Cardiovascular diseases (CVD) are the leading cause of death in most developed countries. MicroRNAs (miRNAs) are highly investigated molecules not only in CVD but also in other diseases. Several studies on miRNAs continue to reveal novel miRNAs that may play a role in CVD, in their pathogenesis in diagnosis or prognosis, but evidence for clinical implementation is still lacking. The aim of this study is to clarify the diagnostic potential of miRNAs in some CVDs.

BACKGROUND: The noninvasive collection of saliva samples for DNA analyses is simple, and its potential for research and diagnostic purposes is great. However, DNA isolates from such samples are often of inferior quality to those from blood. AIM: The aim of this study was to investigate the robustness and sensitivity of the ddPCR instrument for genetic analyses from saliva samples of poor quality by comparing their results to those obtained using an established method from blood samples. METHODS: Blood and saliva were collected from 47 university students, which was followed by manual isolation of DNA and analysis on droplet digital PCR (ddPCR). Results of analyses were supplemented with values of fractional abundances. RESULTS: ddPCR proved to be highly suitable for analysis of even low-quality saliva samples (concentrations as low as 0.79 ng/μL), especially when augmented by fractional abundance data. This combination yielded 100% agreement with results obtained from blood samples. CONCLUSION: This study verified the applicability of ddPCR as a sensitive and robust method of genetic diagnostic testing even from low-quality saliva isolates. This makes it potentially suitable for a wide range of applications and facilitates the performance of large epidemiological studies, even if sampling or sample processing is suboptimal.

DNA methylation is the most studied epigenetic mechanism that regulates gene expression, and it can serve as a useful biomarker of prior environmental exposure and future health outcomes. This study focused on DNA methylation profiles in a human cohort, comprising 125 nonsmoking city policemen (sampled twice), living and working in three localities (Prague, Ostrava and Ceske Budejovice) of the Czech Republic, who spent the majority of their working time outdoors. The main characterization of the localities, differing by major sources of air pollution, was defined by the stationary air pollution monitoring of PM2.5, B[a]P and NO2. DNA methylation was analyzed by a genome-wide microarray method. No season-specific DNA methylation pattern was discovered; however, we identified 13,643 differentially methylated CpG loci (DML) for a comparison between the Prague and Ostrava groups. The most significant DML was cg10123377 (log2FC = -1.92, p = 8.30 × 10-4) and loci annotated to RPTOR (total 20 CpG loci). We also found two hypomethylated loci annotated to the DNA repair gene XRCC5. Groups of DML annotated to the same gene were linked to diabetes mellitus (KCNQ1), respiratory diseases (PTPRN2), the dopaminergic system of the brain and neurodegenerative diseases (NR4A2). The most significant possibly affected pathway was Axon guidance, with 86 potentially deregulated genes near DML. The cluster of gene sets that could be affected by DNA methylation in the Ostrava groups mainly includes the neuronal functions and biological processes of cell junctions and adhesion assembly. The study demonstrates that the differences in the type of air pollution between localities can affect a unique change in DNA methylation profiles across the human genome.

Povinností každého pracoviště, jež v rámci lékařského ozáření nakládá se zdroji ionizujícího záření, je mimo jiné dodržovat principy radiační ochrany včetně respektování pravidla ALARA (As Low As Reasonably Achievable). Cílem této práce bylo posoudit úroveň optimalizace radiační ochrany zdravotnických pracovníků na PET/CT pracovištích s ohledem na obdrženou osobní dávku, používání pomůcek, stavební uspořádání pracoviště a také zkušenosti a zručnost pracovníků. Všechna PET/CT a PET/MR pracoviště v ČR včetně dvou pracovišť, která nepracují s pozitronovými radiofarmaky, byla oslovena, aby se pomocí vyplnění dvou elektronických dotazníků a poskytnutím dalších důležitých informací a dat zapojila do studie. Následně byla získaná data osobní dozimetrie (za respektování GDPR), informace o množství aplikované aktivity, odpovědi na subjektivní hodnocení situace vedoucími pracovníky a také obdržené plány stavebního uspořádání pracoviště zpracovány a porovnány. Pro orientační statistické zpracování byla použita regresní analýza. Sady hodnot byly porovnány neparametrickými testy Mann-Whitney a Kruskal-Wallis. Pro každého pracovníka byla vypočtena jeho průměrná efektivní a ekvivalentní dávka ve vztahu k aplikované aktivitě a také kolektivní dávka pro každé pracoviště. V dalším kroku byla vypočtena obdržená hodnota Hp(10) a HT z 1 GBq na jednoho pracovníka (v μSv). Z celkového počtu 22 oslovených pracovišť ČR a Slovenska odpovědělo na 1. dotazník 18 pracovišť. Návratnost u dotazníku č. 2 byla 55,6 %. Následně mohlo být do studie zahrnuto 10 pracovišť. Výsledky ukázaly rozdíly úrovně radiační ochrany mezi jednotlivými pracovišti. Hlavní roli zde hrají vybavenost a dostatek personálu. Rozdíly byly nalezeny také v obdržené dávce na kůži rukou. Nepotvrdila se domněnka, že práce s pozitronovými radiofarmaky přináší větší radiační zátěž a také se nepotvrdilo, že s pořízením automatických aplikačních systémů automaticky klesne kolektivní dávka. Důležitá je praktická zručnost a zkušenost pracovníků. Pro jednotlivá pracoviště byla navržena konkrétní doporučení pro zlepšení úrovně optimalizace.

