Patients with chronic lymphocytic leukemia (CLL) have a high risk of poor outcomes related to coronavirus disease 2019 (COVID-19). This multicenter cohort study evaluated the impact of COVID-19 infection on the population of CLL patients in the Czech Republic. Between March 2020 and May 2021, 341 patients (237 males) with CLL and COVID-19 disease were identified. The median age was 69 years (range 38-91). Out of the 214 (63%) patients with the history of therapy for CLL, 97 (45%) were receiving CLL-directed treatment at diagnosis of COVID-19: 29% Bruton tyrosine kinase inhibitor (BTKi), 16% chemoimmunotherapy (CIT), 11% Bcl-2 inhibitor, and 4% phosphoinositide 3-kinase inhibitor. Regarding the severity of COVID-19, 60% pts required admission to the hospital, 21% pts were admitted to the intensive care unit (ICU), and 12% received invasive mechanical ventilation. The overall case fatality rate was 28%. Major comorbidities, age over 72, male gender, CLL treatment in history, CLL-directed treatment at COVID-19 diagnosis were associated with increased risk of death. Of note, concurrent therapy with BTKi compared to CIT was not associated with better outcome of COVID-19.

Until recently, a combination of anti-CD20 antibody plus less intensive chemotherapy was a standard of care in elderly population with previously untreated chronic lymphocytic leukemia (CLL). The aim of this observational study was to retrospectively assess efficacy and safety of obinutuzumab + chlorambucil (G-Clb), rituximab + chlorambucil (R-Clb), and bendamustine + rituximab (BR) given as the frontline therapy within routine practice. The final analyzed dataset included 398 consecutive CLL patients from 10 hematology centers cooperating within the Czech CLL Study Group: 63 treated with G-Clb, 78 with R-Clb, and 257 with BR. There were no significant differences in prognostic and predictive markers among the groups. On the contrary, median age at the start of therapy and cumulative illness rating scale (CIRS) score was significantly higher in R-Clb group. Obinutuzumab plus chlorambucil regimen was preferably offered to elderly patients (compared to BR) with less severe comorbidities and lower CIRS score (compared to R-Clb). A time period when a treatment was indicated had also a strong impact on the choice of the regimen. The overall response rate reached 76% (30% complete remissions, CRs) in G-Clb, 75% (22% CRs) in R-Clb, and 85% (47% CRs) in BR group. Median event-free survival was 49.0 months for G-Clb, 20.3 months for R-Clb, and 37.0 months for BR group. Neutropenia grade ≥ 3 developed in 43% of G-Clb, 31% of R-Clb and in 49% of BR patients, grade ≥ 3 infections were recorded in 17% of G-Clb, 6.4% of R-Clb, and 17% of BR patients. In conclusion, real-world therapeutic activity of G-Clb appears to be at least comparable to prospective clinical trial data. R-Clb yields relatively good results in very old and severely comorbid patients.

