Karcinom plic patří celosvětově na čelní místa co do incidence i mortality. U většího podílu nemocných bývá zachycen v inoperabilním pokročilém či metastazujícím stadiu. Imunoterapie checkpoint inhibitory je standardní součástí léčby metastazujícího karcinomu plic, a to bud' samostatně, nebo v kombinaci s chemoterapií. Imunoterapie jednoznačně patří do první linie léčby, kde přináší nejlepší výsledky. U nemalobuněčného karcinomu plic (non-small cell lung cancer, NSCLC) neskvamózní histologie musíme primárně vyloučit řídicí mutace – především mutaci receptoru pro epidermální růstový faktor (epidermal growth factor receptor, EGFR) a traslokaci genu pro anaplastickou lymfomovou kinázu (anaplastic lymphoma kinase, ALK), abychom mohli tuto léčbu indikovat. U NSCLC obecně platí, že imunoterapie je tím účinnější, čím je vyšší exprese ligandu programované buněčné smrti 1 (programmed cell death-ligand 1, PD-L1). U nádorů s vysokou expresí PD-L1 (50 % a více) můžeme používat imunoterapii samotnou, v ostatních případech ji kombinujeme s chemoterapií. Další možností u NSCLC je adjuvantní podání imunoterapie po úspěšné konkomitantní chemoradioterapii. Kombinace chemoterapie a imunoterapie v první linii je standardem léčby i u malobuněčného karcinomu plic, nezávisle na expresi PD-L1. Článek detailně rozebírá možnosti použití (chemo)imunoterapie u nemocných s karcinomy plic v kontextu léčby v České republice hrazené ze zdravotního pojištění.
Lung cancer is one of the leading cancers worldwide in terms of incidence and mortality. In a larger proportion of patients, it is detected at an inoperable advanced or metastatic stage. Immunotherapy with checkpoint inhibitors is a standard part of the treatment of metastatic lung cancer, either alone or in combination with chemotherapy. Immunotherapy clearly belongs in the first line of treatment where it yields the best results. In non-small cell lung cancer (NSCLC) of non-squamous histology, we must primarily rule out controlling mutations (primarily epidermal growth factor receptor [EGFR] and anaplastic lymphoma kinase [AEK]) to indicate this treatment. In NSCLC, in general, immunotherapy is more effective the higher the PD-L1 expression. For tumors with high PD-L1 expression of 50% or more, we can use immunotherapy alone, otherwise we combine it with chemotherapy. Another option for NSCLC is adjuvant immunotherapy after successful concomitant chemoradiotherapy. Combination of chemotherapy and immunotherapy in the first line is the standard treatment also in small cell lung cancer, regardless of PD-L1 expression. The article discusses in detail the possibilities of (chemo)immunotherapy in patients with lung cancer in the context of treatment in the Czech Republic covered by health insurance.
Malobuněčný karcinom plic (small cell lung cancer, SCLC) zůstává jedním z nádorových onemocnění s velmi špatnou prognózou, v komplexu diagnóz karcinomu plic se podílí asi z 15 %. V souborném článku je stručně uvedena epidemiologie, biologie, diagnostika a terapie limitovaných i extenzivních stadií onemocnění. Jde o nádor, který je obvykle diagnostikován v pokročilých stadiích, iniciálně je převážně chemosenzitivní a radiosenzitivní. Léčbou navozené remise bývají zejména v pokročilých stadiích nemoci dočasné, není známa účinná adjuvantní léčba. Terapie druhé a vyšších linií bývá účinná jen krátkodobě. Limitovaná stadia onemocnění mohou být úspěšně léčena konkomitantní chemoradioterapií s profylaktickým ozářením mozku, velmi časná stadia pak chirurgicky a adjuvantní chemoterapií. Novým směrem, který zlepšil výsledky léčby extenzivních stadií SCLC, je kombinace chemoterapie s imunoonkologickou léčbou durvalumabem a atezolizumabem. Perspektivními léky druhé a dalších linií se ukazují být lurbinectedin, talazoparib, dále olaparib, temozolomid a kombinace sintilimab + anlotinib. V současnosti je vývoj diagnostických a léčebných modalit u SCLC z hlediska praktického využití méně příznivý než vývoj diagnostiky a terapie karcinomů nemalobuněčných.