The results revealed differences in the level of radiation protection between individual workplaces. Equipment and sufficient staff play a major role. Differences were also found in the dose received on the skin of the hands. The assumption that utilization of positron radiopharmaceuticals is connected to a greater radiation burden has not been confirmed, as well as that the acquisition of automatic delivery systems does not decrease the collective dose automatically. Practical skills and experience of employees are important. Specific recommendations to improve the level of optimization were proposed for individual departments.
Cílem tohoto článku je shrnout doposud publikované výsledky epidemiologických studií zabývajících se dopadem dlouhodobých expozic znečištěnému ovzduší na zdraví a podat ucelený přehled o metodice hodnocení zdravotních dopadů. Dlouhodobá expozice znečištěnému ovzduší je považována za významnou determinantu zdraví. Dle Světové zdravotnické organizace (WHO) znečištění ovzduší přispívá ročně k 7,6 % předčasným úmrtím (odhad z roku 2016) a ke snížení let prožitých ve zdraví. Výsledky kohortových studií a metaanalýz poskytly řadu důkazů, které byly následně uplatněny při odhadování vztahů mezi expozicí (dávkou) a biologickým účinkem. Pro efektivní řízení rizika znečištěného ovzduší přijala Světová zdravotnická organizace ucelenou metodiku odhadů teoretických zdravotních dopadů, jejichž závěry mohou být využity při komunikaci rizik s veřejností, jakož i podklad pro tvorbu akčních plánů a priorit výzkumu na národní i mezinárodní úrovni. Potenciální zdravotní dopady jsou dle této metodiky vyjadřovány pomocí ukazatelů zátěže onemocněními - "attributable burden", jako jsou roky ztracené z důvodu předčasného úmrtí (Years of Life Lost - YLLs) nebo roky života strávené s disabilitou (Years Lived with Disability - YLDs), které vznikají jako důsledek dlouhodobé expozice polutantům ovzduší, konkrétně polétavým prachovým částicím o aerodynamickém průměru menším než 10 µm a 2,5 µm (PM10, PM2,5) a oxidu dusičitému (NO2). Tyto dopady vyjadřují podíl vlivu znečištění ovzduší na úmrtnost či nemocnost na závažná civilizační onemocnění jako jsou cerebrovaskulární onemocnění (ischemická choroba srdeční, mrtvice), zhoubný novotvar průdušnice, průdušek a plic, chronickou obstrukční plicní nemoc a další.
The aim of this work is to summarize the results of epidemiological studies related to the impact of long-term exposure to air pollution on health and to provide a comprehensive overview of the methodology of health risk and impact assessment. Long-term exposure to ambient air pollution is considered to be a significant determinant of health. According to the World Health Organization (WHO), air pollution contributes annually to 7.6% of deaths (estimate from 2016), and to a decrease in the number of healthy life years. Based upon the results of cohort studies and meta-analyses, the relations between exposure and biological effects were estimated. To effectively manage the risks of air pollution, the World Health Organization has adopted a unified methodology for health impact estimation. The conclusions can be used to communicate about risks with the public, as well as a basis for developing action plans and research priorities at national and international levels. Those health effects are expressed using the Years of Life Lost (YLLs) due to premature deaths or life in disability (YLDs), which originate as a consequence of long-term exposure of inhabitants to air pollutants, especially to particulate matter with aerodynamic diameters less or equal than 2.5 µm and 10 µm (PM2.5, PM10) and to nitrogen dioxide (NO2). The relations between the exposure and consequences have been described in serious diseases of civilization, which include cerebrovascular diseases (IHD, stroke), malignant tumours of the trachea, bronchi, and lungs (LC), chronic obstructive pulmonary disease (COPD), and others.

