BACKGROUND AND OBJECTIVES: The mental health of healthcare workers (HCWs) may have improved after the COVID-19 pandemic. We aimed to model the trajectories of psychological distress, depressive symptoms, and resilience during the COVID-19 pandemic and toward its end in HCWs in Czechia and investigate, which COVID-19 work stressors were associated with these trajectories. METHODS: The study included 322 HCWs from the Czech arm of the international HEROES Study who participated in an online questionnaire in two waves during the pandemic and one wave toward its end. Growth mixture modeling identified trajectory patterns of depressive symptoms (measured with Patient Health Questionnaire), distress (General Health Questionnaire), and resilience (Brief Resilience Scale). Logistic regression was applied to estimate the association of COVID-19 stressors with mental health trajectories, adjusting for baseline characteristics. RESULTS: Trajectory classes revealed both high and low depressive symptoms (high in 61% of participants), distress (high in 82% of participants), and resilience (low in 32% of participants). Depressive symptoms and distress trajectories demonstrated the same shape, first increasing during the pandemic and decreasing toward its end, while resilience remained constant. Exposure to COVID-19 stressors, in particular, the experience of stigmatization, discrimination, and violence, was associated with high depressive symptoms and distress trajectories, but not with resilience. CONCLUSIONS: Interventions provided to HCWs during crises such as pandemic should target distress and depressive symptoms and need to address stigmatization, discrimination, and violence.

Health care workers have been exposed to COVID-19 more than people in other professions, which may have led to stigmatization, discrimination, and violence toward them, possibly impacting their mental health. We investigated (1) factors associated with stigma, discrimination, and violence, (2) the association of stigma, discrimination, and violence with mental health, (3) everyday experiences of stigmatization, discrimination, and violence. We chose a combination of a quantitative approach and qualitative content analysis to analyze data collected at three time points: in 2020, 2021 and 2022. A higher age was associated with lower odds of experiencing stigma, discrimination, and violence, whereas female gender was related to more negative experiences. The intensity of exposure to COVID-19 was associated with greater experience with stigmatization, discrimination, and violence across all three years (for example in 2022: odds ratio, 95% confidence interval: 1.74, 1.18-2.55 for mild exposure; 2.82, 1.95-4.09 for moderate exposure; and 5.74, 3.55-9.26 for severe exposure, when compared to no exposure). Stigma, discrimination, and violence were most strongly associated with psychological distress in 2020 (odds ratio = 2.97, 95% confidence interval 2.27-3.88) and with depressive symptoms in 2021 (odds ratio = 2.78, 95% confidence interval 2.12-3.64). Attention should be given to the destigmatization of contagious diseases and the prevention of discrimination, violence, and mental health problems, both within workplaces and among the public.

OBJECTIVES: Despite growing interest, the cost-effectiveness of eHealth interventions for supporting quality of life of people with dementia and their caregivers remains unclear. This study evaluated the cost-effectiveness of the FindMyApps intervention, compared to digital care-as-usual. FindMyApps aims to help people with dementia and their caregivers find and learn to use tablet apps that may support social participation and self-management of people with dementia and sense of competence of caregivers. METHOD: A randomised controlled trial (Netherlands Trial Register NL8157) was conducted, including people with mild cognitive impairment (MCI) or mild dementia and their informal caregivers (FindMyApps n = 76, digital care-as-usual n = 74). Outcomes for people with MCI/dementia were Quality-Adjusted Life-Years (QALYs), calculated from EQ-5D-5L data and the Dutch tariff for utility scores, social participation (Maastricht Social Participation Profile) and quality of life (Adult Social Care Outcomes Toolkit), and for caregivers, QALYs and sense of competence (Short Sense of Competence Questionnaire). Societal costs were calculated using data collected with the RUD-lite instrument and the Dutch costing guideline. Multiple imputation was employed to fill in missing cost and effect data. Bootstrapped multilevel models were used to estimate incremental total societal costs and incremental effects between groups which were then used to calculate Incremental Cost-Effectiveness Ratios (ICERs). Cost-effectiveness acceptability curves were estimated. RESULTS: In the FindMyApps group, caregiver SSCQ scores were significantly higher compared to care-as-usual, n = 150, mean difference = 0.75, 95% CI [0.14, 1.38]. Other outcomes did not significantly differ between groups. Total societal costs for people with dementia were not significantly different, n = 150, mean difference = €-774, 95%CI [-2.643, .,079]. Total societal costs for caregivers were significantly lower in the FindMyApps group compared to care-as-usual, n = 150, mean difference = € -392, 95% CI [-1.254, -26], largely due to lower supportive care costs, mean difference = €-252, 95% CI [-1.009, 42]. For all outcomes, the probability that FindMyApps was cost-effective at a willingness-to-pay threshold of €0 per point of improvement was 0.72 for people with dementia and 0.93 for caregivers. CONCLUSION: FindMyApps is a cost-effective intervention for supporting caregivers' sense of competence. Further implementation of FindMyApps is warranted.

