High-grade serous carcinoma of the ovary, fallopian tube and peritoneum (HGSC), the most common type of ovarian cancer, ranks among the deadliest malignancies. Many HGSC patients have excess fluid in the peritoneum called ascites. Ascites is a tumour microenvironment (TME) containing various cells, proteins and extracellular vesicles (EVs). We isolated EVs from patients' ascites by orthogonal methods and analyzed them by mass spectrometry. We identified not only a set of 'core ascitic EV-associated proteins' but also defined their subset unique to HGSC ascites. Using single-cell RNA sequencing data, we mapped the origin of HGSC-specific EVs to different types of cells present in ascites. Surprisingly, EVs did not come predominantly from tumour cells but from non-malignant cell types such as macrophages and fibroblasts. Flow cytometry of ascitic cells in combination with analysis of EV protein composition in matched samples showed that analysis of cell type-specific EV markers in HGSC has more substantial prognostic potential than analysis of ascitic cells. To conclude, we provide evidence that proteomic analysis of EVs can define the cellular composition of HGSC TME. This finding opens numerous avenues both for a better understanding of EV's role in tumour promotion/prevention and for improved HGSC diagnostics.

Cílem článku je přinést ucelený přehled rizikových faktorů a jejich vztahu ke vzniku malignity endometria při ultrazvukovém nálezu endometriálního polypu. Přehledová práce byla vytvořená systematickým sběrem a tříděním původních prací, review a metaanalýz se vztahem k problematice endometriálních polypů a riziku přítomnosti malignity. Každý jednotlivý předpokládaný rizikový faktor byl samostatně podroben rozboru literatury. V případě abnormálního děložního krvácení, vyššího věku pacientky a hodnoty indexu tělesné hmotnosti se výsledky studií poměrně jednotně shodují na vysokém riziku přítomnosti malignity v děložním polypu. Vysoké riziko mají také pacientky s Lynchovým syndromem. Co se týče potenciálních rizikových faktorů, jako je počet a velikost polypu, diabetes, hypertenze či pozitivní rodinná anamnéza, není dostatek dat k jasnému závěru a/nebo se publikované výsledky liší.
This article presents a comprehensive review of factors that increase the risk of malignancy in ultrasound findings of an endometrial polyp. We collected original studies, reviews, and meta-analyses that dealt with the topic of endometrial polyps and the risk of developing endometrial cancer. Each presumed risk factor was analysed individually. According to searched studies, abnormal uterine bleeding, old age, and body mass index are valid risk factors for developing endometrial cancer in endometrial polyps. Lynch syndrome patients are also in a high-risk group for endometrial cancer. On the other hand, the number of polyps, their size, diabetes mellitus, hypertension, and positive family history are factors with inconclusive results. There are either not enough data or different results among several studies.
Cíl: Ucelený přehled léčebných přístupů recidiv karcinomu endometria s ilustrativní kazuistikou. Metodika: Přehledová práce podávající základní přehled o využití léčebných modalit u různých forem recidivujícího karcinomu endometria vč. operačního řešení, systémové léčby a radioterapie. Součástí práce je kazuistika prezentující léčbu pacientky s recidivou karcinomu lokalizovanou v břišní stěně. Závěr: Léčebné přístupy u pacientek s recidivujícím karcinomem endometria zahrnují operační přístup, radioterapii a systémovou léčbu v závislosti na předchozí terapii, charakteru a místu recidivy či diseminace, fyzickém stavu a přání pacientky. Rozhodnutí o výběru léčebné modality by mělo být individuálně posuzováno multioborovou onkogynekologickou komisí.
Objective: A comprehensive overview of therapeutical strategies for recurrent endometrial cancer with illustrative case report. Methodology: A review providing basic overview of therapeutical options for different forms of recurrent endometrial cancer including surgical treatment, systemic treatment and radiotherapy. It includes a case report presenting a treatment of patient with an endometrial cancer recurrence in the abdominal wall. Conclusion: Therapeutical strategies in patients with endometrial cancer recurrence include surgical treatment, radiotherapy and systemic treatment depending on previous therapy, type and site of recurrence or dissemination, performance status and wishes of the patient. Decision about choice of treatment should be individually discussed and evaluated by multidisciplinary oncogynecological commission board.