Cíle: Cílem průzkumu bylo zhodnocení optimalizace radiační ochrany při práci s otevřenými pozitronovými zářiči na nově zřízeném PET/CT pracovišti Kliniky nukleární medicíny ve Fakultní nemocnici v Ostravě (KNM FNO). Metody: Pomocí retrospektivní studie byly porovnány hodnoty osobní dozimetrie pracovníků KNM FNO, kteří se v období leden 2014 až duben 2018 podíleli na vyšetření pacientů pomocí PET/CT. Pro monitorování osob na pracovišti byly využity osobní OSL dozimetry a prstové tzv. TLD dozimetry. Výsledky osobní dozimetrie zpracovala služba osobní dozimetrie VF, a. s. Černá Hora. Stavební uspořádání pracoviště bylo zhodnoceno dle projektové dokumentace pracoviště. Výsledky: Hodnota průměrného osobního dávkového ekvivalentu Hp (10) se po spuštění provozu PET/CT v porovnání s měsíci leden až srpen roku 2017 zvýšila o 50,83 %, což představuje průměrné navýšení o 0,0154 mSv u jednotlivého pracovníka. Dávka na ruce se naopak ve sledovaném období v průměru snížila o 0,587 mSv, a tím došlo ke snížení radiační zátěže na kůži rukou o 23,53 %. Hodnota Hp (10) se v průměru za měsíce leden - duben roku 2018 zvýšila o 6,84 % a průměrná dávka na kůži rukou HT ve stejném porovnávaném období klesla o 7,19 %. Závěr: Radiační ochrana nově zavedeného PET/CT pracoviště KNM ve FNO je optimalizována a nebyly překročeny stanovené limity pro radiační pracovníky.
Aim: The aim of the study was to evaluate the optimization of radiation protection related to work with open positron sources on the newly opened PET/CT department in the Clinic of Nuclear Medicine at the University Hospital Ostrava (KNM FNO). Methods: We compared retrospectively the values from the personal dosimetry of employees, who participated on the PET/CT examinations over the time period from January 2014 to April 2018. For monitoring purposes there were used personal OSL dosimeters and finger TLD dosimeters. The results of personal dosimetry were processed by the dedicated service of VF Černá Hora. The construction of the premises was assessed according to the project documentation. Results: The average personal dose equivalent Hp (10) increased by 50.83% after the starting of the PET/CT, when compared with the time period of January-August 2017. This represents an average increase for 0.0154 mSv per employee. On the contrary, the dose to the hands decreased in the observed period on average by 0.587 mSv, this led to the decrease of the radiation dose to the skin by 23.53%. The Hp (10) value increased by 6.84% on average in January-April 2018 and the HT-average skin dose in the same period decreased by 7.19%. Conclusion: The radiation protection of the newly established PET/CT department in KNM FNO can be considered to be optimised; specified limits for the radiation workers were not exceeded.
Stárnutí evropské populace souvisí i s potřebou zlepšení kvality života ve vyšším věku a se zodpovězením otázek, do jaké míry jsou rozdíly zdravotního stavu dány kvalitou životního prostředí a dalšími neovlivnitelnými faktory a do jaké míry je možno ovlivnit zdravé stárnutí vlastním přičiněním. Krátké sdělení přináší první informace o obsahu a metodice pětiletého projektu HAIE zahájeného v roce 2018, který se pokouší najít odpověď na tyto otázky. Projekt HAIE (Healthy Aging in Industrial Environment) - Zdravé stárnutí v průmyslovém prostředí je projektem Excelentního výzkumu v rámci Operačního programu Výzkum, vývoj a vzdělávání Ministerstva školství, mládeže a tělovýchovy. Do jeho řešení jsou zapojena čtyři výzkumná pracoviště - Centrum epidemiologického výzkumu, Lékařská fakulta, Ostravská univerzita - hlavní řešitel projektu; Ústav experimentální medicíny, Akademie věd ČR, v. v. i.; Výzkumný ústav veterinárního lékařství, v. v. i.; a Centrum diagnostiky lidského pohybu, Pedagogická fakulta, Ostravská univerzita.
Aging of the European population warrants efforts to increase the quality of life in older age and to answer questions about the extent to which differences in health status are determined by the quality of the environment and other unchangeable factors, and the extent to which healthy aging can be improved by one's own actions. This brief report provides preliminary information on the content and methodology of the 5-year project HAIE that has been launched in the year 2018 and that attempts to find answers to these questions. The HAIE (Healthy Aging in the Industrial Environment) project is a project of the Excellent research within the framework of the Research and Development for Innovations Operational Programme of the Czech Ministry of Education Youth and Sports. Four research institutes are involved in the project - the Centre for Epidemiological Research, Faculty of Medicine, University of Ostrava (principal investigator); Institute of Experimental Medicine, Academy of Sciences of the Czech Republic; Veterinary Research Institute, v. v. i., and the Human Motion Diagnostic Centre, Pedagogical Faculty, University of Ostrava.