Zdravotničtí pracovníci pečující o pacienty s COVID-19 čelili během pandemie vysokému riziku nákazy, nedostatku personálu, kontaktu se smrtí pacientů a vlastnímu riziku infekce. Tyto faktory výrazně ovlivnily jejich duševní zdraví. Narativní review shrnuje výsledky studií publikovaných v češtině a angličtině mezi lety 2020 a 2024 o dopadu pandemie na duševní zdraví zdravotníků v Česku. Z 20 identifikovaných studií jich splnilo kritéria pro zahrnutí 8. Studie popisují příznaky deprese, stres, syndrom vyhoření, stigmatizaci, diskriminaci a násilí. Výsledky ukazují závažný dopad těchto faktorů na duševní pohodu zdravotníků. Závěrem zdůrazňujeme potřebu preventivních opatření, včetně supervize, psychologické pomoci a podpory ze strany vedení. Důraz na duševní zdraví zdravotníků je klíčový pro jejich ochranu a udržitelnost péče v krizových situacích.
Healthcare workers caring for COVID-19 patients faced a high risk of infection, staff shortages, contact with patient deaths and their own risk of infection during the pandemic. These factors greatly affected their mental health. This narrative review summarizes the results of studies published in Czech and English between 2020 and 2024 on the impact of the pandemic on the mental health of healthcare professionals in the Czechia. Of the 20 studies identified, 8 met the inclusion criteria. Studies describe symptoms of depression, stress, burnout, stigmatization, discrimination, and violence. The results show a severe impact of these factors on the mental well-being of healthcare professionals. Finally, we emphasize the need for preventive measures, including supervision, psychological assistance and support from management. An emphasis on the mental health of healthcare professionals is key to their protection and sustainability of care in crisis situations.
This study investigated the Czech adults' mental health following the COVID-19 pandemic and the potential influence of data collection methodology on prevalence estimates. Separately, it investigated changes in help-seeking and associated barriers. Data from representative surveys on Czech adults, conducted in November 2017 (n = 3,306), in May (n = 3,021) and November 2020 (n = 3,000), and in November and December 2022 (n = 7,311), were used. Current mental disorders were assessed by the Mini International Neuropsychiatric Interview, and the treatment gap was established in individuals scoring positively. In help-seeking individuals, encountering barriers was investigated. In 2017 and 2022, 20.02 % and 27.22 % of individuals had at least one mental disorder, respectively. The 2022 panel sampling and online and telephone interviewing estimates (34.29 % and 26.7 %) were substantially higher than those from household sampling and personal interviewing (19.9 %). Prevalence rates based on household sampling and personal interviewing were broadly consistent in 2017 and 2022. The treatment gap was around 80 % from 2017 to 2022. More than 50 % of individuals encountered structural barriers in help-seeking in 2022. This study showed that prevalence rates were still elevated in 2022, but suggests that data collection methodology influenced the estimates. Separately, the treatment gap remained consistently very high, and encountering structural barriers in help-seeking was common.

