Východiska: Ačkoli nádory centrální nervové soustavy (CNS) nepatří mezi nejčastější nádorová onemocnění, jejich incidence v posledních letech stále roste. Naneštěstí se tato skupina vyznačuje velmi špatnou prognózou. Výběr správné terapie záleží na včasné a přesné diagnóze. Diagnostické postupy v případě nádorů mozku jsou však často limitovány jejich lokalizací a značnou heterogenitou nádoru. Proto je věnováno velké úsilí hledání nových diagnostických přístupů a biomarkerů, které by byly dostatečně robustní, senzitivní a specifické, a jejichž analýza by ve vztahu k pacientovi eliminovala nutnost biopsie. Mozkomíšní mok (cerebrospinal fluid - CSF) přichází do přímého kontaktu s CNS a představuje proto vhodný biologický materiál, který by mohl prostřednictvím biomarkerů reflektovat její aktuální stav. Vhodnými molekulami se v tomto případě zdají být mikroRNA (miRNA), krátké nekódující RNA, které byly již několikrát detekovány v CSF a jejichž deregulované hladiny jsou asociované s různými typy nádorů CNS. Cíl: Bohužel metodické přístupy používané pro analýzy miRNA v CSF nebyly prozatím dostatečně standardizovány. Z těchto důvodů se v předkládaném přehledovém článku snažíme shrnout a zhodnotit doposud používané metodické přístupy k analýze miRNA z CSF s cílem nalézt ty nejvhodnější. Dále zde sumarizujeme výsledky studií popisující miRNA, které by mohly v budoucnu představovat biomarkery různých nádorových onemocnění CNS.
Backround: Although central nervous system (CNS) tumors are not the most common cancers, their incidence rate is constantly growing. Unfortunately, this group of cancers is characterized by a very poor prognosis with a very short average patient survival. Appropriate therapy depends on early and accurate diagnosis. However, this is often limited by brain tumor localization and heterogeneity. Therefore, new diagnostic approaches and biomarkers that are robust, sensitive, specific, and also without need of invasive biopsy, are still being sought. Cerebrospinal fluid (CSF) comes into direct contact with the CNS and becomes a suitable source of biological material that could reflect actual state of CNS. Suitable molecules in this regard appear to be microRNAs (miRNAs), short non-coding RNAs, that have been already detected in CSF and whose dysregulated levels are associated with various types of brain tumors. Purpose: Unfortunately, the methodical approaches used for CSF miRNA analysis have not been sufficiently standardized yet. For this reason, we summarize and evaluate methodical approaches which were previously used for miRNA analysis from CSF in order to find the most appropriate ones. Subsequently, we review studies focused on miRNA with potential to become biomarkers of CNS tumors in the future.

Východiska: Karcinom ledviny tvoří 2–3 % všech zhoubných nádorů. Léčba metastatické formy tohoto onemocnění se opírá zejména o tyrozinkinázové inhibitory (TKI). Pro efektivní indikaci TKI je nevyhnutelná předpověď léčebné odpovědi pacientů. Cílem této práce proto bylo nezávislé ověření výsledků dosud publikovaných studií zabývajících se prediktivním potenciálem mikroRNA (miRNA). Materiál a metody: Zvolili jsme sedm nejvýznamnějších kandidátních miRNA a analyzovali jsme jejich expresi u 44 pacientů rozdělených do skupiny se špatnou a dobrou odpovědí na léčbu sunitinibem, rozdělených dle délky období do progrese. Vzorky nádorové tkáně jsme analyzovali metodou kvantitativní PCR za použití specifických sond TaqMan pro testované miRNA. Výsledky: Potvrdili jsme, že dvě miRNA jsou odlišně exprimovány u pacientů odpovídajících a neodpovídajících na léčbu sunitinibem. Ostatní analyzované miRNA neměly prediktivní potenciál. Závěr: Z dosud studovaných miRNA se nám podařilo potvrdit prediktivní potenciál u dvou studovaných miRNA.
Background: Renal cell carcinoma (RCC) accounts for 2–3% of all malignant tumours. Metastatic RCC (mRCC) is commonly treated with tyrosine kinase inhibitors (TKI). Effective TKIs administration can be achieved only by accurate prediction of therapeutical response. Therefore, the aim of this study was to analyse papers concerning predictive potential of microRNA (miRNA). Material and Methods: We chose seven candidate miRNAs and analysed their expression on 44 patients divided into cohort with poor and good response to sunitinib treatment. Patients were divided into two groups according to progression-free survival. RNA from tissue samples was isolated and expression of selected miRNAs was measured using quantitative PCR with miRNA-specific TaqMan probes. Results: We successfully validated two miRNAs to be differentially expressed in responding and non-responding patients to sunitinib treatment. Other analysed miRNAs have not shown predictive potential. Conclusion: From miRNAs studied so far, two miRNAs had predictive value according to present study.

