Prevence návykových nemocí by měla být komplexní a zahrnovat nácvik sociálních dovedností, dovednosti rozhodování, práci s rodinou atd. Podobně komplexní, systematická a dlouhodobá by měla být i efektivní léčba. Tělesná cvičení a jóga či jiné formy tělesného cvičení jsou užitečnou součástí šířeji založených preventivních a léčebných programů. Na druhé straně ovšem vrcholový sport míru rizikových faktorů spíše zvyšuje. V této práci zmiňujeme praktické zkušenosti s využíváním jógy v rámci komplexního léčebného programu pro závislé na alkoholu, jiných látkách a patologické hráče. K výhodám jógy patří integrace tělesného cvičení a relaxace.
Prevention of addictive diseases should be complex and systematic and it should include training of social skills, decision-making skills, family intervention, etc. Similarly, effective treatment is usually long-term, systematic and complex. Physical exercise and yoga can be useful components of comprehensive prevention and treatment programmes. On the other hand, competitive professional sport rather increases the number of risk factors for substance-related problems. Practical experience with the use of yoga in substance dependent patients and pathological gambles are mentioned. One of the advantages of yoga is the integration of physical exercise and relaxation.

In this article we present an answer to the question how dificult might it be, to define a working CDA report that fulfills the requirements of the Austrian specifications of the nationwide electronic health record called ELGA. We chose the results of standardized cardiopulmonary exercise testing (CPET) results as an example document. We therefore analyzed existing documentation and interviewed sport scientists and medical doctors to find out how this type of medical documentation is best structured and what data must be and can optionally be included. We then worked out the appropriate elements of a CDA report for levels 2 and 3. Only one adaptation had to be made to the oficial Austrian health records stylesheet, which was necessary to be able to integrate scalable vector graphic (SVG) images. After this project we can conclude, that the time and technical efiort to construct documents for the nationwide Austrian electronic health record is quite little. The biggest problem still might be to obtain a consensus of all involved parties when trying to define an oficial report, which was not necessary in our case.

Effective arterial elastance(E(A)) is a measure of the net arterial load imposed on the heart that integrates the effects of heart rate(HR), peripheral vascular resistance(PVR), and total arterial compliance(TAC) and is a modulator of cardiac performance. To what extent the change in E(A) during exercise impacts on cardiac performance and aerobic capacity is unknown. We examined E(A) and its relationship with cardiovascular performance in 352 healthy subjects. Subjects underwent rest and exercise gated scans to measure cardiac volumes and to derive E(A)[end-systolic pressure/stroke volume index(SV)], PVR[MAP/(SV*HR)], and TAC(SV/pulse pressure). E(A) varied with exercise intensity: the E(A) between rest and peak exercise along with its determinants, differed among individuals and ranged from -44% to +149%, and was independent of age and sex. Individuals were separated into 3 groups based on their E(A)I. Individuals with the largest increase in E(A)(group 3;E(A)>0.98 mmHg.m(2)/ml) had the smallest reduction in PVR, the greatest reduction in TAC and a similar increase in HR vs. group 1(E(A)<0.22 mmHg.m(2)/ml). Furthermore, group 3 had a reduction in end-diastolic volume, and a blunted increase in SV(80%), and cardiac output(27%), during exercise vs. group 1. Despite limitations in the Frank-Starling mechanism and cardiac function, peak aerobic capacity did not differ by group because arterial-venous oxygen difference was greater in group 3 vs. 1. Thus the change in arterial load during exercise has important effects on the Frank-Starling mechanism and cardiac performance but not on exercise capacity. These findings provide interesting insights into the dynamic cardiovascular alterations during exercise.

In the present study we aimed to evaluate whether oxidative stress and inflammation induced by strenuous exercise affect glycocalyx integrity and endothelial function. Twenty one young, untrained healthy men performed a maximal incremental cycling exercise - until exhaustion. Markers of glycocalyx shedding (syndecan-1, heparan sulfate and hyaluronic acid), endothelial status (nitric oxide and prostacyclin metabolites - nitrate, nitrite, 6-keto-prostaglandin F(1alpha)), oxidative stress (8-oxo-2'-deoxyguanosine) and antioxidant capacity (uric acid, non-enzymatic antioxidant capacity) as well as markers of inflammation (sVCAM-1 and sICAM-1) were analyzed in venous blood samples taken at rest and at the end of exercise. The applied strenuous exercise caused a 5-fold increase in plasma lactate and hypoxanthine concentrations (p<0.001), a fall in plasma uric acid concentration and non-enzymatic antioxidant capacity (p<10(-4)), accompanied by an increase (p=0.003) in sVCAM-1 concentration. Plasma 6-keto-prostaglandin F(1alpha) concentration increased (p=0.006) at exhaustion, while nitrate and nitrite concentrations were not affected. Surprisingly, no significant changes in serum syndecan-1 and heparan sulfate concentrations were observed. We have concluded, that a single bout of severe-intensity exercise is well accommodated by endothelium in young, healthy men as it neither results in evident glycocalyx disruption nor in the impairment of nitric oxide and prostacyclin production.