Cíl: Ucelený přehled operační léčby karcinomu vulvy vč. recidivujících forem. Metodika: Přehledová práce podávající základní přehled o patogenezi, diagnostice a operační léčbě karcinomu vulvy se zaměřením na možnosti terapie jeho recidiv. Součástí je ilustrativní kazuistika prezentující pacientku s invazivním spinocelulárním karcinomem vulvy s iterativními lokálními recidivami a následným rozvojem nádorové triplicity a vzdáleného metastatického postižení. Závěr: Operační léčba zůstává hlavní modalitou terapie karcinomu vulvy, a to i v případě lokálně pokročilých či recidivujících nálezů. V těchto případech je nezbytností multidisciplinární spolupráce operačních oborů. Důležitou úlohu při včasném odhalování recidiv má disciplinovanost odléčených pacientek s participací na pravidelné dispenzární péči. Případnou alternativou k operačnímu řešení zůstává (chemo) radioterapie, v klinické praxi má pak radioterapie nezastupitelné místo v adjuvantní terapii. Regionální a vzdálené recidivy se vyznačují špatnou prognózou.
Objective: A comprehensive overview of the surgical treatment of vulvar cancer, including recurrent forms. Methodology: A review work providing a basic overview of the pathogenesis, diagnosis and surgical treatment of vulvar cancer with a focus on the possibilities of treatment of its recurrences. It includes an illustrative case report presenting a patient with invasive squamous cell carcinoma of the vulva with iterative local recurrences and subsequent development of tumor triplicity and distant metastatic involvement. Conclusion: Surgical treatment remains the main modality of vulvar cancer therapy, even in the case of locally advanced or recurrent findings. In these cases, multidisciplinary cooperation of operational fields is necessary. The discipline of treated patients with participation in regular dispensary care plays an important role in the early detection of recurrences. (Chemo) radiotherapy remains a possible alternative to the surgical solution; in clinical practice, radiotherapy has an irreplaceable place in adjuvant therapy. Regional and distant recurrences are characterized by a poor prognosis.
Úvod: Pohled na karcinom endometria se v poslední dekádě dramaticky proměnil, došlo k výrazné změně v operačním přístupu a stagingu lymfatických uzlin. Tyto změny prezentujeme na výsledcích Onkogynekologického centra Gynekologicko-porodnické kliniky FN Brno v letech 2012–2021 v kontextu aktuálních národních i evropských doporučení. Metodika: Retrospektivní unicentrická observační studie, přehled národních a evropských doporučených postupů. Výsledky: Ve sledovaném období bylo na klinice léčeno 715 pacientek s karcinomem endometria, operační léčbu podstoupilo 636 z nich (89 %). V rámci stagingu lymfatických uzlin je v první polovině hodnoceného období patrný trend rozšiřování lymfadenektomie o paraaortální oblast, v letech 2018–2019 pak zavádění detekce sentinelové uzliny, nakonec přechod k této metodě jako k hlavnímu stagingovému výkonu v roce 2021, kdy bylo vyšetření provedeno u 73 % operací, a to i u high risk karcinomů splňujících kritéria pro provedení biopsie sentinelové uzliny, tj. nádory omezené na dělohu. S rozšířením biopsie sentinelové uzliny je patrný postupný pokles laparotomických výkonů (maximum 41 % v roce 2016, 18 % v roce 2021), snížení krevní ztráty (2012–2019 medián 100 ml, s poklesem na 50 ml v letech 2020–2021). Délka hospitalizace se ustálila na mediánu 5–6 dnů. Závěr: Operační léčba karcinomu endometria se stala pro většinu pacientek výkonem minimálně invazivním, snížila se průměrná krevní ztráta, délka hospitalizace. Biopsie sentinelové uzliny se stala preferovanou metodou stagingu lymfatických uzlin.
Introduction: In the last decade, the view of endometrial cancer has shifted enormously, and the surgical approach or lymph node staging has changed significantly. We are presenting these changes with the University Hospital Brno Oncogynecology center’s results in the years 2012–2021 in the actual national and European guidelines context. Methods: The retrospective unicentric observational study, national and European guidelines review. Results: In the observation period, 715 endometrial cancer patients were treated in our clinic, and 636 of them underwent surgical treatment (89%). Concerning lymph node staging, firstly, there is a clear trend of expanding lymphadenectomy to the paraaortic area, followed by the sentinel node biopsy introduction in the years 2018–2019, and finally, the complete transition to this method as the main staging procedure in 2021, when this examination was performed in 73% of surgeries, even with high-risk cancers limited to the uterus. Within the sentinel node biopsy expansion, a gradual decrease in laparotomy approach (maximum 41% in 2016, 18% in 2021), and blood loss (2012–2019 median 100 mL, with a decrease to 50 mL in 2020–2021) was evident. A hospitalization length stabilized at a median of 5–6 days. Conclusions: Surgical treatment of endometrial cancer has become a minimally invasive procedure for the majority of patients, the average blood loss and hospitalization length have decreased. Sentinel node biopsy has become the preferred lymph node staging method.