Clear-cell renal cell carcinomas (ccRCCs) are genetically heterogeneous tumors presenting diverse clinical courses. Epithelial-mesenchymal transition (EMT) is a crucial process involved in initiation of metastatic cascade. The aim of our study was to identify an integrated miRNA/mRNA signature associated with metastasis and prognosis in ccRCC through targeted approach based on analysis of miRNAs/mRNAs associated with EMT. A cohort of 230 ccRCC was included in our study and further divided into discovery, training and validation cohorts. EMT markers were evaluated in ccRCC tumor samples, which were grouped accordingly to EMT status. By use of large-scale miRNA/mRNA expression profiling, we identified miRNA/mRNA with significantly different expression in EMT-positive tumors and selected 41 miRNAs/mRNAs for training phase of the study to evaluate their diagnostic and prognostic potential. Fifteen miRNAs/mRNAs were analyzed in the validation phase, where all evaluated miRNA/mRNA candidates were confirmed to be significantly deregulated in tumor tissue. Some of them significantly differed in metastatic tumors, correlated with clinical stage, with Fuhrman grade and with overall survival. Further, we established an EMT-based stage-independent prognostic scoring system enabling identification of ccRCC patients at high-risk of cancer-related death. Finally, we confirmed involvement of miR-429 in EMT regulation in RCC cells in vitro.

The EMT status reflecting miRNAs identified by expression profiling analysis using Real-Time PCR miRNA Arrays or their target molecules on protein level will enable to identify RCC patients with increased risk of metastatic disease, and by that will improve the treatment strategy by individualizing RCC patients therapy.
MiRNA, které reflektují statut EMT, získané pomocí analýzy profilů exprese miRNA pomocí metody Real-Time PCR miRNA Arrays, případně jejich cílové molekuly na proteinové úrovni, umožní u pacientů s renálním karcinomem identifikovat jedince s vysokým rizikem metastatického onemocnění, a tak přispějí k individualizaci jejich léčby.

Serum microRNAs are emerging as a clinically useful tool for early and non-invasive detection of various cancer types including renal cell carcinoma (RCC). Based on our previous results, we performed the study to analyze circulating serum miR-378 and miR-210 in patients with various histological subtypes of RCC. RNA was purified from blood serum samples of 195 RCC patients and 100 healthy controls. The levels of miR-378 and miR-210 in serum were determined absolutely using quantitative real-time PCR. Pre- and postoperative levels of both microRNAs were compared in 20 RCC patients. Significantly increased serum levels of both miR-378 and miR-210 enabled to clearly distinguish RCC patients and healthy controls with 80% sensitivity and 78% specificity if analyzed in combination (p<0.0001), and their levels significantly decreased in the time period of three months after radical nephrectomy (p<0.0001). Increased level of miR-378 positively correlates with disease-free survival (p=0.036) and clinical stage (p=0.0476). The analysis of serum miR-378 and miR-210 proved their potential to serve as powerful non-invasive diagnostic and prognostic biomarkers in RCC.

