The aim of this study was to collect data from patients who underwent liver transplantation (LT) for adenomatosis; to analyze the symptoms, the characteristics of the disease, and the recipient outcomes; and to better define the role of LT in this rare indication. This retrospective multicenter study, based on data from the European Liver Transplant Registry, encompassed patients who underwent LT for adenomatosis between January 1, 1986, and July 15, 2013, in Europe. Patients with glycogen storage disease (GSD) type IA were not excluded. This study included 49 patients. Sixteen patients had GSD, and 7 had liver vascular abnormalities. The main indications for transplantation were either a suspicion of hepatocellular carcinoma (HCC; 15 patients) or a histologically proven HCC (16 patients), but only 17 had actual malignant transformation (MT) of adenomas. GSD status was similar for the 2 groups, except for age and the presence of HCC on explants (P = 0.030). Three patients with HCC on explant developed recurrence after transplantation. We obtained and studied the pathomolecular characteristics for 23 patients. In conclusion, LT should remain an extremely rare treatment for adenomatosis. Indications for transplantation primarily concern the MT of adenomas. The decision should rely on morphological data and histological evidence of MT. Additional indications should be discussed on a case-by-case basis. In this report, we propose a simplified approach to this decision-making process.

Fokální nodulární hyperplazie a adenomy jsou benigní hepatocelulární afekce. Riziko život ohrožujícího krvácení a maligního zvratu u adenomů vyžaduje přesnou diagnózu obou jednotek a jejich odlišení nejen od sebe navzájem, ale i od jiných tumorů jater. Pokrok v oblasti zobrazovacích metod a zejména v histopatologické diagnostice významně zpřesnil odlišení těchto lézí nejen na úrovni zobrazovacích metod, ale i v resekátech a jehlových biopsiích. Magnetická rezonance s hepatospecifickou kontrastní látkou hraje důležitou roli v diagnóze, v odlišení některých subtypů adenomů a v detekci komplikací adenomů; umožňuje také diferenciální diagnózu adenomu a fokální nodulární hyperplazie. Využití genetických a na ně navazujících imunohistochemických metod dovoluje odlišit podskupiny adenomů s různým rizikem krvácení a maligního zvratu – to umožňuje individualizovanou léčbu chirurgickou u vysoce rizikových forem a konzervativní léčbu u malých ložisek s nízkým rizikem maligní transformace. Diagnóza je výsledkem komplexního zhodnocení klinického a laboratorního obrazu, zobrazovacích metod a morfologického zhodnocení. Terapeutický přístup je individuální a vyžaduje multidisciplinární spolupráci zkušených odborníků.
Focal nodular hyperplasia and adenomas are benign hepatocellular lesions. The risk of life‑threatening bleeding and malignant transformation of adenomas require accurate diagnosis of both units and their differentiation not only from each other but also from other liver tumors. The progress in the area of imaging methods and particularly in the histopathological diagnosis leads to a more accurate differentiation using imaging methods, resection specimens and needle biopsies. Magnetic resonance imaging with a contrast medium plays an important role in the diagnostics, in the differentiation of adenoma subtypes and in the detection of adenoma complications; it also allows to establish differential diagnosis of focal nodular hyperplasia and adenoma. The use of genetic and immunohistochemical methods helps to distinguish adenoma subgroups with varying risk of hemorrhage and malignant transformation – this enables the application of an individualized surgical treatment in high risk adenomas and conservative treatment of small tumors with low risk of malignant transformation. The diagnosis is a result of a comprehensive assessment of the clinical and laboratory image, imaging methods and morphological evaluation. The therapeutic approach is individualized and requires multidisciplinary cooperation of experienced professionals. Nejčastějším primárním maligním nádorem jater je hepatocelulární karcinom, který představuje 80–85 % všech maligních nádorů jater. Druhým nejčastějším maligním tumorem je cholangiocelulární karcinom. V poslední době bylo dosaženo velkého pokroku v oblasti molekulární patologie nádorů jater, což vedlo k nové klasifikaci adenomů jater, kde vztah mezi genotypem a fenotypem lze snadno využít v rutinní histopatologické diagnostice. V oblasti diagnostiky časných neoplazií bylo dosaženo dohody v definování a odlišení dysplastických nodulů od časného hepatocelulárního karcinomu. V diferenciální diagnóze se využívá rozpoznání stromální invaze a imunohistochemického průkazu glypicanu-3. Mnoho problémů ale stále existuje v kategorii smíšených hepatocelulárních a cholangiocelulárních karcinomů. I když tato kategorie je zahrnuta ve WHO klasifikaci, diagnostická kritéria i terminologie jsou nejasné a zmatečné. Uvádíme problémy a nástrahy histopatologické diferenciální diagnostiky maligních nádorů jater.