Úvod: U pacientů léčených pro tromboembolickou nemoc asociovanou s nádorem (TENAN) je doporučena léčba nízkomolekulárním heparinem (LMWH) po dobu prvních 6 měsíců; iniciální terapeutická dávka může být po uplynutí 1 měsíce redukována na 60–75 %. Po 6 měsících by měla být terapie přehodnocena a následná léčba LMWH nebo warfarinem by měla být podávána nejméně do doby vyléčení nádoru nebo vzniku závažné kontraindikace. Cíle: Zjistit použití léčebných modalit po uplynutí 6 měsíců od diagnózy TENAN ve vztahu k průběhu nádorového onemocnění. Sledovaná skupina: 87 pacientů (49 mužů a 38 žen) ve věku 27–87 let (průměrný věk 64 let) s léčených pro TENAN. Metodika: Pacienti byli prospektivně sledováni během léčby TENAN. Bylo analyzováno zastoupení pacientů, u nichž byl LMWH nahrazen warfarinem během prvních 6 měsíců a během celého sledování ve vztahu k průběhu nádorového onemocnění. Ke statistickému zhodnocení byly použity testy χ2 a χ2 s Yatesovou korekcí. Výsledky: Medián doby sledování od vzniku trombózy byl 445 dnů, průměrná doba sledování 743 dnů. 6 měsíců po diagnóze trombózy 9/30 (30 %) pacientů v kompletní remisi (CR) bylo léčeno warfarinem, zatímco pouze 3 z 12 (25 %) v parciální remisi (PR); 0 z 9 se stabilním onemocněním (SD) a 1 ze 13 s progresí onemocnění (7,7 %) byl léčen warfarinem. Pacienti v kompletní remisi a v parciální remisi byli léčeni warfarinem častěji než pacienti se stabilním či progredujícím nádorovým onemocněním (P = 0,02). Během celého sledování 25/30 (83,3 %) pacientů, jejichž nádor byl v 6 měsících od diagnózy trombózy v CR, bylo převedeno na warfarin, zatímco jenom 5/12 (41,7 %), 1/9 (11,1 %) a 1/13 (7,7 %) pacientů v PR, SD a progresi bylo převedeno na warfarin. Podíl pacientů v CR, převedených na warfarin, byl vyšší než podíl pacientů v PR (P = 0,007), v SD (P = 0,0003) a s progresí nádoru (P < 0,0001). Medián času od diagnózy trombózy do převodu z LMWH na warfarin byl u pacientů, kteří byli v 6 měsících v CR, 219 dnů a nebyl dosažen u pacientů, kteří v 6 měsících nebyli v CR. Závěr: Průběh maligního onemocnění je důležitým faktorem ve výběru léčebné strategie TENAN po 6 měsících i v následném období. Pacienti v kompletní remisi jsou častěji převedeni na léčbu warfarinem.
Background: The treatment with low-molecular-weight heparin (LMWH) is recommended for patients with cancer associated thrombosis (CAT) during the first 6 months; the initial therapeutic dose can be reduced to 60–75 % after one month. The therapy should be reevaluated after 6 month and subsequent therapy using LMWH or warfarin is recommended indefinitely, unless the cancer is resolved or any major contraindications arise. Aims: To analyze the usage of various treatment modalities in CAT patients after the initial 6 month period according to the course of cancer. Patients and methods: 87 patients with CAT were followed prospectively during the treatment period. The proportion of patients, in whom LMWH was replaced by warfarin during the first 6 months and during the entire follow up, was analyzed in relation to the course of the cancer. The χ2 test and χ2 test with Yates correction were used for the statistical evaluation. Results: The median follow-up was 445 days, mean duration of follow up was 743 days. 6 months after the diagnosis of thrombosis 9/30 (31.0 %) patients with complete remission (CR) of the cancer were treated with warfarin, while only 3/12 (25 %), 0/9 (0 %) and 1/13 (7.7 %) of the patients with partial remission (PR), stable disease (SD) and progression, respectively, were treated with warfarin. Patients with CR and PR were more frequently treated with warfarin than patients with SD or progression (P = 0.02). 13 patients died during the first 6 months, and 8 patients were followed less than 6 months. During the entire follow up, 25/30 (83.3 %) patients with CR switched to warfarin, while only 5/12 (41.7 %), 1/9 (11.1 %) and 1/13 (7.7 %) patients with PR, SD, and progression, respectively, were treated with warfarin. The proportion of patients with CR which switched to warfarin was higher, than the proportion of patients with PR (P = 0.007), with SD (P = 0.0003), and with progression (P < 0.0001). Median time from thrombosis onset to the switch from LMWH to warfarin was 219 days in patients in complete remission after 6 months and was not reached in patients in PR,SD, and progression. Conclusions: The course of malignant disease is important for the choice of appropriate therapy of CAT after the initial period of LMWH therapy. The patients achieving complete remission are more prone to switch from LMWH to warfarin.