Small cell lung cancer (SCLC) remains one of the cancers with a very poor prognosis, accounting for about 15% of lung cancer diagnoses. Epidemiology, biology, diagnosis and therapy of limited and extensive stages of the disease are briefly presented in the comprehensive article. It is a tumor that is usually diagnosed in advanced stages, initially mainly chemosensitive and radiosensitive. Treatment-induced remissions tend to be temporary, especially in advanced stages of the disease, there is no known effective adjuvant treatment, secondand higher-line treatment is only effective in the short term. Limited stages of the disease can be successfully treated either with concomitant chemoradiotherapy with prophylactic brain irradiation, very early stages with surgery and adjuvant chemotherapy. A new direction that has improved the results of treatment of extensive stages of SCLC is the combination of chemotherapy with immuno-oncological treatment durvalumab and atezolizumab. Promising secondand further-line drugs appear to be lurbinectedin, talazoparib, olaparib, temozolomide and the sintilimab-anlotinib combination. Currently, the development of diagnostic and treatment modalities for SCLC is less favorable from the point of view of practical use than the development of diagnostics and therapy for non-small cell carcinomas.
Většina pacientů s nemalobuněčným karcinomem plic přichází k lékaři s lokálně pokročilým či metastazujícím onemocněním. Metastazující onemocnění jsou indikována k systémové léčbě. Pokud je to možné a jsou-li u pacientů s neskvamózní histologií vyloučeny mutace receptoru epidermálního růstového faktoru (epidermal growth factor receptor, EGFR) a anaplastické lymfomové kinázy (anaplastic lymphoma kinase, ALK) (eventuálně další řídicí mutace), je imunoterapie indikována do první linie léčby. Kombinovaná imunoterapie nivolumab, ipilimumab podávaná s úvodními dvěma cykly chemoterapie doložila signifikantně delší přežití ve srovnání s chemoterapií. Podle úhradových podmínek v České republice je jedinou možností léčby u nemalobuněčného karcinomu plic s negativitou ligandu programované buněčné smrti 1 (programmed cell death ligand 1, PD-L1). U pozitivních expresorů PD-L1 1-49 % je možnou alternativou standardních režimů s pembrolizumabem.
Most patients with non-small cell lung cancer come to the doctor with locally advanced or metastatic disease. Metastatic diseases are indicated for systemic treatment. If possible and if epidermal growth factor receptor (EGFR) and anaplastic lymphoma kinase (ALK) mutations (or other driver mutations) are excluded in patients with non-squamous histology, immunotherapy is indicated as the first line of treatment. Combined immunotherapy of nivolumab, ipilimumab administered with the initial two cycles of chemotherapy demonstrated significantly longer survival compared to chemotherapy alone. According to the reimbursement conditions in the Czech Republic, it is the only treatment option for non-small cell lung cancer with programmed cell death ligand 1 (PD-L1) negativity. For positive PD-L1 expressors 1-49% it is a possible alternative to standard regimens with pembrolizumab.

Východiska: Fúze genu neuregulin 1 (NRG1) byla zjištěna u mnoha různých karcinomů. Nejčastěji u adenokarcinomů plic, a to zejména v těch bez mutace genů KRAS a BRAF. Cíl: Článek představuje nově popsanou různorodou skupinu karcinomů s fúzí genu NRG1. Vysvětluje základní molekulárně-genetické mechanizmy související s touto řídicí genovou alterací. Jde o aktivaci receptoru ERBB3 (HER3) a ERBB2 (HER2) s následnou iniciací kanonických onkogenních drah PIK a MAPK. Sumarizujeme dosavadní zkušenosti s novými léčebnými postupy. Závěr: Dosavadní celosvětové výsledky imunoterapie, cytotoxické a zatím zkoušené cílené léčby nejsou uspokojivé. Probíhají další léčebné studie (z toho dvě v Evropě) a výzkum biologie těchto karcinomů. NRG1 rearanžované karcinomy je možné diagnostikovat metodou mRNA sekvenace nové generace (NGS) i v podmínkách ČR.
Introduction: Neuregulin 1 (NRG1) gene fusion was detected in a wide range of carcinomas. It is most frequently present in lung adenocarcinomas, especially in KRAS and BRAF wild-type cases. Purpose: We present a newly described diverse group of NRG1 rearranged carcinomas. The paper explains basic molecular principles associated with this oncogenic driver. It consists of ERBB3 (HER3) and ERBB2 (HER2) receptor activation with downstream activation of PIK and MAPK canonical pathways. The experience with new therapeutic modalities is summarized. Conclusions: So far, the global results of cytotoxic, immune and targeted therapies were disappointing. Further research (including two studies in Europe) is underway, developing new therapeutic strategies and examining this cancer biology. In the meantime, it is possible to diagnose NRG1 rearranged carcinomas in the Czech Republic since mRNA next generation sequencing (NGS) analysis is readily available.