OBJECTIVE: Worldwide, primary schools are the most important segment of childhood education. The main disturbing source of noise in schools is human speech, the primary component of the learning process. School noise has a negative impact on information processing, teachers and pupils, and communication in the classroom. The study aimed at assessing A-weighted sound pressure levels in primary school classrooms. It was concerned with A-weighted sound pressure levels and their fluctuation during various classes. METHODS: The noise was measured with the static calibrated Brüel and Kjær 2260 Investigator sound level meter. The sound pressure levels were measured twice in 12 classes as 11 different subjects were taught to reduce the uncertainty of results due to their potential variability in time. From the instantaneous values, LA (t), equivalent continuous A-weighted sound pressure levels (LAeq,T) and LAmax were calculated. RESULTS: The mean equivalent A-weighted sound pressure level during lessons irrespective of age and subjects taught was 68.0 ± 3.4 dB (uncertainty of measurement). The highest levels of noise were in physical education and arts; the lowest levels were found for English taught as a foreign language. There were no statistically significant differences in LAeq,T depending on the numbers of children in the classes. The main source of noise is pupils. Attention should be paid to negative feelings of noise to reduce LAeq,T in schools. CONCLUSIONS: Continuous noise pollution, in combination with stress and the mentally demanding nature of the teaching profession, may lead to numerous negative effects on human personality.

Na pracovišti PET/CT v Novém Jičíně je v současné době využívána automatická aplikační stanice u většiny vyšetření s výjimkou imobilních pacientů. Před zavedením této pomůcky byla v prvních měsících provozu v roce 2013 zjištěna poměrná průměrná dávka u pracovníka 3,62 µSv/GBq. Výpočet vyjadřuje hodnotu osobního dávkového ekvivalentu Hp (10) vztažený k 1 GBq, jež byl aplikován pacientům. S využitím automatické aplikační stanice došlo ke snížení na 1,69 µSv/GBq ještě během téhož roku. V průběhu dalších dvou let se podařilo snížit průměrnou dávku na 1,35 µSv/GBq v roce 2014 a 1,32 µSv/GBq v roce 2015. Měření bylo prováděno pomocí elektronického dozimetru umístěného na levé straně hrudníku všech pracovníků, kteří přišli do styku s radiofarmakem či naaplikovaným pacientem. Operativně byla zaznamenávána dávka u osoby, která přímo radiofarmakum aplikovala, a to vždy při podání aktivity přibližně 300 MBq. V 90 % byla při jedné aplikaci naměřena hodnota 1 µSv, zatímco po podání radiofarmaka o stejné aktivitě, která byla aplikována tzv. „z ruky“, byla hodnota 2 µSv a více ve všech sledovaných případech. Optimalizace přinesla rovněž snížení ekvivalentní dávky na kůži rukou normalizované na 1 GBq z 278 µSv/GBq na 145 µSv/GBq.
Automatic injection device is currently used for most examinations except immobile patients at PET/CT department in Nový Jičín. Relative average dose of 3.62 μSv/GBq for staff member was measured in the first months of the year 2013 before introduction of this device. Dose calculation express a value of personal dose equivalent Hp (10) in relation to 1 GBq injected to patients. Dose was reduced to 1.69 μSv/GBq after introduction of this device still during this year. Relative average dose was further reduced during next two years to 1.35 μSv/GBq in the year 2014 and 1.32 μSv/GBq in the year 2015. Measurements were performed with an electronic dosimeter placed on the left side of the thorax of all staff members who came into contact with a radiopharmaceutical of injected patient. Operational measurement was performed in any person who injected radiopharmaceutical of approximately 300 MBq. 1 μSv was detected in 90 % during one injection while 2 or more μSv were detected during hand-operated injection of the same activity. Our optimization also decreased skin dose equivalent normalized to 1 GBq from 278 μSv/GBq to 145 μSv/GBq.
