Okultní primární karcinom prsu je velice vzácný typ nádoru, který se projevuje axilární lymfadenopatií bez průkazu ložiska v prsu. S vývojem zobrazovacích metod (magnetická rezonance, PET/CT, PET/MRI apod.) se daří detekovat velkou část nádorů, které jsou na základních vyšetřovacích metodách mamografii a ultrasonografii, neviditelné. V současné době není shoda v názorech na optimální léčebný postup tohoto onemocnění. Obecně se doporučuje léčba stejná jako u primárního karcinomu prsu s metastázou do axilárních lymfatických uzlin, tzn. neoadjuvantní onkologická léčba, disekce axily s adjuvantní radioterapií a mastektomie nebo ozáření prsu. Nicméně několik současných studií tento postup vyvrací a poukazuje na méně radikální postupy se srovnatelnými výsledky. Práce popisuje případ 62leté pacientky s okultním karcinomem prsu, postup a výsledek zobrazovacích vyšetření a následně management léčby. Dále uvádí a diskutuje léčebné postupy a jejich výsledky, které jsou publikovány v současné literatuře.
Occult breast cancer is a very rare type of cancer which presents with axillary lymphadenopathy with no visible mass in the breast. Advances in imaging methods (MRI, PET/CT, PET/MRI, etc.) have enabled the detection of a large number of lesions which are not visible using basic imaging methods, such as mammography and ultrasound. To date, optimal management of this type of cancer is lacking. Generally, treatment of occult breast cancer is that of primary breast cancer with axillary lymph node involvement. This includes neoadjuvant oncological therapy, axillary dissection with adjuvant radiation therapy and either mastectomy or radiation to the breast. However, several recent studies have shown that similar results may be achieved with less radical treatment. The paper describes the case of a 62-year-old patient with occult breast cancer, the procedure and results of imaging assessments, and subsequently the treatment management. Furthermore, the paper reports on current treatment trends published in the literature.

Úvod: Neoadjuvantní terapie (NT) je jedna z možných strategií onkologické léčby u malignit prsní žlázy. Cílem je downstaging nádorového postižení prsu a axily a tím možnost konverze mastektomie na záchovný výkon prsu a provedení šetrnější cílené operace axilárních uzlin. Úlohou radiologa je provést pomocí zobrazovacích metod přesný lokální staging malignity před podáním NT, hodnotit efekt léčby v jejím průběhu a po ukončení léčby provést restaging nádorového postižení prsu a axily. Kazuistiky: Autoři prezentují ve třech kazuistikách pacientek s neoadjuvantní chemoterapií (NCHT) diagnostický postup a zobrazování při stanovení lokálního stagingu malignity před léčbou, při sledování v průběhu podávání a při provedení restagingu malignity po ukončení NCHT. Radiologická odpověď po ukončení NCHT je korelována s patologickou odpovědí. Závěr: Správné stanovení rozsahu nádorového postižení prsu a axily radiologem před léčbou a přesné histologické posouzení nádoru patologem jsou zásadní pro rozhodnutí o následné terapii u pacientů v mamárním týmu.
Introduction: Neoadjuvant therapy (NT) is one of the possible oncological treatment strategies for breast cancer. Its aim is to achieve down-staging of the tumour in the breast and axilla and thus the possibility of converting mastectomy to a breast-conserving procedure, and also to allow for a less burdensome and more targeted operation of the axillary lymph nodes. The role of the radiologist is to utilise imaging procedures for precise local staging of the malignancy prior to NT, to evaluate the effect of treatment during its course and upon its completion, and to perform restaging of the cancer in the breast and axilla. Case reports: The authors present three case reports of female patients with breast cancer who underwent neoadjuvant chemotherapy (NCT). They describe the diagnostic procedure and imaging methods used to establish local staging of the cancer prior to treatment, to monitor the disease during the course of treatment, and to perform restaging of the cancer after completing NCT. The radiological response after NCT completion was correlated with the pathological response. Conclusion: Correct determination of the extent of the cancer in the breast and axilla by the radiologist before NT and precise histological analysis of the tumour by the pathologist are fundamental for selecting the appropriate treatment for patients at the multidisciplinary breast tumour board.