Prasugrel, tikagrelor a kangrelor jsou nové protidestičkové léky působící cestou bloklády destičkových ADP receptorů P2Y12. Neproblematická bioaktivace (prasugrel) či přímý účinek látky (tikagrelor, kangrelor) jsou důvodem vyšší účinnosti nových farmak oproti referenčnímu léku této skupiny klopidogrelu. Akutní koronární syndrom a prevence následných aterotrombotických komplikací u pacientů podstupujících perkutánní koronární intervenci je indikací léčby novými protidestičkovými léky, které jsou podávány v kombinaci s kyselinou acetylsalicylovou s cílem potenciace účinku díky rozdílnému mechanizmu antiagregačního působení. Prasugrel s tikagrelorem jsou již schváleny a plně využívány v běžné klinické praxi. Kangrelor nebyl dosud doporučen ke klinickému využítí a jeho další osud je nejistý. Nejzávažnějším nežádoucím účinkem této skupiny léků zůstávají krvácivé komplikace, jejichž výskyt je vyšší než u klopidogrelu, což však plně koreluje s jejich vyšší účinností.
Prasugrel, ticagrelor and cangrelor are new antiplatelet drugs acting as platelets ADP receptor P2Y12 blockers. The reasons for the higher efficacy of new drugs compared to clopidogrel are unproblematic bioactivation (prasugrel) or a direct effect of the substance (ticagrelor, cangrelor). Acute coronary syndrome and prevention of his subsequent atherothrombotic complications in patiens undergoing percutaneous coronary intervention are an indication for treatment with these new drugs. They are administered in combination with acetylsalicylic acid to incerase the therapeutic effect. Prasugrel and ticagrelor are already approved and used in routine clinical practice. Cangrelor has not been recommended for clinical use and its fate is uncertain. The bleeding complications are the most serious adverse events of therapy with these drugs. The incidence is higher than for clopidogrel, but it fully correlates to their higher efficiency.

Cílem protidestičkové léčby je primární či sekundární prevence aterotrombózy, která je příčinou vážných chorob (akutní koronární syndrom, ischemický iktus, kritické ischemie končetin), jež jsou nejčastějšími příčinami úmrtí ve vyspělých zemích. Kyselina acetylsalicylová a klopidogrel jsou považovány za zlatý standard současné protidestičkové léčby s prokázaným klinickým přínosem. Jejich stinnými stránkami jsou nedostatečný efekt, rezistence k terapii u části populace, lékové interakce a zvýšené riziko krvácení. Nedostatečný účinek stávající protidestičkové léčby je pozorován rovněž u specifických skupin pacientů, jakými jsou např. diabetici 2. typu či pacienti s ischemickou chorobou dolních končetin. V současné době je již k dispozici celá řada nových protidestičkových léků inhibujících různé fáze tvorby destičkového trombu. Preparáty typu blokátorů ADP receptorů P2Y12, antagonistů destičkových trombinových receptorů PAR-1, blokátorů syntézy tromboxanu A2 či antagonisté jeho receptorů a antagonisté povrchových destičkových glykoproteinů (GP IIb/IIIa) prošly celou řadou klinických studií a některé již mají místo v klinické praxi. Lepší účinnost nových léků oproti standardům byla dosud prokázána pouze v některých specifických situacích. Protidestičková terapie je vždy spojena se zvýšeným výskytem krvácivých komplikací.
The aim of antiplatelet therapy is the primary and secondary prevention of atherothrombosis which causes serious diseases (acute coronary syndrome, ischemic stroke, peripheral artery disease) that are among the leading causes of mortality in developed countries. Acetylsalicylic acid and klopidogrel are considered as the gold standard of antiplatelet therapy with verified benefit. The main drawbacks of them are treatment failure, individual resistance to the therapy, drug interactions and increased risk of bleeding. A limited efficacy of classic antiplatelet drugs is observed in some specific groups of patients, for example in patients with type 2 diabetes mellitus or those with peripheral artery disease. Currently, numerous new antiplatelet drugs are available which inhibit different phases of platelet thrombus formation. Antiplatelet drugs targeting ADP receptors P2Y, thrombin receptors PAR-1, thromboxane A2 synthesis and IIb/IIIa glycoprotein receptors have been tested in a large number of clinical trials and some of them are used in clinical practice. But only some of the new drugs in some specific situations have been shown to be superior to aspirin or klopidogrel. Antiplatelet therapy is always associated with an increased incidence of bleeding complications. thromboxane A2 synthesis, IIb/IIIa glycoprotein inhibitors.
