OBJECTIVE: Allergic contact dermatitis (ACD) in the healthcare sector is a major occupational health hazard. There are many reasons for a higher frequency of ACD in healthcare personnel compared to other populations: among others, simultaneous exposure to multiple substances, use of aggressive detergents and wet work. However, studies that systematically correlate skin symptoms with the presence of sensitization investigated through patch tests in specific categories of health workers are very rare and conflicting. Although some studies have reported a correlation between skin disease and night shift, the strength of the evidence is rather limited. The purpose of our study was to investigate by means of patch testing the skin sensitization (SS) to common allergens in the hospital setting in a group of healthcare workers (HCW) reporting symptoms related to dermatitis, according to their job activity and their shift status. METHODS: 132 HCWs visiting a health surveillance centre were investigated by means of specific questionnaire for dermatitis, followed by patch test evaluation including 40 haptens of the SIDAPA 2016 series. RESULTS: Skin sensitization was observed in 1/3 of the subjects investigated by patch tests. The nursing job was strongly associated with cutaneous reactivity after controlling for the confounding of gender, age and other factors. Shift work was related to the prevalence of SS. CONCLUSIONS: In our study, the nurse's role and shift work were significantly associated with the risk of cutaneous sensitization, in particular for common antigens.
BACKGROUND: Chronic skin exposure to a wet work environment, as well as disinfectants are factors contributing to epidermal barrier damage. This disturbs its protective functions and promotes the development of dermatoses. All these factors occur in the work environment of a laboratory diagnostician. The aim of the paper was to analyze selected skin parameters and skin lesions in the hands of laboratory diagnosticians. MATERIAL AND METHODS: Overall, 50 laboratory diagnosticians employed in a laboratory in Kraków, Poland, took part in the study. After the interview, the skin pH and moisture content were examined. Measurements were performed on the dorsal side of the dominant hand using the Skin-ph-Meter® PH 905 and Corneometer® CM 825 devices. RESULTS: The obtained moisture level values were qualified as dry or very dry skin, and the skin pH was within the reference range. No correlation between the skin pH value and the features examined (professional experience, the number of hours spent in protective gloves during the day, the number of hand washing episodes during the day) was not indicated. The level of skin moisture content seems to positively correlate with the time of wearing gloves. The use of hand moisturizers after each hand washing episode correlated with the level of skin moisture content. CONCLUSIONS: During the COVID-19 pandemic, this professional group has been facing huge challenges, and mental and physical stressors (including wet work environment) will be the source of future occupational diseases. Tubera mulgentium (hrboly dojičů) jsou infekčním virovým kožním onemocněním. Původcem je virus paravakcinie, přenáší se přímým kontaktem z nakažených krav na člověka. Autoři popisují případ 50leté pacientky zaměstnané jako dojička v kravíně, u které se vyvinul na ruce dojičský hrbol. Diagnóza byla stanovena na základě anamnézy, klinického vzhledu a histologie. Léčba lokálními antiseptiky vedla k vyléčení během tří týdnů. Onemocnění bylo uznáno jako choroba z povolání. V článku je uveden stručný přehled poznatků o tomto onemocněni.
Tubera mulgentium (Milker´s nodules) is an infectious viral skin disease. It is caused by the paravaccinia virus, transmitted from infected cows to humans by direct contact. The authors describe a case of a 50-year-old patient working as a milker in a cowshed, who developed Milker´s nodules on her hand. The diagnosis was based on the patient´s history, clinical appearance and histology. Treatment with local antiseptics led to healing within 3 weeks. The disease was recognized as an occupational disease. The article provides a brief review of current knowledge about this disease.

V článku jsou uvedeny nejvýznamnější profesionální dermatózy s jejich stručnou charakteristikou, diagnostikou a zásadami léčby. Zdůrazněn je význam správného postupu v rámci léčebně preventivní péče.
In the article we present the most important occupational diseases with brief features , diagnostics and principles of their treatment.The importace of correct process concerning therapeutic and preventive care is emphesized.