The most common primary hepatic malignancy is hepatocellular carcinoma, which constitutes 80–85% of all malignant epithelial neoplasms originating in the liver. The second most common primary hepatic malignancy is cholangiocellular carcinoma. In recent years, remarkable progress has been made in elucidating the molecular pathology of hepatic tumors. Advances in our understanding of molecular subtypes have led to a creation of a new classification system of hepatocellular adenomas, with important genotype-phenotype correlations and easy application to routine diagnostic practice. In the field of early hepatic neoplasia, a consensus on the definition of dysplastic nodules and early hepatocellular carcinoma has been reached. Immunohistochemical detection of glypican-3 and stromal invasion are used for the differential diagnosis. A lot of problems still exist in the category of mixed hepatocellular and cholangiocellular carcinoma. Although this category is recognized in the WHO classification, confusing terminology has been generated to describe tumors with morphological features of both HCC and CC. We further specify problems and pitfalls of the differential histopathological diagnosis of liver malignant tumors.

Adenomy jater a fokální nodulární hyperplazie (FNH) představují benigní hepatocelulární afekce jater a jejich rozlišení v jehlové biopsie a někdy i v resekátu bývalo často velmi problematické. Přestože by se mohlo zdát, že rozlišování benigních afekcí nemá pro klinika a tím i pro pacienta valný význam, opak je pravdou. Riziko život ohrožujícího krvácení a riziko maligního zvratu u adenomů vede k požadavku přesné diagnostiky těchto afekcí. Využití genetických metod a na ně navazujících imunohistochemických průkazů umožňuje nejen přesnější odlišení obou hlavních skupin FNH a adenomů, ale dovolí i odlišit podskupiny adenomů s různým rizikem maligního zvratu. Proto je v současnosti určení subtypu adenomu nezbytnou součástí bioptické diagnózy. Identifikace podskupiny následně umožňuje individualizovanou léčbu s resekcí u vysoce rizikových forem a na druhé straně umožňuje rozhodnutí vyhnout se resekci jater v případě malého ložiska s nízkým rizikem maligní transformace. Adenomas of the liver and focal nodular hyperplasia (FNH) are benign hepatocellular affection and their distinguishing in a needle biopsy sample and sometimes also in a surgical specimen causes often a problem. Although it might seem that the differentiation of the benign conditions is of a low value for the clinicians and also for the patients, the opposite is true. The risk of life-threatening bleeding and risk of the malignant transformation of adenomas leads to request accurate diagnosis of these conditions. New genetic methods followed by immunohistochemical detection of several antigens enables more accurate distinction not only of the two main groups of FNH and adenomas, but allows also to distinguish subsets of adenomas with varying risk of malignant transformation. Therefore, to determine the subtype of adenoma represents now essential part of a biopsy diagnosis. Identification of the subsets of adenomas allows an individualized treatment with resection in high-risk forms and, on the other hand, allows avoiding liver resection in the case of small liver mass with a low risk of malignant transformation. U 53 letého muže byl exstirpován rozsáhlý nádor hrudní stěny, který byl histologický ověřen jako metastáza maligního hepatomu. V druhé době byl odstraněn primární jaterní nádor rozšířenou pravostrannou lobektomií. Kazuistika dokumentuje vzácnou vzdálenou metastázu maligního hepatomu a úspěšné paliativní chirurgické řešení.
In a 53-year-old man an extensive tumour of the thoracic wall was extirpated. It was examined histologically and identified as a secondary of a malignant hepatoma. In the second stage a primary hepatic tumour was removed by extended right-sided lobectomy. The case-history provides evidence of a remote secondary of a malignant hepatoma and a successful palliative approach.
