Venous thromboembolism (VTE) is a frequent cause of morbidity and mortality in hospitalized patients. Although most patients can be treated safely with anticoagulation, inferior vena cava filters (IVCF) represent an important alternative to anticoagulation in a small subset of patients. After the introduction of temporary IVC filters, the number of implantations has had an upward trend. Indications for IVCF insertion have been liberalized despite limited evidence supporting the practice. In addition, IVC filters are associated with a number of serious complications. In this article, we comprehensively review the entire issue of IVCF, including indications, placement technique, and complications.

AIMS: The management of congestion is one of the key treatment targets in heart failure. Assessing congestion is, however, difficult. The purpose of this study was to investigate the safety and dynamic response of a novel, passive, inferior vena cava (IVC) sensor in a chronic ovine model. METHODS AND RESULTS: A total of 20 sheep divided into three groups were studied in acute and chronic in vivo settings. Group I and Group II included 14 sheep in total with 12 sheep receiving the sensor and two sheep receiving a control device (IVC filter). Group III included an additional six animals for studying responses to volume challenges via infusion of blood and saline solutions. Deployment was 100% successful with all devices implanted; performing as expected with no device-related complications and signals were received at all observations. At similar volume states no significant differences in IVC area normalized to absolute area range were measured (55 ± 17% on day 0 and 62 ± 12% on day 120, p = 0.51). Chronically, the sensors were completely integrated with a thin, reendothelialized neointima with no loss of sensitivity to infused volume. Normalized IVC area changed significantly from 25 ± 17% to 43 ± 11% (p = 0.007) with 300 ml infused. In contrast, right atrial pressure required 1200 ml of infused volume prior to a statistically significant change from 3.1 ± 2.6 mmHg to 7.5 ± 2.0 mmHg (p = 0.02). CONCLUSION: In conclusion, IVC area can be measured remotely in real-time using a safe, accurate, wireless, and chronic implantable sensor promising to detect congestion with higher sensitivity than filling pressures.

PURPOSE: To determine normative data for the inferior vena cava (VCI) diameter in euvolemic children and its correlation with different somatic parameters in a pediatric population at one center in Europe. MATERIALS AND METHODS: This prospective observational study enrolled healthy children aged 4 weeks to 18y that visited our outpatient clinic. Weight, height, body surface area, and age were recorded. The children were grouped according to weight, as follows (80 children/group): < 10 kg, 10-19.9 kg, 20-29.9 kg, 30-59.9 kg, and 60-90 kg. Children were placed in a supine position and, during quiet respiration, the maximum and minimum VCI diameters were measured with M-mode ultrasonography. The collapsibility index (CI) was also automatically calculated for each subject: CI = [VCI maximum (expiratory) diameter - VCI minimum (inspiratory) diameter]/VCI maximum (expiratory) diameter. RESULTS: From May 2016 through November 2018 we retrieved data for 415 children that underwent VCI diameter evaluations. 400 children were included (mean age: 7.8y ± 5.8, mean weight: 32 kg ± 24.4, 46 % girls). The VCImax and the VCImin were significantly correlated with age (r = 0.867, p < 0.001, r = 0.797, p < 0.001), height (r = 0.840, p < 0.001, r = 0.772, p < 0.001), weight (r = 0.858, p < 0.001, r = 0.809, p < 0.001), and BSA (r = 0.878, p < 0.001, r = 0.817, p < 0.001). Correlations between the CI and age, weight, height, and BSA were not statistically significant. CONCLUSION: This prospective study provided reference values for sonographic measurements of VCI diameters in euvolemic children and might greatly assist in assessing fluid status in sick children.

BACKGROUND In dialysis patients with exhausted usual central venous access sites, the translumbar hemodialysis catheter (TLC) provides a viable option for dialysis access. The technical success of catheter insertion, associated complications, and long-term patency of TLC were evaluated in this study. MATERIAL AND METHODS This retrospective study included 37 patients with occluded central thoracic veins in whom 39 TLC implantation procedures were performed and 196 patients with internal jugular vein hemodialysis catheters (JVC). TLC implantation was performed as a hybrid procedure with computed tomography (CT)-navigated translumbar inferior vena cava cannulation and subsequent fluoroscopy-guided hemodialysis catheter placement. RESULTS The rates of technical success of the implantations and minor periprocedural complications were 97.4% and 10.3% in the TLC group and 98.6% and 4.2% in the JVC group, respectively. The median follow-up in the TLC and JVC groups was 673 days and 310 days, respectively. The primary-assisted patency at the 1-year and 3-year follow-up was 88.7% and 72.0% in the TLC group and 81.6% and 67.0% in the JVC group, respectively, with no statistically significant difference between the 2 groups. The incidence rate of infection-related and patency-related complications calculated for 1000 catheter-days was 0.15 and 0.11 in the TLC group and 0.33 and 0.25 in the JVC group, respectively. CONCLUSIONS The CT-guided implantation of the TLC is a useful option to create dialysis access with a low complication rate and satisfactory long-term patency in patients without usual venous access.