BACKGROUND: Endometrial cancer is the most common gynecological malignancy in developed countries with no screening available. There is still a tendency to provide invasive bioptic verification in asymptomatic women with abnormal ultrasound findings to diagnose carcinoma in a preclinical phase; even though, it is not supported by European guidelines. Our goal was to determine DFS (disease-free survival), OS (overall survival), and DSS (disease-specific survival) differences between symptom-free and symptomatic (bleeding, or spotting) endometrial cancer patients with similar stage and tumor/clinical characteristics. METHODS: All of our patients with endometrial cancer following surgical treatment between 2006 and 2019 were assessed, evaluating risk factors for recurrence and death while focusing on bleeding using univariable and multivariable analysis. RESULTS: 625 patients meeting the inclusion criteria were divided into asymptomatic (n = 144, 23%) and symptomatic (n = 481, 77%) groups. The median follow-up was 3.6 years. Using univariable analysis, symptomatic patients had a three times higher risk of recurrence (HR 3.1 (95% Cl 1.24-7.77), p = 0.016). OS (HR 1.35 (0.84-2.19), p = 0.219) and DSS (HR 1.66 (0.64-4.28), p = 0.3) were slightly worse without reaching statistical significance. In our multivariable analysis, symptomatology was deemed completely insignificant in all monitored parameters (DFS: HR 2.03 (0.79-5.24), p = 0.144; OS: HR 0.72 (0.43-1.21), p = 0.216). CONCLUSIONS: The symptomatic endometrial cancer patients risk factor of earlier recurrence and death is insignificantly higher when compared with the asymptomatic cohort. However, multivariable analysis verifies that prognosis worsens with other clinically relevant parameters, not by symptomatology itself. In terms of survival outcome in EC patients, we recognized symptomatology as a non-significant marker for the patient's prognosis.

Endometrial cancer is the most common gynecologic malignancy in Europe and usually diagnosed in its initial stage owing to early symptoms of abnormal bleeding. There is no population screening for this disease, although it can sometimes be accidentally diagnosed in asymptomatic patients. Our study aims to determine differences in clinical and tumor characteristics between an asymptomatic and symptomatic group of patients. This unicentric prospective observational study took place in University Hospital Brno between January 2016 and December 2019. A total of 264 patients met inclusion criteria (26% asymptomatic, 74% with reported symptoms). We did not find a statistically significant difference in clinical characteristics (menopausal status, parity, age, BMI, and serum level of CA 125) between groups. According to ultrasound examination, bleeding tumors were larger (19.5 vs. 12.7 mm, p ≤ 0.001). Definitive histology results indicated more frequent lymphovascular space invasion (p < 0.001), along with deep myometrial (p = 0.001) and cervical (p = 0.002) invasion. There was no difference in advanced stages of the tumor. We did not substantiate statistically significant difference in immunohistochemical profile (estrogen and progesterone receptors, L1 cell adhesion molecule, tumor protein p53), which is relevant for tumor recurrence risk and survival capacity. Our conclusions affirmed that bleeding occurs more often among patients with local tumor invasion into the myometrium and cervical stroma. Final International Federation of Gynecology and Obstetrics (FIGO) stage, histology, and immunohistochemical characteristics do not significantly affect symptom appearance.

Circulating tumor markers are not routinely used in patients with endometrial cancer (EC). This pilot study evaluated the role of monitoring new biomarkers DJ1 and L1CAM, in correlation with CA125 and HE4, for the effects of anticancer treatment and preoperative management in EC patients. Serial serum levels of DJ1, L1CAM, CA125 and HE4 were collected in 65 enrolled patients. Serum DJ1, L1CAM, CA125 and HE4 levels were significantly higher at the time of diagnosis compared to those measured during follow-up (FU). In patients with recurrent disease, serum DJ1, CA125 and HE4 levels were significantly higher at the time of recurrence compared to levels in disease-free patients. Serum L1CAM levels were also higher in patients with recurrence but without reaching statistical significance. While DJ1 levels were not affected by any of the observed patient-related characteristics, L1CAM levels were significantly higher in patients with age ≥60 years who were overweight. At the time of EC diagnosis, DJ1 and L1CAM serum levels did not correlate with stage, histological type or risk of recurrence. This is a preliminary description of the potential of serial DJ1 and L1CAM serum level measurement for monitoring the effects of treatment in EC patients.