The pandemic due to COVID-19 brought new risks for depression of health care workers, which may have differently influenced men and women. We aimed to investigate (1) whether health care workers in Czechia experienced an increase in depression during the COVID-19 pandemic, (2) which factors contributed the most to this change, and (3) whether the magnitude of the associations differed by gender. We studied 2564 participants of the Czech arm of the international COVID-19 HEalth caRe wOrkErS (HEROES) Study. Online questionnaire was administered to health care workers in summer 2020 (wave 0) and spring 2021 (wave 1). Depression was defined by reaching 10 or more points on the Patient Health Questionnaire. Logistic regression investigated the association of participant ́s characteristics with depression and multivariable decomposition for non-linear models assessed, to what extent the characteristic explained the change in depression occurrence. The prevalence of depression increased twice during the pandemic (11% in wave 0 and 22% in wave 1). Stress accounted for 50% of the difference, experience of death due to COVID-19 for 15% and contact with COVID-19 patients for 14%. Greater resilience and sufficient personal protective equipment were strongly associated with lower occurrence of depression. The protective association of resilience with depression was stronger in men than in women. We conclude that interventions to promote mental health of health care workers in future health crisis should aim at decreasing stress and enhancing resilience. They should be delivered especially to individuals who have contact with the affected patients and may face their death.

Hlavním cílem příspěvku je seznámit odbornou veřejnost s konceptem dementia-friendly community – společnosti vstřícné životu s demencí, který odráží nejnovější pohled na život s demencí, tj. pohled na demenci jako na každé jiné zdravotní postižení s důrazem na rovnost v lidských právech a otázky smyslu a kvality života poznamenaného demencí. Článek přináší zkušenosti se zaváděním tohoto konceptu v rámci komunit i zdravotnických zařízení v zahraničí i s aktuálním vývojem na národní úrovni. Seznamuje s Národním akčním plánem pro Alzheimerovu chorobu 2020–2030 (NAPAN)[1], který je dobrým signálem, že se vláda, ministerstva, odborníci různých specializací, poskytovatelé služeb, samotní pečující a lidé žijící s demencí dokázali spojit a rozhodli se hledat cesty k lepšímu životu s demencí v naší společnosti. Účelem článku je mimo jiné přiblížit koncept společnosti vstřícné k životu s demencí lékařům a jiným zdravotnickým pracovníkům a podpořit přátelský a vstřícný přístup k lidem s demencí při poskytování zdravotní péče. Zdůrazňuje roli multidisciplinární spolupráce v péči o člověka s demencí včetně role praktických lékařů, geriatrů, neurologů, psychiatrů a dalších zdravotníků. Přináší příklad projektu, který bude realizován ve Středočeském kraji a nastaví kritéria a způsoby hodnocení přínosu komunit přátelských k lidem s demencí pro jejich zdraví a kvalitu života[2].
Principal goal of the article is to inform health care professionals about the concept of dementia-friendly community (DFC) reflecting the contemporary thinking of living with dementia, i.e. dementia as a disability, equal human rights, a sense of meaning. Experiences from abroad and from the Czechia are discussed with special attention to implementation of this concept in the health care facilities. The National Action Plan for Alzheimer´s Disease 2020–2030 (NAPAN) is presented, which shows that the Czech Government, ministries, specialists, care providers, informal careers and even the people with dementia worked together on the improvement in this field. The purpose of this paper is to bring the principles of DFC closer to the physicians and other health care professionals and support friendly and helpful approach to people living with dementia when providing health care to them. The multidisciplinary cooperation of GPs, geriatricians, neurologists, psychiatrists etc. is emphasized. An example of a regional project in Middle Bohemia is presented looking for criteria and ways of evaluation of the effect of DFC.

