BACKGROUND: Sutherland et al. recently published the Practical Guidelines for high-sensitivity detection of paroxysmal nocturnal hemoglobinuria (PNH) clones by flow cytometry (FCM), containing concise protocols for PNH testing. METHODS: Using this approach, we studied the intra- and interlaboratory variability observed in a multicenter study in which fresh blood samples containing three clinically relevant PNH clone sizes within the granulocytic, monocytic, and red blood cell (RBC) populations were shipped to each participating center. RESULTS: Coefficients of variation (CVs) for precision/reproducibility analysis ranged from 0.01%/0.02% to 0.48%/0.45% (big clone), from 0.69%/1.52% to 4.24%/5.80% (small-intermediate clone), from 1.47%/3.91% to 15.01% /17.83% (minor clone) for PNH white blood cells (WBCs) and from 0.24%/0.48% to 1.76%/1.83% (big clone), from 0.80%/1.14% to 2.39%/4.45% (small-intermediate clone), from 1.09%/3.36% to 10.54%/10.23% (minor clone) for PNH RBCs, respectively. Linear regression analysis showed excellent performance correlation between centers (r > 0.99), Wilcoxon rank test revealed no statistically significant differences for PNH granulocytes, monocytes, and RBCs (P > 0.05%), Bland-Altman analysis demonstrated good performance agreement for all target PNH clones (mean bias ranging from -1.47 to 0.71). CONCLUSION: Our results demonstrate very good intra- and interlaboratory performance characteristics for both precision and reproducibility analyses and excellent correlation and agreement between centers for all target PNH clone sizes. Our data confirm the reliability and robustness of the recently published Practical Guidelines approach for high sensitivity PNH testing by flow cytometry and suggest that such an approach represents an excellent basis for standardization of PNH testing by flow cytometry.

Paroxysmální noční hemoglobinurie (PNH) je získané klonální onemocnění krvetvorby. Patří do skupiny získaných hemolytických anémií a projevuje se hemolýzou červených krvinek různé intenzity a nadměrnou tendencí ke vzniku trombóz. Vzniká z důvodu proliferace kmenové buňky, které chybí povrchové antigeny vázané na buněčné membráně pomocí glykosylfosfatidylinositolové kotvy (GPI). Průtoková cytometrie se používá na stanovení PNH klonů od roku 1986 a od roku 1996 se považuje za metodu volby pro diagnostiku onemocnění. Vyšetření v současné době zahrnuje též monitorování velikosti PNH klonů v průběhu onemocnění – ve vztahu k progresi, regresi, remisi, transformaci či odpovědi na terapii – a dále je též zaměřeno na detekci nízkých počtů PNH buněk (< 1,0 %) v rámci monitorování pacientů s aplastickou anémií (AA) a myelodysplastickým syndromem (MDS). V praxi se používají odlišné metodické postupy. Průtoková cytometrie je stále nedostatečně standardizovanou metodou, závěry externích kontrol kvality navíc poukazují na různorodost jednotlivých výsledků. Cílem následující práce je poskytnout přehled současných možností vyšetření PNH buněk průtokovou cytometrií v souladu s naší dlouholetou zkušeností a v kontextu aktuálních trendů a doporučení pro diagnostiku a monitorování PNH.
Paroxysmal nocturnal hemoglobinuria is an acquired clonal disease characterized by proliferation of stem cells, deficient of proteins linked to the membrane via glycophosphatidylinositol (GPI) anchors. PNH cell characterization by flow cytometry was introduced in 1986, since 1996 is considered as method of choice for PNH diagnosis. Flow cytometry PNH analysis is nowadays crucial for disease monitoring in terms of progression, regression, remission or response to therapy and screening for small PNH clones (< 1.0%) in patients with aplastic anemia or myelodysplastic syndrome. Flow cytometry is unfortunately still poorly standardized, there is a variety of different methodological approaches for PNH evaluation and results from external quality assurances schemes reveal heterogeneous results. The aim of this work is to review the applicability of flow cytometry for the diagnosis and monitoring of PNH with respect to our experience and in the context of the recent trends and guidelines for PNH evaluation by flow cytometry.
In the present study, we compared three single platform methods for CD34+ hematopoietic stem cell (HSC) enumeration by flow cytometry. For this purpose, we analyzed the performance characteristics and results obtained from different HSC sources. Interlaboratory coefficients of variation (CV) for precision/reproducibility analysis varied from 4.0% to 6.7% / 6.7% to 9.2% for the low and 3.2% to 4.1% / 4.3% to 6.7%, respectively, for the high stem cell control. Correlation between methods ranged from 0.92% to 0.99%; Wilcoxon test showed no significant differences (p > 0.05); Bland-Altman analysis confirmed good agreement between assays (mean bias ranging from -0.48 to 6.91). Our results demonstrate very good intralaboratory correlation and agreement between methods, confirm the major impact of single platform strategy for accurate and reproducible HSC enumeration and suggest that high interlaboratory variability could be influenced by incorrect performance of validated methods.