BACKGROUND: A detailed analysis of the expression of 440 cancer-related genes was performed after the combined treatment of medulloblastoma cells with all-trans retinoic acid (ATRA) and inhibitors of lipoxygenases (LOX) and cyclooxygenases (COX). The combinations of retinoids and celecoxib as a COX-2 inhibitor were reported to be effective in some regimens of metronomic therapy of relapsed solid tumors with poor prognosis. Our previous findings on neuroblastoma cells using expression profiling showed that LOX/COX inhibitors have the capability of enhancing the differentiating action of ATRA. Presented study focused on the continuation of our previous work to confirm the possibility of enhancing ATRA-induced cell differentiation in these cell lines via the application of LOX/COX inhibitors. This study provides more detailed information concerning the mechanisms of the enhancement of the ATRA-induced differentiation of medulloblastoma cells. METHODS: The Daoy and D283 Med medulloblastoma cell lines were chosen for this study. Caffeic acid (an inhibitor of 5-LOX) and celecoxib (an inhibitor on COX-2) were used in combined treatment with ATRA. The expression profiling was performed using Human Cancer Oligo GEArray membranes, and the most promising results were verified using RT-PCR. RESULTS: The expression profiling of the selected cancer-related genes clearly confirmed that the differentiating effects of ATRA should be enhanced via its combined administration with caffeic acid or celecoxib. This effect was detected in both cell lines. An increased expression of the genes that encoded the proteins participating in induced differentiation and cytoskeleton remodeling was detected in both cell lines in a concentration-dependent manner. This effect was also observed for the CDKN1A gene encoding the p21 protein, which is an important regulator of the cell cycle, and for the genes encoding proteins that are associated with proteasome activity. Furthermore, our results showed that D283 Med cells are significantly more sensitive to treatment with ATRA alone than Daoy cells. CONCLUSIONS: The obtained results on medulloblastoma cell lines are in accordance with our previous findings on neuroblastoma cells and confirm our hypothesis concerning the common mechanism of the enhancement of ATRA-induced cell differentiation in various types of pediatric solid tumors.

MikroRNA (miRNA) jsou početnou skupinou krátkých nekódujících RNA (cca 18–25 nukleotidů), které tlumí translaci vazbou na cílové mRNA, případně vedou k jejich degradaci. Tyto endogenní molekuly RNA jsou vysoce sekvenčně konzervované, a to i napříč nepříbuznými organizmy, což poukazuje na jejich roli v základních biologických procesech, jako je vývoj, diferenciace, proliferace a apoptóza. Rovněž se podílejí na regulaci kmenových vlastností buněk, imunitního systému nebo nádorové transformaci. Tento přehled podává ucelené informace o miRNA u karcinomu jícnu, a to jak o jejich roli v základních pochodech vzniku a rozvoje nádorového onemocnění, tak o možnostech jejich využití v klinické praxi při zpřesnění diagnózy, stanovení prognózy onemocnění a predikci léčebné odpovědi. Díky regulaci klíčových signálních drah v maligní transformaci představují miRNA rovněž slibné terapeutické cíle.
MicroRNAs are an abundant class of non‑coding RNAs (approx. 18–25 nucleotides in length) that suppress translation through binding to their target mRNAs, eventually leading to mRNAs degradation. Sequences of these endogenous RNA molecules are highly conserved, even among unrelated species, indicating their involvement in basic biological processes, such as development, differentiation, proliferation or apoptosis. MiRNAs also participate on regulation of cancer stem cell functioning, immune system and malignant transformation. This review provides a comprehensive overview of miRNAs functions in esophageal cancer, their roles in key pathogenetic pathways and disease development, as well as their potential usage in clinical routine as biomarkers improving diagnosis, prognosis and prediction of therapeutic response. Through regulation of signaling pathways important in malignant transformation, miRNAs present also promising therapeutic targets.