Úvod: Pandemie COVID-19 byla globální krizí s extrémním tlakem na zdravotnické systémy. Zátěžové faktory byly shodné ve všech zasažených zemích, i když se mohly projevovat s různou intenzitou podle vyspělosti zdravotnictví. Většina publikovaných studií o dopadech na psychiku zdravotníků je kvantitativních a mapuje výskyt depresivní symptomatologie, příznaků vyhoření, míry zátěže a dalších faktorů. Naším cílem bylo pomocí kvalitativní analýzy zjistit, jaké potřeby mají pracovníci ve zdravotnictví. Metody: Na základě otevřených odpovědí ze tří vln dotazníkového šetření českého ramene mezinárodní studie HEROES z let 2020, 2021 a 2022 jsme analyzovali 1105 odpovědí z celkových 2880. Z dotazníku potřeb v rámci pilotní intervenční studie HEROES z roku 2023 jsme analyzovali 118 odpovědí. Použili jsme přístup obsahové analýzy. Po vytvoření kódovacího schématu byla data kódována v programu Atlas.ti. Kódy byly následně roztříděny do kategorií. Výsledky: Potřeby zdravotníků byly kategorizovány do následujících témat: pracovní podmínky, týmová spolupráce, podpora ze strany nadřízených a vedení, práce s pacienty, slaďování práce a osobního života a potřeby spojené s duševní pohodou. Závěr: Naše analýza prokázala, že zdravotníci začínají formulovat svoje potřeby jak v oblasti pracovních podmínek a organizace práce, ale i v kontextu celého svého života. Začínají klást důraz na vztahy na pracovišti, na zajištění psychologických potřeb a na rovnováhu mezi osobním životem a prací.
Introduction: COVID-19 pandemic was a global crisis with extreme pressure on healthcare systems. The stressors were the same in the affected countries even if they could manifest in different intensity due to varied development and resources in the healthcare systems. Most of the published studies about mental health consequences on healthcare workers are quantitative studies mapping the occurence of depressive symptoms, symptoms of burnout, level of stress and other facotrs. In the presented paper, our aim was qualitative analysis of needs of healthcare workers. Methods: We have analysed 1105 responses from a total of 2880 free answers in the three waves of the Czech HEROES study questionnaire from years 2020, 2021, 2022 and 118 responses from the survey of needs in the 2023 HEROES pilot intervention study. We used content analysis approach, and after creation of the code scheme, the data was coded in Atlas.ti programme. The codes were subsequently categorised. Results: The needs of healthcare workers were categorised in following themes: working conditions, teamwork, support from superiors and management, patient care, work-life balance, and needs of well-being. The details regarding each category and example quotes from the respondents are presented in the paper. Conclusion: Our analysis proved that healthcare workers start to exprress their needs not only in the field of working conditions and organization of their work but also in the context of their entire lives. They put emphasis on relations at the workplace, on support of their psychological needs and on work-life balance.

Mental health literacy (MHL) grows in importance as the prevalence of mental health conditions rises worldwide. There is a need for valid MHL measures especially among adolescents. The current study aims to develop and validate a Universal mental health literacy scale for adolescents (UMHL-A), a self report scale to assess MHL of children between ages 10 and 14 across all four components within Kutcher's framework for MHL. The development of the UMHL-A was initiated by a literature review and followed by item generation. Then, we tested the face and content validity of the scale via a delphi study, focus groups and cognitive interviews with the target group. Next with a convenience sample, we established the factor structure and construct validity of the scale using exploratory and confirmatory factor analyses. Lastly, we tested the scale convergent validity with other measures. The final form of the scale was refined on a representative sample of Czech asolescents. The UMHL-A scale comprises 17 items measuring four components of MHL. The scale has substantial methodological advantages in comparison to existing measures of MHL, especially considering its brevity and comprehensive coverage of MHL. Further studies are needed to test its application and validity globally.
