Úvod V posledních letech v České republice narůstá incidence kolorektálního karcinomu. Pro další osud a léčbu nemocných je důležité rozpoznání tohoto onemocnění v počátečním stádiu, což jsou většinou malignizované polypy. Materiál a metodika V naší kohortové studii jsme provedli analýzu nemocných, kteří byli na našem pracovišti vyšetřováni koloskopicky, v letech 1991 – 2014 a u kterých byla provedena polypektomie. Nemocní po těchto výkonech jsou u nás dispenzarizováni a vyšetřováni v pravidelných intervalech. Výsledky Za uvedená léta jsme u 840 nemocných provedli koloskopii s polypektomií, u některých nemocných byly polypy vícečetné. Histologicky se jednalo v 511 případech o hyperplastický polyp, 248 x adenom s lehkou nebo střední dysplazií a ve 141 případech byl zjištěn v polypu karcinom, z toho 98 x nález klasifikován jako karcinom in situ nebo intramukozní karcinom a intramukozní karcinom s invazí do podslizniční svaloviny (Haggitt 0 a 1). U 12/98 (12,2 %) nemocných byly zjištěny metastázy jaterní. Závěr Na pacienty, jimž byl snesen polyp tlustého střeva a byl klasifikován jako Haggitt 0 a 1, nutno pohlížet jako na nemocné s maligním onemocněním. A je tedy nutné vyloučit diseminaci onemocnění. Je nutná dispenzarizace s vyšetřením koloskopickým, sonografickým, eventuelně CT a PET CT, a to vždy po roce. V rámci dispenzarizace jsou prováděny odběry onkomarkerů.
Introduction The incidence of colorectal cancer in the Czech Republic has been increasing in the last years. For prognosis and treatment of patients, is important to diagnose the disease in its early stage, which is most often malignant colorectal polyps. Material and method Our study analyzed a set of patients who underwent colonoscopy at our department between the years 1991 – 2014 and who then underwent a polypectomy. Patients who undergo these procedures are then dispensarized at our department and undergo regular check-ups. Result In the aforementioned period, colonoscopy with polypectomy was performed in 840 patients; multiple polyps were observed in some patients. Histological findings showed hyperplastic polyp in 511 cases, adenoma with mild to moderate dysplasia in 248 cases and carcinoma was discovered in 141 patients; of these 98 were classified as carcinoma in situ or intramucosal carcinoma and intramucosal carcinoma invading through muscularis mucosa (Haggitt 0 and 1). Liver metastases were seen in 12/98 patients, which is 12,2 %. Conclusion It is necessary to consider patients who underwent colorectal polypectomy and the polyps were classified as Haggitt 0 and 1 as patients with malignant disease. Therefore it is necessary to rule out disease dissemination. Dispensarization is essential, which includes colonoscopy, ultrasound examination, possibly PET CT examination, once a year. Tumor marker testing should also be performed as part of dispensarization.
AIM: The rate of rectal cancer locoregional recurrence following radical surgery varies from 4% to 33%. Though the causes are unclear, likely factors include microscopic tumor residues in the lymphatics, positive resection margins and exfoliation of tumor cells and their subsequent intraluminar spread during operation. Other significant factors include type and technique of surgical procedure. Recently, it has been demonstrated that local recurrence may also be associated with the biological behaviour of the tumor and/or with the composition of the cellular microenvironment which creates optimal conditions for the growth and spread of tumor cells. CASE REPORT: The presented case here is interesting because the tumour recurred early following a curative surgical procedure with negative resection margins, without positive lymph nodes, without infiltration of the pelvic wall and without distant metastases. CONCLUSION: In patients with a determined risk of genetically altered tumor field encompassing epithelial or stromal changes, a different treatment strategy, including gene therapy, anti-inflammatory or anti-angiogenic therapy should be chosen to minimize increased tumor risk.

Úvod: Svízelnou oblastí chirurgie tlustého střeva je řešení striktur kolorektální anastomózy. Toto do jisté míry souvisí i se zavedením staplerů a vzrůstajícím počtem resekčních svěrač zachovávajících výkonů. Na našem pracovišti při řešení striktur kolorektální anastomózy využíváme s dobrým efektem operační rektoskop. Materiál a metodika: Retrospektivně jsme hodnotili soubor 27 nemocných s benigní strikturou kolorektální anastomózy středního a horního rekta, kteří byli operováni na našem pracovišti v období 1/2004–12/2011 (od ledna 2004 do prosince 2011). Z celkového souboru 27 nemocných u 23 pacientů vznikla striktura na podkladě dehiscence anastomózy a u 4 nemocných se jednalo o strikturu anastomózy jiné etiologie. Výsledky: V souboru 27 nemocných bylo 14 pacientů se strikturou kolorektální anastomózy vyřešeno radiologicky pomocí vysokotlakého balonu. Celkem 13 pacientů bylo operováno, z toho 10 pacientů operačním rektoskopem a 3 pacienti podstoupili resekční výkon abdominálním přístupem. Závěr: Metodou první volby při řešení striktury kolorektální anastomózy je balonová dilatace. Při jejím neúspěchu můžeme použit operační rektoskop jako minimálně invazivní, přesto však efektivní a bezpečnou metodu.
Introduction: The treatment of the stenoses of colorectal anastomoses represents a difficult area of colonic surgery. This is partly connected to the introduction of staplers and an increasing amount of sphincter-preserving surgeries. At our clinic, we solve the stenoses of colorectal anastomoses using a surgical rectoscope with a good effect. Material and methods: We analysed retrospectively a group of 27 patients with benign stenosis of colorectal anastomosis of the medial and upper rectum who underwent surgery at our clinic in the period between January 2004 and December 2011. From the total amount of 27 patients, in 23 patients the stenosis was caused by the dehiscence of anastomosis, and in 4 patients the stenosis had a different etiology. Results: In the group of 27 patients, in 14 patients the stenosis of colorectal anatomosis was solved radiologically using a high pressure balloon. Total of 13 patients underwent surgery, in 10 of them we used the surgical rectoscope and 3 patients underwent an open abdominal procedure. Conclusion: The first method of choice in treatment of the stenoses of the colorectal anastomoses is a balloon dilatation. If this is not successful we can use the surgical rectoscope as a minimally invasive though effective and safe method.
