Cíl: Prezentovat případ heterotopické gravidity po intrauterinní inseminaci s akutně vzniklou bolestí v podbřišku. Metodika: Kazuistika s přehledem literatury. Závěr: Heterotopická gravidita je raritní diagnóza v gynekologii, která zůstává potenciálně nebezpečnou a život ohrožující komplikací gravidity. Včasná operační intervence je zásadní a současně umožňuje perzistenci a další vývoj intrauterinně lokalizovaného těhotenství.
Aim: To present a case of heterotopic pregnancy in a patient after intrauterine insemination with acute onset of abdominal pain. Methods: Own observation and literature review. Conclusion: Heterotopic pregnancy is a rare diagnosis in gynecology which remains a life threatening complication of pregnancy. Early and timely diagnosis is fundamental and allows for further development of intrauterine pregnancy.
Cíl práce: Popis případu intrakraniálního krvácení u rodičky v důsledku dekompenzace hypertenze při závažné preeklampsii s nutností provedení akutního císařského řezu s následnou hysterektomií. Typ práce: Kazuistika. Název a sídlo pracoviště: Gynekologicko-porodnické oddělení, Oblastní nemocnice Kladno, a.s. Vlastní pozorování: Prezentujeme případ 36leté duodecimgravidy ve 35. týdnu těhotenství bez prenatální péče. V osobní anamnéze měla jeden císařský řez před třemi roky a šest spontánních porodů bez komplikací, z toho jednou spontánně porodila gemini, dále měla tři miniinterupce a jeden spontánní abort. Pacientka byla přivezena na naše pracoviště ve velmi zavážném stavu s kritickými hodnotami krevního tlaku, silnou cefaleou, poruchou vidění, dysartrií a pravostrannou hemiparézou. Neurologem bylo doporučeno provedení urgentního nativního CT mozku, na kterém bylo zjištěno čerstvé intrakraniální krvácení. Nález podle neurochirurga nebyl vhodný k neurochirugickému operačnímu řešení. Pacientka byla indikována k akutnímu císařskému řezu. Vzhledem k silnému krvácení byla peroperačně připojena hysterektomie jako výkon nezbytný k záchraně života. Závěr: Prezentací tohoto závažného případu preeklampsie se autoři snaží poukázat na nutnost včasné diagnózy, mezioborové spolupráce, nasazení adekvátní terapie a ovládání techniky chirurgické intervence včetně abdominální hysterektomie, jako výkonu nezbytného k záchraně života při závažném peripartálním život ohrožujícím krvácení. Objective: To present a case of intracranial haemorrhage due to decompensated hypertension in case of severe preeclampsia which resulted in acute caesarean section followed by hysterectomy. Design: Case report. Setting: Department of Obstetrics and Gynaecology, Hospital Kladno. Case report: We report the case of a 36-year old multiparous woman in the 35th week of pregnancy with no prenatal care. According to her medical history, she had a caesarean section three years earlier and six previous spontaneous deliveries without complications (including spontaneously delivered twins). She underwent three abortions and one spontaneous miscarriage. This patient arrived to our department in serious condition with critical values of blood pressure, strong headache, impaired vision, dysarthria and right hemiparesis. In cooperationwith a neurologist we performed an acute native brain CT which revealed an acute intracranial haemorrhage. According to the neurosurgeon, this finding was not suitable for neurosurgical intervention. The patient was indicated for acute caesarean section. Due to heavy bleeding during the caesarean section, a hysterectomy was performed. Conclusion: In presenting this serious case of preeclampsia, the authors want to emphasize the need for early diagnosis, interdisciplinary cooperation, sufficient treatment and knowledge of surgical interventions such as abdominal hysterectomy, as a final solution for a life-threatening perinatal haemorrhage. Cíl práce: Popis případu úmrtí pacientky s fulminantním průběhem syndromu HELLP ve 32. týdnu gravidity. Typ práce: Kazuistika. Název a sídlo pracoviště: Gynekologicko-porodnické oddělení, Oblastní nemocnice Kladno a.s. Vlastní pozorování: Prezentujeme případ 29leté primigravidy ve 32. týdnu gravidity, která byla na naše pracoviště přivezena již ve velmi závažném stavu s cefaleou, zvracením a progredujícími celotělovými otoky; při příjmu dochází ke ztrátě vědomí pacientky, klinickému nálezu dominují kritické hodnoty krevního tlaku, je proveden urgentní císařský řez. Laboratorní obraz odpovídá plně rozvinutému HELLP syndromu, který postupně progreduje ze III. třídy až do I. třídy klasifikace Mississippi. Další stav pacientky si vyžádal opakované chirurgické intervence včetně provedení hysterektomie. I přes intenzivní terapii a multioborovou spolupráci však dochází k postupnému selhání životních funkcí a úmrtí pacientky. Závěr: Prezentací tohoto infaustního průběhu HELLP syndromu se autoři snaží o zdůraznění nutnosti časné diagnostiky a následného komplexního řešení ve spolupráci s ostatními obory při péči o kriticky ohroženou pacientku na porodním sále.
