OBJECTIVE: Judging the validity of CTG monitoring as a method of acute foetal hypoxia diagnosis (AFH), quantifying the number of cesarean section (SC), performed from the indication of acute fetal hypoxia and point out a number of surgeries based on false positive CTG results. DESIGN: Retrospective study. SETTING: 1st Department of Obsterics and Gynaecology Faculty of Medicine MU, Brno. METHODS: 100 women, patients of the 1st Department of Obstetrics and Gynaecology in Obilní trh, Brno, who had cesarean section due to foetal hypoxia indicated on the base of pathological or suspect CTG, were involved in the collection. In the next stage newborns were divided into 2 clinical groups: "Acidosis" and "Normal" depending on the pH value from a. umbilicalis and statistically analysed. RESULTS: From 100% suspect or pathological CTG, only 36.19% were valid (newborns showed depression after birth). Remaining 63.18% of newborns, although showed CTG signs of hypoxia were born normal and probably were not necessary to be born by cesarean section. CONCLUSION: Although CTG investigation is in most places the only "objective" method for diagnosis of AFH, this investigation cannot precisely determinate whether hypoxia is present or it is not. For true objectivity of foetal intrauterine condition and correct indication of SC is suitable to use further diagnostic methods of AFH in future, for example intrapartal foetal pulse oximetry or blood analysis from foetal scalp.

AIM: The association between chronic venous disease (CVD) and atherosclerosis has been intensively investigated. Because data regarding cardiovascular disease in women are sparse, we evaluated the potential association between the symptoms of CVD and cardiovascular risk factors including markers of preclinical atherosclerosis in middle-aged women. METHODS: A questionnaire for chronic lower limb venous insufficiency was completed by 902 women, aged 45-54 years, from the general population. At the same time, all women were examined for the presence of cardiovascular risk factors, including the ankle/brachial systolic blood pressure index (ABI) and carotid intima media thickness of the common carotid arteries measured by ultrasound. Differences in cardiovascular risk factors and markers of preclinical atherosclerosis between women with and without symptoms of CVD were evaluated. RESULTS: Symptoms (cramps, aching, edema, disturbance of sleep) suggestive of CVD in the lower extremities were reported by 606 (67.2%) women and 114 (12.6%) reported that those symptoms were severe enough to limit their usual daily activities. A higher prevalence of ABI of less than 0.91 was observed in women with any (P=0.005) or severe (P=0.029) CVD symptoms. A significantly higher prevalence of the following were observed in women with any or severe CVD symptoms: coronary artery disease, history of diabetes mellitus, increased body mass index, waist circumference, serum triglycerides, serum C-reactive protein and lower serum HDL cholesterol. CONCLUSION: CVD symptoms were strongly associated with a higher prevalence of pathological values of ABI and several other manageable cardiovascular risk factors. These findings support the data that chronic venous disease might also indicate increased risk for atherosclerosis.

The defininition of the corrective surgery is the performance of the surgical procedures that modify the form and the functions of the tissues and organs. In the case of external genital organs in women the delimitation of the area of interest is the upper limits of vulva cranialy, laterally fossae genitocruralis and dorsaly the perineum. From the visceral point of view the margin is the hymen. This type of surgery is an interdisciplinary one as what concerns the area of interest and the medical speciality. The external genital organs in women neighbor with the distant parts of the urinary system and the aboral end of the digestive tract. The surgical procedures may be performed either by gynecologist or in collaboration with esthetic surgeon or urologist. The targets are the following: to reestablish the normal functions of the external genitalia and the neighboring organs namely the sexual functions and the the functions of the excretory organs, the elimination of the pathologic resistance, is the part of the oncogynecological therapy, the esthetics of genital organs and to reach the psychological well being of the patient. Among the numerous surgical procedures performed on the female external genitals the following are the most frequent: enlargement of the hypoplastic vaginal outlet, surgical treatment of condylomata accuminata, pseudocystis of the Bartolin gland, sutura episiototomiae male sanata, descensus vaginae, the hypotrophy of labia minora, adrenogenital syndrome, vaginal agenesis, hymen malformations, female genital mutilation (FGM) in African women, and vulva tumors.