Impaired diabetic wound healing is an important current medical issue, mainly concerning patients recovering from complicated operations or patients with ulcers on their feet. The obese Zucker diabetic fatty rat, with a mutation in leptin receptors, may be a good choice for studying impaired wound healing. Male and female rats were fed a diabetogenic high-fat diet. Wound size changes of air-exposed excisional 2 cm circular wounds were measured until Day 10. Wound tissue was analyzed morphologically, histologically, and immunohistochemically. The hydroxyproline content in the granulation tissue (GT) was determined. mRNA expression was assayed by DNA-array analysis and real-time reverse transcription-polymerase chain reaction. Wound-size changes were retarded in diabetic rats and differed between the sexes. Diabetic wounds were characterized by impaired contraction, abundant crust production, increased inflammation, and pus formation. On Day 10, the GT contained a significantly increased amount of intercalated fat tissue and showed an irregular arrangement of GT and collagen fibers. Interestingly, the length of new epithelium was increased in diabetic wounds. The concentration of hydroxyproline in the GT of diabetic animals was significantly decreased to about one half when compared with the nondiabetic controls. The expression of interleukin-6, myeloperoxidase, stromelysin-1, and collagenase-3 was increased in the GT of diabetic rats on Day 10, while the expression of type I collagen and elastin was decreased. Taken together, Zucker diabetic fatty rats exhibited impairments in wound-size reduction, inflammatory response, tissue organization, and connective tissue turnover and are thus proposed as a new model for studying impaired repair.

Průtoková cytometrie hraje významnou roli v charakterizaci krevních destiček a v analýze jejich funkcí ve vztahu k riziku trombózy a krvácení. V klinické praxi se uplatňuje v diagnostice vrozených poruch funkce destiček, imunitních a megakaryocytárních trombocytopenií, v analýze polymorfie systému lidských trombocytárních antigenů (HPA) ve vztahu k aloimunizaci, neonatální trombocytopenii, potransfuzní purpuře nebo refrakternosti na trombocytární přípravky. Klinicky významná je analýza aktivity cirkulujících destiček a stanovení trombocytárních mikročástic u pacientů s tranzitorní ischemickou atakou, akutním koronárním syndromem, kardiopulmonálním bypassem. V posledních letech se průtoková cytometrie uplatňuje úspěšně v měření účinnosti antiagregační léčby inhibitory receptoru P2Y12 a antagonisty komplexu IIb/IIIa. Relativním nedostatkem běžně používaných metod je nedostatečná standardizace a reprodukovatelnost. Cílem této práce je poskytnout přehled současných možností vyšetření krevních destiček a jejich funkcí metodou průtokové cytometrie v kontextu zavedených standardních postupů na našem pracovišti.
Flow cytometry is a significant tool for platelet characterization and functional analysis in relation to the risk of bleeding and thrombosis. In clinical practice it is useful for the diagnosis of inherited platelet disorders, immune and megakaryocytic thrombocytopenias, for platelet HPA-1 polymorphism analysis in association with alloimmunization, neonatal alloimmune thrombocytopenia, post-transfusion purpura or refractoriness to platelet infusion. The analysis of platelet reactivity and platelet microparticles in patients with transitory ischemic attack, acute coronary syndrome or cardiopulmonary bypass is clinically significant. In recent years, flow cytometry plays an increasing role in monitoring of antiplatelet therapy with P2Y12 receptor inhibitors and IIb/IIIa antagonists. Relative limitation of the majority of methods in use is poor standardization and reproducibility. The aim of this work is to review the potential role of flow cytometry analysis of platelets and their function in the context of the standardized techniques adopted in our laboratory.