Objective: To present a case of death of a patient with fulminant course of HELLP syndrome in 32nd week of pregnancy. Design: Case report. Setting: Department of Obstetrics and Gynecology, Hospital Kladno. Case report: We report a case of 29 years old primigravida in 32nd week of pregnancy, which was admitted to the hospital in a serious condition with cephalea, vomiting and progressive full-body swelling. During the entrance examination the patient looses consciousness, due to critical pressure an emergency caesarian section is performed. Laboratory values correspond to a fully developed HELLP syndrome, which gradually progresses from class III to class I of Mississippi classification. The patient´s condition required further surgical interventions including hysterectomy. Despite intensive therapy and multidisciplinary cooperation a gradual failure of vital functions and death of the patient followed. Conclusion: Presenting this critical situation the authors want to emphasize the need of early diagnosis and subsequent comprehensive treatment in cooperation with other disciplines in the care of critically endangered patient on delivery room.

BACKGROUND: We assessed the results and impact of blockage of utero-ovarian anastomoses (UOA) on clinical outcome in women treated by laparoscopic uterine artery occlusion for uterine fibroids. METHODS: Between 2004 and 2005, we prospectively analyzed the clinical data for 23 laparoscopic uterine artery occlusion cases combined with blockage of utero-ovarian anastomoses (Group A) and 67 laparoscopic uterine artery occlusion cases alone (Group B). RESULTS: Of these 23 patients with UOA (mean age, 36.7+/-2.8 years), 10 patients (43.4%) had anastomoses bilaterally and 13 patients (56.6%) had unilateral anastomoses. Mean fibroid size reduction after LUAO and anastomoses blockage was 32.5% from baseline (P<0.001). In patients with LUAO, the mean DF size after surgery was estimated at 38.7+/-19.2 mm, which translated to a mean fibroid size reduction of 30.6% from baseline (P<0.001). No case of clinical failure or recurrence was found in Group A patients with UOA (mean follow-up, 15.6 months), who were treated with combined surgery. At a mean clinical follow-up of 18.2 months (Group B), 6 patients (8.9%) elected to undergo further surgical intervention for clinical failure and recurrence, including 4 myomectomies and 2 hysterectomies. The statistical difference between groups was not significant (P=0.33). CONCLUSION: Laparoscopic blockage of utero-ovarian anastomoses combined with uterine artery occlusion is a safe, feasible surgical procedure in women with symptomatic fibroids. Combining the uterine artery occlusion and blockage of UO anastomoses may be a useful procedure for the decreasing rate of clinical failure and recurrence. This premise should be confirmed in a larger prospective multicenter study.