Metastazující či lokálně pokročilý nemalobuněčný karcinom plic (non-small cell lung cancer, NSCLC) má obecně velmi špatnou prognózu. Pětileté přežití ve stadiu IV se pohybuje v řádu jednotek procent (méně než 5 %). Nádory plic s přítomnými řídicími mutacemi jsou „jiné" nádory, s odlišným chováním a prognózou. Léčba by měla být zahajována vždy se znalostí mutačního statusu a exprese ligandu programované buněčné smrti 1 (programmed cell death-ligand 1, PD-L1). Exprese PD-L1 by měla být v době diagnózy patologem stanovena reflexně u všech NSCLC a mutace EGFR, ALK, ROS reflexně u neskvamózních NSCLC. Článek předkládá kazuistiku nemocného, kde opomenutí tohoto vyšetření může vést k chybné volbě léčebného schématu, minimálně však k prodlení zahájení léčby. Článek dále rozebírá možnosti biologicky cílené léčby u jednotlivých typů řídicích mutací u karcinomu plic - EFGR, ALK, ROS, BRAF, KRAS atd.
Metastatic or locally advanced non-small cell lung cancer (NSCLC) generally has a very poor prognosis. Five-year survival in stage IV is in the order of percentage units (less than 5%). Lung tumors with control mutations present are „different" tumors, with different behavior and prognosis. Treatment should always be initiated with knowledge of the mutation status and expression of programmed cell death-ligand 1 (PD-L1). PD-L1 expression should be reflexively determined in all NSCLC at the time of diagnosis by a pathologist and EGFR, ALK, ROS mutations reflexively in non-squamous NSCLC. The article presents a case report of a patient, where the omission of this examination may lead to the wrong choice of treatment scheme, but at least to a delay in the start of treatment. The article further discusses the possibilities of biologically targeted treatment for individual types of control mutations in lung cancer - EFGR, ALK, ROS, BRAF, KRAS, etc.

Cystická fibróza (CF) se řadí mezi závažné, progredující, zatím nevyléčitelné nemoci. Klinicky se manifestuje zejména obrazem chronického sinopulmonálního onemocnění. Základní terapeutické principy založené na komplexní symptomatické léčbě jsou nově rozšířeny o léčbu modulátory chloridového kanálu a zintenzivnění respirační fyzioterapie využitím přístroje PhysioAssist (Simeox). Vzhledem k postupné progresi ireverzibilních plicních změn s věkem je nezbytné se zaměřit již od raného věku na techniku provádění mukociliární očisty dýchacích cest a u nemocných s dobrou compliance se snažit o rozšíření standardních metod airway clearance přístrojovou podporou s cílem zajistit efektivní expektoraci a minimalizovat tak rozvoj akutních exacerbací. Účinné odhlenění umožňuje snížit počet hospitalizací a potřebu antibiotické terapie, zpomaluje zhoršování plicních funkcí a vede ke zlepšení kvality života. PhysioAssist zaujímá nezastupitelnou roli v péči o nemocné s CF s poruchou expektorace, jak dokládá námi prezentovaná kazuistika 18leté dívky.
Cystic fibrosis (CF) is one of the serious, progressive, as yet incurable diseases. It is clinically manifested mainly by the image of chronic sinopulmonary disease. The basic therapeutic principles based on complex symptomatic treatment are extended by treatment with chloride channel modulators and intensification of respiratory physiotherapy using the PhysioAssist device (Simeox). Due to the age-related gradual progression of irreversible pulmonary changes, it is necessary to focus on the mucociliary airway clearance technique from an early age and to extend standard airway clearance methods with instrumental support in patients with good compliance to ensure effective expectoration and minimize the development of acute exacerbations. Effective attenuation leads to a reduction in the number of hospitalizations and antibiotics, to a slowdown in the deterioration of lung function, and thus to an improvement in the quality of life. PhysioAssist plays an irreplaceable role in the care of CF patients with expectorant disorder, as evidenced by the presented case report of an 18-year-old girl.