AIM: The aim of this retrospective study was to analyse the preoperative ultrasound findings in patients with minimal or almost no morphological changes of axillary lymph nodes (LN) and to correlate these findings with the results of sentinel node (SN) biopsy. MATERIALS AND METHODS: Between January 2014 and September 2018, 289 female patients with newly diagnosed breast cancer and negative preoperative axillary staging were examined with preoperative ultrasound evaluation of axillary LNs. Patients with no evidence of LN metastases underwent primary surgical treatment with SN biopsy. Negative predictive value (NPV) of preoperative ultrasound was evaluated and the histopathological findings in positive SN biopsies were correlated with tumour type and preoperative ultrasound LN imaging. RESULTS: Of 289 patients with negative preoperative axillary staging who had primary surgical treatment, 268 patients had negative SN biopsy while SN metastases were detected in 21 patients. Of patients with positive SN biopsies, 2 patients had negative core biopsy of axillary LN before surgery. The preoperative ultrasound examination was negative in the remaining 19 patients with SN metastases. CONCLUSIONS: Preoperative ultrasonography is very accurate in the detecting of axillary LN metastases. Patients with primary tumour size ≥ 1 cm, with grade ≥ 2 no special type carcinomas (NST - no special type, also known as invasive ductal carcinoma) or multicentric lobular invasive cancer should undergo a more thorough ultrasound evaluation.
Mezi nejčastěji diagnostikované lokálně pokročilé nádory prsu s exulcerací patří maligní nádory prsní žlázy - nejčastěji invazivní duktální karcinom (dle novější histologické klasifikace invazivní karcinom NST - "no special type"), ev. invazivní lobulární karcinom prsu. V tomto sdělení je popsán histologicky vzácný případ lokálně pokročilého maligního adenomyoepiteliomu prsu. Pacientka s lokálně pokročilým exulcerovaným nádorem prsu, který ze dvou předoperačních biopsií vychází jako benigní adenomyoepiteliom, podstoupila na našem pracovišti radikální mastektomii. Z definitivní histologie vyplývá, že šlo o benigní adenomyoepiteliom s předpokládanou malignizací epitelové složky. U lokálně pokročilých nádorů benigního histologického typu je nutné předem myslet na přechodové formy těchto nádorů nebo na možnou malignizaci jedné ze složek histologicky různorodých nádorů. Vzhledem k nejisté povaze adenomyoepiteliomů by vždy měla následovat chirurgická resekce s negativními resekčními okraji.
Malignant tumors of the mammary gland - most commonly invasive ductal carcinoma (according to the new histological classification invasive carcinoma NST - no special type) and invasive lobular cancer are most frequently seen in the locally advanced breast tumors with ulceration of the skin. This report describes a histologically rare case of locally advanced malignant breast adenomyoepithelioma. A patient with locally advanced exulcerated breast tumor, with benign preoperative histopathological finding of benign adenomyoepithelioma, underwent a radical mastectomy at our department. The definitive histology suggests that it was a benign adenomyoepithelioma with supposed malignant epithelial component. In locally advanced tumors of benign histological type, it is necessary to think about the transient forms of these tumors or the possible malignancy of one of the components of histologically diverse tumors. According to uncertain nature of adenomyoepitheliomas, surgical resection with negative resection margins should always follow.

Východiska: Triple negativní karcinomy prsu (triple-negative breast cancer - TNBC) jsou považovány za morfologicky a geneticky heterogenní skupinu karcinomů prsu charakterizovanou nízkou či nulovou expresí estrogenových i progesteronových receptorů, HER2/neu/ERBB2 proteinu, specifickým biologickým chováním a odpovědí na léčbu. Cílem naší práce bylo hodnocení intenzity lymfo/plazmocytární infiltrace (tumor-infiltrating lymphocytes/plasmocytes - TILs) v nádorovém parenchymu, jeho stromatu a v okolí invazivní fronty u časného karcinomu prsu bez předchozí chemoterapie a její korelace s morfologií nádoru, proliferační aktivitou, expresí Bcl-2 a dalšími vybranými klinicko-patologickými parametry. Materiál a metody: Z celkového počtu 3 544 karcinomů prsu diagnostikovaných na našem pracovišti mezi lety 2007 a 2017 bylo 413 TNBC (11,65 %). Z toho jsme získali pouze 61 případů, které byly primárně léčeny chirurgicky a které neměly morfologii poškozenou chemoterapií. U nich jsme korelovali intenzitu infiltrace TILs s morfologií, expresí Bcl-2 proteinu, proliferační aktivitou měřenou expresí Ki-67, věkem pacientek, stupněm diferenciace (grade) a výskytem metastáz. Výsledky: V našem souboru TNBC dominovaly invazivní karcinomy NST (no special type) s medulárními rysy (80,32 %), které formovaly poměrně dobře ohraničené léze průměrné velikosti 13,4 mm (rozmezí od 5 do 20 mm, medián 15 mm) s centrální nekrózou či fibrózou, často s vřetenobuněčnou transformací a/nebo alespoň v malém úseku s ložisky podobnými apokrinní diferenciaci a variabilně intenzivním TILs. V souboru se však vyskytly i nádory, jejichž morfologie není typická pro TNBC. Prokázali jsme statisticky významnou pozitivní korelaci infiltrace TILs s premenopauzálním stavem (p = 0,003), expresí Ki-67 (p = 0,015), gradem (p = 0,002), hraniční pozitivní korelaci s velikostí nádorů (p = 0,065) a statisticky významnou negativní korelaci s expresí proteinu Bcl-2 (p = 0,035). U mladších žen (do 50 let) s velikostí nádorů do 20 mm (pT1a-pT1c) jsme zaznamenali nižší počet případů s metastatickým postižením lymfatických uzlin (p = 0,001). Závěr: Intenzita a lokalizace TILs infiltrace vyhodnocovaná v kontextu morfologických změn a dalších klinicko-patologických znaků u terapií neovlivněných TNBC může mít prognostický význam a poskytnout podklad pro plánování efektivnější terapie.
Background: Triple-negative breast cancers (TNBCs) are considered a morphologically heterogeneous group of breast carcinomas characterized by the absence or low protein expression of hormone receptors and HER2/neu/ERBB2 with a specific biological behavior and therapeutic response. This study aimed to evaluate correlations of the density of tumor-infiltrating lymphocytes/plasmocytes (TILs) in the tumor parenchyma, stroma, and invasive margins with tumor morphology, the proliferation rate, Bcl-2 expression, and selected clinical and pathological parameters in early breast cancer patients prior to mastectomy who had not received initial chemotherapy. Materials and methods: Samples of 3,544 breast cancer patients investigated in our department between 2007 and 2017 were re-examined. In total, 413 (11.65%) patients were diagnosed with TNBC. Only 61 cases did not undergo neoadjuvant therapy prior to mastectomy. Correlations between the density of TILs and tumor morphology, Bcl-2 expression, proliferative activity measured by Ki-67, patient age at diagnosis, tumor grade, and metastases were investigated. Results: The samples were predominantly relatively well-localized invasive carcinomas of no special type with medullary features (80.32%) that measured on average 13.4 mm (range 5-20 mm, median 15 mm) and exhibited central necrosis or fibrosis, a tendency to undergo spindle cell and/or apocrine-like differentiation, and intensive infiltration of TILs. There were significant positive correlations between TILs and premenopausal status (p=0.003), Ki-67 expression (p=0.015), and tumor grade (p=0.002), a marginal positive correlation between TILs and tumor size (p=0.065), and a significant negative correlation between TILs and Bcl-2 expression (p=0.035). In younger patients (< 50 years) with tumor size less than or equal to 20 mm (pT1a-pT1c) we recorded a lower number of women with metastatic lymph node involvement (p=0.001). Conclusion: The density and location of TILs in non-therapeutically influenced TNBCs, evaluated in the context of morphological changes and other clinicopathological parameters, may have prognostic significance and assist effective therapy planning.