[Extraction of caval filter 7 years after placement]
Plicní embolie je závažné, mnohdy život ohrožující postižení kardiovaskulárního systému. Jako prevence plicní embolie se kromě medikamentózní terapie provádí také implantace kaválního filtru do dolní duté žíly. Představuje mechanickou překážku pro průnik krevních sraženin do plic. Předkládáme kazuistiku nemocné, které byl ALN kavální filtrimplantován před 7 lety po porodu, kdy byl prokázán vlající trombus v dolní duté žíle, trombus zasahoval až k odstupu pravé rená lni žíly. Proto byl filtrimplantován atypicky nad odstupy renálních žil. Sedm let po implantaci kaválního filtru přichází pacientka s neurčitými bolestmi v pravém podžebří k vyšetření. Protože nebezpečí plicní embolie pominulo, bylo rozhodnuto pokusit se o endovaskulámí odstranění tohoto filtru. Filtr byl úspěšně odstraněn, výkon proběhl bez komplikací, jen bylo nutné použít o 1 F větší sheath, nežje doporučeno, což lze patrně vysvětlit tím, že po dlouhé době implantace byly kotvící nožičky filtru potaženy fibrinem. Popisovaná kazuistika patří k případům, kde byl kavální filtr úspěšně odstraněn po poměrně dlouhé době po implantaci.
Pulmonary embolia is serious, often life threatening disease. As a pulmonary embolia prevention anticoagulation therapy is usually used, but in some cases inferior vena cava filter placement is employed. Filter creates a mechanical barrier preventing the passage of the embolus into lungs. We present a case of female patient with ALN caval filter placed 7 years ago after child delivery. She had floating thrombus in inferior vena cava, the top of the thrombus was up to ostium of right renal vein. It was the reason why the filter was placed atypically above renal veins level. Seven years after filter placement the patient complains on non-specific pain in right upper abdominal area. As the risk of pulmonary embolia is not present anymore it was decided to remove the filter by endovascular approach. The filter removal was successful and was complication-free. We had to use a sheath 1 F larger than recommended, most probably due to fibrin coating of the filter anchoring struts. Presented case belongs to the few presented case reports when the filter was removed after relatively long time after placement.

Hlboká žilová trombóza (HŽT) je závažné ochorenie, pri ktorom môže byť ohrozená vitalitakončatiny. Phlegmasia coerulea dolens predstavuje najzávažnejší stav HŽT s kompletnoutrombózou hĺbkových aj povrchových žíl zasahujúcou celú končatinu, zastavuje sa žilovýodtok, v dôsledku čoho dochádza k prerušeniu tepnového toku a ischémii končatiny. V takomprípade je koža cyanotická, tepnové pulzácie nie sú hmatné, môžu vzniknúť buly na distálnychčastiach končatiny, gangréna žilového pôvodu. Práca prezentuje kazuistiku pacientky s rý-chlym rozvojom phlegmasia coerulea dolens ľavej dolnej končatiny.
Deep vein thrombosis is a serious disease in which limb vitality may be compromised. Phleg-masia coerulea dolens is the most severe stage of deep vein thrombosis with a completethrombosis of deep and superficial veins, and therefore, the whole limb is affected. Exceptthe swelling and pain, the affected limb is cyanotic and there are no peripheral pulsations dueto absent venous and therefore arterial flow and subsequent ischaemia. Also, a venous gan-grene may develop. This case report presents a female patient with rapidly developing phleg-masia coerulea dolens.

BACKGROUND: In clinical medicine, little is known about the use of allografts for portal vein (PV) reconstruction after pancreaticoduodenectomy (PD). Portal and caval systems are physiologically different, therefore the properties of allografts from caval and portal systems were studied here in a pig model. MATERIALS AND METHODS: PD with PV reconstruction with allogeneic venous graft from PV or inferior vena cava (IVC) was performed in 26 pigs. Biochemical analysis and ultrasonography measurements were performed during a 4-week monitoring period. Computer simulations were used to evaluate haemodynamics in reconstructed PV and explanted allografts were histologically examined. RESULTS: The native PV and IVC grafts varied in histological structure but were able to adapt morphologically after transplantation. Computer simulation suggested PV grafts to be more susceptible to thrombosis development. Thrombosis of reconstructed PV occurred in four out of five cases in PV group. CONCLUSION: This study supports the use of allografts from caval system for PV reconstruction in clinical medicine when needed.
