Úvod: Respektování důstojnosti seniorů představuje aktuální výzvu v přístupu všeobecných sester. Cíl: Zhodnotit míru důstojného přístupu všeobecných sester k seniorům v domácí péči, identifikovat subškálu, která je nejvíce ohrožena pochybením sester, a zároveň posoudit souvislosti mezi respektováním důstojného přístupu k seniorům a demografickýma údaji všeobecných sester (věk sester, dosažené vzdělání, rok ukončení studia, délka praxe ve zdravotnictví a v agentuře domácí péče). Metodika: Dotazníkový průzkum byl realizován v 19 agenturách domácí péče kraje Vysočina (n = 92 sester). Na zjištění úrovně respektování důstojného přístupu byl použit autorizovaný dotazník (Tabaková?) obsahující 26 položek rozdělených do pěti subškál (informovanost pacienta, autonomie, důvěrnost, princip spravedlnosti, intimita). Výsledky: Nejnižší skóre bylo zjištěno v subškále autonomie pacienta a nejvyšší skóre bylo zjištěno v subškále důvěrnost a ochrana informací. Celkově sestry nižšího věku a s kratší praxí v ADP dosáhly vyššího skóre v hodnocení škály celkového přístupu k důstojnosti pacienta (p ≤ 0,05). Závěr: Doporučujeme, aby sestry na seniory nahlížely jako na aktivní subjekty péče a aby jim umožnily participovat na rozhodování o sobě. Ke změně postojů je nezbytná kontinuální edukace a auditní činnost.
Introduction: Respecting the dignity of clients represents a current challenge in the nursing approach. Aim: To evaluate the degree of dignified nursing approach towards elderly patients in home care agencies, to identify subscale, which is mostly endangered by the nurses' misconduct and simultaneously to record the link between the respect of a dignified approach and general nurses' demographic data (nurses' age, educational achievements, the graduation year, the length of professional experience in the healthcare and in a home care agency). Methods: The research was carried out in 19 home care agencies in the Vysočina region, the questionnaire survey was attended by 92 nurses. To determine the level of a respectful, dignified approach, the authorized questionnaire (Tabaková?) was used, which consists of 26 items and was divided into five subscales (the awareness of patient, autonomy, confidence, principle of justice, and intimacy). Results: Specifically, the lowest rating was found in the subscale of patient autonomy and the highest rating in the subscale of confidence and protection of information. Generally, the dignified approach to the elderly increases with the nurse's younger age and with less practice in the home care agency (p ≤ 0,05). Conclusion: We recommend that nurses look at each patient as an active subject of the care to enable those patients to participate in decisions about themselves. There is need for continuous educational and audit activities to ensure the change of attitudes.

Příspěvek se zaměřuje na ošetřovatelskou péči o pacientku s pokročilým karcinomem prsu. Prostřednictvím kazuistiky jako empirické metody je prezentována ošetřovatelská péče o pacientku v průběhu hospitalizace i v domácí péči.