OBJECTIVE: To assess pregnancy outcomes and deliveries after laparoscopic uterine artery transsection (LTUV) in symptomatic women with fibroids. SETTING: Department of Obstetrics and Gynecology, Endoscopic Training Center, Baby Friendly Hospital, Kladno, Czech Republic. DESIGN: One hundred and fifty three patients underwent laparoscopic transsection of uterine vessels during a 4-year period. RESULTS: Nine of the 21 women desiring pregnancy conceived spontaneously and one after anovulation treatment. The average age of the women was 32.4 years, and the range was 26-39 years. Two women had vaginal delivery at term and one delivered vaginally at 31 weeks secondary to premature preterm rupture of membrane (PROM). Four others delivered at term by cesarean section. One woman with placenta previa was delivered by cesarean section 3 weeks before term. Mean birth weight was 3199 g (range 1710-3910 g). One spontaneous abortion was reported in the first trimester of pregnancy. One case of undesired pregnancy occurred. An extrauterine pregnancy was reported in this woman. CONCLUSION: LTUV is a minimally invasive operative procedure, that preserves the uterus and ovarian blood supply and allows for the achievement of pregnancy in women with symptomatic fibroids. Fetal growth and umbilical Doppler findings remained normal in all cases. An increased risk for preterm delivery and cesarean section was found in this small series.

BACKGROUND: We assessed the results and impact of lateral uterine artery dissection on clinical outcome following laparoscopic myomectomy. METHODS: We retrospectively analyzed the clinical data for 27 laparoscopic myomectomy cases (Group I) and 54 laparoscopic myomectomy cases combined with lateral uterine artery dissection (Group II) between January 2001 and August 2004 in one center. Only 81 patients who had dominant fibroids between 4 cm and 10 cm in diameter were included in the study. We assessed the clinical outcomes: perioperative blood loss, operating time, hospital stay, complications, hemoglobin decrease, inflammatory response, and tissue markers (C-reactive protein, white blood cells, creatinine kinase) changes. RESULTS: The mean operating time was 70.37 minutes in group I and 78.61 minutes in group II. The mean length of hospital stay was 2.7 days versus 2.2 days, respectively (P>0.05). The difference in intraoperative blood loss was 70.1 mL (147.7 mL vs 77.3 mL, Group I) and 33.9 mL (105 mL vs 71.1 mL, Group II); estimated postoperative blood loss was statistically significant (P<0.001, P<0.05, respectively). Group 2 demonstrated a less intense stress response in C-reactive protein (P<0.001) and white blood cell count (P<0.05). CONCLUSION: The dissection of the uterine artery in laparoscopic myomectomy is a feasible operative procedure with a low rate of complications. The procedure reduced perioperative blood loss and resulted in significant improvement in fibroid-related symptoms.

Cíl studie: Prezentace dvou neobvyklých případů pánevní aktinomykózy. Typ studie: Kazuistika. Název a sídlo pracoviště: Gynekologicko-porodnické oddělení, Nemocnice Kladno. Předmět a metoda studie: Popis průběhu pánevní aktinomykózy komplikované v jednom případě šířením a fistulací do břišní stěny a v druhém případě do pochvy u pacientek léčených na našem oddělení. Závěr: Aktinomykóza je chronické onemocnění s tendencí k šíření do okolních tkání per continuitatem a k tvorbě fistul. Popisované jsou dvě méně obvyklé lokalizace šíření, a to břišní stěna a pochva. I v pokročilém stadiu je při adekvátní léčbě pravděpodobné vyhojení onemocnění ad integrum.
Objective: The presentation of two unusual cases of pelvic actinomycosis. Subject: Case reports. Settings: Department of Obstetrics and Gynecology, Hospital Kladno. Subject and Method: The observation of two cases of pelvic actinomycosis. These cases were complicated by the abdominal wall fistula in one case and vaginal fistula in the other. Conclusion: Actinomycosis is a chronic disease with tendency to progress per continuitatem into neighbouring tissues and with tendency to formation of fistulas. Two less common localisations of progress of the disease are described; it means the abdominal wall and vagina. In spite of a more advanced stadium, if adequately treated, a recovery ad integrum is common.