Kožní nemoci z povolání patří k důležitým nemocem z povolání a jejich počet v roce 2017 dosáhl téměř 300. Představují tak téměř 25 % všech nemocí z povolání. Kontaktní alergická dermatitida je se 131 případy na 3. místě ve frekvenci diagnóz; jejími hlavními příčinami jsou plastické hmoty, guma a její složky, kovy, dezinfekční prostředky, barvy a detergenty. Mezi infekčními kožními nemocemi byl nejčastější scabies s 95 případy. Naproti tomu byl zaznamenán významný pokles některých kožních nemocí, např. acne oleosa a kontaktní iritační dermatitida, způsobené prací s minerálními oleji, organickými rozpouštědly nebo detergenty. Článek detailně popisuje diagnostická kritéria, legislativu a postup ověřování profesní expozice jakožto kauzálního faktoru vzniku onemocnění. Jsou popsána i diagnostická úskalí posuzování kožních nemocí z povolání. Pracovní podmínky se stále mění díky inovativním technologiím a technickému pokroku. Tyto změny jsou také důvodem, proč seznam nemocí z povolání nemůže být neměnný a "otevřený" seznam nejlépe postihuje preventivní charakter oboru pracovní lékařství.
Occupational skin diseases are one of the most important occupational diseases and their number reached nearly 300 in the Czech Republic in 2017. They represent almost 25% of all occupational diseases. Allergic contact dermatitis was the 3rd most frequently reported occupational disease with 131 cases; it is most often caused by plastic materials, rubber and rubber components, metals, disinfectants, paints and detergents. Among infectious skin diseases, scabies was the most frequent with 95 cases. On the other hand, a significant decrease of several skin diseases was recorded, such as acne oleosa and irritant contact dermatitis that are most frequently caused by mineral oils, organic solvents or detergents. The diagnostic criteria, legislation and the process of verification of occupational exposure as the key factor leading to the disease, are described in the article, as well as diagnostic difficulties of occupational skin diseases assessment. Working conditions are constantly changing due to innovative technologies and technical progress. These changes are the main reason, why the "open list" of occupational skin diseases best reflects the preventive needs of occupational medicine.

Přecitlivělost na epoxidové pryskyřice je běžnou příčinou kontaktní alergické dermatitidy, velmi často dochází k senzibilizaci při práci s nevytvrzenými pryskyřicemi. Vytvrzené pryskyřice ztrácejí iritační i senzibilizační potenciál. Kontaktní alergie se může vyvinout na epoxidy, ale i na tvrdidla a další látky epoxidových systémů. Nejčastěji postiženými profesemi jsou lakýrníci, podlaháři a dělníci v elektrotechnice. Je uveden typický případ profesionální kontaktní alergické dermatitidy na epoxidové pryskyřice u pacienta, který pracoval jako lakýrník. Při práci používal barvy na bázi epoxidů. V klinickém obraze dominovaly ekzémové projevy na obličeji a na předloktích. Epikutánními testy byla prokázaná kontaktní přecitlivělost na epoxidovou pryskyřici – bisfenol A, který je obsažen v Evropské standardní sadě epikutánních testů. Po vyřazení z kontaktu s epoxidy došlo k postupnému odhojení projevů kontaktní alergické dermatitidy.
Sensitivity to epoxy resins is a common cause of allergic contact dermatitis which often occurs during work with uncured epoxies. Hardened epoxy resins loose their irritant and allergic potential. Contact allergy may appear to epoxy resins, hardeners and other substances of epoxy systems. The most affected professions are lacquerers, floor workers and workers in electronic industry. A typical case of professional contact dermatitis to epoxy resins in pacient who worked as lacquerer with epoxy paints is reported. The patient presented with eczema eczema on the face and on the forearms. The contact allergy to epoxy – resin Bisphenol A which is included in European Standard Series of Patch Test Allergens, was proved by patch testing. After elimination of contact with epoxy resins the contact allergy symptoms gradually disappeared.