BACKGROUND: The management of internal mammary nodes (IMNs) during multidisciplinary treatment of breast cancer has been debated for the last four decades without unequivocal conclusion. PATIENTS AND METHODS: We retrospectively reviewed patients with breast cancer who underwent sentinel lymph node biopsy at our center from 2008 until 2012. IMN drainage was assessed as a potential risk factor for local and distant disease recurrence. RESULTS: We identified 712 patients, with incidence of drainage to IMNs of 18.4%. No detrimental effect of the pattern of drainage to IMNs was found after a median follow-up of 58 months. A similar outcome was observed when drainage to IMNs was evaluated as a risk factor for patient survival. The potential risk factors for drainage to IMNs during sentinel lymph node biopsy were younger age (p=0.002) and tumor location in lower-outer, lower-inner, and upper-inner versus upper-outer quadrant (p<0.0001). CONCLUSION: The drainage to IMNs is unlikely to have a detrimental effect on patient outcome.

Pathologic complete response is an indicator of long-term disease control after neoadjuvant chemotherapy in breast carcinoma. In our earlier study, patologic complete response was observed significantly more often in patients with tumors expressing HER-2 or triple negative tumors. The aim of the present study is to study biologic response to neoadjuvant chemotherapy in patients treated with standard regimens that will not be affected by the project. In a retrospective study on material from patients in 2 centers difference in tumor infiltrating lymphocytes in patients with different tumor phenotype as well as the association between indoleamine 2,3-dioxygenase expression and the presence of tumor infiltrating lymphocytes will be evaluated together with tumor phenotype using immunohistochemistry. Alterations of iron metabolism and citrulline, an indicator of intestinal toxicity, will also be assessed.
Dosažení patologické kompletní odpovědi po neoadjuvantní chemoterapii je ukazatelem dlouhodobé kontroly onemocnění. V předchozím projektu byla patologická kompletní odpověď pozorována významně častěji u nemocných s nádory expresí HER-2 a bez exprese HER-2 i hormonálních receptorů. Cílem předkládaného projektu je studovat biologickou odpověď na neoadjuvantní chemoterapii. Použity budou standardní režimy chemoterapie používané v jednotlivých centrech a volbu léčby projekt neovlivní. V retrospektivní studii na materiálu nemocných z 2 center budou vyhodnoceny rozdíly tumor infiltrujících lymfocytů u nemocných s různým fenotypem nádoru, bude také studován vliv exprese indoleamin 2,3-dioxygenázy na přítomnost tumor infiltrujících lymfocytů a bude provedeno imunohistochemické zhodnocení fenotypu nádoru. Budou rovněž sledovány změny metabolismu železa a citrulin, ukazatel toxicity léčby.