BACKGROUND: To prospectively assess LLL incidence among cervical cancer patients treated by uterine surgery complemented by SLN biopsy, without PLND. METHODS: A prospective study in 150 patients with stage IA1-IB2 cervical cancer treated by uterine surgery with bilateral SLN biopsy. Objective LLL assessments, based on limb volume increase (LVI) between pre- and postoperative measurements, and subjective patient-perceived swelling were conducted in six-month periods over 24-months post-surgery. RESULTS: The cumulative incidence of LLL at 24 months was 17.3% for mild LLL (LVI 10-19%), 9.2% for moderate LLL (LVI 20-39%), while only one patient (0.7%) developed severe LLL (LVI > 40%). The median interval to LLL onset was nine months. Transient edema resolving without intervention within six months was reported in an additional 22% of patients. Subjective LLL was reported by 10.7% of patients, though only a weak and partial correlation between subjective-report and objective-LVI was found. No risk factor directly related to LLL development was identified. CONCLUSIONS: The replacement of standard PLND by bilateral SLN biopsy in the surgical treatment of cervical cancer does not eliminate the risk of mild to moderate LLL, which develops irrespective of the number of SLN removed.

Cíl: Rozbor našeho přístupu k rekonstrukci prsou po mastektomii u žen s karcinomem prsu a/nebo BRCA mutací. Onkoplastická chirurgie umožňuje výkony dostatečně radikální a s velmi dobrým kosmetickým efektem. S rozvojem programů na genetické testování narůstá i nutnost profylaktických výkonů. Lze využít jednostranného kurativního výkonu a současně profylaktické operace na prsu druhém. Metodika: Využíváme možnosti okamžité rekonstrukce prsou v jedné době se subkutánní kůži šetřící mastektomií i modifikované mastektomie. Rekonstrukce řešíme buď expandérem a v druhé době vložením silikonového implantátu, nebo přímo vložením implantátu samotného či v kombinaci s využitím autologní tkáně podle toho, jak je plánována další onkologická léčba (chemoterapie nebo radioterapie). Výsledky: Bylo provedeno 103 rekonstrukčních operací u 58 žen s karcinomem prsu a/nebo BRCA mutací od dubna 2017 do května 2020. Z toho bylo 52 okamžitých rekonstrukcí u nepokročilých nádorů. Tkáňový expandér byl vložen u 27 žen (46,6 % souboru) s lokálně pokročilými nádory a nutností následné radioterapie (18 okamžitých a 9 odložených rekonstrukcí). Prsní implantáty byly použity u 52 žen (89,7 % souboru) v celkovém počtu 80 implantátů. Rekonstrukce prsu vlastní tkání byla provedena u 8 žen, z toho 5 výkonů v rámci okamžité rekonstrukce. Pooperační komplikace byla u 11 žen a bylo provedeno 15 korekčních výkonů (12,7 % operací). Závěr: Rekonstrukce prsu je komplexní soubor technik, kterými lze jakékoli pacientce vytvořit prs, tak aby nebyla odkázána na epitézu. Větší riziko komplikací je u pacientek s lokálně pokročilým onemocněním, které podstupují neoadjuvantní chemoterapii a radioterapii. S rostoucím počtem rakoviny prsu stoupá poptávka po rekonstrukčních výkonech, zejména okamžitých rekonstrukcích.
Objective: Analysis of our approach to breast reconstruction after mastectomy in women with breast cancer and/or BRCA mutations. Oncoplastic surgery enables procedures that are sufficiently radical and with a very good cosmetic effect. With the development of genetic testing programs, the need for prophylactic procedures is also increasing. One-sided curative performance and at the same time prophylactic surgery on the other breast can be used. Methods: We use the possibility of immediate breast reconstruction simultaneously with subcutaneous and skin-saving mastectomy. We solve the reconstruction either with an expander and in the second time by inserting a silicone implant, or directly by inserting the implant alone or in combination with the use of autologous tissue, depending on further oncological treatment (chemotherapy or radiotherapy). Results: One-hundred and three reconstructive surgeries were performed on 58 women with breast cancer and/or BRCA mutations from April 2017 to May 2020. Of these, there were 52 immediate reconstructions for untreated tumors. A tissue expander was inserted in 27 women (46.6% of the group) with locally advanced tumors and the need for subsequent radiotherapy (18 immediate and 9 delayed reconstructions). Breast implants were used in 52 women (89.7% of the group) in a total of 80 implants. Breast reconstruction of own tissues was performed in 8 women, of which 5 operations had immediate reconstruction. Postoperative complications occurred in 11 women and 15 corrective procedures were performed (12.7% of operations). Conclusion: Breast reconstruction is a comprehensive set of techniques by which any patient can obtain a breast so that it does not depend on the epithelium. Patients with locally advanced disease who receive neoadjuvant chemotherapy and radiotherapy are at greater risk of complications. With the growing number of breast cancers, the demand for reconstructive procedures, especially immediate reconstructions, is increasing.
